A STUDY ON THE REFERRAL PROGRAM FROM PRIMARY HEALTH CARE FACILITIES IN A RURAL AREA (일부 농촌지역의 의뢰환자 프로그램 운영에 관한 연구)

  • This study was conducted to follow the patient referral system operated from the primary health care facilities to the hospital in a rural area of Korea. The subject for this study was sampled from a community health development project carried out by the Korea University in Yeoju Kun, Kyonggi Province. The data of referred patients from primary health care facilities were collected during the period from January 1989 to December 1989. The data was sorted out by a computer system using Database package. The results of this study were summarized as follows: 1. Characteristics of the referred patients were: males $32.0\%$. and females $68.0\%$. The more elderly of the patients visited to the hospital after having been referred there by CHPs or public physicians, $25.9\%$ has been to hospital on at least one previous occasion as against $74.1\%$ for whom it was there first visit. 2. The majority of patients who were referred to a hospital where: medicine $44.3\%$ and orthopedics$16.4\%$, major diseases were : diseases of digestive system $(21.3\%)$ ; symptoms and ill defined conditions $(17.3\%)$ ; diseases of the muscular skeletal system and connective tissue$(14.2\%)$. chronic illness was $82.0\%$ and acute illlness was $18.0\%$. 3. From Community health practitioners more patients referred than the public physicians. Categoris of diseases of the referred patients were different between community health practitioners and public physicians. Due to the. respective differences between the medical restrictions put on the nursing staffs at the community health practitioners and public physicians. From this study it was recommended to define the reason of differences between ~he two groups in futher study. Study as to 1) why one group should be referring more for hospital treatment than the other. And 2) why the two agencies should be referring different diseases.

Improvement of Public Health Services in Korea (우리나라 공공보건의료 발전방안)

  • For the longest time, our government has played an inconsiderable role in the public health services of Korea, especially as it relates to their investment. Voices have cried out against increases in national health expenditure and for more establishment of public medical facilities. In light of this, the necessity and importance of public medical facilities have come into focus amidst the recent medical crisis. When public medical facilities filled in the gap created by the suspension or closure of private hospitals and clinics as a result of this national crisis and acted as a safety net, the demand for more establishment of such facilities increased. Although patient diagnosis and treatment are the first priority of public medical facilities, they must also deal with scopes that private medical facilities do not deal with, dislike, or have difficulty with. In this respect, the closure or privatization of public hospitals to reduce their number just because of their low profits or financial burdens that must be carried by the government is to ignore their innate importance and social role; therefore, we must do all we can to block such efforts and further empower these public health facilities according to demands of the time. The improvement of public health services can be realized by redefining its goals and roles, increasing government funding, strengthening of existing public health facilities and reorganizing the public health services system. Even if public health facilities were to increase their medical services and be reinforced, they cannot take on all the services related to public health services, Therefore, in a country like ours where public health services come second to private health services in the health care system, the health of citizens can be safeguarded only when private and public facilities cooperate and private medical facilities share the social responsibilities. Only the show of interest and effort by government, politicians, health professionals, professional organizations and public can initiate the improvement that is sought.

Relationships between Perceived Stress, Mental Health, and Social Support in Community Residents (일 도시지역 주민의 스트레스 지각, 정신건강 및 사회적 지지)

  • Purpose: The study investigated the relationships among perceived stress, mental health, and social support in community residents. Method: A self-reporting questionnaire was completed by 302 community residents aged 19-64-years-of-age from October 7 to November 30, 2008. Assessment tools were an The established perceived stress scale, standardized mental health scale, and established social support scale. The data was analyzed by descriptive statistics, t-test, one-way ANOVA, Scheffe's test, and Pearson's correlation coefficient using the SPSS WIN 12.0 program. Result: Significantly negative correlations were evident between perceived stress and social support, and between social support and mental health. But, there was a significantly positive correlation between perceived stress and mental health. Conclusion: It is necessary to develop a strategy to decrease perceived stress, improve mental health, and increase social support for community residents.

