Background : The objectives of this study were to examine patients' satisfaction with the DRG based payment method and its association with their awareness of the method, to examine patient reported changes in doctors' caring attitude, level of their out-of-pocket payments, providers' acceptance of patients' request for additional services after the program, and to examine changes in service utilization recorded in medical records. Method : One hundred-four patients who had cesarean sections before and after the demonstration program at two hospitals located in Seoul participated in the study. Patients were surveyed before discharges when their charges were finalized. Their medical records were reviewed as well to collect data for service utilization during hospital stays. The association between patients' satisfaction with the payment method and their awareness of the method was analyzed by ${\chi}^2$-test, and the significance of changes in providers' acceptance of patients' request for additional services and service utilization after the program were examined by ${\chi}^2$-test and t-test, respectively. Results : A large proportion of patients did not know of the DRG based payment method at the time of survey and a significantly larger proportion of patients who came to the hospitals with the knowledge satisfied with the method. About the same proportion of patients reported improvement and deterioration in doctors' caring attitude compare to the previous hospitalizations and a similar result was found concerning out-of-pocket payments. Providers' acceptance of patients' request for medication, PCA and painless delivery decreased significantly after the program whereas the acceptance for additional hospital days and laboratory and radiology tests did not. Length of stay, the numbers of days on antibiotics and antianemic medication, and the number of blood tests decreased significantly after the program, however, decreases in the rate of antianemic medication and the number of urine analyses were not statistically significant. Re-operation, in-hospital death, and complications were not observed before and after the program. Conclusion : The study findings indicated a need for better patient education and publicity about the newly introduced payment method to improve their satisfaction with the system. Other study findings concerning service utilization and quality of care indicators were consistent with the government funded evaluation studies.
Purpose: This study was to investigate the cancer screening rates and influence factors in island residents. Methods: The participants were 1,223 Shinan gun island residents. Data were collected using structured questionnaires from June 23th to September 8th, 2007 and analyzed using the SAS win 12.0 program. Results: The cancer screening rate was 49.9%. There were significant differences for sex, age, living with family, economic level, smoking, exercise, private health insurance, familial history, health concern. The highest practice rate was of stomach cancer (55.9%), which is gastric endoscopic exam. The most common motivation of getting a screening test was the concern of health (40.8%), and many had no recommender of the screening test (30.0%). 58.4% of the subjects were satisfied with the screeing tests and the most frequent reason of the satisfaction was 'rapid result report' (33.1%). The msot common reason of unsatisfaction was 'long waiting time' (25.7%). Most participants agreed with the necessity of cancer screening (74.9%). More than half participants said they would participate in another cancer screening tests in the future (51.9%). Private health insurance, exercise, health concern and smoking showed significant predictors (20.6%) of obtaining cancer screening. Conclusion: The results suggest that health care professionals should give more attention to help the residents obtain cancer screening tests. A further study is necessary to develop any effective intervention for people who do not practice cancer screening tests.
Purpose: This study was to understand the meaning of women's experience of spontaneous delivery with midwives at midwifery clinics or home. Purpose: This study aimed to investigate the maternal health effects of internet-based education interventions on parturients during the postpartum period through a systematic review of randomized controlled trials. Methods: An electronic literature search of the Cochrane Library, CINAHL, EMBASE, Eric, PsycINFO, PubMed, RISS, and KISS databases was performed, using the combination of keywords such as 'parenthood education', '*natal education', '*birth intervention', 'internet-based intervention', 'randomized controlled trial'. The inclusion criteria were peer-reviewed papers in English regarding randomized controlled trials of internet-based postnatal education interventions. Educational interventions were delivered through any web, mobile, eHealth, mHealth, virtual reality, short message service, or social networking service platform. Quality appraisal was performed using the Risk of Bias 2 (RoB 2) for randomized controlled trials. Nine articles were yielded, and the intervention effects were analyzed. Results: Internet-based education interventions during the postpartum period affect maternal self-efficacy, postpartum depression, and successive breastfeeding; however, they do not affect maternal satisfaction and parenting confidence. Conclusion: This study demonstrated that internet-based education interventions affect maternal health status in terms of psychological, emotional, and physical wellness. Therefore, maternal health care professionals can utilize remote education using the internet or mobile-based interventions during the postpartum period.
