간호·간병통합서비스 병동 간호사의 환자안전문화인식, 감정노동, 직무스트레스가 환자안전간호활동에 미치는 영향 (Effects of Awareness of Patient Safety Culture, Emotional Labor and Job Stress on Patient Safety Nursing Activities by Comprehensive Nursing Care Medical Service Ward Nurses)

  • Purpose : This study aimed to examine the effects of the awareness of patient safety culture, emotional labor, and job stress on patient safety nursing activities by comprehensive nursing care medical service ward nurses. Methods : The participants were 112 nurses from the comprehensive nursing care service wards from six hospitals. Data were collected using structured questionnaires and analyzed using a t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and multiple regression analysis using SPSS version 20.0. Results : The factors that significantly influenced patient safety nursing activities in the comprehensive nursing care medical service ward nurses included job stress (𝛽=-0.18, p<.032) and awareness of patient safety culture (𝛽=0.54, p<.001). The explanatory power of these factors was 31.0% (F=13.43, p<.001). Conclusion : This study showed that the factors that influenced patient safety management activities of comprehensive nursing care medical service ward nurses included job stress and awareness of patient safety culture.

  • 본 연구는 간호간병통합병동과 일반병동에 근무하는 간호사의 업무수행능력 차이를 확인하기 위한 서술적 조사연구이다. D광역시의 S병원에 근무하는 간호사 185명을 대상으로 자료를 수집하여 간호전문직관은 윤은자 등(2005)이 개발한 간호전문직관으로, 업무수행능력 정도는 이원희 등(1990)이 개발한 임상수행능력 측정도구를 이용하여 측정하였다. 자료는 SPSS 20.0 프로그램을 이용하여 기술통계, t-test, ANOVA, Duncan test, Pearson's correlation coefficients, multiple regression을 이용하여 분석하였다. 본 연구결과 간호간병통합병동 간호사가 일반병동 간호사보다 간호전문직관이 높게 나타났고, 간호간병통합병동과 일반병동의 간호사 모두 간호전문직관이 업무수행능력 향상에 영향을 미치고 있음을 확인할 수 있었다. 그러므로 향후 간호사의 간호전문직관을 정립하는 방안을 모색하여 간호업무의 수행능력을 향상시키는 방안으로 활용할 수 있을 것이다.

  • 본 연구는 간호간병통합서비스병동 간호사의 이직의도에 감정노동, 회복탄력성, 간호근무환경이 미치는 영향을 확인하고자 시도되었다. 자료는 간호간병통합서비스병동 간호사 186명을 대상으로 구조화된 설문지를 이용하여 자료수집 하였고, SPSS WIN Version 22.0을 이용하여 분석을 실시하였다. 연구결과, 간호간병통합서비스병동 간호사의 감정노동, 회복탄력성, 간호근무환경, 이직의도의 정도는 각각 평균 3.66점, 3.52점, 2.62점 및 3.82점으로 나타났다. 간호간병통합서비스병동 간호사의 이직의도는 감정노동과는 양의 상관관계, 회복탄력성 및 간호근무환경과는 음의 상관관계가 있는 것으로 나타났고, 이직의도의 영향요인은 감정노동, 회복탄력성, 간호근무환경, 성별로 나타났다. 따라서, 간호간병통합서비스병동 간호사의 이직의도를 감소시키기 위해서는 감정노동, 회복탄력성, 간호근무환경, 성별을 고려하여 중재프로그램을 개발하여 적용하는 것이 필요하다고 사료된다.

급성기병원에서의 간호 · 간병통합서비스 운영 전후 낙상 및 욕창 발생 변화 (Performance of Comprehensive Nursing Care Service in an Acute Care Hospital: Focusing on Accidental Falls and Pressure Injuries)

  • Purpose: This study aimed to investigate whether the comprehensive nursing care service positively affected accidental falls and pressure injuries. Methods: This study was a retrospective study that analyzed the accidental falls and pressure injuries cases in an acute care hospital located in Seoul and compared the rates of accidental falls and pressure injuries before and after the comprehensive nursing care service was operated. Results: Comparing the accidental fall incidence rates per 100 person-months between a comprehensive nursing care ward and a general ward, it showed fewer accidental falls by 0.44 in comprehensive nursing care wards, but the result was not statistically significant. In the case of pressure ulcers, the incident rate per 100 person-month was 6.17 in general wards and 4.77 in comprehensive nursing care wards, which showed that the number of pressure ulcer patients was lower in comprehensive nursing care wards, however it was also not statistically significant. Conclusion: It is not confirmed that the operation of the comprehensive nursing care service contributes to the reduction of accidental hospital falls or pressure injuries. Follow-up studies are recommended to determine the effectiveness of comprehensive nursing services in quality indicators.