Organizational Capacity and Performance of Local Public Health in Korea (지역공공보건조직의 역량과 조직성과)

  • Objectives: The purpose of this study was to investigate the differences of capacity of local health organization to regional characteristics and the influence of organizational capacity on organizational performance. Methods: The study used the secondary data for 160 local public health organizations from $5^{th}$ Community Health Plans and 2009 Community Health Survey. The collected data were analyzed using one-way ANOVA and multiple regression analysis. Results: Work force and budget showed differences in regional size and elderly population rate. And consumer satisfaction and health care utilization showed differenced in work force and budget. The regression model with total number of employee, number of registered nurses, number of doctors and budget against consumer satisfaction was statistically significant (F=14.70, p=<.001), and number of registered nurses was identified as a factor influencing consumer satisfaction. This model also explained 20.5% of service satisfaction. The regression model for consumer satisfaction was statistically significant (F=45.98, p=<.001), and total number of employee nurses was identified as a factor influencing health care utilization. This model also explained 53.1% of utilization. Conclusions: The findings of this study imply that organizational capacity as work force and budget should be increased to improve the organizational performance as consumer satisfaction and health care utilization.

The Function of Health Center in the New Public Health Age (공중보건의 역할변화와 보건소의 기능)

  • Public health system and public health practice have changed over the past decades as the result of social and epidemiologic changes. New public health concept emphasizes leadership, strategical thinking, systematic planning and effective performance to identify and solve complex health problem In Korea, the role of Health Center has been expanded rapidly. However there is strong suspicion that Health Center can achieve their mission. This paper aims at understanding what is the needed functions of Health Center and which of its aspects needs improvement. The main results of this study are summarizes as follows. District Health Law does not address the core functions of Health Center. The staffs of Health Center have difficulties in prioritizing their services. The recent attempt to restructure Health Center and change delivery pattern of public health services ended with only limited success. To effectively confront threats to the public's health, the three major function of Health Center must be ${\circled}1$ modifying individual behavior and lifestyle, ${\circled}2$ improving social and economic conditions, and ${\circled}3$ reforming health policies. Better results do not come from setting new functions only; they come from understanding and improving the processes that will then leads to better outcome. We recommend that policy-makers focus economic evaluation of public health programs, building and spreading of the scientific evidence of programs, linkage of public health research and public health program. The criteria of delegation of public health service to private sector is urgently needed. Making community health information data available on a routine basis to providers, managers and researchers of public health services helps promote the efficiency of the overall operation of public health system.

Analysis of the Nursing Interventions Performed by Public Health Nurses in Health Centers Using the NIC (보건소 간호사의 간호중재 분석 - 간호중재분류[NIC]의 적용 -)

  • Purpose: The purpose of this study was to identify nursing interventions performed by public health nurses in health centers. Method: Data was collected by the taxonomy of Nursing Intervention Classification(NIC 3rd: 486 nursing interventions) from 131 public health nurses in health centers and analyzed using descriptive statistics. Result: As its result, more than 50% of public health nurses performed 137 nursing interventions at least monthly. The most frequently used intervention class was 'activity and exercise management', followed by 'physical comfort promotion', 'community health promotion', 'life span care', 'coping assistance', 'Self care facilitation', 'information management', 'nutrition support', 'community risk management' and 'patient education'. One hundred twenty nursing interventions were rarely performed by 90% or more of the nurses. Most of them were the physical complex domain. Conclusion: In conclusion, 137 interventions were performed by public health nurses at least monthly. NIC is helpful to build a standardized language for public health nursing.

Comparative Research of the Medical Cost, The Quality of Life, The Family burden of the Mentally III before and after the Community Mental Health Service (지역사회 정신보건서비스 제공 전$\cdot$후 정신장애인의 의료비용, 삶의 질, 가족 부담감 비교연구)

  • Community mental health management system emphasizing on the rehabilitation and the return to the community has been established and carried out for many years. The study has been demanded to prove that the decreasing rate of the recurrence of the mentally ill resulted to lower their medical costs, to enrich the quality of life, and to reduce the psychological burden of their family. This study tried to prove that the mental health services to the mentally ill which were registered in community mental health center of A city have an influence on the medical cost, the quality of their lives. the family burden. The subject group of this study were 39 home-based mentally ill patients and their 37 family members, totally 76 people registered in mental health center of A city and participated in its program. This research had been measured twice, the first before the intervention and the second after at least a year. The measuring tools in the research were the medical cost measurment tools developed by the researcher, the quality of life index by Yoo ja, Noh(1988) and the family burden by Montgonery(1985). The methods were modified and supplemented in this study. This research made use of SPSS Win 10.0. The results of this study are the same as followings. 1) There were the significant difference in the medical cost before and after the mental health service delivery. 2) The quality of lives of the mentally ill, after the mental health services delivered were significantly higher than before. 3) The family burden were significantly reduced after the delivery of community mental health services. Community mental health services brought out efficient results to the social return and rehabilitation. And these results means that the mentally ill changed highly the quality of life and their burden of family and medical cost were reduced. So the public organization and the private society should help positively the mentally ill and their family through mental health policy and social service agency to live healthy lives and to be valuable member of society.

