Objectives: Community organization is a resident-led movement aimed at creating fundamental social changes in the community by resolving its problems through the organized power of its residents. This study evaluated the effectiveness of health community organization (HCO), Gangwon's Health-Plus community program, implemented from 2013 to 2019 on residents' health behaviors. Methods: This study had a before-and-after design using 2011-2019 Korea Community Health Survey data. To compare the 3-year periods before and after HCO implementation, the study targeted areas where the HCO had been implemented for 4 years or longer. Therefore, a total of 4512 individuals from 11 areas with HCO start years from 2013 to 2016 were included. Complex sample multi-logistic regression analysis adjusting for demographic characteristics (sex, age, residential area, income level, education level, and HCO start year) was conducted. Results: HCO implementation was associated with decreased current smoking (adjusted odds ratio [aOR], 0.73; 95% confidence interval [CI], 0.57 to 0.95) and subjective stress recognition (aOR, 0.79; 95% CI, 0.64 to 0.97). Additionally, the HCO was associated with increased walking exercise practice (aOR, 1.39; 95% CI, 1.13 to 1.71), and attempts to control weight (aOR, 1.36; 95% CI, 1.12 to 1.64). No significant negative changes were observed in other health behavior variables. Conclusions: The HCO seems to have contributed to improving community health indicators. In the future, a follow-up study that analyzes only the effectiveness of the HCO through structured quasi-experimental studies will be needed.
Objectives: A great number of the elderly are not aware of having metabolic syndrome, and this pattern differs among regions of the nation. This issue is particularly important for the elderly people with chronic diseases. This study was conducted in order to identify the prevalence, predicting factors of perception and exercise behaviors of among the elderly population. Methods: Using data from the 2009 Korean Community Health Survey, we employed two-level logistic regression models to determine whether individual and community factors are associated with perception and exercise behaviors among the elderly population. Results: Results from weighted two-level logistic regression analyses indicated the individual-level factors of smoking, education level, health center education, community exercise program participation and unmet needs in men, and marital status (being separated), health center education women predicted the elders' perception. As for the community-level predictors, higher smoking and living in regions with high suicide rates significantly predicted the elders' perception and exercise behaviors of metabolic syndrome. Conclusions: These findings demonstrate that education according to stage would increase the perception and motivation toward healthier behavior particularly for the elderly with lower perception levels of metabolic syndrome.
Purpose: The purpose of this study was to identify factors influencing health promoting behavior of school-aged children on community child center. Methods: Participant were 207 elementary school student located in Mokpo. For data analysis descriptive statistics, t-test, ANOVA pearson correlation coefficients and stepwise multiple regression were used with SPSS/WIN ver 18.0 program. Results: The mean score for health promoting behavior was below the lower point at $3.39{\pm}0.61$. There were significant differences in health promoting behavior according to gender (t=9.41, p=.002), father's job (F=2.28, p=.048), perceived health status (F=5.70, p=.001), There were significant correlations between health promoting behavior and self-esteem (r=.655, p=.001), self-efficacy (r=.530, p=.001). The explanatory power of these variables accounted for 47.1% of health promoting behavior. The most significant variable was self-esteem (t=7.60, p=.001) and explained 42.8%. Conclusion: The finding indicate that self-esteem of children on community child center are important variables for health promoting behavior. This result suggests that interventions focusing on self-esteem, on enhance health promoting behavior.
Purpose: This study was conducted to compare the hypertension management between a non-elderly group and elderly group of hypertension patients in Community residents. The study also sought to generate strategies for increasing the hypertension management of residents using Community health center. Methods: Data on the general characteristics and hypertension management from 381 hypertension patients between non-elderly and elderly, living in P city, Gyeonggi Province and C city, Chungnam Province. South Korea, were collected based on a structured questionnaire, The data were analyzed using the SPSS 20.0 statistics program. Results: The use of a Community health center in the non-elderly and elderly groups showed a statistically significant difference in facility excellence and cheaper cost. Hypertension management was measured every day, The daily blood pressure and physician counseling was performed according to the changes in blood pressure. The management of hypertension medication in a community health center provided for hypertensive patients can be evaluated as an efficient service. Conclusion: The self-management ability of hypertension needs to be improved. In particular, especially, the elderly managed by the Community health center have good accessibility and a good alternative for the treatment cost. Therefore, it is necessary to provide support and measures to make hypertension management safer.

