환자대면서비스에 대한 의사의 중요도 인식과 실천간 차이에 미치는 요인 분석 (Analysis of the Gap Between Physician's Perceived Importance and Performance of Interpersonal Care)

  • To examine the gap between physician's perceived importance and performance of care and to identify factors associated with the gap. A self-administered questionnaire survey was conducted with 91 physicians working in a University hospital in Seoul. The respondents were asked about their perceived importance and actual performance of interpersonal care on a 5-point Likert-type scale, indicating a higher score as higher importance and performance. Interpersonal care was measured by questions modified from the Korean Standard Service Quality Index, which are grouped into 6 categories: basic services, extra services, reliability, courtesy, convenience, and tangibles. Multiple regression analysis was conducted to find out physician characteristics associated with the gap. All of the 6 interpersonal care categories showed lower performance than perceived importance. The respondents tended to have a worse performance than perceived importance as the number of patients per outpatient care session ($\beta$=-0.0204, p<0.05) and the need for customer satisfaction education increase ($\beta$=-0.2226, p<0.05). Female physicians ($\beta$=0.2336, p<0.05) and those with higher job satisfaction($\beta$=0.0096, p<0.05) showed a better performance than perception. Overall, it appears that lower quality of interpersonal care was delivered to patients than the desired level considered by the responding physicians. Based on the regression analysis results, it is suggested that reducing patient volume per session, fulfilling education need for customer satisfaction, and improving job satisfaction may contribute to reduce the gap between physician's perceived importance and performance of interpersonal care.

요양보호사의 대인태도가 의사소통능력에 미치는 영향 (Influence of Interpersonal Attitude on Communication Competence in Care Workers for Frail Elderly)

  • Purpose: The purpose of this study was to identify the influence of interpersonal attitude (I+, I-, U+, U-) on communication competence in care workers for frail elderly. Methods: This study was a cross-sectional survey. The data were collected from 153 care workers for frail elderly using a structured questionnaire. The data were analyzed using multiple regression with the SPSS/WIN 20.0 program. Results: The interpersonal attitude style of subjects was I+U+, and the score of communication competence was 3.53. There were significant differences in interpersonal attitude (I+, I-), and communication competence depending on age, education level, experience of personality type test (yes). Factors influencing on communication competence in care workers were interpersonal attitude (I+, I-, U+) and experience of personality type test with $R^2$ value of 48.5% (F=23.47 p<.001). The most influencing factor was I+ (${\beta}$=.36), followed by I- (${\beta}$=-.22), U+ (${\beta}$=.20), and experience of personality type test (yes) (${\beta}$=.16). Conclusion: It is needed to maintain the interpersonal attitude style (I+U+) of care workers. Continuing education program is needed for increasing communication competence especially for those fifties and over, and experienced care workers. Giving an opportunity for personality test is helpful to increase communication competence in care workers.

다문화가정 아동을 위한 대인 돌봄 기반의 오르프 음악 활동 프로그램 개발 (Development of an Orff Musical Activity Intervention Program based on Interpersonal Care for Children from Multicultural Families)

  • 본 연구에서는 우리나라 다문화가정 아동을 위한 대인돌봄 기반의 오르프 음악활동 프로그램을 개발하고자 한다. 대인 돌봄 기반의 오르프 음악 활동 프로그램 구성은 자아 탄력성, 또래 관계, 행복, 대인 돌봄 인식, 불안 및 스트레스를 조절할 수 있는 프로그램으로 구성하였으며, 분석(Analysis), 설계(Design), 개발(Development), 실행(Implementation)의 ADDI 모형을 토대로 개발하였다. 그 결과, 돌봄의 표현 영역, 핵심 주제, 돌봄 목표, 대인 돌봄 기법, 오르프 기법에 근거한 10차시의 프로그램이 개발되었다. 개발된 본 프로그램의 연구 결과를 통해 다문화가정 아동에게 대인돌봄간호가 효과적일 수 있으며, 본 프로그램이 아동의 정신건강 증진을 위한 중재로 활용될 수 있는 기초자료로 활용될 수 있을 것이다.

