The Comparison Study of Ankle Joint Mobilization and Elongation on the Difference of Weight-bearing Load, Low Back Pain and Flexibility in Flat-foots Subjects (편평족에 대한 관절가동술과 자가신장이 요부의 신장성, 요통과 하지 체중부하 차이에 대한 비교 연구)

  • Background: The purpose of this study was to investigate the relationship between the spine and the flat-foot, the stability and the alignment of the posture of the neck to prevent the alignment of the ankle joint operation and the lower back flexibility of the lumbar region according to the type of treatment using active stretching of the triceps, back pain, and to see how they affect weight bearing differences. Methods: The subjects of this study were 24 chronic low back pain patients. They were randomly divided into experimental group and control group. In the experimental group, ankle joint mobilization and active scraping of triceps were performed three times a week for a total of 6 weeks. The control group was performed in the same way without articulation. The range of flexion and extension motion of the lumbar spine and pain degree and difference of weight-bearing were measured before and after the experiment. Results: The model of ankle joint mobilization and calf muscle elongation of flat foot significantly improved the range of flexion and extension motion of the vertebrae (p<.05) and the VAS and distribution of weight-bearing were decreased in both of two groups (p<.05). In other words, the exercise and mobilization help to recover of the balance of the whole musculoskeletal, the vertebrae. Conclusions: The active exercise of the triceps muscle of the lower leg in this study It affects the flexibility of the lumbar spine, the pain and the difference in the weight support of the lower extremities, when we performed ankle joint mobilization for exercise and cramping, pain and the difference in weight support between the two lower limbs.

Effect of Rehabilitation Exercise on Lumbar Muscle Function and Pain Relife for the Middle-Aged Women with Low Back Pain (중년여성 요통환자들의 재활운동이 요부 근기능과 통증완화에 미치는 영향)

  • This study is aimed at determining the effects of rehabilitation training on lumbar extension strength and relief of back pain in middle-aged women of low back pain. Twenty-nine subjects(total 29 people; CLBP 16, HLD 13) were trained twice per week for eight weeks and completed a maximum isometric test at various flexion angle(the degree of $0^{\circ}$, $12^{\circ}$, $24^{\circ}$, $36^{\icrc}$, $48^{\circ}$, $60^{\circ}$, $72^{\circ}$) by lumbar extension machine. The result showed that. 1. Patient group of CLBP were increased the maximum lumbar extension strength at a range of 7 flexion angles after rehabilitation rather than no rehabilitation(on the average 60.75%). The operated patient group in HLD also showed an increase of 56.55%. In view of these cases, all of two groups showed a significant increase of muscle strength(p<.05). But there is no difference between pre-exercise and post-exercise groups. 2. Patient group of CLBP were increased higher rate than 41% for maximum lumbar extension strength(91.79% at $0^{\circ}$, 79.41% at $12^{\circ}$, 65.89% at $24^{\circ}$) at all angles after 8 weeks training. Both groups indicated a significant increase(p<.05) of lumbar extension strength at all degrees. There is no difference between pre-exercise and post-exercise groups. 3. Relief of back pain in Patient group of CLBP showed a decrease of average 105.2% and patient group of HLD indicated a decrease of average 64.57% two groups showed a significant reduction (p<.05, p<.05). But in case of a decrease of pain, CLBP group got 3.44 points and HLD group got 4.77 points. In view of these results, two groups showed remarkable reduction of back pain, however HLD group had residual pain relatively.

Treatment of Iatrogenic First Metatarsal Dorsiflexion Deformity After Hallux Valgus Surgery -A Case Report- (무지 외반증 환자의 근위부 절골술 후 발생한 의인성 제 1 중족골의 족배 굴곡증의 치료 -1예 보고-)

  • Purpose: We report a case that iatrogenic dorsiflexion deformity after hallux valgus surgery treated successfully with crescenteric plantar flexion metatarsal osteotomy. Materials and Methods: 43 years old female who suffered from left fore foot pain and deformity after hallux valgus surgery was evaluated. Results: Preoperatively she did not put on ordinary shoes and had had persistent pain and discomfort on 1st metatarsal area. She also had a callus on plantar surface of 2nd metatarsal head. Simple AP and Lateral x-ray identified that 1st metatarsal bone had a 23 degree dorsiflexion deformity. For correction of deformity, plantarflexion crescenteric osteotomy was performed on proximal 1st metatarsal area. After operation, All of symptom eliciting patient was gone and 43 points of AOFAS scale preoperatively improve 100 points and the patient very satisfied. Post operative x-ray was showing complete correction of deformity. Conclusion: As a treatment of iatrogenic dorsiflexion deformity after hallux valgus surgery, the crescenteric plantar flexion osteotomy can be good and safe modality for correction.

