A Study on the Architectural Planning for Integrated Nursing Care Service in Ward of Public Hospital - Focused on the Regional Public Hospital in Korea - (간호·간병통합서비스를 위한 국내 공공병원 병동부에 대한 건축 계획적 연구 - 국내 지역거점 지방의료원을 중심으로 -)

  • The study is to identify the status of the ward of public hospital for integrated nursing care service. Integrated nursing service has expanded nationwide from 2016 but 10 out of 34 public hospitals are currently unable to operate as nurses due to the shortage. Analyze each floor plan and space program of ward and provide the basic data for the construction of the architecture plans for Integrated nursing care service. For this study, selecting 14 wards of public hospital in Korea and then analyzes space organization, distance from NS to bedroom and NS to core(Elevator) and spatial hierarchy of them. Among the 14 public hospitals, there are 7 types of double-loaded corridor, 4 types of double corridor and 3 types of single corridor. If NS is placed centrally, the control of the outsider and the patient's control would be efficient, but most of NS are lean on one side of floor. the placement of NS should be independent of individual arrangements, but it should be smooth alignment with patient spaces, staff spaces, and public spaces. This study could serve as basic research for the architectural plan for future integrated nursing care in ward of public hospital.

  • The purpose of this study is identify the status of the ward of public hospital for integrated nursing care service. Integrated nursing care service has been expanding on a national scale from 2016 but most of public hospitals are currently unable to operate due to nursing shortage. In this study 8 wards of public hospital have been chosen and analyzed. The measure of space program and area distribution(patient area percentage, staff area percentage, circulation area percentage), nurses's walking distance(distance from ns to patient room, distance from ns to core) were calculated by depthmapX and autocad programs. The result of this study is as follows. Nurse's walking distance is more than 24m so the efficiency of nurse's patient care is reduced. The percentage of patient area in double-corridor is higher and the patient feels more comfortable but the Nurse's walking distance is longer and the accessibility is lowered. NS should be located in the center of the ward and close to the core but some wards are not composed of proper space-separation and flow of human trrafic is overlapped. This study may serve as basic research for the architectural plan for future integrated nursing care ward.

Perception on Nursing Practice of Total Care Service in Nursing Students (간호간병 통합서비스 간호업무에 대한 간호대학생의 인식)

  • Purpose: This study aims to identify the factors that affect the expected frequency of performance and the importance in nursing practice of total care service recognized by nursing college students. Methods: This is a descriptive study. A self-administered questionnaire survey was conducted on 288 nursing students of E university and data was collected and analyzed using the SPSS program. Results: The average scores for the expected frequency of performance and the importance in nursing practice of total care service were $4.07{\pm}0.58$ and $4.00{\pm}0.50$ respectively. The expected frequency of performance was found to have a significantly positive correlation with the importance in nursing practice. Additionally, the factors that influenced the degree of recognition in the expected frequency of performance and the importance in nursing practice of total care service among nursing students were comfortable care and respiratory care, respectively. Conclusion: The major finding of this study shows a significantly positive correlation between the expected frequency of performance and the importance in nursing practice. These results suggest that effective and systematic education is needed on factors affecting the expected frequency of performance and the importance in nursing practice for nursing college students.

Development of Staffing Levels for Nursing Personnel to Provide Inpatients with Integrated Nursing Care (간호·간병통합서비스 제공을 위한 간호인력 배치기준 개발)

  • Purpose: To develop staffing levels for nursing personnel (registered nurses and nursing assistants) to provide inpatients with integrated nursing care that includes, in addition to professional nursing care, personal care previously provided by patients' families or private caregivers. Methods: A time & motion study was conducted to observe nursing care activities and the time spent by nursing personnel, families, and private caregivers in 10 medical-surgical units. The Korean Patient Classification System-1 (KPCS-1) was used for the nurse manager survey conducted to measure staffing levels and patient needs for nursing care. Results: Current nurse to patient ratios from the time-motion study and the survey study were 1:10 and 1:11, respectively. Time spent in direct patient care by nursing personnel and family/private caregivers was 51 and 130 minutes per day, respectively. Direct nursing care hours correlated with KPCS-1 scores. Nursing personnel to patient ratio required to provide integrated inpatient care ranged from 1:3.9 to 1:6.1 in tertiary hospitals and from 1:4.4 to 1:6.0 in general hospitals. The functional nursing care delivery system had been implemented in 38.5% of the nursing units. Conclusion: Findings indicate that appropriate nurse staffing and efficient nursing care delivery systems are required to provide integrated inpatient nursing care.

