TREATMENT OF SELF-INJURIOUS LIP BITING WITH POSTERIOR BITE BLOCK APPLIANCE IN A BRAIN LESION PATIENT (교합 거상판을 이용한 뇌병변장애 환아의 자해성 구강 손상의 치료)

  • Brain disorder disability is assessed when organic brain lesion such as cerebral palsy, traumatic brain injury, or stroke causes physical impairment which limits daily activites substantially according to its level and range of paralysis or the presence of involuntary movement. According to the disabled person welfare law in Korea, grade I brain disorder is assessed when one is in continuous irreversible state of coma without the ability to prolong one's life without other's help. Self-injurious behavior is defined as the non-suicidal intentional or unintentional injury to one's own body which can occur repetitively temporarily or chronically. People with decreased consciousness often exhibit increased self-injurious behavior which is most commonly associated with tongue or lip biting. This report documents a successful self -injurious lip biting treatment of a brain lesion patient within a short time by applying a removable acrylic resin appliance including posterior bite block.

An Exploratory Study on Decisions by foster Parents to care for Children with Special Needs : The Case of 'H' Foster Agency (특수욕구를 가진 아동에 대한 일반위탁부모의 위탁의사: 'H위탁보호기관'의 사례를 중심으로 한 탐색적 연구)

  • It is generally blown that foster children often have special needs such as disability and psycho-behavioral problems. It is also known that foster parents will provide better care with those children when they are coping well with foster children with special needs. The purpose of the study was to explore the perceptions foster parents have and what factors affect their decisions to care for children with special needs. Literature review reveals that successful foster parent care is related to the foster parents' motivation, role perception, and social support systems. foster parents' motivation is related to 'altruism toward foster child', 'the egocentrism of the foster parent', 'previous experience with adoption' and 'the past experiences of the foster parent'. Role perceptions of the foster parent consist of 'supporting the internal system of the foster child' and 'supporting the external system of the foster child'. The social support system is characterized as the limited notions of a family oriented concept. In addition, a foster parent's decision to look after special-needs children is related to their motivation while the decision to foster disabled children is related to the levels of available social support systems as well as motivation. The results of the study implies that the motivation level and that of social support system are closely related to different aspects of the special-needs foster child; this factor should also be considered when recruiting foster parents or placing special needs children.

Effects of Interactive Metronome Training on the Plantar Pressure and Fall Efficacy in Chronic Stroke Patients (상호교환식 메트로놈 훈련이 만성 뇌졸중 환자의 족저압과 낙상효능감에 미치는 영향)

  • PURPOSE: This study examined the effects of Interactive Metronome training on the plantar pressure and fall efficacy in chronic stroke patients. METHODS: Twenty-two hemiplegic patients were allocated randomly to an experimental group and control group. The experimental group received conventional physical therapy and emphasized weight-bearing interactive metronome training, whereas the control group received conventional physical therapy. The training was performed three times per week, 40 minutes per each session, for a total of seven weeks. The plantar pressure was assessed using the contact area and contact pressure, whereas the fall efficacy was assessed using the FES (Fall Efficacy Scale), ABC (Activities-specific Balance Confidence scale) and FOFQ (Fear of Falling Questionnaire). RESULTS: After training, a significant increase was observed in the paretic side of the contact area and the paretic and non-paretic side of contact pressure in both groups (p<.05). The between-group differences in the changes before and after training were statistically significant in the paretic side of the contact pressure (p<.05). After training, both the FES of the between-group and ABC of the experimental group were increased significantly (p<.05), but the between-group differences in the changes before and after training were not statistically significant in the FES, ABC, and FOFQ (p>.05). CONCLUSION: Interactive Metronome training is considered an effective treatment for improving the contact pressure of the paretic side in chronic stroke patients.

Study of Life History of Elderly Women who had Six Times of Imprisonment (여섯 번의 수감 생활을 한 여성 노인의 생애사 재구성)

  • This study was to explore the life history of an elderly woman who had six times of imprisonment and entered a Samchung re-education camp. This study of life history followed the analysis of Mandelbaum(1973) pointing three perspectives of life: dimensions, turnings, and adaptations. Participant's dimensions of life were exploitation of labor, hostess life for U.S. military, prison life, Samchung re-education camp, marriage with the disabled, life of a farm worker. Turnings of life were serving as a maid, confinement of prison, life of hostess for living, being remanded to Samchung re-education camp by state violence, marriage and divorce, denial of social welfare service. Adaptations of life were downright adaptation in early life, exaggerated act in juvenile reformatory, prostituted women as a simple fortune-maker, adaption as a good wife and wise mother after marriage, resistive adaption as a self-employed. and farm worker. Based upon this results outcome, discussions and implications were suggested.

Anodal Effects of Transcranial Direct Current Stimulation on the Excitability of Central Neuron (양극 경두개 직류 전기 자극이 중추신경원의 흥분성에 미치는 영향)

  • Purpose : This study is to examine the effects of transcranial direct current stimulation on the excitability of the central neuron. Methods : This study selected 24 suitable women in their twenties. A positive electrode of transcranial direct current stimulation was placed on the primary motor area (M1) C4 and a negative electrode was placed on the left supraobital. A stimulation of 0.04mA/$cm^2$ was applied for 20 minutes. H-reflex and V wave used diagnostic electromyography. An active electrode was placed at the muscle belly of the medial gastrocnemius muscle at a prone posture. An electrical stimulation was given to the posterior tibial nerve. Measurements were made before and after the stimulation. All data were analyzed with SPSS 12.0 and between each measuring before and after the change of the H-reflex and V wave amplitude. Results : There were no significant differences in all H wave, M wave, and V wave amplitude before and after transcranial direct current stimulation. There were no significant differences in the change of H/M ratio and V/M ratio before and after transcranial direct current stimulation. Conclusion : We know that transcranial direct current stimulation cannot have an influence on a normal grown-up person's central neuron.

