병원간호사의 직무 스트레스, 윤리적 딜레마 및 소진이 이직의도에 미치는 영향 (Influences of Occupational Stress, Ethical Dilemma, and Burnout on Turnover Intention in Hospital Nurses)

  • Purpose: The aim of this research was to identify the relationships between occupational stress, ethical dilemma, burnout and turnover intention and the factors influencing turnover intention in hospital nurses. Methods: The participants, 215 nurses, were recruited from two university hospitals located in two provinces of Korea. Data were collected through self-report questionnaires. Collected data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression with SPSS/WIN 22.0. Results: Participants' scores for occupational stress, ethical dilemma, burnout and turnover intention were $3.96{\pm}0.45$, $2.60{\pm}0.39$, $3.71{\pm}0.70$, and $3.50{\pm}0.90$ respectively. The results show that the higher the score for occupational stress, ethical dilemma, and burnout the higher the score for turnover intention. The most important factor influencing turnover intention was burnout (${\beta}=.42$, p<.001), followed by position (${\beta}=.21$, p=.001), educational level (${\beta}=.21$, p=.007), ethical dilemma (${\beta}=.13$, p=.020) in that order. These factors explained 43.5% of total variance in turnover intention (F=19.34, p<.001). Conclusion: Changes in hospital structural management systems and support strategies should be developed and operated, taking into account burnout, ethical dilemma, and personal characteristics of clinical nurses in order to reduce turnover.

Development of Korean Version Burnout Syndrome Scale (KBOSS) Using WHO's Definition of Burnout Syndrome

  • Background: Burnout syndrome (BOS) is defined by the World Health Organization (WHO) as a syndrome conceptualized as resulting from chronic workplace stress that has not been successfully managed. This study aims to create the Korean version burnout syndrome scale (KBOSS) that conforms to WHO's definition of BOS and present the cut-off points for screening. Methods: We developed the KBOSS based on WHO's definition of BOS. An online survey was conducted through a specialized online research company. We recruited 444 workers for this research. The validity of the KBOSS was assessed using factor analysis and Pearson's correlation. The KBOSS reliability was assessed using Cronbach's alpha coefficient. The cut-off points for each of the three dimensions were derived using the upper quartile score. Results: The validity and reliability of the KBOSS were good. Regarding reliability, the scale's overall Cronbach's alpha was 0.813. Cronbach's alpha of each three-dimension was as follows: exhaustion, 0.916; cynicism, 0.865; and professional inefficacy, 0.819. The cut-off points of BOS three dimensions are exhaustion S 21; cynicism S 18; and inefficacy S 15. Conclusion: The developed questionnaire (KBOSS) can be a useful tool for screening of BOS.

아이돌보미의 감정노동과 직무 스트레스가 소진에 미치는 영향 (The Effects of Child Care Helpers' Emotional Labor and Job Stress on Their Burnout)

  • Purpose: This study aims to understand baby care helpers' degree of emotional labor, job stress, and burnout and analyze the effects of emotional labor and job stress on their burnout. Methods: Data were collected from August 16 to September 6, 2016 for three weeks with child care helpers working in C city, D city, and S city as subjects. Multiple regression analysis was used in order to examine the factors influencing the subjects' burnout. Results: Burnout in general characteristics differed significantly according to age (F=4.81, p=.011). As the most influential factors for burnout, emotional labor (${\beta}=.43$) had statistically significant correlation with it, along with job stress (${\beta}=.16$) and age (${\beta}=.14$). These variables explained burnout ($Adj.R^2=0.29$, p<.001) at a rate of 29%. Conclusion: It is judged that strategies and continuous management aimed at reducing child care helpers' emotional labor and job stress are necessary. As an approach from different aspects, it is required that variables related to emotional labor, job stress, and burnout are identified and there would be more researches in order to establish some national policies for child care helpers.

