임상간호사의 직장 내 괴롭힘에 대한 대처 경험: Q 방법론적 접근 (A Study on Clinical Nurses' Coping to Workplace Bullying: Q Methodological Approach)

  • Purpose: The purpose of this study was to provide basic data to understand the organizational culture of nurses by categorizing nurses' experience of coping with bullying in the workplace through Q methodology and analyzing the characteristics of each type, and to induce correct policy measures and interventions to create an atmosphere created in the nursing clinical field to be more advanced and positive. Methods: To form the Q population, focus group interviews were conducted with nurses working for more than six months at two general hospitals in Seoul and Gyeonggi. Interviews were conducted by 12 nurses introduced to participants who can provide researchers with a wealth of information on workplace bullying experiences without filtration. In addition, the Q population was extracted by reviewing the results. Based on the results derived from this, 38 Q statements in total were extracted. Forty clinical nurses were required to classify Q sample statements, and the data collected through this were analyzed using the pc-QUANAL program. Results: As a result of the analysis, a total of five types of clinical nurses' experiences of coping with bullying in the workplace were identified: 'tense emotion-based tolerance response,' 'positive thinking-based self-effort response', 'individualistic thinking-based passive response', 'support system-based emotional expression response' and 'active response centered on problem-solving'. Conclusion: The derived response types are expected to be guidelines for suggesting strategies to eradicate bullying in the workplace at the organizational level, individual level, prevention level, and organizational culture level.

병원조직에서 간호사가 경험한 태움에 관한 현상학적 연구 (A Phenomenological Study on Tae-um Experienced by Nurses in Hospital Organizations)

  • 본 연구는 간호사를 대상으로 병원조직에서 겪은 태움에 관한 경험의 의미와 본질적인 구조를 파악하기 위한 질적 연구이다. 참여자는 해당 병원의 협조를 받아서 직장내 SNS를 통하여 연구에 참여하겠다고 동의한 자를 대상으로 10명을 선정하여 면담을 진행하였다. 수집된 자료는 Colaizzi의 현상학적 방법을 적용해 분석하였다. 연구 결과 간호사의 병원조직 내 태움은 4개의 범주(지독한 첫발 내딛기, 끝날 것 같지 않은 힘듦의 연속, 스스로 버텨봄, 적응을 위한 비상의 날갯짓 하기)와 16개의 주제들로 도출되었다. 결론적으로 태움은 신규간호사 뿐만 아니라 경력을 쌓은 간호사들조차도 여전히 현재진행형이므로 힘든 병원조직에 적응을 하고 있음을 확인할 수 있었다. 따라서 간호사들을 위한 심리적 지원프로그램을 도입한다면 태움 문제를 해결하고, 간호사들이 존중 받는 조직문화를 이루는데 기여할 것이다.

  • Purpose : The aims of this study was to provide basic data to enhance the professional self-concept of nurses in intensive care unit and identify the relationship between job stress, working bullying, self-efficacy and professional self-concept and identify the factors influencing professional self-concept in intensive care unit nurses. Methods : This study was conducted from June to August, 2018 at five general hospitals over 500 beds. Structured questionnaire were completed by 120 nurses who had agreed to understand and participate in the study. The collected data were analyzed using the SPSS win 24.0 program. Results : Factors affecting the professional self-concept of the subjects were self-efficacy, working bullying, unsatisfaction and satisfaction of nursing job satisfaction, age. The explanatory power of these variables on professional self-concept was 55.1%. Conclusion : The self-efficacy, working bullying, nursing job satisfaction, and age of the intensive care unit nurses were identified as the major factors influencing the professional self-concept. Therefore, it is necessary to develop and apply an educational program to improve self-efficacy and nursing job satisfaction and reduce working bullying, which are influencing factors of professional self-concept of intensive care unit nurse.

  • Purpose: The purpose of this study was to identify the relationship between organizational culture and workplace bullying of intensive care unit (ICU) nurses. Methods: A secondary analysis was conducted using three different data sets on workplace bullying of hospital nurses. This analysis focused on the relationship between organizational culture and workplace bullying of 240 ICU nurses. The original data were collected using self-reporting questionnaires and were analyzed using hierarchical multiple regression. Results: ICU nurses were aware of their organizational culture as being rank-oriented, relationship-oriented, innovation-oriented, and task-oriented in that order. The results showed that 15.0% of the ICU nurses reported being victims of workplace bullying, mostly work-related bullying. The perception of a relationshiporiented culture explained a variance of 29% in the workplace bullying of ICU nurses. Conclusion: The findings suggest that ICU nurses who are strongly aware of a relationship-oriented culture would experience less workplace bullying. Further research is needed to develop interventions that can foster a relationship-oriented culture in ICUs.