The Effect of 12 week Isotonic Exercise to Change Stabilization Ratio of Elderly Female Low Back Pain Patients (12주 등장성 운동이 여성노인 요통환자의 요부 안정화 비율 변화에 미치는 영향)

  • This study is that the elderly female patients having low back pain(34 people) show a change of stabilization ratio after isotonic rehabilitation of 4 types(before exercise, after 4weeks, 8weeks, 12weeks). Under the cover of lumbar extension machine, isometric extension strength and stabilization ratio is measured and analyzed at some flexion angles(the degree of $0^{\circ}$, $12^{\circ}$, $24^{\circ}$, $36^{\circ}$, $48^{\circ}$, $60^{\circ}$, $72^{\circ}$). The conclusion is as follows. 1. The maximum lumbar extension strength at a range of 7 angles have an increase of 73.92% in case of 12weeks isotonic exercise(p<.001). 2. After 12weeks exercise, the lumbar flexion angle maximum extension strength, have an higher increase than 54% on the average at all angles(p<.001). These statistically show a meaningful increase of muscular strength. 3. After 12weeks isotonic exercise, the lumbar stabilization ratio have a decrease of 50.27% at a statistically meaningful level(p<.001). In case of 12weeks exercise comparing with 8weeks, the stabilization ratio decrease at the level of 1.85 versus 1. This figure is similar to that of a normal person. In view of this study, 12weeks exercise for the elderly female patients having low back pain has much influence on the change of lumbar stabilization ratio and this is the scientifically verified result of a long term exercise.

The Effects of Transcranial Direct Current Stimulation in Motor Performance of Serial Reaction Time Task (경두개직류전류자극이 시열반응과제에 대한 운동 수행 능력에 미치는 영향)

  • Purpose: The aim of this study is to investigate whether motor cortex excitability by transcranial direct current stimulation (tDCS) over primary motor cortex (M1) affects motor performance of serial reaction task. Methods: Cathodal, anodal and sham tDCS (1 mA) are applied over right M1 of 24 subjects for 30 minutes including 11minutes for task period time. We applied two electrodes at the same position to both an experimental group and a sham-controlled group, and we made 2 groups recognize to be applicated of stimulation. Flexion, extension of wrist and thumb flexion are carried out following colors of arrows on the monitor. Serial reaction time task was applied to confirm the difference of the reaction time between 2 groups. Results: Reaction time is decreased in both tDCS-group and Sham-controlled tDCS group, and the degree of reduction is much greater in the post-test than pre-test. Reduction of reaction time between groupsis statistically significant. Conclusion: We consider that anodal tDCS increased the cortical excitability of the underlying motor cortex and it can be helpful to modulate motor performance. It seems that tDCS is an effective modality to modulate brain function, and it will be great help to mediate strategy for the brain injury patients.

Development of Exoskeleton-Type Data Glove for Position/Force Feedback (위치/힘 피드백이 가능한 외골격 구조의 데이터 글로브 개발)

  • In this paper, we present a new exoskeleton-type data glove that can sense the movement of the human finger and reflect the force to the finger. The data glove is designed on the basis of the skeletal structure of the human hand, and the finger module has 1 degree-of-freedom because it includes three four-bar mechanism joints in series and a wire-coupling mechanism. In addition, the transmission ratio of the finger module is maintained at 1:1.4:1 over the entire movement range, and hence, the module can perform both extension and flexion. In addition, to enable adduction/abduction motion of the human hand, a unique MCP joint is designed by using two universal joints. To validate the feasibility of the data glove, master-slave control experiments based on force-position control between the data glove and the robot hand are conducted.

