요양병원 간호인력의 카바페넴 내성 장내세균속균종 감염관리 수행 정도에 미치는 요인 (Factors Affecting Performance of the Infection Control of Carbapenem Resistant Enterobacteriaceae of Nursing Staffs in Long-term Care Hospitals)

  • Purpose: This study aimed to identify factors affecting the carbapenem-resistant enterobacteriaceae (CRE) infection control performance of nursing staff, who closely contact patients with CRE in long-term care hospitals. Methods: A cross-sectional study design was used. A total of 135 nursing staffs working in seven long-term care hospitals in the southern and northern areas of the K province in Korea were included. We measured the CRE infection control general characteristics, knowledge, perception, and performance. Results: The main factors affecting the CRE infection control performance were education, knowledge, and perception. The model explained the 60.8% total variance in CRE infection control. Conclusion: Appropriate infection control strategies should be prepared to provide high quality nursing care and prevent the spread of CRE infection in long-term care hospitals. Establishing an efficient infection control system in long-term care hospitals is necessary.

요양병원 간호사의 임종간호수행 구조모형 (A Structural Equation Model of Terminal Care Performance for Long-term Care Hospital Nurses)

  • Purpose: This study developed a structural model for explaining and predicting terminal care performance in long-term care hospital nurses. The model was based on the stress integration model of Ivancevich and Matteson(1980) and the results of previous studies. Method: Data was obtained from August to September 2022 from 267 nurses in 13 long-term care hospitals in G-do. Results: Results of model verification for this study, revealed that factors directly affecting the terminal care performance of long-term care hospital nurses were nursing work environment(β=0.43, p<0.001), death anxiety(β=-0.29, p<0.001), and terminal care stress(β=0.22, p=0.003). However, the attitude toward nursing care of dying(β=0.07, p=0.287) had no effect on the terminal care performance. Conclusion: The results of this study, confirmed the necessity of improving an individual's perceived nursing work environment, continuous education related to terminal care to reduce death anxiety, and an interventional approach for enhancing terminal care performance.

노인요양병원 간호사의 섬망에 대한 지식, 섬망간호스트레스 및 섬망간호수행간의 관계 (Relationship between Knowledge, Stress, and Nursing Performance about Care for Delirium in Geriatric Hospital Nurses)

  • Purpose. The purpose of this study was to investigate knowledge, stress and nursing performance about care for delirium in geriatric hospital nurses. Methods. For this study, data were collected from 242 nurses employed at geriatric hospitals with the capacity of more than 100 beds which were located in A, P and Y cites. Frequency analysis, t-test and one-way ANOVA were utilized to analyze the data and Scheffe test was used to conduct post-hoc tests. Results. Nurses showed significant differences in the level of Knowledge, as well as terminal care performance by marital status, nursing experience. A significant relationship was found between knowledge, stress and Delirium care performance. Conclusion. This study found nurses perform delirium care with a high level of stress but with insuffiecient understanding, and thus, showed great needs for related training. Such findings can be useful to develop educational programs to improve knowledge on delirium and plans to reduce stress from delirium for geriatric hospital nurses.

간호사의 직무이행도, 업무만족도 및 생산성에 대한 예측인자 (Predictors of Job Performance, Work Satisfaction, and Productivity among Korean Hospital Nurses)

  • Purpose: The purpose of this study was to examine the levels of burnout experienced by Korean hospital nurses (N=198), and to identify predictors of their nursing outcomes such as job performance, work satisfaction, and productivity. Method: Hierarchical multiple regression was used to identify predictors of each nursing outcomes. Included predictors were nurses' general characteristics, work- related characteristics (role stress and perceived control), and burnout. Results: Korean nurses experienced higher levels of burnout compared to the cutoffs suggested by Maslach and Jackson (1986) and to those in the USA. For each nursing outcome, predictor variables explained 39% of the variance in role performance, 30% of the variance in work satisfaction, and 38% of variance in productivity. Higher personal accomplishment, lower role ambiguity, being staff nurses, and lower emotional exhaustion were related to higher job performance, and higher productivity. Lower role conflict and role ambiguity were also related to higher work satisfaction. Conclusion: Based on the findings of this study, managemental interventions for nurses to reduce their burnout experience are needed. Further study in this area is warranted.