간호사의 직장 내 약자 괴롭힘, 리더-구성원 교환관계가 이직의도에 미치는 영향 (Influence of Workplace Bullying and Leader-Member Exchange on Turnover Intention among Nurses)

  • Purpose: The purpose of this descriptive study was to identify the impact of workplace bullying and LMX (Leader-Member Exchange) on turnover intention among nurses. Methods: The participants were 364 nurses from the Seoul metropolitan area who were attending a continuing education program. A structured questionnaire was used for data collection and data were analyzed using the SPSS/Window program. Hierarchical multiple regression analysis was performed to verify the effect of variables on turnover intention. Results: Higher workplace bullying was associated with higher turnover intention. Workplace bullying was negatively correlated with leader-member exchange. The most influential factors for turnover intention were LMX (${\beta}=-7.22$, p<.001), work load (${\beta}=2.96$, p=.003), and workplace bullying (${\beta}=2.64$, p=.009). These factors accounted for 28% of the variance in turnover intention. Conclusion: The study results indicate that there is need to develop strategies to prevent workplace bullying and cultivate a good relationship between nursing managers and nurses to lower nurses' turnover intention.

신규간호사의 직장 내 괴롭힘 경험에 따른 대처적응과정과 조직사회화의 차이 (Differences in Coping and Adaptation Processing, Organizational Socialization according to the New Nurse's Experience of Bullying in Workplace)

  • Purpose: This study is a descriptive study to identify the degree of workplace bullying experience of newly graduated nurses and the difference in coping and adaptation processing, organizational socialization in relation to workplace bullying experience. Methods: The Study participants were newly graduated nurses with 3 months to 1-year clinical experience (N=186) and conveniently sampled from a small to medium-sized hospital located in G Metropolitan City. Data were collected in July 2018, using structured questionnaires. Results: Half of the participants (50.0%) reported workplace bullying experience. Most bulling experiences were person or work related. Newly graduated nurses who experienced bullying in the workplace demonstrated significantly lower coping and adaptation process (t=3.34, p=.001) and organizational socialization (t=7.46, p=<.001) than nurses who did not experience bullying in the workplace. Conclusion: Orientation programs for newly graduated nurses need to include contents that can improve the coping and adaptation process. Nursing managers at small and medium-sized general hospitals should actively support the mentor-mentee program to promote the organizational socialization of newly graduated nurses.

병원간호사의 직장 내 괴롭힘, 사회적 지지, 회복탄력성이 재직의도에 미치는 영향: 전문직 삶의 질의 매개효과 (Influences of Workplace Bullying, Social Support and Resilience on Retention Intention among Hospital Nurses: The Mediating Effect of Professional Quality of Life)

  • Purpose: The purpose of this study is to examine the mediating effect of professional quality of life among hospital nurses in the effects of workplace bullying, social support and resilience on retention intention. Methods: A descriptive and cross-sectional study was conducted on 414 hospital nurses who worked for at least two months at a general hospital in the Seoul metropolitan area. The data were analyzed using IBM SPSS 23.0/AMOS 22.0 programs and employed reliability verification, descriptive statistics include frequency analysis, confirmatory factor analysis, structural equation modeling, and bootstrapping. Results: As a result of the study's structural modeling verification, workplace bullying had a negative effect, social support had a positive effect, resilience had a positive effect on professional quality of life, and professional quality of life had a positive effect on retention intention. Also, workplace bullying, social support, and resilience did not directly impact on retention intention. However, the professional quality of life had a complete mediating effect in the relationships between workplace bullying, social support, resilience and retention intention. The study results verified the mediating effect of professional quality of life affecting the retention intention. Conclusion: In order to improve professional quality of life and retention intention, strategies to prevent workplace bullying and promote social support and resilience are needed.

