A Survey on the Use of Visiting Nurses in Child Day Care Centers (지역사회 보육시설에서의 방문간호사 활용을 위한 기초조사연구)

  • Purpose: The purpose of this study was to contribute children's health care and health promotion by surveying the use of visiting nurses in child day care centers. Methods: This study is a descriptive study, and the study period covers July to September, 2010. The research subjects were 27 public health center managers, 166 visiting nurses and 137child care teachers. Results: Teachers' need of visiting health care services and visiting nurses' work performance of were statistically significantly different. The teachers' need of visiting health care was higher in all areas (health examination, health life practice, infectious disease control, safety accidents and disaster management, emergency measures and linkage, nutrition, parent education) but the visiting nurses had a low level of work performance. Conclusion: Child care teachers are not health professionals for child health care, and therefore they are in need of professional help. Thus, for the current public health centers in need of customized visit health care, new visiting nurses in charge of professional child health care need to be developed. Also, new models need to be developed for visiting nurses and child care teachers through the connection of community child health care.

Estimating the Cost of Visiting Nursing Service by Visiting Nursing Model for Urban Public Health Center in Korea (우리나라 대도시 보건소 방문간호 사업유형별 방문간호원가 및 소요재정 추계 분석)

  • Purpose: This study focused on analysing costs per visiting nursing care based on nursing activities in a public health center. Method: The Easley-Storfjell Instrument(1997) was used for a prospective descriptive analysis of self-records for workload data from 10 visiting nurses during 4 weeks on all nursing activities. In addition, analysis of the 478 visiting nursing records and cost data from 5 home visiting departments in public health centers during one year of 2003 was done. Result: The workload of visiting nurses by the type of model was identified as follows: Type I showed that caseloads made up 32.9 % of all nurse activities, and type II showed that the caseloads made up 45.8 %. Second, The cost per visit in type I was 33,088 won and 31,323 won in type II. Third, the estimated budgets were 1,902,436 won to 12,057,696 won for the type I model. and 4,151,316 won to 17,432,712 won for the type II model for one year. Conclusion: This study's results will contribute to baseline data used to establish on infrastructure for visiting nursing program and visiting nursing agencies based on the budget of visiting nursing services.

Job Performance and Self Confidence by Visiting Nurses who are engaged in the Consolidated Health Promotion Program in Gangwon-Province (강원도 통합건강증진사업 방문간호사의 업무수행정도와 자신감)

  • Purpose: This study was designed to determine visiting nurses' job performance and self confidence by career and number of visiting households. Methods: The subjects were 120 visiting nurses fully in charge of the visiting health care program working in 18 community health care centers, who were checked for 5 domains of visiting nurses' job, 39 tasks of job performance and self confidence. ANOVA test, Scheffe test and Pearson correlation coefficient were used for data analysis. Results: The level of job performance and self confidence indicated highest in understanding of the visiting health care program among 5 domains of visiting nurses' job. The level of job performance and self confidence indicated lowest in community resource connection. When nurses were classified by a career with approximately 5 years of working experience, nurses with a career of more than 5 years were highly significant. Job performance and self confidence were positively correlated. Conclusion: Based on the result of this study, it is recommended that the education program for newly hired nurses should be differentiated from that for visiting nurses with a career in order to improve quality of the visiting nursing program and capacity of residents' health care.

Evaluation of Cognitive Functions, Depression, Life Satisfaction among the Elderly Receiving Visiting Nursing Services (방문간호 대상 노인의 인지기능, 우울 및 생활만족도 평가)