응급실 간호사의 직무 스트레스, 스트레스 대처방식, 강인성이 소진에 미치는 영향 (Impact of Job Stress, Coping Behavior, Hardiness on Burnout in Nurses in the Emergency Department)

  • Purpose: This study aimed to identify the effect of job stress, coping behavior, and hardiness on burnout in nurses in the emergency department (ED). Methods: Data were acquired via self-reporting questionnaires from 148 nurses in EDs at one general hospital, four university hospitals, and six hospitals with >200 beds. Data were analyzed using the SPSS 23.0 program through descriptive statistics, t-test, analysis of variance, Pearson's correlation coefficient, and stepwise multiple regression. Results: A statistically significant difference in burnout was observed with respect to the age of the nurses. Burnout was positively correlated with job stress and wishful thinking and detachment, among coping behaviors, and was negatively correlated with hardiness. Job stress and wishful thinking, among coping behaviors, were significant predictors of burnout and were observed in 14.9% of the nurses who experienced burnout. Conclusion: According to our results, job stress was identified as the most significant factor which influenced burnout in ED nurse's. Therefore, it is necessary to develop and implement educational programs for reducing job stress and minimizing the use of wishful thinking as a coping behavior in order to alleviate the burnout in ED nurses.

임상간호사의 인문학적 소양과 직무 스트레스가 전문직 삶의 질에 미치는 영향 (Influence of Humanistic Knowledge and Occupational Stress on Professional Quailty of Life of Clinical Nurses)

  • 본 연구는 임상간호사의 전문직 삶의 질에 영향을 미치는 요인을 확인하여 전문직 삶의 질을 높이기 위한 기초자료 제공을 위해 수행되었다. 연구대상자는 일 광역시 3개 종합병원에 근무하는 간호사 217명으로, 2019년 6월 3일부터 6월 14일까지 구조화된 설문지를 사용하였고, 수집된 자료는 SPSS Statistics 23.0 program을 이용하여 기술통계, t-test, ANOVA, Scheffé test, Pearson's Correlation Coefficient, Stepwise multiple regression으로 분석하였다. 연구결과, 공감만족은 인문학적 소양과는 양의 상관관계, 소진과는 음의 상관관계가 있었고, 소진은 인문학적 소양과는 음의 상관관계, 직무 스트레스와 이차 외상성 스트레스와는 양의 상관관계가 있었으며, 이차 외상성 스트레스는 직무스트레스와 양의 상관관계가 있었다. 공감만족에 영향을 미치는 요인으로는 소진, 간호만족, 임상경력으로 나타났으며 48.5%의 설명력을 보였고, 소진에 영향을 미치는 요인에는 공감만족, 이차 외상성 스트레스, 간호만족, 기혼, 직무 스트레스로 나타났고 68.8% 설명력을 보였으며, 이차 외상성 스트레스에 영향을 미치는 요인은 소진과 급여로 나타났으며 21.9%의 설명력을 보였다. 이상의 연구 결과 소진은 공감만족과 이차 외상성 스트레스 모두에 영향을 미치므로 소진을 예방하는 노력이 요구된다.

  • Purpose: The purpose of this descriptive, cross-sectional study was to examine relationships among emotional labor, self-efficacy, and burnout of employees in public health centers. Factors that influence burnout of workers were also assessed. Methods: One hundred sixty six workers in public health centers completed a pack of self-report questionnaires. Collected data were analyzed using descriptive statistics, t-test, one-way ANOVA along with $Scheff{\acute{e}}$ test, and stepwise multiple regression. Results: Employees in public health centers seemed to experience emotional labor to some extent. There were statistically significant differences in burnout depending on the age of workers. Employees' burnout had a positive relationship with emotional labor and a negative relationship with self-efficacy. Factors influencing burnout of employees in public health centers were emotional dissonance, surveillance & monitoring of organization, age, and self-efficacy. Conclusion: Considering the findings from this study, health care providers need to develop effective interventions which increase the level of self-efficacy and decrease certain types of emotional labor for employees in public health centers and to help them effectively manage burnout.

