Purpose: The purpose of this study was to evaluate food carving among hemodialysis patients and to identify factors affecting the food carving. Methods: A descriptive correlational study was conducted. Data were collected from 129 patients receiving hemodialysis between March 15 and April 15, 2017. Measurement instruments included the general food craving questionnaire trait, the center for epidemiological studies-depression scale, and stress response inventory-modified form. The statistical analysis included t-test, analysis of variance, pearson correlation analysis, and stepwise multiple regression analysis. Results: Mean food craving score was $53.00{\pm}12.36$. Food craving was higher in patients younger than 40 years (F=4.36 p=.006) and having occupation (t=2.18 p=.031). Patients receiving hemodialysis demonstrated higher levels of depression ($21.37{\pm}9.62$) and stress ($39.68{\pm}15.95$). Factors influencing food craving were depression (${\beta}=.52$, p<.001), stress (${\beta}=.65$, p<.001), and age 50~59 years old (${\beta}=-.28$, p=.001) and 60~69 years old (${\beta}=-.19$, p=.026), which accounted for 25.6% of total variance. Conclusion: Food craving among hemodialysis patients was influenced by patients' depression, stress and age. Assessment and management of depression and stress needs to be incorporated as a nursing strategy for dietary management for hemodialysis patients.
본 연구는 혈액투석 기간 동안 경험하는 혈액투석 환자의 음식 갈망 현상을 깊이 있게 이해하기 위함이다. 참여자는 일개 의료기관으로부터 말기신부전 진단을 받고 혈액투석 치료를 받는 성인 환자 7인을 임의로 선정하였다. 수집된 자료는 현상학적 분석방법으로 분석하였다. 분석결과 10개 주제와 5개의 주제 모음과 2개의 범주로 구조화할 수 있었다. 두 범주는 '인지적 영역에서의 갈망경험', '정서적 영역에서의 갈망경험'이었으며 '갈증으로 물과 수분이 많은 음식을 찾게 됨', '제한된 음식으로 종일 음식 생각이 남', '항상 허기지고 배고픔', '매시간 식욕을 유발하는 상황에 맞닥뜨림', '식사 조절 실패로 부정적인 정서를 경험함'의 5가지 주제 모음이 포함되었다. 그동안 연구되지 않았던 혈액투석 환자의 관점에서 음식 갈망을 조명한 본 연구결과는 혈액투석 환자의 식욕조절과 수분 제한 및 식이요법 이행에 대한 이해를 증진하고 환자에게 맞는 식이요법 교육과 간호 중재를 적용할 때 혈액투석 환자의 음식 갈망 속성을 이해하고 그 정도에 맞춰 최적화된 맞춤형 교육이 이루어질 수 있도록 해야 할 것이다.
Objectives In the current study, we quantitatively estimated changes in appetite and eating behavior of bipolar disorder patients during the pharmacotherapy. We also investigated their contribution to the weight gain and their association with specific food-craving characteristics of the patients. Methods Subjects included forty-one bipolar disorder patients and fifty-six controls. Currently sustained natures of food craving were assessed using the General-Food Craving Questionnaire-Trait (G-FCQ-T) and changes in appetite and eating behavior were measured using the Drug-Related Eating Behavior Questionnaire (DR-EBQ). Results Compared to the control group, the patients' group showed significantly higher body mass index (t=2.028, p=0.045). The patients' group had significantly higher 'Preoccupation with food' factor score of G-FCQ-T (p=0.016) than that of the control group. Hierarchical multiple regression analysis showed that only 'preoccupation with food' factor independently predicted psychotropic medication-induced appetite change. Conclusions Appetite change while receiving psychotropic medication seems to be related to the weight-gain and associated with craving natures of 'preoccupation with food' in bipolar disorder. Appetite and/or eating behavioral changes measured by G-FCQ-T and DR-EBQ could be regarded as an important mediating factor in future studies exploring biological mechanisms of weight gain related with pharmacotherapy for bipolar disorder.
