A Study on Impact of Nursing Work Environment, Interpersonal Skills, and Grit on Field Adaptation of Nurses Who Have Experienced Department Transfers (부서이동을 경험한 간호사의 간호근무환경, 대인관계능력, 그릿이 현장적응에 미치는 영향)

  • Purpose : This study aims to identify the factors influencing field adaptation among nurses who have undergone department transfers, by examining the levels of and the relationships between their nursing work environment, interpersonal skills, grit, and field adaptation. Methods : The study involved 137 nurses who had experienced at least one department transfer within the last three years across two hospitals. Data analysis was performed using SPSS/WIN 29.0, involving descriptive statistics, means, standard deviations, independent t-tests, one-way ANOVA, Scheffé post-hoc tests, Pearson correlation coefficients, and multiple regression analysis (enter method). Results : Factors affecting field adaptation included the nursing work environment (β=.37, p<.001), interpersonal skills (β=.19, p=.021) and grit (β=.18, p=.025), with a total explanatory power of 31% (F=12.16, p<.001). Conclusion : This study contributes insights into effective nursing personnel management, offers foundational data for managing department transfers, and aims to improve the overall quality of nursing care.

Hydrochemistry and Noble Gas Origin of Various Hot Spring Waters from the Eastern area in South Korea (동해안지역 온천유형별 수리화학적 특성 및 영족기체 기원)

  • The purpose of this study is to characterize the hydrogeochemical characteristics of hot spring waters and to interpret the source of noble gases and the geochemical environment of the hot spring waters distributed along the eastern area of the Korean peninsula. For this purpose, We carried out the chemical, stable isotopic and noble gas isotopic analyses for eleven hot spring water and fourteen hot spring gas samples collected from six hot spring sites. The hot spring waters except the Osaek hot spring water show the pH range of 7.0 to 9.1. However, the Osaek $CO_2$-rich hot spring water shows a weak acid of pH 5.7. The temperature of hot spring waters in the study area ranges from $25.7^{\circ}C$ to $68.3^{\circ}C$. Electrical conductivity of hot spring waters varies widely from 202 to $7,130{\mu}S/cm$. High electrical conductivity (av., $3,890{\mu}S/sm$) by high Na and Cl contents of the Haeundae and the Dongrae hot spring waters indicates that the hot spring waters were mixed with seawater in the subsurface thermal system. The type of hot springs in the viewpoint of dissolved components can be grouped into three types: (1) alkaline Na-$HCO_3$ type including sulfur gas of the Osaek, Baekam, Dukgu and Chuksan hot springs, and (2) saline Na-Cl type of the Haeundae and Dongrae hot springs, and (3) weak acid $CO_2$-rich Na-$HCO_3$ type of Osaek hot spring. Tritium ratios of the Haeundae and the Dongrae hot springs indicate different residence time in their aquifers of older water of $0.0{\sim}0.3$ TU and younger water of $5.9{\sim}8.8$ TU. The ${\delta}^{18}O$ and ${\delta}D$ values of hot spring waters indicate that they originate from the meteoric water, and that the values also reflect a latitude effect according to their locations. $^3He/^4He$ ratios of the hot spring waters except Osaek $CO_2$-rich hot spring water range from $0.1{\times}10^{-6}$ to $1.1{\times}10^{-6}$ which are plotted above the mixing line between air and crustal components. It means that the He gas in hot spring waters was originated mainly from atmosphere and crust sources, and partly from mantle sources. The Osaek $CO_2$-rich hot spring water shows $3.3{\times}10^{-6}$ in $^3He/^4He$ ratio that is 2.4 times higher than those of atmosphere. It provides clearly a helium source from the deep mantle. $^{40}Ar/^{36}Ar$ ratios of hot spring water are in the range of an atmosphere source.

Lamellar Body Counts in Fetal Rabbits' Experimental Diaphragmatic Hernia and Tracheal Ligation (기도결찰을 시행한 토끼 태자의 실험적 횡격막탈장에서 Lamellar body counts)

