  • Purpose : The purpose of this study investigate method and effect of Hwangtoh as Korean traditional therapy. Methods : This is literature study with books, and thesis for Korean traditional Hwangtoh therapy. Result : The physical therapy that is in use nowadays, especially thermotherapy has been around since human started to live on the earth. This primitive Korean traditional physical therapy with Hwangtoh have used in common. Historical records show that some kings in those days recommended to their men these methods to treat any chronic disease or illness. This was often thought of as positive means of treatment. Conclusion : Korean traditional physical therapy has been developed, and its scientific effects have been proven. Korean traditional physical therapy is used extensively in hospital and clinics to treat many kinds of diseases and illness and need for scientifical research and data regarding any unproven physical therapy technique.

  • Even though the automation of industrial machinery has dramatically increased, most industries still require human hands for the production of goods. Our hands are essential to our existence. For this reason, hand injuries caused by industrial accidents have become an important issue lately. Most hand injuries are external wounds and constitute 38.6% of disabling industrial accidents. This significant statistic cannot be ignored by rehabilitation programs. The rate of success in operations of finger and hand injuries has increased, but relatively less consideration has been given to physical therapy, rehabilitation, research, or systematic installment in terms of welfare, which would help the injured carry on the nominal life they lived before their injury. Therefore, it is necessary to study systematically all related aspects to provide patients with physical therapy, rehabilitation, and social welfare, in order to restore their social, professional, and economical capabilities. Physical considerations, functional usability, and cosmetic restoration, as well as the patients' mental state must be addressed. The results of the study shows several problems. Medical appliances, manpower, and the environment of the treatment rooms are very poor. The patients cannot receive treatment early enough nor enough treatment because of too many patients for too few physical therapists. Close cooperation between physical therapists and doctors, nurses, or related departments is lacking. Furthermore, it is irrational that industrial accident premiums for the exert; e treatment of hand injury patients are not itemized. Lack of recognition of the importance of specialty in hand injury therapists leads to the lack of professionalization of systematic hand injury treatment In order to solve these problems, the professonalism of physical therapy should be acknowledged and particular treatment courses should be available and/or required. Based on the understanding of the hand injury patients' needs, new theories of physical therapy should be developed and modem medical appliances and facilities should be available, in order to provide the patients with qualitative medical treatment, which then will facilitate the patients' recovery.

  • This research is due to know the transformation after Physical Therapist(PT) was educated the Comunity Based Rehabilitation(CBR) at the strong point by Public Health tenter(PHC) of the CBR. It was the education of physical therapist of strong point by PHC of the CBR from April 17 to April 29 of 2000 in National Rehabilitation Center. The fellowing is the result of Questionnaire educated 39 peoples. First, male and female are same rate, average age, 33 years old, In the position, regular position $86.3\%$. irregular position $13.2\%$. Second. the $76.3\%$ was only working PHC one PT. the main patients were $73.7\%$ of chronic degeneration and over 65 years old who are treated, they are treating $60.5\%$ under 20 person in each day. Third. We asked thru Questionnaire, these are first, need CBR education. second. participating of the PT of the CBR projects. third. suggesting the CBR the another in the PHC. The most of $97.4\%$ was answered the positive. $89.5\%$ agreed that they can offer physical therapy to Handicapped man without responsibility. And all of these answered $100\%$ that they want to offer the physical therapy when they meet the handicapped. Are you can aviable to assess the part of handicapped they answered $71.1\%$, to therapeutic exorcise $52.6\%$, to transfer and family education $47.4\%$, to gait training and family education $55.3\%$. In the welfare policy, introduce job and rehabilitation information side. having answer$44.7\%$. It was negative. In the filed of social welfare also is it possible for them to educate far the another medical staffs and volunteers is PHC. It was $52.6\%$. In Conclusion, needs and participant's rate was high all the people but it was the less in professional physical therapy.

Effect of Microcurrent Taping Therapy on Lower Back Pain (미세전류 테이핑 치료의 만성 요통에 대한 효과)

  • Objectives: The aim of this study was to evaluate the efficacy of microcurrent taping therapy and kinesio taping therapy. Methods: We included 58 participants who met the inclusion criteria and assigned them into two groups after the randomization. We allocated 29 participants to the microcurrent taping therapy group (MCT) and the other 29 to the general kinesio taping therapy group (GT). 27 participants completed the trial in the MCT group, 27 participants in GT. We attached "I" shaped 20cm tape along the erector muscle of the spine starting from the level of S2, and another 30cm tape on the iliac crest horizontally. These "I" shaped tapes were attached on two parts of the body and were performed the same way for both groups. This procedure was done 8 times and participants visited a total of 9 times including a final visit for evaluation. We measured Visual Analog Scale (VAS), Range of Motion (ROM) and Schober's test on every visit. Participants completed a questionnaire of Oswestry Disability Index (ODI) and Beck's Depression Inventory (BDI) on the first and last visits. Results & Conclusion: There was a significant effect of microcurrent taping therapy on lower back pain, but we could not establish the superior effect of microcurrent taping therapy compared with general kinesio taping therapy.