Effect of Educational Nurse Practitioners at Comprehensive Nursing Care Service Units (간호·간병 통합서비스 병동 교육전담간호사 운영 효과)

  • Purpose: This study aimed to investigate the effect of educational nurse practitioners at comprehensive nursing care service units by comparing the educational satisfaction of general nurses and the occupational stress of preceptor nurses before and after education by educational nurse practitioners. Methods: Participants were 71 general nurses and 71 preceptor nurses working at comprehensive nursing care service units of G University hospital in I City. The level of educational satisfaction of general nurses and occupational stress of preceptor nurses were measured and compared. The differences in educational satisfaction and occupational stress before and after the education program were verified by paired t-test and one-way ANOVA using SPSS/WIN 27.0 statistical program. Differences before and after the education program according to the general characteristics were analyzed with repeated measure analysis of variance after Sapiro-Wilks normality test, and post-hoc analysis was conducted using Scheffétest. Results: It was inferred that there was a significant difference (t=-9.32, p<.001) in the educational satisfaction of general nurses, with an average of 3.72±0.77 before the education and 4.47±0.60 after the education. However, there was no significant difference in occupational stress in the preceptor nurses. Conclusion: It could be concluded that introduction of educational nurse practitioners at comprehensive nursing care service units has a positive effect on the educational satisfaction of general nurses. This result will serve as basic data to implement educational nurse practitioners for general nurses in all medical institutions as well as comprehensive nursing care service units.

The Effects of Organizational Commitment and Perceived Patient Safety Culture on Patient Safety Nursing Activities among Nurses in Comprehensive Nursing Care Units (간호·간병통합서비스 병동 간호사의 조직몰입과 환자안전문화인식이 환자안전간호활동에 미치는 영향)

  • Purpose: The purpose of this study is to identify the effects of organizational commitment (OC) and perceived patient safety culture (PPSC) on patient safety nursing activities (PSNA) among nurses in comprehensive nursing care units. Methods: Participants were 173 nurses working at five general hospitals in Chungcheong area. Data were analyzed using descriptive statistics, 𝑥2 test, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis with SPSS/WIN 23.0 programs. Results: The mean scores of the OC and PPSC were 3.28±0.50 and 3.85±0.35, respectively. The mean score of PSNA was 4.55±0.41, and PSNA was significantly different by the experience of participating in hospital's safety culture campaigns (t=2.70, p=.008). The results of the multiple regression analysis showed that 'patient safety knowledge and attitudes' (β=.27, p=.006) and 'unpunished environment to error' (β=.22, p=.004) as the sub-categories of PPSC were affecting factors on PSNA with an explanatory power of 26.0% (F=6.40, p<.001). Conclusion: The results of this study suggest that in order to promote PSNA among nurses in comprehensive nursing care units, it is necessary to develop a program to enhance patient safety-related knowledge and attitudes. In addition, the hospital's organizational efforts such as operating safety campaigns and creating an unpunished environment to error should be needed.

Influencing Effects of Job Stress, Professional Autonomy, Reciprocity on the Job Embeddedness of Comprehensive Nursing Care Unit Nurses (간호·간병통합서비스 병동 간호사의 직무 스트레스, 전문직 자율성, 호혜성이 직무착근도에 미치는 영향요인)

  • Purpose: The purpose of this study was to identify the influencing effects of job stress, professional autonomy, and reciprocity on the job embeddedness among comprehensive nursing care unit nurses. Methods: The participants in this study were 147 nurses who have worked for over 6 months in Comprehensive Nursing Care Unit. Data were collected from January 3 to January 31, 2022 from six general hospitals with more than 300 beds in three cities in G, G and P. Results: As the results of hierarchical regression analysis, job embeddedness was lower when nurses had clinical experiences for 1 to under 5 years (β=-.49, p<.001), 5 to under 10 years (β=-.27, p=.035), 10 to under 20 years (β=-.54, p<.001) compared to those who had clinical experiences for more than 20 years. Also, job embeddedness was higher when there was greater balance within team caregiving of reciprocality (β=.27, p<.001) and intrinsic reward (β=.22, p=.003), and lower role conflict (β =-.27, p<.001). Conclusion: The results of the study showed that job embededness would increase if the role conflict of comprehensive nursing care unit nurses reduces, if there was a culture that the members can make cooperative relationship with other health care professionals and, if there was an appropriate intrinsic reward depending on their work experience.