  • Experimental tracheal ligation (TL) has been shown to reverse the pulmonary hypoplasia associated with congenital diaphragmatic hernia (CDH) and to normalize gas exchange. The purpose of this study was to determine whether the TL would correct the surfactant deficiency present in the fetal rabbit model of CDH by using lamellar body count. Lamellar bodies are synthesized and secreted by the type II pneumocytes of fetal lung. The phospholipids present in these bodies constitute the major component of pulmonary surfactant. Twenty-one pregnant New Zealand rabbits underwent hysterotomy and fetal surgery on gestational day 24. Two fetuses of each pregnant rabbit were operated. In the fetus of one end of bicornuate uterus, left DH was created by excision of fetal diaphragm through open thoracotomy (DH Group). In the fetus of the other end of bicornuate uterus, left DH and TL were created (TL Group). The fetuses were delivered by Cesarean section on gestational day 31. Fourteen in control group, 12 in the DH group and 13 in TL group were born alive. En bloc excision of lungs, bronchi and trachea was done in all newborn rabbits. A five Fr catheter was inserted through trachea and repeated irrigations with 10 cc normal saline were done. The irrigated fluid was centrifuged at $280{\times}g$ for 5 minutes and the lamellar bodies were counted with the upper level fluid in platelet channel of electronic cell counter. The average lamellar body counts were $37.1{\pm}14.2{\times}10^3/{\mu}L$ in control group, $11.5{\pm}4.4 {\times}10^3/{\mu}L$ in DH group, and $6.5{\pm}0.9{\times}10^3/{\mu}L$ in TL group. Lamellar body count in DH group was lower than in control group and did not increase after TL. This study shows TL has no therapeutic effect on decreased surfactant level of CDH and the pregnant rabbit is appropriate for the animal model of CDH.

Influence of the Duration of Smartphone Usage on Flexion Angles of the Cervical and Lumbar Spine and on Reposition Error in the Cervical Spine (스마트폰 사용시간이 목뼈 및 허리뼈의 굽힘각도와 목뼈의 재현오차에 미치는 영향)

  • The purpose of this study was to assess the influence of the duration of smartphone usage on cervical and lumbar spine flexion angles and reposition error in the cervical spine. The study included 18 healthy smartphone users (7 males and 11 females). We measured the kinematics of the upper and lower cervical and lumbar spine flexion angles and the reposition error of the upper and lower cervical spine after 3 s and 300 s smartphone use in sitting. A paired t-test was used to compare the effects of the duration of smartphone usage on the kinematics of cervical and lumbar spine flexion angles and reposition error. The flexion angles of the lower cervical and lumbar spine and the reposition error in the upper and lower cervical spine were significantly increased after 300 s smartphone of use (p<.05). However, the flexion angle of the upper cervical spine was not significantly different between the 3 s and 300 s smartphone of use (p>.05). These findings suggest that prolonged use of smartphones can induce changes in cervical and lumbar spine posture and proprioception in the cervical spine.

Effects of Axillary Crutch Length on EMG Activity of the Trunk Muscles and Range of Motion of the Lumbar Spine, Pelvis, and Hip Joint in Healthy Men

  • This study investigated the effects of axillary crutch length on trunk muscle activity and lumbo-pelvic-hip complex movements during crutch gait. Eleven healthy men participated in this study. The participants performed a three-point gait with optimal, shorter, and longer crutch lengths. Weight-bearing (WB) side was determined as the dominant leg side. The electromyography (EMG) activity of the bilateral rectus abdominis (RA) and erector spinae (ES) muscles and lumbo-pelvic-hip complex movements were monitored using a three-dimensional motion system with wireless surface EMG. Differences in the EMG activity of RA and ES muscles and range of motion (ROM) of lumbar spine, pelvis, and hip among conditions were analyzed using one-way repeated-measures analysis of variance, and a Bonferroni correction was conducted. There was less RA muscle activity on the WB side under the optimal crutch length condition compared with shorter and longer crutch length conditions (p<.05). The EMG activity of the RA muscle on the non-WB side and ES muscle on the WB side were significantly decreased under the optimal crutch length condition compared with shorter crutch length condition (p<.05). No significant differences in the EMG activity of the ES muscle on the non-WB side and ROM of lumbo-pelvic-hip complex were found among conditions (p>.05). These findings indicate that the optimal crutch length improves the trunk muscle efficiency during crutch gait.

Evaluation of Photoneutron During Radiation Therapy when Using Flattening Filter and Tracking Jaw with High Energy X-ray (고 에너지 X선 방사선치료 시 Flattening Filter와 Tracking Jaw 사용에 따른 광중성자 발생 평가)

  • Radiation therapy is usually using linear accelerator and used X-ray energy is also getting higher. Recently linear accelerators has been developed 3F mode and tracking jaw technology and that was applied for patient therapy. This study aims at measuring photoneutrons depending on the use of 3F and tracking jaw system when radiation is irradiated using a linear accelerator. The generation of photoneutrons of 3F system was 70% smaller than 2F system and that of tracking jaw system was 83% higher than static jaw system. Photoneutron value is relatively low. However, it must be minimized for Photoneutron exposure during radiation therapy.