백내장 환자의 수술후 진료만족도의 다면적 평가와 결정요인 (Determinants of Multidimensional Outcomes of Patient Satisfaction in Operated Cataract Patients)

  • This study was to compare multidimensional outcomes of patient's satisfaction after cataract surgery and to identify factors influencing satisfaction after operation. Patient's satisfaction was measured with three dimensions : interpersonal care, physician explanation and hospital care. Overall satisfaction was measured as means of three dimensional scores. For the study, a prospective study was performed with 389 patients who had undergone cataract surgery for either one eye or both eyes. The surgery was performed by 20 ophthalmologists who were practicing at university hospitals and general hospitals. Patients were interviewed and clinical data (the visual acuity of operated eye, visual function, symptom score and satisfaction with vision) were obtained. The doctors were questioned with self-reported questionnaire forms. Medical records were also examined to understand surgery Process. The survey was conducted before(389) and after operation(327). Alter excluding cases with incomplete data, 3n cases were enrolled In this study. Both the overall satisfaction and the satisfaction with physician explanation increased after the operation whereas the satisfaction with interpersonal care and hospital care did not change significantly. Multiple regression analysis showed that the level of education, baseline satisfaction scores and the degree of vision improvement were statistically significant variables. The preoperative lower level of education, higher level of overall satisfaction (interpersonal care, physician explanation, hospital care scores) and the more the satisfaction with vision improvement were associated with the improvement of postoperative satisfaction scores.

간호·간병통합서비스병동과 일반병동 간호사의 피로, 수면의 질, 대인관계 스트레스 비교 (Comparison of Nurses's Fatigue, Sleep Quality and Interpersonal Relationship Stress between Comprehensive Nursing Care Service Wards and General Wards)

  • Purpose: The purpose of this study was to compare the differences in fatigue, sleep quality and interpersonal relationship stress levels between nurses in comprehensive nursing care wards and in general wards. Methods: Data were collected using self-reported structured questionnaire from 14 August to 17 September 2018. The participants were 220 nurses from 6 hospitals in Seoul and Gyeonggi-do. The data were analyzed using the IBM SPSS 23.0 program, descriptive statistics, t-test, ${\chi}^2-test$, Fisher's exact test, and Pearson's correlation coefficient. Results: The subjects' fatigue, sleep quality, and interpersonal relationship stress were not significantly different between the comprehensive nursing care services wards and the general wards. The interpersonal stress with a superior was statistically higher among general wards than comprehensive nursing care services wards. The relationship stress between patient and caregiver was higher in comprehensive nursing care services wards than in general wards. Conclusion: A follow-up study is needed to develop a strategic method to reduce interpersonal stress in nurses.

간호대학생의 비판적 사고성향, 대인관계능력이 인간중심간호 역량에 미치는 영향 (Effects of Critical Thinking Disposition and Interpersonal Relationship on Person Centered Care Competency in Nursing Students)

  • 본 연구는 간호대학생의 인간중심간호 역량에 영향을 주는 요인을 파악하기 위한 연구이다. D시에 소재하는 2개 대학 간호대학생 294명을 대상으로 2019년 9월 1일부터 9월 25일까지 설문조사하였고, SPSS 21 프로그램을 이용하여 분석하였다. 간호대학생들의 비판적 사고성향은 5점 만점에 3.72점, 대인관계능력은 5점 만점에 3.90점, 인간중심간호 역량은 5점 만점에 4.01점이었다. 전공만족도가 높은 학생들이 낮은 학생에 비해 비판적 사고성향, 대인관계능력, 인간중심간호 역량 수준이 높았으며, 인간중심간호 역량은 비판적 사고성향, 대인관계능력과 순 상관관계에 있었다. 인간중심간호 역량에 미치는 요인은 비판적 사고성향, 대인관계능력, 학년임을 확인하였으며(F=20.103, p<.000), 변인들의 설명력은 33%이었다. 간호대학생의 인간중심간호 역량 향상을 위해서는 비판적 사고성향, 대인관계능력을 향상시킬 수 있는 교과내외의 다양한 프로그램개발과 적용이 필요하다.

보육교사의 의사소통 능력, 대인관계 유능성, 그리고 가정연계 효능감이 부모-교사 협력에 미치는 영향 (The Effects of Early Child Care Teachers' Communication Competence, Interpersonal Competence and Teacher Self-efficacy to Enlist Parental Involvement on Parent-Teacher Partnerships)

  • This study examined the characteristics of early child care teachers and the ways this influences their abilities to promote the parent-teacher partnerships. 266 early child care teachers participated in this study. Parent-teacher partnerships, communication competence, interpersonal competence, and teacher efficacy in enlisting parental involvement were measured by means of The Caregiver-Parent Partnership Scale, Global Interpersonal Communication Competence Scale, Interpersonal Competence Questionnaire and selected elements of the Teacher Self-efficacy Scale. The results indicated that parent-teacher partnerships were significantly different in terms of the characteristics of teachers. The hierarchical multiple regression analysis revealed that the communication competence, interpersonal competence, and teacher self-efficacy significantly predicted the type and quality of parent-teacher partnerships. In conclusion, improving the effectiveness of parent-teacher partnerships seems heavily dependent upon the quality and personal abilities an beliefs of early child care teachers.