천식아동 간호 시뮬레이션 학습의 효과 (Effects of Simulation-Based Learning in the Nursing Care of Children with Asthma)

  • Purpose: The purpose of this study was to examine the effects of simulation-based learning on the knowledge about and performance confidence in the nursing care of children with asthma. Methods: A one-group pre- and post- study design was used. A total of 70 nursing students participated in the study. In groups of five, the participants experienced simulation-based learning during their child health nursing practicum. The given scenario was about nursing care for children with asthma. The simulation learning was provided for 3 hours, and included a group discussion before the simulation and a debriefing. Results: After the simulation, knowledge about and performance confidence in the nursing care of children with asthma significantly increased. The nursing students reported a high level of satisfaction with the simulation-based learning. There was a significant positive correlation between knowledge and performance confidence. Conclusion: Simulation-based learning was effective for nursing students in the nursing care of children with asthma. Our results suggest utilizing this new way of learning to strengthen the clinical experience of child health nursing in nursing students.

COVID-19 이후 임상실습을 경험한 간호대학생의 대인관계능력, 사회심리적 건강, 간호술기수행자신감이 임상수행능력에 미치는 영향 (The Impact of Interpersonal Skills, Psychosocial Health, and Confidence in Performing Nursing Skills on Clinical Performance of Nursing Students Who Experienced Clinical Practice after COVID-19)

  • 본 연구는 COVID-19로 이후 임상실습을 경험한 간호대학생을 대상으로 임상수행능력에 미치는 영향요인을 파악하여 임상수행능력 향상을 위한 기초자료제공을 위해 수행되었다. 본 연구는 G도 소재의 간호학과 2곳의 재학생 144명을 대상으로 2023년 10월 10일부터 10월 27일까지 자료수집하였다. 수집된 자료는 서술통계와 차이분석(t-est, on-way ANOVA), 상관관계, 위계적 회귀분석으로 분석하였다. 연구결과 모델 1에서는 성격유형 중 외향적과 혼합적이 임상수행능력을 설명하는 유의한 예측요인으로 나타났다. 모델 1의 적합도는 통계적으로 유의하였고, 설명력은 9.2%였다(F=8.256, p<.001). 모델 2에서는 대인관계능력과 간호술기자신감이 임상수행능력을 설명하는 유의한 예측요인으로 나타났다. 간호술기자신감이 임상수행능력을 가장 잘 예측하는 요인이었고, 그 다음으로 대인관계능력이었다. 모델의 설명력은 50.1%였고, 모델 1에 비해 41.3% 증가하였다. 모델적합도도 통계적으로 유의하였다. 직접간호 수행의 기회를 높이고 간호대학생의 간호역량을 향상시키기 위해 다양한 상황을 재현하는 시뮬레이션 교육이 강화되어야 한다. 시뮬레이션교육을 통해 간호술기자신감과 대인관계능력을 향상시키면 결국 임상수행능력이 향상되고 이는 간호사로 취업되었을 때 간호역량으로 발휘될 수 있을 것이다.

자기주도형 중환자간호 이러닝 프로그램 개발 및 적용 (Development and Implementation of a Self-directed Critical Care Nursing e-Learning Program)

  • Purpose: The purpose of this study was to develop a self-directed e-learning program for ICU nurses and to evaluate how the proposed e-learning program affects the level of knowledge, nursing performance and job satisfaction of ICU nurses. Methods: The e-learning program was developed with the Computer Assisted Instruction Design Model using sources of self-efficacy which included the inactive attainment, vicarious experience, and verbal persuasion of ICU nurses. The program was evaluated by experts. Following the revision of the program, it was applied to a total of 59 ICU nurses (experimental group n=29, control group n=30) from three hospitals. Four weeks later, we measured the level of knowledge, nursing performance and job satisfaction. Results: The level of knowledge significantly improved in the experimental group (t=5.691, p<.001). Moreover, the level of nursing performance significantly increased in the areas of circulatory diagnostic test (t=2.143, p=.039), EKG (t=2.911, p=.006), aortic balloon pump (t=2.491, p=.017), and nebulizer therapy (t=2.085, p=.044). Overall, job satisfaction did not significantly increase (t=1.664, p=.105); however, job satisfaction in terms of relationships with coworkers (t=2.371, p=.023) and the needs of nursing performance (t=2.940, p=.006) were significantly increased. Conclusion: The results showed that the proposed e-learning program using sources of self-efficacy from ICU nurses was an effective learning method to increase the level of knowledge and nursing performance. The propose de-learning program would be a useful teaching tool for ICU nurses improving the quality of ICU patient care.