  • 본 연구는 전국적으로 확대 시행하고 있는 간호 간병통합서비스병동 간호사와 일반병동 간호사의 직무만족, 직무스트레스, 직무소진 및 이직의도를 조사하고 비교하여 간호 간병통합서비스 병동의 효율적인 간호인력 관리를 위한 기초자료를 제공하기 위한 비교조사 연구이다. 대상자는 G광역시 소재 간호 간병통합서비스를 시행하고 있는 6개 병원에서 간호 간병통합서비스병동 간호사와 일반병동 간호사 181명으로, 자료수집은 구조화된 설문지를 이용하여 2017년 5월 16부터 6월 30일까지 실시하였고, t-test, one-way ANOVA, Pearson 상관관계를 이용하여 분석하였다. 연구결과, 간호 간병통합서비스병동 간호사와 일반병동 간호사의 직무만족, 직무스트레스, 직무소진 및 이직의도에서 집단 간의 차이는 없는 것으로 나타났으나, 간호 간병통합서비스병동 간호사가 직무만족 점수는 더 높았고, 직무스트레스, 직무소진 및 이직의도 점수는 더 낮았다. 또한 간호 간병통합서비스병동 간호사는 직무스트레스(r=.39, p=<.001)와 직무소진(r=.43, p=<.001)이 높을수록 이직의도가 높게 나타났고, 직무만족(r=-.66, p=<.001)이 높을수록 이직의도가 낮게 나타났다. 간호 간병통합서비스병동 간호사는 일반병동 간호사에 비해 직무 관련해서 더 긍정적으로 생각하고 있었다. 따라서 본 연구결과는 간호 간병통합서비스병동의 효율적인 간호인력 관리를 위한 기초자료로 활용될 수 있을 것이다.

  • Background: The purpose of this study was to identify role ambiguity of comprehensive nursing care unit nurses. Methods: A concept analysis method by Walker and Avant was used to understand role ambiguity of comprehensive nursing care unit nurses. Results: The antecedents of role ambiguity of nurses at comprehensive nursing units were shortage of nurses, unclear admission criteria, and demands for customized nursing care according to severity. Attributes include ambiguity in role delegation, patient placement ambiguity, and professional ambiguity among nursing staff. The consequences were diminished job satisfaction due to excessive workload, difficulty in resolving role ambiguity due to the lack of work analysis studies, and poor outcome of nursing indicators. Conclusion: Improvement of nationwide awareness for comprehensive nursing care unit is required. Clear division at scope of practice for nursing staff in accordance of each medical institution's characteristics is essential. Nurses at comprehensive nursing care unit should understand nature of role ambiguity that occurs as they work in large groups. Nurses should promote communications between nursing staff and they must have volition to improve status quo. An additional research of comprehensive nursing care on the causes of role ambiguity in the practice of nursing care for ward nurses is needed, and management measures should be sought at the organizational level.

교육전담간호사가 주도하는 교육 프로그램에 대한 간호·간병통합서비스 병동 간호사의 교육 만족도 측정 도구 개발 및 타당도 검정 (Development and Validation of a Education Nurse Specialist-Led Education Satisfaction Scale for Nurses Working at Comprehensive Nursing Care Service Wards)

  • Purpose: The purpose of this study was to develop and validate a education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service wards. Methods: A methodological study was conducted. A total of 237 nurses working at comprehensive nursing care service wards in a general hospital participated in this study. The scale was developed through literature reviews, in-depth interview, development of preliminary items, verification of content validity, development of secondary items, evaluation of construct validity, and extraction of final items. Analysis included exploratory factor analysis, pearson's analysis, and reliability analysis using cronbach's α. Results: The education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service ward consisted of 21 items. Two factors (critical thinking disposition and clinical competency) were identified which explained 63.5% of the total variance. Cronbach's α of each factors were >.95. Conclusion: The results suggested that the education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service wards demonstrated acceptable validity and reliability. Items of the instrument can assess the level of satisfaction with regards to education led by education nurse specialist among nurses working at comprehensive nursing care service wards.

