의사결정분석을 이용한 우리나라 노인의 요양시설서비스 이용 결정요인에 관한 연구 (A Study on Factors of Elderly Residential Care Service Utilization for using Decision Tree Regression)

  • 본 연구는 노인장기요양보험제도 2차 시범사업 결과를 바탕으로 우리나라 노인의 요양시설 서비스 이용에 영향을 미치는 요인을 탐색해 보고자 하였다. 이론적 모형으로는 앤더선과 뉴만(Andersen and Newman)의 행동주의 모델을 토대로 하여 영향요인을 구분하였으며, 연구방법은 의사결정분석을 이용하였다. 분석결과 서비스 이용에 가장 큰 영향을 미치는 것은 대상 노인의 서비스 이용의향이었으며, 이러한 선행요인 다음 중요한 요인으로 자원 요인(수발자 있음 및 동거가족 여부)으로 나타났다. 욕구요인으로는 인지장애가 있는 경우, 치매로 인한 문제행동이 있는 경우, 기본적 일상생활수행능력에 있어 완전도움을 받아야 하는 경우로 나타났다. 이러한 분석결과는 앤더슨과 뉴만의 행동주의 이론의 과정을 그대로 보여주고 있음을 알 수 있는데, 즉 선행요인과 자원요인에 의해 서비스 이용형태가 달라지며, 이러한 선행요인과 자원요인은 욕구요인과 결합하여 실제 서비스 이용에 영향을 주는 것으로 나타났다. 이를 통해 본 연구는 시설 서비스 이용 대상자의 욕구요인을 정확히 파악하여 이에 적절한 서비스 내용을 구축하고 지역사회 내 시설서비스 이용노인을 선별하는데 기초 자료를 제공하는데 기여하고자 한다.

  • 최근 복지서비스의 확대에 힘입어 여러 정부기관에서 복지 관련 서비스를 다양하게 제공하고 있으며 이에 따라 복지관련 서비스의 양과 질의 향상이 동시에 이루어지고 있다. 복지서비스 향상과 더불어 부적정 신청자로 인한 부정·중복수급율도 증가하고 있다. 특히 여러 기관에서 유사하게 추진되는 사업의 경우 기관들 간 시스템 불일치 및 자격검증 지연의 문제 등에 기인한 중복신청 확인이 미흡할 수 있으며 이로 인한 중복수급이 발생할 수 있다. 본 논문에서는 여러기관에서 진행되는 복지서비스 유사사업 관련 중복수급 방지를 위하여, 기관 간 데이터 관리 및 공유의 문제점을 검토하고 블록체인기술의 적용방법을 단계별로 제안하였다. 본 논문의 제안을 통하여 수급대상자의 투명한 데이터 관리가 가능하며 이를 통하여 신뢰기반의 복지급여 관리가 가능할 것이다.

재가노인을 위한 가정배달 급식관리체계 및 급식서비스 현황 조사 (Foodservice Management Systems of Home-Delivered Meal Service Program for Home-Bound Elderly)

  • The purposes of this study were to : a) examine the current foodservice management practices of twenty-one seniors centers in charge of hoke-delivered meal programs, b) evaluate the attitudes of one hundred and ten recipients of meal service program, and c) provide feedback for the efficient and effective foodservice management for the elderly. Statistical data analysis was completed using the SAS package program for descriptive analysis, T-test, and ANOVA. The results of quantitative analysis indicated that the costs of meals, containers and special foods were mostly dependent on the support from local government(Seoul city). The centers where the volunteers were over seventy five p ercent of the workers were frtty-eight percent and sixty-seven percent of the subjects in food preparation and food delivery to the homebound clients, respectively. Meal preparation and food purchasing were mainly practiced by social welfare worker. Standardized recipes were not developed and meal preparation was controlled under the cook' and volunteers' experiences. The survey results of recipients who participated the home-delivered meals program showed that the mean of meal satisfaction score was rated over three point five in the five-point scale. There were significant differences between dependent variables(volunteer's kindness, plate waste, menu variety) and independent variables(sex, receiving periods and family type of the subjects). 의료급여 과다 이용 수급권자 중 장애인의 의료이용 (The Medical Use of the Disabled Among Overusers of Medical Aid in Korea)