Needs Assessment of Nutrition Programs in Public Health Center by General Characteristics and Body Mass Index of Community Residents (지역사회 주민의 일반적 특성 및 체질량 지수에 따른 보건소 영양사업 요구도 평가)

  • The purpose of this study was to assess the needs for nutrition programs in the public health centers by general characteristics and body mass index (BMI) of community residents. Information of general characteristics of study participants including age, education, income, marital status, residence, and job, and resident's interest in nutrition programs, the preferred educational methods and the willingness to participate in nutrition programs were collected by an interview. Weight and height were measured and were used to calculate the BMI. The program that resident's were most interested in was 'diet therapy for chronic disease and counseling' ($28.5\%$), followed by 'nutritional management for the elderly' ($21.1\%$), and 'obesity and weight control' ($17.1\%$). 'Education and counseling by nutrition professionals' was the most preferred educational method. Among the programs that the subjects would participate in, if they were offered in public health centers, $65.8\%$ subjects would participate in 'diet therapy for chronic disease and counseling' programs, $64.9\%$ would participate in 'nutritional management for the elderly' programs and $52.2\%$ would participate in 'obesity and weight control' programs. The contents of programs that the subjects were interested in, the preferred methods and their willingness to participate nutrition programs differed significantly by age, income, education, marital status, and body mass index. The results imply that the planning of nutrition interventions in the public health centers must be tailored and targeted group specific by taking the participants general characteristics and body mass index into consideration. This would surely increase the nutrition program's effectiveness

Analysis of Working Time of Nurses in Urban Public Health Center Branches in South Korea.: Focused on Nurses for Visiting Health Service and Chronic Disease Management (도시보건지소 간호사의 업무활동 소요시간 분석 - 방문보건 및 만성질환관리를 중심으로 -)

  • Purpose: The purpose of this study is to investigate working time of nurses in urban public health center branch, especially for nurses for visiting health service and chronic disease management. Method: Daily note. which was developed by this research team. was distributed to nurses at 7 urban public health center branches to be filled out them for 2 weeks during 2 month from June 2007 to August 2007. We analyzed 121 daily notes recorded by visiting nurses and 65 daily notes written by chronic disease management nurses were analyzed. Result: The total working time for visiting nurses at urban public health center branches was 589.85 minutes per day on the average. They spent 147.13 min in actual visiting nursing services, 149.36 min in documenting, 66.94 in preparing, 77.69 min in transferring, and 11.84 min in referring. The total working time for chronic disease management nurses at urban public health center branches was 582.92 minutes per day on the average. They spent 148.77 min in actual chronic disease management services, 120.62 min in documenting, 42.46 min in group education, 37.38 in preparing, and 10.38 min in referring. Conclusion: Based on the results of this study, it is recommended to improve documenting systems and to increase community resources linkage were recommended through the results of this study. The results of this study are expected to be used to plan staffing at urban public health center branches in the future.

Relationships of Depression Symptom, Self-Esteem, and Stress to Health-Related Quality of Life in Patients with Hypertension Registered to a Community Health Center (보건소를 이용하는 고혈압 환자의 우울증상, 자아존중감, 스트레스 및 건강관련 삶의 질 관계)

  • Purpose: This study was conducted to examine the relationships of depression symptom, self-esteem, and stress with health-related quality of life(HRQOL) in patients with hypertension registered to a community health center. Methods: This study was a correlational survey using a convenience sampling. A total of 110 patients diagnosed with hypertension were recruited from a health center in Gyeonggi-do. The questionnaires used were the Cardiovascular Disease Specific-HRQOL questionnaire, Center for Epidemiologic Studies Depression, Rosenberg Self-Esteem Scale, and Perceived Stress Scale. The acquired data were analyzed using IBM SPSS version 22.0. Multiple linear regression analysis was performed. Results: Moderate depression symptom(${\beta}=-.368$, p<.001), severe depression symptom (${\beta}=-.450$, p<.001), stress(${\beta}=-.339$, p=.001), and gender(${\beta}=-.148$, p=.049) were significant predictors for the HRQOL. Multiple linear regression showed that 51.8% ($R^2=.518$) of the variance in the HRQOL was explained. Conclusion: Based on these results, development of an intervention or education program, to decrease depression symptoms and stress is recommended. This may improve the HRQOL in patients with hypertension registered to a community health center.