간호조직문화, 셀프리더십이 공공병원 간호·간병통합서비스 병동 간호사의 직무만족에 미치는 영향 (Effects of Nursing Organizational Culture and Self-leadership on Job Satisfaction for Nurses Working in the Comprehensive Nursing Care Service Ward in Public Hospitals)

  • Purpose: This study investigated the effects of nursing organizational culture and self-leadership on job satisfaction for nurses working in the comprehensive nursing care service ward in public hospitals. Methods: The participants were 138 nurses working in Gyeonggi-do medical center. The collected data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, and regression analysis with SPSS/WIN 23.0. Results: Factors affecting job satisfaction were innovation-oriented culture (${\beta}=.29$, p<.001), want to work in this department (${\beta}=-.25$, p<.001), the cognitive strategy of self-leadership (${\beta}=.23$, p=.002), hierarchical-oriented culture (${\beta}=-.21$, p=.002), and marital status (${\beta}=.18$, p=.005). These factors accounted for 42.0% of the variability in job satisfaction. Conclusion: In order to improve the job satisfaction of nurses in the comprehensive nursing care service ward in public hospitals, institutional-level efforts should be made to encourage an innovation-oriented culture and avoid a hierarchy-oriented culture. In addition, education and program development are needed to improve the cognitive strategy of self-leadership.

  • Objectives: The purpose of this study is to evaluate the factors affecting patient satisfaction for the nursing service of comprehensive nursing care service unit in Tertiary hospital Methods: This study employed a descriptive survey to investigate the nursing satisfaction and the study data was collected from 74 patients among 83 patient who were admitted to the nursing care integration service ward of an tertiary hospital for 10 days from May 9, 2018 to May 19, 2018. Data was analyzed by descriptive statistics, t-test, ANOVA, and multiple linear regression using SPSS 21 program. Results: The average with nursing satisfaction was significantly different according to marriage type and KPCS level. It was found that physical factor and educational factor have statistically significant effect on nursing satisfaction. As physical care increase by one unit, patient satisfaction increase by 0.226(95% CI; 0.061-0.390) and also satisfaction increase by 0.443(95% CI; 0.070-0.816) as education care increase by one unit. Conclusions: As a result of study, it was found that it is necessary to provide patients with more careful nursing service in physical and educational perspective, in order to improve the service quality of comprehensive nursing care service unit.

간호·간병 통합병동에 입원한 노인환자의 낙상예방교육 효과 (Effect of Fall Prevention Education for Older Patients in Comprehensive Nursing Care Service Ward)

  • Purpose: This study was conducted to examine the effect of a fall prevention education on the fall related knowledge and fall prevention behavior of the elderly patients in comprehensive nursing care service wards. Method: A quasi experimental study with control group and experimental group was used. A total 62 elderly patients in comprehensive nursing care service wards were randomly divided into the experimental group(n=30) and the control group(n=32). The data collection period was from August to December, 2017. Data were analyzed with $X^2-test$, paired t-test, independent t-test and Pearson's correlation coefficient by using SPSS 25.0. Results: Experimental group reported higher score in fall related knowledge than the control group(t=-10.28, p<.001). Participants who viewed DVD and received the leaflet education reported higher score than those with routine admission education(t=-6.51, p<.001). The experimental group showed significant improvement in fall related knowledge and fall prevention behavior(r=.21, p=.015). Conclusion: The fall prevention program was effective in improving fall-related knowledge and fall prevention behavior among elderly patients. Since the DVD plus leaflet education was effective in improving prevention behaviors, it might be included in routine orientation on admission for the elderly patient in comprehensive nursing care service wards.