  • Objectives: In Korea, the top 10% of Medical Aid recipients represent nearly 60% of total payment, with the costs for those disabled for over 365 days representing approximately 30% of total payment. The purpose of this study was to compare Medical Aid use of the disabled with non-disabled recipients, and to identify contributing factors to the total payment in the top 2% of recipients identified as Medical Aid overusers. Methods: Subjects (n=2,211) selected were ${\geq}18$-years-of-age and received >1000 days of co-payment-free type I Medical Aid. Case managers (n=200) conducted interviews in December 2006, and collected data from Health Insurance Review & Assessment Service. Amounts over the 9 months from January September 2006 were analyzed descriptively and using Chi-square, ANCOVA, and robust multiple linear regression. Results: Disabled individuals (mean age 61.3 years) composed 36.6% of subjects; 44.8% of the disabled were male. On a monthly basis per capita, the disabled group averaged 10.5 outpatient days, total payment of 523,000 Korean Won(₩), inpatient payment of ₩359,000, and outpatient payment of ₩183,000. All values exceeded the monthly average for non-disabled individuals. Contributing factors were identified as male gender (₩82,000), elementary school or lower educational level (₩64,000), residence in a small city (₩82,000), lack of family support (₩61,000), kidney disability (₩673,000), intellectual disability (₩151,000), and multiple disabilities (₩119,000). Conclusions: The identification of contributing factors to Medical Aid use by those defined as disabled supports the adoption of comprehensive alternative policies such as strengthening of education and consultation services, provision of alternative facilities, and promotion of self-care.

성 주류화의 관점에서 본 저소득 실업자에 대한 분석과 자활사업 정착을 위한 정책 제안 (An Analysis on the Low- income Unemployed and a Policy Development for the Self-supporting Program on Gender-mainstreaming Perspective)

  • In the last two decades, the welfare states have undergone the changes of restructuring towards two ways. One is a restructuring of workfare and the other is a restructuring of the gender model of welfare state. In Korea, the workfare is reflecting on the DJ Welfarism 'Productive Welfare', but the gender model has a little effect on the public policies. By the way, It is imported that has the gender perspective in approaching the self-supporting program in the public assistance representing of the DJ Welfarism. Because almost 60% of the beneficiaries of the program are women. The Gender-mainstreaming strategy criticizes for the laws, the public policies, the public programs that considered gender-neutral or gender-blinded, and then complete the equality between women and men through the modification or development of those. The approach of Gender-mainstreaming is very important strategy for not only women who are self-reliance recipient but also successful settlement of self-supporting program. Nowadays that program is not execute yet. Then we hardly have information about recipients and the results that expected from that. At this point of gender-mainstreaming, this report suggest the strategy to development and settlement of the self-supporting program in the basics of analysis for the low-income unemployed and the government policy response to unemployment. For the gender models of the self-supporting program, the most important thing is throwing the male breadwinner model and adoption the dural-earner model (the gender model) about that program. Then we must produce gender-statistics data, develop programs for public work, job replacement, job training, evaluating system, etc. with gender perspective.

사회복지서비스 이용에 영향을 미치는 요인 - 일반계층과 저소득층의 비교연구 - (A comparative study on factors of social welfare service utilization between general and low-income group)

  • 본 연구에서는 사회복지서비스의 이용여부에 영향을 미치는 요인과 이용수준에 영향을 미치는 요인을 소인성 요인, 가능성 요인, 욕구요인으로 구분하여 각각의 영향관계를 살펴보고 이를 일반계층과 저소득층으로 나누어서 비교해 보았다. 분석결과, 사회복지서비스 이용여부에 있어서는 일반계층과 저소득층 모두 배우자가 있는 경우, 동거가족이 많을수록, 건강이 안좋다고 느낄수록, 만성질환이 없는 경우에 사회복지서비스를 이용할 가능성이 높았다. 사회복지서비스 이용수준에 있어서는 배우자가 없는 경우, 가구소득이 일반계층에서는 적을수록, 저소득층에서는 높을수록, 동거가족수는 많을수록, 건강이 좋지 않다고 느낄수록, 만성질환이 있는 경우가 사회복지서비스 이용수준이 높았다. 이 외에 저소득층에서는 여자보다 남자일 경우, 연령이 적을수록, 고졸이상보다는 중졸이하 집단이, 취업자보다는 미취업자가, 거주지역이 서울 광역시일 경우, 생활만족도가 낮을 경우 서비스 이용수준이 높아졌다. 이상의 연구결과는 각 계층별 특성과 욕구가 다르며, 서비스의 목적과 내용에 따라 잠재적인 대상자들의 욕구조사를 통해 보다 체계적이고 적절한 서비스를 제공할 필요가 있음을 시사한다.

