Giant cell tumor(GCT) occurs very unusually in the rib(less than 1% of GCT). We present the cytologic features of GCT of the rib. It showed multiple cellular clusters composed of characteristic, benign looking osteoclast-like multinucleated giant cells and fibroblast-like mononuclear cells. The multinucleated giant cells contained numerous nuclei (average, 30 to 40 per cell, which were closely packed. The nuclei in giant cells were remarkably uniform and round to oval. The mononuclear, neoplastic stromal cells were elongated and spindle-shaped. There was no cytologically malignant portion in the tumor.

An unusual case of papillary carcinoma of the thyroid revealing numerous multinucleated giant cells in the aspiration biopsy cytology is reported. Papillary carcinoma is the most common malignant neoplasm of the thyroid and is frequently diagnosed by aspiration biopsy cytology. Recently, we experienced a case of papillary carcinoma with many multinucleated giant cells in a 55-year-old woman. The cytologic features are described.

Malignant tumors of the breast with stromal multinucleated giant cells are rare entity of uncertain clinical significance. There have been few reports on the fine needle aspiration cytologic(FNAC) findings about these rare tumors. We report a FNAC case of invasive mammary carcinoma with osteoclast-like giant cells not only for its rare occurrence but in particular for its distinctive cytologic picture on aspirated material. The patient was a 40-year-old woman who presented with a right breast mass for one month. Mammography showed a well-demarcated rounded mass density without calcification. The aspirates of FNAC were highly cellular and two main cell types were seen; malignant epithelial cells and osteoclast-like multinucleated giant cells. The carcinoma cells occurred singly or arranged in loose clusters with ill-defined cytoplasm, oval nuclei, coarse chromatin and small but distinct nucleoli. The multinucleated giant cells showed variable number of nuclei with prominent nucleoli and abundant dense oxyphilic cytoplasm. The immunocytochemical studies suggested that osteoclast-like giant cells were not of epithelial origin, but rather of histlocytic origin.

Anaplastic carcinoma of the thyroid is one of the most malignant tumors and survival for longer than three years after diagnosis is exceptional. Multinucleated giant cells of osteoclastlike appearance are seen un some of the anaplastic carcinoma, but only three cases in which the diagnosis was made by fine needle aspiration (FNA) cytology are reported in the international literature. We experienced a case of anaplastic carcinoma with osteoclastlike giant cells in a 66-yr-old female, diagnosed by FNA cytology. The smears revealed two cell populations: multinucleated giant cells and large polygonal or spindle shaped malignant cells. The FNA cytodiagnosis of anaplastic thyroid carcinoma containing osteoclastlike giant cells was substantiated by subsequent biopsy.

Patients with rheumatoid arthritis of any degree of severity or duration may develop necrotizing granulomatous pleuritis, a morphologic replica of the inflammatory reaction characteristic of rheumatoid synovitis and rheumatoid nodules. The principal feature is the background composed of granular, amorphous, particulate material or debris of various hues. The material is sometimes eosinophilic sometimes more cyanophilic, or even green in the Papanicolaou stain. Within this background are elongated, fibroblast-like epithelioid cells, numerous multinucleated giant cells and degenerating leukocytes. The combination of the debris, spindle epithelioid ceils, and multinucleated giant cells in fluid is pathognomonic for rheumatoid pleuritis. We experienced a hcase of rheumatoid pleuritis showing these characteristic cytologic findings. The patient was a 63 year-old man with positive rheumatoid factor. The pleual fluid specimen revealed elongated epithelioid cells and multinucleated giant cells on a background of amorphous granular material.

A case of avian tuberculosis is reported in a 10-year-old female Mandarin duck (Aix galericulata). Numerous multi focal to coalescing, caseo-necrotic granulomas in variable sizes were scattered throughout the major parenchymal organs. Histologically, the granulomas have caseonecrotic and occasionally mineralized centers that are surrounded by macrophages, epithelioid cells and Langerhan's type multinucleated giant cells and then by lymphocytes and a few plasma cells and fibrosis. Ziehl - Neelsen's stain revealed myriads of acid-fast bacterial rods present in the cytoplasms of macrophages and multinucleated giant cells.

Osteoclast-like giant cell tumor of the liver is an extremely rare malignancy with poor prognosis. To our knowledge, 5 cases have been reported in English literatures, but there was no report about fine needle aspiration cytologic(FNAC) features. We experienced a case of osteoclast-like giant cell tumor of the liver obtained by computed tomography(CT)-guided FNAC and needle biopsy. The cytologic findings mimicked slant cell tumor of the bone. A large hepatic mass of the left lobe with abdominal wall invasion was found by CT in a 46- year-old female complaining of epigastric pain. The FNAC showed moderately cellular smears consisting of osteoclast-like giant cells and mononuclear cells, which were individually scattered or intermingled in clusters. The osteoclast-like giant cells had abundant cytoplasms and multiple small round nuclei with fine chromatin and distinct nucleoli. The mononuclear cells had moderate amount of cytoplasm and relatively bland-looking oval nuclei with single small nucleoli. All of the cytologic features recapitulated the histologic findings of bland-looking osteoclast-like multinucleated giant cells evenly dispersed throughout the background of mononuclear cell. The immunohistochemical study showed positive reaction for CD68 and vimentin, but negative for cytokeratin in both osteoclast-like slant cells and mononuclear cells.

A 5-year-old Pekinese dog was presented for evaluation of a back mass of 1 year duration. Fine needle aspiration cytology of the mass revealed numerous keratinocytes, and a lesser number of multinucleated giant cells along with a low number of spindle cells, which were suggestive of epidermal cyst or hair follicle tumor with secondary granulomatous inflammation. The mass was surgically removed and examined histologically. Microscopically there was partially encapsulated well-demarcated nodule in deep dermis and subcutis. The nodule consisted of central cyst and secondary cysts that were lined by a single or multiple layers of basaloid cells and squamous epitheliums with central laminated keratin. Multifocal aggregation of numerous macrophages and multinucleated giant cells were present. The final diagnosis was keratoacanthoma with granulomatous inflammation. The mass did not recur 5 months after surgery.

A case of enteric pythiosis in the jejunum is reported in a 28-month-old, male Jindo dog. Grossly, a 20cm segment of the cranial jejunal wall was markedly thickened to a thickness of 10cm and the lumen was narrowed. Histological examination of the jejunum reveals transmurally affected multifocal to coalescing granulomatous inflammation. The granulomatous foci were composed of a necrotic center with neutrophilic infiltration and occasionally non-parallel, branching, septate hyphae surrounded by numerous epithelioid macrophages, multinucleated giant cells, lymphocytes and fibrosis. Multinucleated giant cells occasionally contain fragment of hyphae. The diagnosis was based on light microscopical studies and positive PAS and immunostaining for Pythium insidiosum. This is believed to be the first reported case of canine enteric pythiosis in Korea.

Multinucleated giant cells of osteoclast-like appearance can be seen in a type of anaplastic carcinoma of thyroid and only a few case reports for fine needle aspiration cytologic findings are found in the literatures. Recently, we experienced a case of anaplastic carcinoma of thyroid with many osteoclast-like giant cells in a 72-year-old woman. The cytologic features and immunohistochemical results are described with special emphasis on differential diagnosis.