간호과정 적용 평가도구 개발 -입원 및 퇴원 시 간호를 중심으로- (Development of Performance Measures Based on Nursing Process for Admission and Discharge Care)

  • Purpose: This study was conducted to develop standards ensuring nursing process-based care of patient admission and discharge, to develop a performance measurement tool evaluating the care applied according to the standards, and finally to determine validity of the standards and the tool. Methods: The standards and the tool were reviewed by a panel of experts and refined based on the panel's suggestions. Validity of the standards and the tool were examined through surveying a total of 302 hospital nurses. Results: The mean validity scores of the performance measurement standards and the tool were 4.11 and 4.09, respectively, out of 5.00. So the performance measurement standards and the tool in this study were found to be acceptable in evaluating quality of nursing care provided at patient admission and discharge. Conclusion: This result indicates that the performance measurement standards and the tool developed in this study are valid instruments to monitor and improve quality of nursing care for patient admission and discharge.

아동간호사의 말초정맥관 관련 교육 및 간호수행에 영향을 미치는 융합적 요인 (Convergence factors affecting on education and nursing performance related to peripheral intravenous catheterization among pediatric nurses)

  • 본 연구는 아동간호사의 말초정맥관 관련 간호교육프로그램의 기초자료를 얻기 위해 125명의 아동간호사를 대상으로 시행된 서술적 조사연구로, 최희강 외(2016)의 아동의 말초정맥주사관리 프로토콜과 간호교육평가원의 핵심 기본간호술 중 정맥주사프로토콜의 일부를 사용하여 수집된 자료를 SPSS 21.0으로 분석하였다. 아동간호사의 말초정맥관 관련 교육수행이 가장 낮았고, 부작용관리수행이 가장 높았다. 일반적특성에 따른 간호수행 하위영역에 부분적으로 유의한 차이가 있었다. 교육과 간호수행 간 정적 상관관계가 있었고(r=.486, p<.001), 간호수행에 영향을 미치는 요인은 교육(${\beta}=.472$), 근무부서(${\beta}=.216$) 및 결혼상태(${\beta}=.169$)이었다(adj $R^2=0.314$, p<.001). 본 연구결과는 대상자 수를 확대하여 반복 연구 후 아동간호사의 말초정맥관 관리 교육프로그램의 기초자료로 활용될 수 있다.

간호사의 유머감각, 자존감과 간호업무성과와의 관계 (Relationship among Sense of Humor, Self-esteem, and Performance in Nurses)

  • Purpose: The study was done to identify relations among sense of humor, self-esteem, and performance in nurses. Methods: Data were collected by questionnaires from 198 nurses in Seoul and Gyeonggi province. Measures were sense of humor, self-esteem, and performance. Data were analyzed using Pearson correlation coefficients, t-test, ANOVA, and stepwise multiple regression with SPSS/WIN 13.0 version. Results: This study shows that performance of nurses has significant difference according to age, marriage, education level, income and clinical experience. The performance of nurses has positive correlation with their sense of humor and self-esteem. The higher clinical experience, sense of humor, and self-esteem they have, the better performance they make. The factors that affect performance of nurses are clinical experience, sense of humor and self-esteem. The degree of explanation is 36.5%. Conclusion: Knowing that sense of humor and self-esteem of nurse are significant factors which can cause nurses' better performance, we suggest an education program that can help nurses to develop and manage their sense of humor.