  • Purpose: The purpose of this study was to examine cognitive functions, depression, and life satisfaction among the elderly receiving visiting nursing services. Methods: The subjects of this study consisted of 221 elderly individuals who were receiving visiting nursing services from a public health center located in Gyeonggi-do. The Mini-mental State Examination Korea version (MMSE-K), Geriatric Depression Scale-short form-Korea version (GDS-K), and Life Satisfaction Scale were utilized to determine cognitive functions, depression, and life satisfaction, Collected data was analyzed by the SPSS PC Win 12,0 program. Results: The prevalence of cognitive impairment (MMSE-K$\leq$24) was 86.4%, and the proportion of severe depression (GDS-K$\leq$10) was 22,6%, Older age, lower education, and low income were associated with cognitive impairment, and low education, no religion, and low income were associated with depression. Cognitive function negatively correlated with depression. Life satisfaction level was fairly low and was not associated with any variables. Conclusion: The high prevalence of cognitive impairment and low life satisfaction status were determined among low-income elderly who were registered at a visiting health care center. Thus, in the future at visiting health care services, efficient and various programs for the elderly should be tried, to improve cognitive functions and reduce depression. Additionally, consistent evaluation studies for those programs will be required.

A Study on Occupational Satisfaction and Stress of Visiting Nurses at District Health Centers in Chungnam Province (충남지역 보건소 방문간호사의 직무만족도와 직무 스트레스)

  • Purpose: The purpose of this study is to identify visiting nurses' occupational satisfaction and their stress who are working at district health centers in Chungnam province. Methods: The subjects of the study were 109 visiting nurses of 16 district health centers in Chungnam province. The data were collected by self report questionnaires from March 1 to 31, 2010. The data were analyzed by Cronbach's ${\alpha}$, t-test, ANOVA (Sheffe's test), and correlation coefficient using the SPSS program. Results: The results of this study were as follows. The occupational stress level of the subjects was 46.7 and the level of subjects' occupational satisfaction was 2.46. There was a significant negative correlation between occupational satisfaction and occupational stress (r=-.627). Conclusion: The occupational stress level of the subjects, and stress levels of 'work environment', 'job insecurity' and 'organizational structure' were higher than those of the Korean women workers' standard. The level of subjects' occupational satisfaction was lower than that of the nurse practitioners. There was a significant correlation among occupational stress and 'burden of workload' and 'visiting nurse career'. The study showed that occupational stress was correlated with occupational satisfaction. The results suggest that further follow-up study on visiting nurse is necessary to relieve their occupational stress and increase occupational satisfaction.

Community Care and Visiting Nursing Services in Denmark, Sweden, the United States, and Japan (국외 커뮤니티 케어의 방문간호서비스 사례 분석)

  • Purpose: This study compared visiting nursing services of Denmark, Sweden, Japan, and the United States to provide baseline data for the development of models for community care nursing services in South Korea. Methods: A review of the literature was performed that include journal articles, government reports, institutional reports, and national/international statistics. Site visits were performed to explore the visiting nursing services of Denmark. Results: Government centered visiting nursing services were provided in Denmark and Sweden mostly by public organizations, while private services prevailed in the United States and Japan. Nursing services included case management or care coordination services, while nurse practitioners or nurse specialists provided visiting nursing services in all of the four countries and the services were provided 24 hour a day. Conclusions: Based on the review of visiting nursing services in foreign countries, the development of models is needed to provide integrated visiting nursing services in Korea that encompass home care nursing, visiting nursing, and visiting health care services.

Job Analysis of the Nurses Who Work in Customized Visiting Health Care Services using DACUM Technique (맞춤형 방문건강관리사업 내 방문간호사의 직무분석)

  • Purpose: This study was performed to analyze the visiting nurses' job using Developing a curriculum (DACUM). Methods: The DACUM committee with 6 visiting nurses participated in the workshop. In this workshop, the visiting nurses role was defined and their duties and tasks were identified. After content validation via the experts, the pilot test was conducted to 78 nurses. Results: A total of 13 duties and 82 tasks were identified on the DACUM chart which represented the importance, difficulty, and frequency of tasks with alphabet A, B, and C as its higher degree. Regarding duties, 'visiting nursing for high risk group' is the highest, while the lowest was 'public welfare events'. Regarding to tasks, 'discovering a new patient' was the highest, while 'selecting patients and offering medical supplies (nutritional supplement, patch)' was the lowest one. Conclusion: The results showed that visiting nurses working in the visiting health care service center were doing more various duties and tasks than those working according to 'Act on Long-term Care Insurance for the Aged'. The results can be used to develop training programs for visiting nurses and evaluation-scale of their job performance.