임상간호사의 간호조직문화, 스트레스 대처, 직장 내 괴롭힘이 이직의도에 미치는 영향 (Effect of Nursing Organizational Culture, Stress Coping, and Bullying on Clinical Nurses' Turnover Intention)

  • Purpose: This study aimed to the investigate the relationship between nursing organizational culture, stress coping, bullying, and turnover intention among Korean hospital nurses; and to identify factors influencing turnover intention. Methods: The participants were 264 nurses working at three general hospitals in a metropolitan area in; South Korea. Data were collected using structured questionnaires from March 20 to June 21, 2021; and analyzed using the SPSS/WIN program. Results: Nurses' turnover intention was positively correlated with hierarchical -oriention (r=.28, p<.001), work -oriention (r=.14, p=.012), and bullying (r=.48, p<.001), whereas turnover intention was negatively correlated with relationship -oriention (r=-.41, p<.001), innovation -oriention (r=-.39, p<.001), and stress coping (r=-.09, p=.009). The factors influencing turnover intention were nursing organizational culture, bullying, age, position, and total working period; these had 44.0% explanatory power for turnover intention (F=14.00, p<.001). Conclusion: According to this study, addressing bullying and strengthening nursing organizational culture is essential to lower turnover intention among clinical nurses.

병원간호사의 직장 폭력 경험 실태 및 대응 체계에 대한 인식 (Prevalence of workplace violence against registered nurses and their perceptions of relevant management systems in acute care hospitals)

  • Purpose: This study investigated the prevalence and perceptions of workplace violence against registered nurses (RNs) in hospitals. Methods: One thousand RNs replied to a nationwide survey from November 14 to December 22, 2022. They reported their general characteristics, prevalence of and their responses to different types of workplace violence from patients, family caregivers, physicians, and nurse peers, as well as their perceptions of workplace violence management systems. Results: A total of 71.1% of the RNs reported that they had experienced workplace violence in the last six months. The violence and sexual harassment experienced from patients and family caregivers were 57.3% and 19.1%, respectively. Furthermore, violence, sexual harassment, and workplace harassment from physicians were experienced by 24.6%, 4.0%, and 7.4%, respectively, of the RNs, and those from nurse peers by 21.4%, 3.1%, and 11.3%, respectively. The RNs stated that they often responded to workplace violence with passive and inactive behaviors, which were more serious toward physicians' workplace violence or sexual harassment from different perpetrators. Only 69.5% were aware of their hospital's workplace violence management systems, while only 14.7%~27.4%, according to the type of hospital (p=.471), perceived the systems as effective. Multiple important strategies were identified to prevent workplace violence. Conclusion: Critical suggestions are discussed for the prevention of workplace violence, including protecting the human rights of healthcare professionals, inter-organizational collaboration, and a culture of person-centered healthcare, and training nurse managers' competency in managing workplace violence.

중환자실 간호사의 간호조직문화, 진정성 리더십, 자아존중감이 직장 내 괴롭힘에 미치는 영향에 대한 구조모형 (The Structural Equation Model of Organizational Culture, Authentic Leadership, Self-Esteem, and Bullying in Nurses at Critical Care Units)

  • Purpose: The purpose of this study were to identify the influential factors of bullying of intensive care nurses and to suggest a final structural model based on identified relationships between nursing organizational culture, authentic leadership, self-esteem, and bullying in the workplace. Methods: Data were collected from 221 nurses at intensive care units in eight general hospitals using structured questionnaires and analyzed by structural equation modeling. Results: In this study, the average of bullying in the workplace was 1.34±0.40, nursing organizational culture was 3.31±0.47, self-esteem was 2.79±0.44, and authentic leadership was 3.61±0.60. The factors affecting nursing organizational culture were authentic leadership (β=.54, p<.001) and self-esteem (β=.24, p=.002) that had direct positive effects on the nursing organizational culture. The nursing organizational culture had also a direct effect on reducing workplace bullying (β=-.45, p<.001). Authentic leadership (β=-.24, p=.004) and self-esteem (β=-.11, p=.004) had indirect effects on workplace bullying, which was mediated by the nursing organizational culture. Conclusion: To understand and reduce workplace bullying, evaluating a nursing organizational culture should be preceded. Based on the finding of this study, an intervention for increasing authentic leadership and self-esteem of nurses can positively help to create the nursing organizational culture and then reduce workplace bullying.
