  • Purpose: This study aimed to evaluate economic viability of public health center visiting nurse services for the low-income elderly with long-term care needs. Method: The sample consisted of 252 community dwelling elderly who enrolled in public health center visiting nurse services for three months or more. Data was collected on physical (ADL and IADL) and cognitive impairments of the elderly, contents and frequency of visiting nurse services, cost per visit, and costs of alternative services for long-term care. Result: The mean score of ADL and IADL levels of the elderly was 2.80.4904, which indicated these patients were mostly independent. Eighty four percent of the elderly subjects were cognitively intact. Among visiting nurse services supplied, providing assessment was 34%, followed by education and counseling 26%, medication 22%, and referral. The mean cost per visit was 17,824.1 won, which transformed into a total cost per person per year of 161,130.2 won. Comparing the cost of a visiting nurse service with those of other long-term care alternatives, the visiting nurse service was the least costly alternative, followed by an outpatient clinic, hospital based home care, and nursing home. Conclusion: Overall, the results of the study provide evidence of the economic viability of visiting nurse services for the low-income elderly among long-term care alternatives.

  • Objectives : This study examined the influence of socio-demographic, health-related, and oral health-related characteristics on adolescent DMFTs. Methods : The subjects in this study were 1,129 adolescents selected from the 6th national health and nutrition examination survey data. To determine the factors influencing dental caries, a regression analysis using a complex-sample generalized linear model was conducted after adjusting for confounding factors. Results : DMFTs were smaller among boys than girls, and smaller in the "13-15 age group" than in the "16-18 age group." In terms of household income, DMFTs were larger in the "lower," "lower-middle," and "upper-middle" income brackets than in the "upper" income bracket. DMFTs were smaller among adolescents with a history of smoking than those who had never smoked. Conclusions : In order to improve the oral health of adolescents, oral health-promotion programs should be provided for girls, high school students, students from low-income families, smokers, and those who consider themselves to have poor oral health.

  • A study was conducted to determine whether there is a difference in the incidence of kidney cancer according to income level and the difference in delayed diagnosis. To this end, the incidence of kidney cancer in Korea was analyzed by income level and by stage. From 2015 to 2017, a national kidney cancer cohort was established by linking the KCCR(Korea Central Cancer Registry), NHISS(National health insurance sharing service), and the HIRA(Health insirance review and assessment service) database to calculate the kidney cancer incidence by stage and income level. During the study period, the incidence of kidney cancer in Korea increased in all income deciles, but decreased only in the medical aid population. The incidence of kidney cancer in Korea was 7.35 per 100,000 people, and 83.54% of them were locoregional kidney cancer. In the top 20% of the income decile, there was a high incidence of 21.46 cases per 100,000 people, among which 18.37 cases were locoregional kidney cancer. On the other hand, even after adjusting for risk factors related to kidney cancer, it was confirmed that the lower the income level, the higher the risk of being diagnosed with kidney cancer with distant metastasis (lowest income 20% adj.OR 1.807, 95% CI 1.411-2.222). In the insured population, the risk ratio of being diagnosed with unknown stage was 1.926 (95% CI 1.317, 2.816). The higher the income level, the higher the frequency of early cancer diagnosis, but the lower the income level, the higher the risk of being diagnosed with metastatic kidney cancer or an unknown stage, so health inequality according to income level was observed.

  • This paper is an exploratory study aimed at finding factors that influence the penetration of cellular mobile telecommunications which have seen rapid development since 1980s. The paper tries to find economy and policy variables to influence the penetration by means of the multiple regression analysis. This paper shows a model that explain the penetration ratio of cellular service. The model is based on data from 39 upper middle income and 39 high income countries. The result shows that GDP per capita and the service period of the cellular mobile telecommunication have influenced on the penetration ratio positively, and the tariff of the service negatively. But, it is the open area to study the impact of the telecommunications policy changes on penetration, and to set a model which could explain the nation-specific characteristics.