Effects of Modified Fasting Therapy Using Fermented Herbal Medicine on Changes of Body Compositions (발효한약을 이용한 절식요법이 체성분 변화에 미치는 영향)

  • Objectives : The purpose of this study was to investigate the influence of modified fasting therapy using fermented herbal medicine on the changes of body compositions. Methods : This study was carried out on 11 patients who carried out modified fasting therapy using fermented herbal medicine. They went through reducing food intakes period(7 days), fasting period(10~14 days) and refeeding period(10~14 days). Body compositions(weight, BMI(body mass index), skeletal muscle mass, body fat mass, percent body fat, basal metabolic rate, waist-hip ratio, visceral fat area) were measured at each state. And then the data was analyzed. Results : 1. The weight and BMI decreased during the reducing food intakes period and the fasting period, and increased during the refeeding period. But the weight and BMI decreased during the fasting therapy period, as a whole. 2. The skeletal muscle mass decreased during the fasting period and increased during the refeeding period. As the final outcome, for the whole fasting therapy period, decrease of skeletal muscle mass didn't show significance. The body fat mass and percent body fat decreased during the reducing food intakes period, the fasting period and the refeeding period. 3. The basal metabolic rate decreased during the fasting period and increased during the refeeding period. As the final outcome, for the whole fasting therapy period, decrease of basal metabolic rate didn't show significance. 4. The waist-hip ratio decreased during the fasting period and the refeeding period. The visceral fat area decreased during the fasting period and refeeding period. Conclusions : Results from this investigation showed that modified fasting therapy using fermented herbal medicine have positive effects on changes of body compositions. This results are expected to compensate the defects of existing fasting therapy.

Utility of Korean Modified Barthel Index (K-MBI) to Predict the Length of Hospital Stay and the Discharge Destinations in People With Stroke (뇌졸중환자에서 재원기간과 퇴원장소 예측을 위한 K-MBI의 유용성)

  • The purpose of this study was to utilize the K-MBI (Korean Modified Barthel Index) and subscales of K-MBI in predicting the length of hospital stay (LOS) and the discharge destinations for stroke patients. The study population consisted of 97 stroke patients (57 men and 40 women) admitted to the Seoul National University at the Bundang Hospital. All participants were assessed by K-MBI at admission and discharge after rehabilitation therapy and the information available was investigated at admission. The data were analyzed by using the Mann-Whitney U test, the stepwise multiple regression and the logistic regression. The median LOS was 30 days (mean, 32.8 days; range, 22 to 43 days). The K-MBI score at initiation of rehabilitation therapy (p<.001), the type of stroke and living habits before a stroke were the main explanatory indicators for LOS (p<.05). Within the parameters of K-MBI measured at initiation for rehabilitation, feeding and chair/bed transfer were the explanatory factors for LOS prediction (p<.01). Confidence in the prediction of LOS was 20%. Significant predictors of discharge destination in a logistic regression model were the discharge K-MBI score, sex and hemiplegic side. Dressing in items of discharge K-MBI was the significant predictor of discharge destination. The K-MBI score was the most important factor to predict LOS and discharge destination. Knowledge of these predictors can contribute to more appropriate treatment and discharge planning.

Effects of Low Power Laser for the Expression of EGF after Muscle Crush Injury (저강도레이저 조사가 근육압좌손상 후 척수분절의 EGF 발현에 미치는 영향)

  • Low energy laser irradiation(LELI) therapy in physical therapy is widespread but the mechanisms are not fully understood. The purpose of the present study was to examine the epidermal growth factor(EGF)'s expression within lumbar spinal cord which corresponding with crushed extensor digitorum longus(EDL) of rats after low-power laser irradiation applied. After a crushed injury on the right EDL, low-power laser irradiation was applied by using 2000mW, 2000Hz, 830nm GaAlAs(Gallium-aluminum-arsenide) semiconductor diode laser. The laser treatment was performed with 10 minutes daily for 3days. After EDL crush injury, EGF immunoreactive positive neurons in experimental group were progressively decreased from the first to third days. Especially 1 day subgroup is highly expressed in dorsal horn(Lamina I, II, III) and around of central cannal of spinal cord(Lamina VII). Control group was only expressed slightly at 3 days. This study suggests that LELI stimulate that release and migration of EGF in spinal cord, which distict to wound site, therfore promote wound healing of EDL crush injury.