Development and Evaluation of Geriatric Visiting Nurses' Educational Program (노인전담 방문간호사 교육 프로그램 개발 및 평가)

  • Purpose: The purpose of this study was to evaluate the validity of an educational program developed for geriatric visiting nurses in Seoul by assessing their satisfaction level and level of learning goal achievement. Methods: This descriptive research study was conducted to develop, implement, and evaluate the educational program in accordance with the ADDIE Instructional System Model. Participants were 170 nurses hired for the 2016 Seoul Metropolitan Government visiting service for older people. Based on Kirkpatrick's Training Evaluation Model, reaction and learning evaluations were conducted during and after the educational program. Data were analyzed using descriptive statistics. Results: The developed educational program consisted of basic and professional courses. The evaluations showed that participants were highly satisfied with the lectures and field placement. Over 90% of the participants achieved the learning achievement goals. Conclusion: The program developed for geriatric visiting nurses in Seoul is considered a valid educational program because of the participants' high levels of satisfaction and academic achievement.

A Survey on Visiting Nurses' Management for Elders with Cognitive Impairment Living in a Community: Focused on Health Centers in a Metropolitan and Medium-sized Cities (방문간호사의 재가노인 인지기능장애 관리에 대한 실태조사: 광역시보건소와 중소도시보건소를 중심으로)

  • Purpose: The purpose of this study was to investigate the status and characteristics of visiting nurses' management for elders with cognitive impairment living in a community focused on health centers in a metropolitan city and five medium-sized cities. Methods: Data were collected from 47 visiting nurses working in a metropolitan city and 47 visiting nurses working in five medium-sized cities from November to December 2012. Results: There were no statistically significant differences in knowledge, attitudes, and nursing behaviors of cognitive impairment between two groups (F=2.13, p=.148; F=3.64, p=.060; F=0.28, p=.595). Among the elders referred to a physician in a metropolitan city by visiting nurses, 42.4% were diagnosed as mild cognitive impairment and 15.2% were diagnosed as severe dementia. The major intervention programs which visiting nurses currently applied for elders were medication and exercise intervention programs, and the intervention programs which they would want to apply in the future were playing, music and recall intervention programs. Conclusion: The cognitive impairment screening test can be done effectively by visiting nurses. This study also suggests to develop various kinds of intervention programs to improve cognitive function for elders living in a community.

The Development and Effect of Doctor-Visiting Nurses ICT Communication System for the Elderly: Focusing on Visiting Nursing Services for Long-term Care Patients (고령자 대상 의사-의료인간 ICT활용 협진 모델개발 및 실증에 관한 연구: 장기요양서비스 중 재가노인대상 방문간호서비스를 중심으로)

  • Purpose: The purpose of this study was to examine effectiveness and usefulness of Information Communication Technology (ICT) in communication between physician and visiting nurses who provide visiting nursing services under long-term care insurance. Methods: Structured questionnaires were used to measure usefulness and satisfaction of the system, both accessibility and convenience to visiting nurses (31 people) and users (182 people). Results: From the user perspective, accessibility and convenience in terms of service users were both satisfactory as shown by users' percentage. No statistically significant difference was found for satisfaction between before and after using the system. The usefulness of the system for visiting nurses was satisfactory for most the nurses. Also most nurses answered that the system is needed and is very useful. Most of the participants (both visiting nurse and service users) were satisfied with use of the ICT system. However, there was no statistically significant difference in satisfaction between the pre and post service because the service provision period was too short (three months). Conclusion: The consensus from both users and service providers is that an ICT based visiting nursing system needs to be introduced but a more user - friendly environment for system development will be needed.