Objective : This study was designed to test the anti-craving effect of Gigaltang(extracts of Oriental medicinal herbs ; Hovenia dulcis and Puerariae flos) in C57BL/6 mice. Anti-craving effect was substituted by alcohol intake amount of C57BL/6 mice. Method : 60 C57BL/6 mice was randomized into vehicle(water) group and Gigaltang group. Only 39(vehicle group(21) and Gigaltang group(18)) was eligible for this study, and 21 C57BL/6 mice was dropped out. The procedure was composed of 5 days of adjustment period and 32 days of alcohol exposure and withdrawal period with limited access paradigm and 10 days of treatment period. Alcohol was offered only for 2 hours a day from 2 to 4PM and water was offered for 22 hours left. Food was offered for 24 hours a day. 1140mg/Kg/day of Gigaltang was administrated to 18 mice of Gigaltang group for 10 days whereas water to 21 mice of vehicle group. The amount of alcohol intake, water intake, food intake, body weight were measured every other day. Result : There weren't significant differences in 2 hours of alcohol intake, 22 hours of water intake, 24 hours of food intake and body weight for ten days between Gigaltang and vehicle group. Conclusion : Anti-craving effect of Gigaltang on C57BL/6 mice wasn't verified. To verify the anti-craving effect, further study with advanced animal model, various dose of Gigaltang and diverse period of drug administration should be taken.
During abstinence from alcohol, craving is elicited by the cues and contexts previously associated with alcohol, and contributes to relapse. To prevent the craving and relapse experienced by alcoholics, cue-exposure therapy (CET) has been used to extinguish the association between alcohol and alcohol-related cues and contexts. This study applied CET, using a virtual reality (VR) system, to eight members of an Alcoholics Anonymous group, in eight sessions. Cues and contexts most likely to elicit an urge to drink were selected through a preliminary survey in order to compose VR-CET scenarios: a glass, bottle, food, and a bar were judged to be the most tempting for people in alcohol dependence and abstinence. By these cues and contexts, a Japanese style pub and a western bar were composed. Each session was administered for 30 minutes by a psychiatrist and included an introduction, immersion, VR navigation, interviews about feelings, and self-report questionnaires about cravings. The eight sessions consisted of initial and closing sessions, and six cue- and context-focused sessions. As a result, a reduction in cue-elicited craving after VR-CET was reported. A mean score of 15.75 (SD = 10.91) on the Alcohol Urge Questionnaire in the first session decreased to 11.57 (SD = 6.88) in the final session.
본 연구는 한국인 인구집단에서 폭식행동, 음식중독을 식별하고, 해당 증상들이 비만 및 섭식행동, 정신건강, 인지적 특성과 어떠한 연관성을 보이는지 규명하고자 하였다. 이를 위하여 정상체중 및 비만체중에 해당하는 한국인 성인 257명을 대상으로 섭식문제(예: 폭식, 음식중독, 음식갈망), 정신건강(예: 우울), 인지기능(예: 충동성, 정서조절)에 관한 임상심리검사 척도를 측정하였다. 비만 여부와 성별에 따라 그룹을 나누었을 때, 비만체중 여성에서 폭식행동이 46.6%, 음식중독이 29.3%로 가장 빈도가 높았다. 성향점수 매칭 후 데이터로 독립성 검정을 수행한 결과, 폭식행동 및 음식중독이 비만체중 집단에서 정상체중 집단보다 더 많이 나타나는 것을 확인하였다. 또한 폭식행동과 음식중독 유무에 각 심리검사 척도 요인이 미치는 영향력을 파악하고자, 전진선택법을 적용한 로지스틱 회귀모델을 구축하였다. 로지스틱 회귀분석 결과, 폭식행동에는 섭식장애, 음식갈망, 상태불안, 정서조절(인지적 재해석) 및 음식중독이 주로 관여하였고, 음식중독에는 음식갈망, 폭식행동과 함께 비만과 연령의 교호작용, 교육년수가 유의하게 작용하는 것으로 나타났다. 본 연구는 한국인 성인을 대상으로 한 체계적 연구로서, 폭식행동과 음식중독이 여성 및 비만인에서 특히 더 많이 나타남을 확인하였다. 폭식행동과 음식중독에는 일부 섭식문제(예: 음식갈망)가 공통되게 관여하나, 정신건강 및 인지적 위험요인에는 차이가 있었다. 따라서 음식중독과 폭식행동은 서로 구별되는 개념으로 두고, 각각의 기질적·환경적 위험요인을 깊이 있게 탐구하는 것이 필요하다.