Background: University health services have provided comprehensive medical care, counseling, health promotion, and public health services to their students and several other local institutions. To their faculty and staff, university health care centers have served occupational health services and employee assistant program. Purpose: We performed this study to review the health promotion services on two kinds of health care center with different style of university formate. Methods: We tried to collect the data by literature review and interview with executive and provider at health care center in University of California at Berkeley and San Jose State University. Results: Our results were as followed. First, students could use the medical services just as they would their regular doctor's office and urgent care center. Second, the health promotion unit offered programs and services for keeping students healthy and safe, including many opportunities for students to get involved in shaping the public health of the campus. Third, the health promotion recommendation offered from ACHA was useful guideline to improve health status of their member in university campus. Finally, the student satisfaction surveys were used for evaluation and quality improvement. Conclusions: The systematic approach to improve health status of students, faculty and staff can use to maintain a state of optimum health among the diverse student community in support of academic excellence. Coupled with health promotion and public health programs, university health service have to reach all segments of the healthy campus community. To achieve study goals in university, the health care center contributes to promote accountability and responsibility for the health and well being of the members in their campus.
Recently, there has been an increasing need for long-term care and comprehensive health care services in community settings. The Ministry of Health and Welfare introduced the Hospital-Based Home Nursing Care Program in 2000. Before this initiative, there was a Home Nursing Demonstration Center, affiliated with the Seoul Nurse Association, had offered home nursing services with the financial support from the local government. since 1993, the Center's nursing staff has been engaged in a general hospital in an effort to provide home nursing care services within Korea's health care system. The purpose of this study was to analyze and identify characteristics of community-based home nursing care supplied by a community-based home nursing team engaged in a general hospital. Also. visit nursing care services provided by public health centers were evaluated in terms of accessibility and supply versus demand, to enhance the accessibility of low-income patients living in Seoul to home nursing care services. Data were collected from home nursing insurance reimbursement claims submitted by the community-based home nursing care team from March 1 to October 30 in 2001 and a questionnaire survey on home-visit nursing services of 25 public health centers in Seoul. The subjects consisted of 197 patients and 12 public health centers. The result were as follows. First, medical institution's community-based home nursing care program was better in technical quality than health-center-based home-visit nursing care. In addition. the pattern of the subject patients was similar to that of hospital-based home nursing care program. Second, there was a high demand for community-based home nursing care while only a small number of home-visiting nurses served at public health centers in Seoul. As a result, many patients could not receive adequate care. Finally, we suggest that community-based home nursing care program should be introduced in the national health system to meet the at-home care needs of severely ill low-income patients. Furthermore, to better utilize home nursing and visit-nursing care resources and offer continued care for patients in community settings, an efficient referral network should be built among related institutions. This would require improvement of reimbursement system and amendment of the law related to health insurance system and community-based home nursing care services.

Objectives : The purpose of this study was to examine the results of the 2013 students' mental health school-community cooperative model. This study investigated teachers' awareness of change in school environment and analyzed the follow-up conditions of students. Methods : The sample consisted of 55 schools which participated in the 2013 students' mental health school-community cooperative model. Two questionnaires were used : one was composed of items about mental health resources, school policy and practice, school climate and community referral, and the other was on management of follow-up conditions for high risk students. Results : First, teachers' awareness regarding school mental health resources, school policy and practice, school climate, and community referral showed positive change. That is, the 2013 students' mental health school-community cooperative model works for improving teachers' interest, awareness and effort regarding mental health. Second, students, who were found by screening test and observation, were managed continually. Most students received follow-up interventions at school, and half of the students were referred to a specialized institution such as a mental health clinic, Wee center, personal counseling center, community welfare center, and so on. Conclusion : This study was a preliminary research, therefore follow-up studies are required in order to accomplish the 2013 students' mental health school-community cooperative model.