  • The purpose of this study is to suggest improvement ways of outpatient process via a simulation model and to improve operational efficiency. Three experimentation scenarios were implemented into the simulation model to determine which proposed scenario provides better improvement in terms of the following performance measures: LOS(Length of Stay), patient waiting time, patient travel time, and staff utilization. The hospital medical data collection and statistical tools used to analyze the process mining tools. And the PIOS simulation tool was used and the validity of the model was verified by using t-test. The simulation results demonstrated that oupatient process of center type is most efficient. Simulation approach is a powerful technique that supports efficient decision-making compared to traditional healthcare management approach based on past experience, feelings, and intuition. Therefore, the proposed experimentation model has wide applicability in healthcare systems.

Observation of the Sea Surface Skin Current Using a GPS-Drifter (GPS 뜰개를 이용한 해양 표면류 관측)

  • A GPS-drifter was newly designed to observe the sea surface skin current and to estimate the direct wind effect on the sea surface. After conducting a test to establish and verify the accuracy of the GPS itself in the laboratory, in-situ experimental campaigns at Saemangeum in Gunsan city and Haeundae in Busan city, Korea, were carried out to ascertain the drifter track and to estimate the velocity data set on Oct. 3, 15, 23, 27 and Nov. 25, 2011. The current meters, RCM9 and ADCP, were moored together to remove the background current field, and the wind data were obtained from several marine stations such as towers and buoys in these areas. The drifter-observed velocity show good agreement with the flow obtained by the HF radar in the Saemangeum area. The direction of the wind-driven current extracted from the drifter-observed velocity was completely deflected to the right, however the degree of the angle was different according to the drift types. The average speed of the wind-driven current matched with 2.19~2.81% of the wind speed and the deflection angle was about $8.0{\sim}10.9^{\circ}$ without adjustment for the land-sea effect, and about 2.19~2.84% and $4.1{\sim}6.0^{\circ}$ with the adjustment for the land-sea effect.

Comparison of Perceived Nurses' Cultural Competence of Nurses and Foreign Patients (간호사의 문화적 역량에 대한 외국인 환자와 병원 간호사의 인식 비교)

  • Purpose: This study aimed to examine the differences in Korean nurses' cultural competence perceived by nurses and foreign patients in a general hospital. Methods: The study participants were recruited from outpatient clinics and wards in H General Hospital located in Busan. Cultural competence was measured by the Cultural Competence Scale for Korean Nurses, consisting of 4 subscales: cultural perception, cultural knowledge, cultural sensitivity, and cultural skills. Data were collected from March 31 to April 30, 2014 by distributing and collecting a self-administrated questionnaire set. Data of the 90 foreign patients and 90 nurses were analyzed using PASW statistics 18.0 for independent t-test and ANOVA. Results: The level of cultural competence of Korean nurses perceived by foreign patients was significantly higher than that of nurses. On the cultural awareness dimension, nurses rated themselves more highly than foreign patients did while ratings on the dimensions of cultural knowledge and cultural skill were the opposite. Conclusion: This study indicates that there was a statistically significant difference in the perception of nurses' cultural competence between nurses and foreign patients. The findings can be used for developing education programs to enhance nurses' cultural competence, integrating the perspectives of foreign patients.

Can Urinary Cotinine Predict Nicotine Dependence Level in Smokers?

  • Background: Although nicotine dependence plays a role as a main barrier for smoking cessation, there is still a lack of solid evidence on the validity of biomarkers to determine nicotine dependence in clinical settings. This study aimed to investigate whether urinary cotinine levels could reflect the severity of nicotine dependence in active smokers. Materials and Methods: Data regarding general characteristics and smoking status was collected using a self-administered smoking questionnaire. The Fagerstr$\ddot{o}$m test for nicotine dependence (FTND) was used to determine nicotine dependence of the participants, and a total of 381 participants were classified into 3 groups of nicotine dependence: low (n=205, 53.8%), moderate (n=127, 33.3%), and high dependence groups (n=49, 12.9%). Stepwise multiple linear regression model and receiver operating characteristic (ROC) curves analyses were used to determine the validity of urinary cotinine for high nicotine dependence. Results: In correlation analysis, urinary cotinine levels increased with FTND score (r=0.567, P<0.001). ROC curves analysis showed that urinary cotinine levels predicted the high-dependence group with reasonable accuracy (optimal cut-off value=1,000 ng/mL; AUC=0.82; P<0.001; sensitivity=71.4%; specificity=74.4%). In stepwise multiple regression analysis, the total smoking period (${\beta}$=0.042, P=0.001) and urinary cotinine levels (${\beta}$=0.234, P<0.001) were positively associated with nicotine dependence, whereas an inverse association was observed between highest education levels (>16 years) and nicotine dependence (${\beta}$=-0.573, P=0.034). Conclusions: The results of this study support the validity of using urinary cotinine levels for assessment of nicotine dependence in active smokers.