뇌졸중환자 간병인의 감정노동이 소진에 미치는 영향 (The Impact of Emotional Labor on Burnout for Caregivers of Stroke Patients)

  • Purpose: The purpose of this study was to explore the impact of emotional labor on burnout in stroke patients' caregivers. Methods: Data were collected from 141 paid caregivers working at 3 hospital across Gyeonggi province from January to April 2014. Results: The caregivers' level of emotional labor showed 2.9/5.0 points and burnout, 2.5/5.0. A significant correlation was found among attentiveness to required display rules (p=.007), emotional dissonance (p=.001) and burnout. In a multiple regression, age (${\beta}$=.279, p=.001) and emotional dissonance (${\beta}$=.193, p=.046) were associated with burnout in caregivers. These factors attributed to 16.9% of variance in the burnout of caregivers. Conclusion: These results suggest that burnout in stroke patients' caregivers can be influenced by emotional dissonance as well as physical condition such as age. Accordingly, it is necessary to be develop support program to deal with mental labor to improve their mental health and lighten their workload for caregivers.

병원 간호사의 감정노동, 임파워먼트, 직무소진 및 이직의도 간의 관계 (The Relationship of Emotional Labor, Empowerment, Job Burnout and Turnover Intention of Clinical Nurses)

  • Purpose: The purpose of this study was to identify the relationships among emotional labor, empowerment, job burnout, and turnover intention. Methods: The design of this study was a cross-sectional descriptive study. The subjects of this study were 358 nurses working for over 6 months in Seoul and Gyeonggi-do. The data were collected from 4th to 30th of September, 2009 through one-to-one interviews. Survey data were analyzed using t-test, one-way ANOVA, Pearson's correlation coefficient, and hierarchical regression. Results: We find the positive correlation of turnover intention with emotional labor and with job burnout, but the negative correlation of empowerment with job burnout and with turnover intention. The nurses' turnover intention scores were relatively low with 1.30. Job burnout, shift work, and frequency of turnover were predictors of turnover intention. The most powerful predictor variable was job burnout (${\beta}$=.420, p<.001). We find that emotional labor and empowerment as determinants of turnover intention are not significant. Conclusion: These results suggest that manager should consider shift work of nurses and job burnout in order to manage nurse organization and resource effectively.

응급실 간호사의 외상사건 경험, 공감피로, 자아존중감, 공감만족 및 소진 (Effects of Traumatic Events, Compassion Fatigue, Self-esteem, and Compassion Satisfaction on Burnout of Nurses in Emergency Department (ED))

  • Purpose: The purpose of this study is to verify the factors that determines burnout of nurses in emergency department. Methods: The survey was given to 170 ED nurses which are located in Busan from July 1st to August 31st of 2012. The results were analyzed by descriptive statistics, t-test, ANOVA, pearson correlation coefficients and stepwise multiple regression using SAS 9.2 program. Results: There were statistically significant differences in burnout depending on gender, position, workplace satisfaction, job satisfaction. Significant positive correlation between compassion fatigue and burnout was found. Also significant negative correlation was found between self-esteem, compassion satisfaction and burnout. Factors influencing burnout were self-esteem, compassion satisfaction and compassion fatigue with $R^2$ value 58.0%. Conclusion: Considering these results, it seems that great efforts will be needed for reducing burnout by mitigating the emergency nurses' compassion fatigue and introducing programs to increase the self-esteem and compassion satisfaction.

직무요구와 직무자원이 병원행정직 간호사의 소진과 조직몰입에 미치는 영향 (The Effects of Job Demand and Job Resources on Burnout and Work Engagement of Hospital Nurse Administrators)

  • Purpose: This study aims to investigate the degree of job demand, job resources, burnout, and the organizational commitment of administrative nurses based on the job demands-resources model. Further, it seeks to confirm the influencing factors affecting nurses' burnout and organizational commitment. Methods: The participants were 188 administrative nurses working at hospitals (one tertiary hospital and six general hospitals) located in D City. The collected data were analyzed with IBM SPSS Statistics 23.0 using frequency, percentage, mean, standard deviation, t-tests, ANOVA, Pearson's correlation coefficient, and multiple regression analysis. Results: The influential factors of burnout were role conflict (β=.50), job demand (β=.18), job position (β=-.17, team leaders and above), and social support (β=-.15). The regression model had an explanatory power of 59%. The influential factors of organizational commitment were appropriate rewards (β=.59), job position (β=.15, team leader or above), working department (β=.14, referral center and health screening administration department), and social support (β=.18). The regression model had an explanatory power of 59.5%. Conclusion: The results support the job demands-resources model, and interventions should be developed to decrease job demand and provide sufficient job resources.
