  • Purpose : This study aimed to investigate the impact of clinical reasoning competence, positive psychological capital, and nursing work environment on nursing performance of intensive care unit (ICU) nurses in tertiary general hospitals. Methods : Data were collected from 169 ICU nurses in tertiary general hospitals in Busan, South Korea, from July 27 to August 15, 2023, using a self-report questionnaire. After excluding 12 questionnaires with insufficient responses, 157 responses were analyzed. Descriptive statistics, independent t-test, one-way ANOVA, Pearson's correlation coefficient, and stepwise multiple regression analysis were employed. Results : Factors influencing nursing performance included clinical reasoning competence (β=.51, p <.001), hope as a subfactor of positive psychological capital (β=.31, p <.001), and the nursing foundations for quality of care in the nursing work environment (β=.21, p <.001). The explanatory power of the regression model was 62% (F=87.41, p <.001). Conclusion : The study underscores the importance of improving clinical reasoning competence among ICU nurses to enhance nursing performance and foster a hopeful attitude through reflective practices and personal growth. Organizational interventions, such as educational and psychological support programs, along with creating a supportive nursing environment, are crucial for improving nursing performance among intensive care unit nurses.

  • Purpose - This study is to propose discriminative management strategies to long-term care facilities based on the empirical analysis after reviewing the effects of social support, perceived by long-term care facility employees, to service orientation. Research design, data, and Methodology - The research model designed social support, job stress, organizational commitment, and service orientation. The survey collected data from 453 customers in a long-term care facility in jeju. The SPSS 18.0 package was used for analysis. Results - First, social support for long-term care facility employees has a negative(-) effect to job stress. Test results, social support factors except appraisal support had a negative impact on job stress. Second, social support has a positive(+) effect to organizational commitment. Test results, informational support, tangible support and appraisal support had significant effects on organizational commitment. However, emotional support had a positive impact on affective commitment and normative commitment. Third, social support has a positive(+) effect to service orientation. Test results had a positive impact. Fourth, job stress has a negative(-) effect to organizational commitment. In the test results, employee's continuance commitment and normative commitment had significant negative effects in job stress. However, affective commitment had no significant impact. Fifth, job stress has a negative(-) effect to service orientation. Test results showed a negative impact. Conclusions - The study implies the following. First, that there should be a change in the social perception of long-term care facilities. 'Long-Term Care Insurance for The Elderly' was enacted to emphasize this responsibility for the elderly problems as a new system. Enactment of this Act was expected to improve the quality of life of the people by stabilizing the elderly life and reducing the burden of families. Therefore, long-term care facility system should be as efficient as possible for making plans for systematic and organizational support. • Purpose: The purpose of this study was to develop a valid tool for measuring the work environment of Korean nurses and test the validity and reliability of the instrument. Methods: This is a methodological study for developing and testing a measurement tool. The conceptual framework for the tool was formed through a literature review and in-depth interviews with clinical nurses. 260 meaningful statements were derived through this process, and 59 of the primary items were generated out of these statements. On the basis of the content validity test, 41 items were selected for the preliminary tool. A total of 350 nurses from 7 hospitals responded to the preliminary questionnaire in October 2011. Results: After the item analysis and the principal component analysis using varimax rotation methods, 11 items were deleted. Considering the conceptual framework and the scree plot, we extracted 4 factors from the final 30 items. These factors were organizational support, head nurse's leadership, support system, and relationship with coworkers, and their total variance was 54.4%. The value of Cronbach's ${\alpha}$ of the final tool was 0.92. Conclusion: The above findings revealed that the Korean Nursing Work Environment Scale (K-NWES) is an appropriate, valid, and reliable tool for evaluating the work environment of Korean nurses. Further research to perform a criterion validity test of the K-NWES is needed.

  • Purpose: The purpose of this study was to identify factors affecting professional burnout in intensive care unit (ICU) nurses. Method: Data were obtained through questionnaires from 240 ICU nurses working in hospitals of 500 beds or more in two cities in South Korea. Data analysis was done with t-test, ANOVA, Scheffe test, Pearson correlation coefficients and multiple stepwise regression with SPSS WIN 14.0. Result: Work stress was positively correlated with level of professional burnout and there was a negative correlation between hardiness and professional burnout. Social support was negatively correlated with level of professional burnout. Hardiness had a positive correlation with social support and a negative correlation with work stress. The explained variances for burnout was 44.7% and factors affecting professional burnout in the ICU nurses' were work satisfaction, work stress, social support, and hardiness. Conclusion: The findings indicate that high levels of stress coupled with low levels of hardiness and social support in ICU nurses result in high levels of professional burnout. Reducing work stress and the level of professional burnout in ICU nurses and increasing work satisfaction can be done by enhancing the mediating factors of hardiness and social support through educational efforts and nursing and administrative support.