대학생의 대인관계문제와 신체불만족, 메이크업관리행동 간의 연계성 (Influence of Interpersonal Problems on College Students' Body Dissatisfaction and Makeup Behavior)

  • This study investigated correlations among interpersonal problems, body dissatisfaction, and makeup behavior among college students. The results provide basic data which are helpful for students to build healthy interpersonal relationships and suggest a direction for beauty care behavior. This study derived the following results: A close correlation was identified between international problems and body dissatisfaction and body dissatisfaction and makeup behavior. In other words, college students suffering from interpersonal problems also experienced body dissatisfaction. In addition, those with body dissatisfaction have made efforts to overcome such problem through makeup behavior. This could be viewed as meaning that the greater the body dissatisfaction, the more active college students are in makeup management behavior, and the intention to resolve their physical dissatisfaction through makeup management behavior was included. Therefore, society or colleges need to recognize the necessity of counseling or treatment programs for college students suffering from interpersonal problems or body dissatisfaction and help them enter into society through diverse programs. Additionally, appearance management programs, such as the aforementioned makeup behavior, that can make them satisfied with their body should be developed. Ultimately, diverse suggestions that can help students improve their self-esteem and interpersonal relationships and enhance sociality through such social efforts should be formulated.

유아의 대인관계 형성능력에 미치는 정서조절능력과 대인문제해결사고의 영향 (The Effect of Emotional Control and Interpersonal Cognitive Problem Solving Ability on Children's Interpersonal Relationship Formative)

  • 본 연구는 유아의 대인관계 형성능력에 정서조절능력과 대인문제해결사고가 어떠한 영향력이 있는지에 대하여 알아보고자 실시되었다. 연구대상은 G광역시의 어린이집과 유치원 3곳의 유아 133명이다. 자료의 분석은 t-test, ANOVA, Pearson의 적률상관관계, 회귀분석을 실시하였다. 본 연구결과는 첫째, 유아의 성별에 따라서는 대인관계 형성능력 전체와 하위요인인 사회적 반응성에서만 여아가 높은 것으로 나타났다. 형제 수와 어머니의 취업여부에 따라서 형제가 1명인 경우, 취업모인 경우가 대인관계 형성능력, 정서조절능력, 대인문제해결사고 모두에서 가장 높게 나타났다. 둘째, 대인관계 형성능력은 정서조절능력과 대인문제해결사고와 정적상관이 있는 것으로 나타났다. 셋째, 대인관계 형성능력에 영향을 미치는 요인은 정서조절의 자기조절과 타인조절력이었으며, 대인문제해결사고에서는 원인적 사고가 정적인 영향을 미치는 것으로 나타났다.

재가방문 요양보호사의 대인관계스트레스가 이직의도에 미치는 영향 - 소진의 매개효과 검증 (The Study on Modification Methods of Residents Registration Number System)

  • 본 연구는 재가방문 요양보호사가 현장에서 혼자 감당해야하는 대인관계스트레스가 이직의도에 주는 영향력과 소진의 매개효과 정도를 파악하여 재가방문 요양보호사의 역량강화를 위한 제언을 목적으로 한다. 분석결과를 요약하면, 첫째, 재가방문 요양보호사의 연령, 배우자유무는 소진과 유의미한 상관관계가 있으며, 연령과 이직경험은 이직의도와 유의미한 상관관계가 있는 것으로 확인되었다. 둘째, 대인관계스트레스는 소진과 이직의도에 각각 정(+)적인 상관관계를 갖는 것으로 나타났다. 셋째, 대인관계스트레스는 소진으로 인해 더 증가됨을 확인할 수 있었고, 소진의 개입이 요양보호사의 이직의도에 정(+)적 영향을 미치는 것으로 나타났다. 본 연구의 결과를 담은 제언은 첫째, 재가방문 요양보호사들이 직면하는 대인관계스트레스에 대한 대처방안으로 대상자 및 대상자가족과의 다양한 상황에 대한 이해와 대처기술 등이 향상될 수 있도록 전문적이고 체계적인 직무교육 등이 확대되고 개선되어야 할 것이다. 둘째, 업무적 특성과 환경적 한계에 대해 관심을 갖고, 이를 지원해주는 센터의 역할이 강화되어야 할 것이다. 셋째, 대인관계스트레스가 이직으로 영향을 미치는 것을 줄이기 위해서는 소진에 대한 연구를 통해 이를 뒷받침해 줄 수 있는 법적, 제도적 지원이 필요하다.