The Clinical Study on the Effect of Reduction in Body Weight and the Change of Body Composition after Fasting Therapy (절식요법의 체중감량효과와 체성분 변화에 대한 관찰)

  • Objectives : This study was aimed to investigate the change of the reduction in body weight and the change of body composition after fasting therapy, Chulsikyopub, which was put into practice in Dept. of oriental rehabilitation medicine at Kyung Hee oriental hospital. Method : We measured body weight, body composition, and the degree of obesity 2 weeks after fasting therapy and 10 weeks after fasting therapy with body composition analyzer(Inbody 2.0). Statistical comparisons between pre-therapy group and post-therpay group were done by paired t-test and Wilcoxon's signed rank test. Results : 2 weeks after fasting therapy, body weight was decreased 7.33kg(9.18%), body fat was decreased 2.47Kg(7.24%), muscle weight was decreased 4.66kg(11.37%), body fat ratio was increased 1.08%, and body metabolic rate(BMR) was decreased 118.23kcal(8.22%). 10 weeks after fasting therapy, each of body weight and fat was decreased 10.4Kg, muscle weight was increased 0.3Kg(0.7%), body fat ratio was decreased 7.9%, and BMR was increased 6.8Kcal(0.47%). But muscle weight and BMR did not satisfy statistical significance. Conclusions : fasting therapy for 10 weeks reduced body weight and body fat statistically significant without loss of muscle weight and BMR.

  • Objectives: The objective of this study is to analyze and observe what kinds of changes in Patients who visited a Korean Medicine Hospital according to the Application of Chuna Therapy Health Insurance and Medical benefits. Methods: In this study we collected data of Patients who first visited the ${\bigcirc}{\bigcirc}{\bigcirc}{\bigcirc}$ Korean Medicine Hospital from March 8 to May 8. we collected data of 1074 Patients who first visited the Daejeon Jaseng Korean Medicine Hospital from March 8 to May 8. Based on the medical charts, Computer order, we analyze and observe statistical data of 1074 patients putting them into six groups. Outcuomes: 1.The increase in the rate of receiving chuna therapy was significant in Group A and Group B(${\rho}<0.05$). 2.The increase in the onset of the disease for more than one year was significant in Group C and Group D(${\rho}<0.05$). 3.The increase in the proportion with disease Corporal was significant in Group E and Group F(${\rho}<0.05$). Conclusions: According to according to the Application of Chuna Therapy Health Insurance and Medical benefits, the number of patients receiving Chuna Therapy increased, and people with old illnesses were more likely to receive Chuna Therapy.

Comparison of Supraspinatus Muscle Architecture During Three Different Shoulder Strengthening Exercises Using Ultrasonography

  • Background: Strengthening the supraspinatus is an important aspect of a rehabilitation program for subacromial impingement and tendinopathy. Many authors recommended empty-can (EC), full-can (FC), and prone full-can (PFC) exercises to strengthen the supraspinatus. However, no ultrasonography study has yet investigated supraspinatus muscle architecture (muscle thickness; MT, pennation angle; PA, fiber bundle length; FBL) in relation to supraspinatus strengthening exercises. Objects: The purpose of this study was to compare the architecture (MT, PA, and FBL) of the supraspinatus muscle during three different types of exercises (EC, FC, and PFC) using diagnostic ultrasound. Methods: Participants performed three different exercises: (A) EC; the arm was maintained at $60^{\circ}$ abduction with full internal rotation in the sitting position, (B) FC; the arm was maintained at $60^{\circ}$ abduction with full external rotation in the sitting position, and (C) PFC; the arm was maintained at $60^{\circ}$ abduction with full external rotation in the prone position. Ultrasonography was used to measure the MT, PA and FBL of the supraspinatus. One-way repeated analysis of variance with Bonferroni's post-hoc test was used to compare between the three exercises and the initial position of each exercise. Results: Compared with each initial position, the FC exercise showed the greatest mean difference in muscle architecture properties and the PFC exercise showed the least mean difference. Conclusion: The findings suggest that the FC exercise position may have an advantage in increasing the amount of contractile tissue or producing muscle power and the PFC exercise position may be useful in a rehabilitation program because it offers the advantage of maintaining the muscle architecture properties.