  • This study investigated the performance of the child care policies implemented by the former government and analyzes some changes in child care policies by the new government in Korea. The criteria for evaluating child care policies of both governments were based on suggestions gleaned from OECD policy review papers on Early Childhood Education and Care (ECEC) in 2004 and 2006. As suggested by the OECD, the input of financial investment in public sectors, universal approaches to child care, measures of quality programs, efforts for improving the work environment of child care workers and selecting effective ways of providing financial support for child care were applied in order to evaluate the child care policies in Korea. A number of policy documents and literature published by both governments from 2003-2009 were reviewed in order to evaluate and compare the former child care policies with the more recent ones. The child care policies enacted by the former Korean government were characterized as the remarkable increases in financial investment to establish a child care infrastructure for quality programs and services, and efforts to enact universal approaches to child care, policy making based on scientific data on child care. These advances were tempered by the observation that despite all these improvements, both investment and expansion in the public child care sector were far below sufficient levels. In contrast, some changes in the child care policies by the new government were criticized in terms of weakening public child care, reinforcing private child care and it's conservative financial support system.

  • Purpose: This study was done to identify differences in work environment, work stress, turnover intention and burnout and investigate the relationship among these variables in nurses in Korean Neonatal Intensive Care Units. Method: Participants were 242 nurses working in 13 general hospitals. Burnout was measured by the Maslach Burnout Inventory (MBI), work stress with the instrument by Gu & Kim (1994), and turnover intention with the scale by Kim & Lee (2001). Size of the NICU, nurse to patient ratio, and communication satisfaction were included in work environment. Data were analyzed using Pearson correlation coefficients and multiple regression analysis with SPSS WIN program. Results: The mean score for work stress in NICU nurses was 3.43 points, for burnout, 2.72 points, and for turnover intention, 4.64 points. Burnout and turnover intention level of participants were moderate-high. Work stress, communication dissatisfaction with physician, and clinical career accounted for 33% of variance in burnout. Significant differences were found between size of NICU and staffing related to environmental characteristics in turnover intention and burnout. Conclusions: Results indicate that effective communication with coworkers and institutional support for appropriate staffing according to number of beds will help to prevent work stress, burnout, and ultimately, nurses' resignations.

  • Caring for patients with cancer is highly stimulating and rewarding, attracting health professionals to the field who enjoy the challenge of managing a complex illness. Health professionals often form close bonds with their patients as they confront ongoing disease or treatment impacts, which may be associated with multiple losses involving function and/or eventual loss of life. Ongoing exposure to patient loss, along with a challenging work setting, may pose significant stress and impact health professionals' well-being. The prevalence rates of burnout and compassion fatigue (CF) are significant, yet health professionals have little knowledge on these topics. A 6-week continuing education program consisting of weekly small-group video-conferencing sessions, case-based learning, and an online community of practice was delivered to health care providers providing oncology care. Program content included personal, organization and team-related risk and protective factors associated with CF, grief models, and strategies to mitigate against CF. Content analysis was completed as part of the program evaluation. In total, 189 participants (93% nurses) completed the program, which was associated with significant improvements in confidence and knowledge of CF and strategies to support self and team resilience. Qualitative themes and vignettes from experiences with the program are presented. Key themes included knowledge gaps, a lack of support related to CF and strategies to support resilience, organization-and team-based factors that can inhibit expression about the impacts of clinical work, the health professional as a "person" in caregiving, and the role of personal variables, self-skill practices, and recommendations for education and support for self and teams.

  • Purpose: The aim of this study was to investigate and identify work environment, job embeddedness, and burnout among general hospital nurses in Korea. Methods: The participants were 563 clinical nurses working in 13 general hospitals across the country. Data were analyzed using SPSS and Microsoft Excel programs. Results: Mean scores were 2.62 for nurses' work environment, 2.97 for job embeddedness, and 3.61 for burnout. Nurses' work environment showed a positive correlation with job embeddedness (r=.70, p<.001), but a negative correlation with burnout (r=-.49, p<.001). Subcategories of nurses' work environment that predicted job embeddedness included satisfaction and happiness, hospital support for the work environment, patient care environment, satisfaction with work schedule, manager leadership, supportive environment for nurses' work, and computer problems. Subcategories of nurses' work environment that predicted burnout included satisfaction and happiness, violence within ward, hospital support for work environment, and patient care environment. Conclusion: Findings from this study indicate the need to evaluate and improve the work environment for nurses to increase job embeddedness and control burnout. Future studies should explore ways in which turnover intention can be decreased by changing nurses' work environment.