The purpose of this study was to explore the personality and eating behavior among adult women. The subjects for the study were 237 adult women, who live in Seoul. The survey instruments were to develop for personality and eating behavior. Data were collected between November 6 and November 27 in 2000 and analyzed using the SPSS program. The result of this study are as fellows: 1. Six factors (perfection, emotion, autonomy, consideration for other, evaluate from other, dependency) emerged from personality, and these contributed 55.29% of the variance in the total score. 2. Three factors(craving, food intake according to emotion, eating habit) emerged from eating behavior, and these contributed 49.23% of the variance in the total score. 3 There were significant correlation between personality and eating behavior(r=.32, p=.00). 4. In the relationships between craving and personality, perfection, emotion, autonomy, consideration for other, evaluate from other, dependency showed a statistically significant correlation. 5. In the relationships between food intake according to emotion and personality, perfection, evaluate from other, dependency showed a statistically significant correlation. 6. In the relationships between eating habit and personality, perfection, autonomy, consideration for other, evaluate from other, dependency showed a statistically significant correlation. This study suggest that perfection, evaluation from other, dependency dimension of personality may be important factor for regulation of eating behavior.
The efficacy of a medicinal herb tea (MHT) for assisting smoking cessation and for reducing withdrawal symptoms was evaluated by a placebo controlled subjective test in 100 male smokers. The MHT group experienced less craving for the taste of tobacco, and was more successful at quitting smoking. Among the subjects continuing to the final stage, 23 subjects (38.3 %) who took the MHT and 2 subjects (10.0 %) who took the placebo tea succeeded in quitting smoking. In the group taking MHT, urinary cotinine concentration increased during the first 2 weeks and then greatly decreased from weeks 2 to 4. The urinary cotinine concentration in the MHT group at week 0 was 38.6 mol/L but decreased to 24.5 mol/L by the end of week 4. MHT also reduced withdrawal symptoms, a frequent reason for failure in reducing or quitting smoking. The effects of the herbal tea on smoking cessation and withdrawal symptoms may be attributed to increased metabolism of nicotine and other effects of unidentified components of the herbal ingredients.
Purpose: This study was performed to measure the influence of premenstrual syndrome coping strategies and food cravings on premenstrual syndrome symptoms among female college students. Methods: A cross-sectional survey was done with the participation of 509 female college students in D city. Data were analyzed using a t-test, ANOVA, Scheffé test, Pearson's correlation coefficients, and Hierarchical multiple regression. Results: The average scores for PMS coping mechanisms, food cravings, and premenstrual syndrome symptoms were 1.58, 2.32, and 2.82, respectively. There were significant differences in PMS symptoms according to food, menarcheal age, menstrual amount, menstrual pain and whether their mother or sister has dysmenorrhea. The factors influencing premenstrual syndrome symptoms of female college students were identified as avoidance (β=-.16, p<.001), menstrual cycle specific (β=-.11, p=.013), and cognitive restraint on eating (β=.13, p=.026). This factor explained 35% of female college students' premenstrual syndrome symptoms. Conclusion: Therefore, it is necessary to take appropriate stress coping measures for the subjects.
This study aimed to elicit the variability of appetite and food intake patterns in female college students during different menstrual phases. The craving for certain foods and physiological and psychological symptoms of menstrual phase (pre- and post-menstrual) were investigated by self-administered questionnaire. Three hundred and sixty six students who were 20.9 years old and had $19.8kg/m^2$ of BMI volunteered to participate in this study. Most of the subjects (89.5%) experienced the premenstrual syndrome (PMS) such as irritation, bodily fatigue, nervosity. Symptoms such as tiredness, stomachache, changes in taste and increased appetite were pointed out to be experienced at the onset of the menstrual cycle. The variability of food intake in premenstrual phase, 11.0% of subjects had decreased food intake where as 68.8% had experienced increased intake. The postmenstrual phase, 20.1% had decreased food intake while 45.2% had experienced increased intake due to changes in the appetite. Before starting menstruation, most of the subjects craved for sweets. The group who had experienced abnormal appetite during menstrual phase was significantly high ratio in overweight and obese students (p<0.05). We also observed an association between the PMS score and the variability of eating patterns during the menstrual phase. The students who experienced changing appetite and food intake had significantly high PMS score in the premenstrual phase (p<0.01) and postmenstrual phase (p<0.05). These results suggested a need for future study related to changes in the actual nutrient intake and activity level during the menstrual phase.