Purpose: This study was conducted to evaluate the economic efficiency of a community-based nursing care center to help policy makers determine whether or not to invest in similar facilities. Methods: The subjects were 101 elderly people over 65 years who participated in a health management program from February 1 to July 31, 2007. Direct cost was estimated with center operations cost, medical cost for out-patients and pharmacy cost. Indirect cost was measured by transportation cost. Direct benefit was calculated by saved medical cost for out-patients, saved pharmacy cost, saved transportation cost, and reducing hospital charges. Indirect benefit was estimated with prevention of severe complications. Economic efficiency was evaluated by cost-benefit ratio and net benefit. Results: Operating a community-based nursing care center was found to be cost-effective. Specifically, the cost of operating the center evaluated here was estimated at 135 million won while the benefit was estimated at 187 million won. Benefit-cost ratio was 1.38. Conclusion: The Community-based nursing care center that was described here could be a useful health care delivery system for reducing medical expenditures.

Objectives: This study addressed the relationship among communication skill, job satisfaction, and mental health level. Methods: The participants in this study were 222 public health center staff members at 9 community public health centers located in Seoul city and Gyeonggi province. Descriptive analysis, Pearson's correlation, and multiple regression were used for statistical analysis of the data. Results: The main findings are as follows: First, communication skill and job satisfaction showed a significant positive correlation with mental health level. Secondly, some components of mental health level generally showed positive correlations with communication skill and job satisfaction. Third, multiple regression showed that 22% of the variance in the mental health level was explained. Conclusions: Based on these results of this study, we suggested that specialized programs should be established to help in development of social arbitration for public health center staff.

The purpose of this research is to examine how the village community centers are being used and find how to improve leisure activities of elders in rural areas by using them. In order to achieve the purpose of this research, we did convenience sampling from elders using 92 village community centers in one of the counties in Gyeong-gi province, and did field survey to 343 persons. The collected data was analyzed by SPSS 18 program, and the results are as follows: The survey on population sociological peculiarity of elders using village community centers shows that most of elders are over 70 years old. As for health conditions, there are more elders who are physically weaker than healthy elders. And the survey shows that most of the elders are suffering from illness such as hypertension, arthritis, neuralgia, and indigestion so such programs related to health are critically required. First, the survey on using village community centers by elders in rural areas shows that they think the village community centers are important for them and use them often. Most of respondents said that they use the village community center so as to meet friends, to enjoy leisure, and to spend time in rural areas where they have difficulty enjoying cultural facilities. Meanwhile, some people dislike to use the village community centers mainly because there is few programs for leisure or each one has a different personality or they don't have enough money to pay for the membership fee. Second, according to the survey on leisure for the elders in village community centers, there are few leisure programs. For men, they spend time by playing Baduk, Janggi, Hwatu and drinking. Meanwhile, women spend time by meeting with friends, listening to radio and TV watching. There are almost nothing to improve leisure activities such as hobby, recreation and health etc. Male elders using centers are suffering from health problems, economic difficulty, and lack of leisure activities, meanwhile, female elders are suffering from loneliness, health problems, and economic difficulty. Third, according to the survey on improving the leisure program of village community center, elders preferred health related programs to improve leisure activities. The most necessary leisure activities in village community center are hobby classrooms, health programs and group travelling. They preferred health related programs for leisure in village community center. Based upon the reality described above, improving the village community centers as leisure facilities is considered to be important for the welfare well of elders in rural areas.