농업인 복지정책 요구도 분석 (A Study on Farmer's Requisition on Welfare Policy)

  • The purpose of the present study is to seek measures to increase the quality of farmers lives by considering the results of a survey on the welfare demand for them when setting up policy. The results are as follows: First, though many concerns and policies to control the rural exodus have been exerted in terms of maintaining agricultural communities, which is one of critical components for the development of Korean agriculture and farming villages, the rural exodus became more and more serious. It is urgently required, therefore, for the Korean administration to correspond actively to the reasons why farmers depart their villages, such as bad prospects, low incomes or problems with their children's education. Second, as the number of females who take part in farming has increased, it is necessary to reform the rural educational environment. In addition, as the agricultural community develops, and other industries flow in, the greater the worries about crime, and consequently, it is necessary to equip a local safety system, share information on job opportunities, and provide educational opportunities. Third, since rural districts nay have financial difficulties because of old-aged and small-sized farmhouse, it is necessary to mainly support the welfare of the aged and lower-income farmers, considering the characteristics of each group and area. Fourth, over half of the farmers responded that they cannot bear to pay the national pension and health insurance premiums, and therefore it should be considered to reduce the pension and premium, or to readjust the insurance value appropriate to the level of the farmer's income. Fifth, despite a number of agricultural policies now in place, farmers desire a policy which can promise a prosperous vision for Korean agriculture and the facilities needed for education and health. It is necessary for the administration to set up policies that are characterized by locality with the view that farmers are not mere recipients, but participants in the policies.

미국 TANF정책이 소득 및 빈곤에 미친 영향: 하부조항별 분석 (The Impact of TANF on Income and Poverty : analyses by sub-component)

  • 우리나라 국민기초생활보장제도는 여전히 노동시장참여를 유인할 만한 체계가 불충분하여 취업을 통한 탈빈곤을 꾀하기에는 역부족이라는 문제가 지적되어 왔다. 반면 미국은 1996년 AFDC를 TANF로 대체하고, 근로의무나 수급기간제한 조항 등을 강화함으로써 대상자들의 탈빈곤을 달성할 것이라 인정받고 있다. 미국의 TANF가 우리제도에 대안적일 수 있는지에 대한 상반된 의견이 존재하는 가운데, 본 논문은 TANF가 과연 탈빈곤 효과가 있는지를 하부조항별로 분석하였다. 분석결과 TANF의 근로의무조항은 오히려 소득을 감소시키고 빈곤발생을 증가시키는 것으로 나타났다. 수급기간제한은 이와 반대의 결과가 발견되었으나 아직은 그것의 효과를 확신할 수 없는 단계이다. 이 분석 결과는 TANF도 탈빈곤이라는 목표를 달성하려면 대상자들을 무조건 노동시장으로 내모는 것으로는 불완전하고, 저임금 노동자를 둘러싼 근본 문제를 동시에 해결해야 함을 시사한다.

여성독거노인가구의 과부담 의료비 지출에 관한 연구 (The Financial Burden of Catastrophic Health Expenditure Among Older Women Living Alone)

  • Older women who live alone are among society's most vulnerable people, since they experience increased risk of multiple chronic diseases and have limited financial protection. This can lead older women living alone to catastrophic health expenditure(CHE), which is defined as a healthcare expenditure that exceeds a certain portion of a household's ability to pay. Using the Korean Longitudinal Study of Ageing(KLoSA), this study investigated the incidence of CHE among older women living alone and identified the factors related to this incidence. Applying health expenditure thresholds of 10%, 20%, 30% and 40% of ability to pay, the proportions of those with CHE were 41.3%, 22.9%, 14.6%, and 9.4%, respectively. Logistic regression models were used to identify factors related to CHE incidence, which include demographics, income, the number of chronic diseases, perceived health status, and health insurance type. The results show that the health care safety net in South Korea is insufficient for older women living alone. The findings can guide policymakers in improving healthcare and welfare policies to protect people from catastrophic payments. Particularly, welfare policies should be established for poor non-recipients who are not included within the benefits scope of the National Basic Livelihood Security System due to the unrealistic criteria of income recognition and family support obligation.

"Leisure Life Satisfaction:" Will It Have a Beneficial Impact on Depression Among Older Adults in Community Care Settings in Korea?

  • Objectives: While the risk of depression is expected to substantially increase among older adults receiving community care, leisure life satisfaction can be regarded as a key component in enhancing the mental health of those receiving community care. However, it is not yet known whether community care utilization increases the risk of depression, or what role is played by leisure life satisfaction in these settings. This study investigated the relationship between community care utilization and depression, as well as the main effect and the moderating role of leisure life satisfaction on the link between community care utilization and depression among older adults. Methods: This study, using the 2019 Korean Welfare Panel Survey, conducted multiple regression analysis on data from 4494 elderly people aged 65 years or older. Results: After controlling for potential covariates, older community care recipients were more likely to report symptoms of depression than those who did not receive community care. Meanwhile, leisure life satisfaction was negatively associated with depression in older adults. The test for interaction between community care utilization and leisure life satisfaction revealed that leisure life satisfaction significantly attenuated the link between depression and community care utilization. Conclusions: The findings of this study imply that leisure life satisfaction could play a meaningful role in improving the mental health of older adults receiving community care. Welfare policies affecting older adults should consider leisure life satisfaction as an important resource for reducing depression in community care settings.