  • Objective : The purpose of this study was to conduct a biomechanical analysis of a one-legged jump in a traditional Korean dance (Wae Bal Ddwigi) according to breathing method. Method : Participants for this study were 10 dancers with experience for at least 10 years in traditional Korean dance. Independent variables for this test were two different types of breathing methods. Dependent variables were ground reaction force and lower extremity kinematic variables. The jumping movement was divided into three separate stages, take off, flight, and landing. The subjects were asked a questionnaire regarding the degree of impact force and stability of landing posture after the experiment. The Kistler Force Plate (9281B, Switzerland) was used to measure ground reaction force. A digital camera was used to look into angles of each joint of the lower part of body. SPSS was used for statistical analysis via the dependent t-test(p<.05). Results : There were significant differences in jumping according to breathing method. The inhalation & exhalation method yielded significantly longer flight times combined with greater ground reaction force. The breath-holding method required more core flexion during landing, increasing movement at the hips and shoulders. Conclusion : Consequently, there was more flexion at the knee to compensate for this movement. As a result, landing time was significantly higher for breath-holding.

Quantification of Degree of Fatigue in Back Muscle and Its Influence on Back Injury Using Electromyography Measurement (근전도를 통한 척추 근육의 피로도의 정량화 및 척추손상에 미치는 영향 연구)

  • 본 연구는 몸통의 반복적인 flexion/extension 동작 수행시 근전도신호를 측정하고 이들 신호에서 나타난 EMG parameters(median frequency, median power, RMS) 와 피실험자의 주관적 불편도의 변화양상간의 정량적인 관계를 알아보고자 수행되었다. 피실험자별로 3-5회 정해진 업무를 수행하였고 주관적불편도와 근전도 신호를 측정하였다. 실험결과 반복횟수가 증가할수록 주관적 불편도는 증가하였으며 각 task 별로 EMG parameter와 불편도의 증감율 간의 회귀분석과 상관분석을 수행한 결과 실험과 EMG parameter에 따른 변이는 존재했지만 대체로 RMS에 비해 MF와 MP의 증감율이 불편도 증감율과 상관관계가 높은 것으로 나타났다.

Gait Analysis of Bilateral Lower Limb Amputee with Incline Training on Treadmill (트레드밀에서 경사 훈련을 실시한 양쪽 하지절단환자의 보행분석)

  • The purpose of this report was to describe the gait pattern and parameters of the complicated bilateral amputee with right transtibial and left tarsometatarsal amputation. Using a Vicon 370 three dimensional gait analysis system, the gait analysis was performed at pre and post-test. Treadmill Training with 15 degree, incline was practiced for 8weeks, 3times per week. In linear parameters, the Velocity, Stride length and Single limb support were increased than pre-test. but Cadence and Double limb support were less post-test than pre-test. In kinematics, the maximal pelvic tilt angle showed right side $21.87^{\circ}$, left side $20.67^{\circ}$ at pre-swing phase, and decreased as compared with pre-test. Especially, the inimal hip flexion angle showed right side $-6.83^{\circ}$, left side $1.52^{\circ}$ at pre-swing phase and increased as compared with pre-test. The maximal knee flexion angle disclosed right side $2.66^{\circ}$, left side $21.71^{\circ}$ at stance phase, and decreased as compared with pre-test. In kinetics, the hip extension moment on initial contact stage was right side 0.938NM/Kg, left side 0.09NM/Kg, which was impaired compared with normal person.

The Effect of Badminton Shoe Forefoot Flexibility during the Under Clear Quick Lunge from a Jump Smashing (배드민턴화의 굴곡성(Flexibility) 차이가 점프 스매싱 후 언더클리어 동작시 하지에 미치는 영향)

  • The purpose of this study was to investigate the effect that difference in forefoot of shoe flexibility during the quick lunge from a jump smashing on the lower limbs and the plantar pressure distribution. For this 10 elite badminton players with over 10 years experience and right handed participated. Two kinds of badminton shoes were selected and tested mechanical testing for the forefoot flexibility. Motion analysis, ground reaction forces and plantar pressure distribution were recorded. It was required to conduct lunge movement after jumping smashing as possible as high. Photo sensor was located in 3 meter away from standing position and its height was 40 cm. Subjects were conducted to return original position after touching the sensor as under clear movement as possible as fast. Forefoot stiffness had an effect on shoe peak bending degree and peak bending angular velocity in propulsion phase. Forefoot flexibility had an effect on ankle plantar flexion and knee flexion moment. It appears that joint power on lower limb and peak plantar pressure were not influenced by the flexibility of shoes.