A Study on Robot Arm Control System using Detection of Foot Movement (발 움직임 검출을 통한 로봇 팔 제어에 관한 연구)

  • The system for controlling the robotic arm through the foot motion detection was implemented for the disabled who not free to use of the arm. In order to get an image on foot movement, two cameras were setup in front of both foot. After defining multiple regions of interest by using LabView-based Vision Assistant from acquired images, we could detect foot movement based on left/right and up/down edge detection within the left/right image area. After transferring control data which was obtained according to left/right and up/down edge detection numbers from two foot images of left/right sides through serial communication, control system was implemented to control 6-joint robotic arm into up/down and left/right direction by foot. As a result of experiment, we was able to get within 0.5 second reaction time and operational recognition rate of more 88%.

A SURVEY OF PATIENTS WITH DISABILITIES VISITING THE DEPARTMENT OF PEDIATRIC DENTISTRY OF CHONBUK NATIONAL UNIVERSITY DENTAL HOSPITAL IN 2016 (2016년 전북대학교 치과병원 소아치과를 방문한 장애인환자에 관한 조사연구)

  • According to the Ministry of Health and Welfare, the portion of physical disability was 51.4% which was the highest among the registered disabled in December 2015, followed by visual disability(10.1%), disability of the brain lesions(10.0%), hearing disability(10.0%). The aim of this study is to investigate the types of disability in patients with disabilities visiting the Pediatric Dentistry, Chonbuk National University Dental Hospital in 2016. Of the patients who visited the pediatric dentistry, 196 patients (7.5%) had disabilities, and 19 patients had two or more disabilities. Twelve patients (6.1%) had both disabilities of brain lesion and intellectual disorder. Of the 177 patients with only one disability, 54 patients (30.5%) had physical disorders and 123 patients (69.4%) had psychiatric disorders. In psychiatric disorders, intellectual disorder was the most with 98 patients (55.3%), followed by autistic disorder with 23 patients (12.9%). In physical disorders, disability of brain lesion was the most with 37 patients (20.9%), followed by speech disability with 8 patients (4.5%). In this study, as a result of analyzing the types of disability with the patients who visited the Pediatric Dentistry of Chonbuk National University Dental Hospital, most of them had either intellectual disorder, disability of brain lesion, or autistic disorder. Dentists need to understand their general and dental characteristics to treat children and adolescents with disabilities. Since the most chief complaint of the patients was dental caries, early interventions should be made by dentist to reduce the incidence of the disease.

A Study on the Improvement of Accessibility Guidelines for Korean Mobile Applications (한국형 모바일 애플리케이션 접근성 지침 개선 방안 연구)

  • In this study, we propose a method to improve the guideline for accessibility of mobile applications based on the results of manual evaluation of 8 mobile online shopping applications which are widely used in Korea. 13 items of mobile online shopping application were found to have low compliance rate, and 5 items such as response time control were observed and there was no corresponding items. In the current mobile application accessibility guidelines, it is necessary to delete the check items that are not relevant or meaningless, and to provide additional logical items such as logical ordering and form creation, and to improve the accessibility of mobile applications, The government should make efforts to develop and disseminate the development guide, and to resolve the digital devides.

Accessibility Analysis of Mobile Applications for Open Platforms (개방 플랫폼용 모바일 애플리케이션 접근성 분석)

  • In this study, we analyzed the accessibility of open platform Android mobile banking applications, based on Korean mobile application accessibility KS standard, Section 508 technical standard of Rehabilitation Act, and European BBC guidelines. Experimental result showed that there are no mobile banking applications of 8 commercial banks in Korea that satisfy all guidelines of three standards. Typical violations included missing alternative text, keyboard focus violations, control size and spacing non-compliance, and low contrast ratio. These violations are fatal in that they make the accessibility of the blind, the disabled and the low vision and the elderly impossible. The reason that mobile banking applications do not comply with accessibility is that mobile application developers and providers have low awareness of accessibility and do not know how to implement accessibility properly. Comparing Korea mobile application accessibility guidelines with the revised standard of the Section 508 of Rehabilitation Act and the BBC standard, many guidelines are missing. Also, evaluation criteria are ambiguous and abstract, making it difficult for developers to refer specifically. Therefore, improving mobile application accessibility requires developer and government efforts and complementation of standards.

A Effectiveness Study of Public Funds Assistive Technology Delivery System Through the Professional Assistive Technology Center (공적급여 보조기구 전달체계에서 전문보조기구센터의 실효성 연구 : 광주광역시 보조기구센터의 장애인보조기구 교부사업 중심으로)

  • In this study, we present how our assistive technology service worked in the public funds delivery system through the center of professional assistive technology in Gwangju. In detail, among total 253 applicants for assistive device support business for disabled, 28 clients and 28 clients failed to get the support because of self-withdrawl and insuitability, respectively. The left 225 clients finally got the chance to use the assistive device; especially, 28 clients will get the alternative device which was recommended through the assistive technology professionals. Therefore, this work is believed to be a good example of how mediation of professionals in assistive technology center leads to the efficient use of public funds.