The Emotional Labor Status and Improvement Plans of Nurses Working in the Integrated Nursing Service Ward: Applying Focus Group Interviews (간호⋅간병통합서비스 병동 간호사의 감정노동 파악 및 개선방안: 초점집단인터뷰 적용)

  • Purpose: The purpose of this study was to investigate the status of emotional labor of nursing personnel working in comprehensive nursing service ward and to suggest the way service improvement can be achieved. Methods: A total of 28 nurses working in comprehensive nursing service ward were divided into four groups to conduct focus group interview. All interviews were recorded and transcribed after the interview to perform data analysis in the order of data classification, topic categorization, and keyword derivation. Results: The five categories of subjects and relating keywords drawn from the focus group interviews are as follows: 1) Emotional labor experience: suppressing emotions, expressing emotions or actions that are different from reality, 2) Situations of emotional labor: verbal abuse and assault, sexual harassment, personal needs and errands, 3) Responses to emotional labor: responding directly, responding directly, receiving senior's help, using the organizational system, persevering, 4) Problems caused by emotional labor: work exhaustion, job change intention, job stress, 5) Protection plan against emotional labor: manual or education for nurses, education for patients and carers, compensation, tough sanctions though system strengthening. Conclusion: This study shows that although nurses working in comprehensive nursing service ward generally experience high levels of emotional labor, the problem solving of them relies mainly on personal response. Therefore, it is necessary to develop various measures to protect nurses in an organizational level response, thus to improve the comprehensive nursing service system.

Effects of Nursing Organizational Culture and Self-leadership on Job Satisfaction for Nurses Working in the Comprehensive Nursing Care Service Ward in Public Hospitals (간호조직문화, 셀프리더십이 공공병원 간호·간병통합서비스 병동 간호사의 직무만족에 미치는 영향)

  • Purpose: This study investigated the effects of nursing organizational culture and self-leadership on job satisfaction for nurses working in the comprehensive nursing care service ward in public hospitals. Methods: The participants were 138 nurses working in Gyeonggi-do medical center. The collected data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, and regression analysis with SPSS/WIN 23.0. Results: Factors affecting job satisfaction were innovation-oriented culture (${\beta}=.29$, p<.001), want to work in this department (${\beta}=-.25$, p<.001), the cognitive strategy of self-leadership (${\beta}=.23$, p=.002), hierarchical-oriented culture (${\beta}=-.21$, p=.002), and marital status (${\beta}=.18$, p=.005). These factors accounted for 42.0% of the variability in job satisfaction. Conclusion: In order to improve the job satisfaction of nurses in the comprehensive nursing care service ward in public hospitals, institutional-level efforts should be made to encourage an innovation-oriented culture and avoid a hierarchy-oriented culture. In addition, education and program development are needed to improve the cognitive strategy of self-leadership.

A Study about Compassion Fatigue, Compassion Satisfaction and Burnout in Comprehensive Nursing Care and General Ward Nurses (간호·간병통합서비스 병동과 일반병동 간호사의 공감피로, 공감만족, 소진 비교)

  • Purpose: The purpose of this study was to compare the differences in compassion fatigue, compassion satisfaction and burnout between nurses working at comprehensive nursing care unit and general ward. Methods: The subjects were 35 nurses in general ward and 42 nurses in the Comprehensive Nursing Care unit in one hospital. Measurement instrument included the Stamm's professional quality of life (ProQOL) version 5-Korean. Descriptive statistics, t-test, paired t-test, ANCOVA, and Pearson correlation were used to analyze the data. Results: Nurses working at the Comprehensive nursing care unit demonstrated significantly lower compassion fatigue (F=17.00, p<.001), higher compassion satisfaction (F=14.39, p<.001), and lower levels of burnout (F=40.07, p<.001) than control group. Conclusion: Compassion fatigue and burnout were lower and compassion satisfaction was higher among nurses working at comprehensive nursing unit than general ward. In order to improve quality of the comprehensive nursing care services, there is a need to be concerned with the nurse's compassion fatigue, compassion satisfaction, and burnout.
