The Analysis of Patterns and Risk Factors of Newly Developed Vertebral Compression Fractures after Percutaneous Vertebroplasty

  • Objective : The purpose of this study was to investigate the patterns and the risk factors of newly developed vertebral compression fractures (VCFs) after percutaneous vertebroplasty (PVP). Methods : We performed a retrospective review of the 244 patients treated with PVP from September 2006 to February 2011. Among these patients, we selected 49 patients with newly developed VCFs following PVP as the new VCFs group, and the remaining 195 patients as the no VCFs group. The new VCFs group was further divided into 2 groups : an adjacent fractures group and a nonadjacent fractures group. The following data were collected from the groups : age, gender, body weight/height, body mass index (BMI), bone mineral density (BMD) score of the spine and femur, level of initial fracture, restoration rate of anterior/middle vertebral height, and intradiscal cement leakage, volume of polymethylmethacrylate (PMMA). Results : Age, gender, mean body height/weight, mean BMI and volume of PMMA of each of the group are not statistically significantly associated with fractures. In comparison between the new VCFs group and the no VCFs group, lower BMD, intradiscal cement leakage and anterior vertebral height restoration were the significant predictive factors of the fracture. In addition, new VCFs occurrence at the adjacent spines was statistically significant, when the initial fracture levels were confined to the thoracolumbar junction, among the subgroups of new VCFs. Conclusion : Lower spinal BMD, the greater anterior vertebral height restoration rate and intradiscal cement leakage were confirmed as risk factors for newly formed VCFs after PVP.

Soft computing-based slope stability assessment: A comparative study

  • Analysis of slope stability failures, as one of the complex natural hazards, is one of the important research issues in the field of civil engineering. Present paper adopts and investigates four soft computing-based techniques for this problem: Patient Rule-Induction Method (PRIM), M5' algorithm, Group Method of data Handling (GMDH) and Multivariate Adaptive Regression Splines (MARS). A comprehensive database consisting of 168 case histories is used to calibrate and test the developed models. Six predictive variables including slope height, slope angle, bulk density, cohesion, angle of internal friction, and pore water pressure ratio were considered to generate new models. The results of test studies are used for feasibility, effectiveness and practicality comparison of techniques with each other, and with the other available well-known methods in the literature. Results show that all methods not only are feasible but also result in better performance than previously developed soft computing based predictive models and tools. It is shown that M5' and PRIM algorithms are the most effective and practical prediction models.

Comparison of Predictive Value of Obesity and Lipid Related Variables for Metabolic Syndrome and Insulin Resistance in Obese Adults

  • In this study, obese adults were compared for their ability to predict obesity and lipid related variables and their optimal cutoff values to predict metabolic syndrome and insulin resistance. In this study, 9,256 adults aged 20 years or older and less than 80 years old, who were in the Gyeonggi region from January 2014 to December 2016 and who were examined at a general hospital, were enrolled. The diagnostic criteria for obesity were WHO (World Health Organization), and BMI $25kg/m^2$ or more presented in the Asia-Pacific region. Metabolic syndrome was diagnosed based on the criteria of American Heart Association / National Heart, Lung, and Blood Institute (AHA / NHLBI). According to the results of receiver operating characteristic curve (ROC) analysis, Triglyceride / HDL-cholesterol (TG / HDL-C), Triglyceride and Glucose (TyG) index, lipid accumulation product (LAP) and visceral adiposity index (VAI) showed high predictive power for diagnosing metabolic syndrome. The diagnostic accuracy of LAP (AUC: 0.854) for males and VAI (0.888) for females was the highest. The optimal cutoff value of LAP was 42.71 for male and 35.44 for female, and the cutoff value of VAI was 1.92 for male and 2.15 for female. In addition, WHtR (waist to height ratio), TyG index, and LAP were used as predictors of insulin resistance in obese adults. Therefore, LAP and VAI were superior to other indicators in predicting metabolic syndrome in obese adults.

한국 성인의 인슐린저항성 및 대사증후군 위험 예측인자로서 체형지수와 허리둘레/신장 비율의 효용성 (Assessing a Body Shape Index and Waist to Height Ratio as a Risk Predictor for Insulin Resistance and Metabolic Syndrome among Korean Adults)

  • WHtR과 ABSI는 기존 비만지표의 단점을 보완하기 위해 개발된 복부비만을 반영한 지표이다. 이 연구는 성인남녀를 대상으로 ABSI와 WHtR의 인슐린저항성과 대사증후군에 대한 예측능력을 허리둘레, WHR과 비교하여 알아보고자 하였다. 이 연구는 횡단면 조사에 기초하여 2017년 1월부터 2017년 9월까지 경기지역 일개 종합병원에서 건강진단을 받은 20세 이상 4,395명을 대상으로 하였다. 대사증후군은 AHA/NHLBI (American Heart Association/National Heart, Lung, and Blood Institute)의 진단기준에 따라 정의하였다. 인슐린저항성은 HOMA-IR값이 3.0 이상인 경우 인슐린저항성으로 판정하였다. WHtR 및 ABSI와 대사적 위험요인간에 상관계수를 비교한 결과 남성과 여성 모두에서 ABSI보다 WHtR과 대사적 위험요인간의 상관성이 더 높았다. 대사증후군을 예측하기 위한 WHtR의 AUC 값은 0.849, ABSI의 AUC 값은 0.676 이었다(각각 p<0.001). 인슐린저항성을 예측하기 위한 WHtR의 AUC 값은 0.818, ABSI의 AUC 값은 0.641 이었다(각각 p<0.001). 결론적으로, 한국인을 대상으로 ABSI가 허리둘레, WHR, WHtR 지표보다 인슐린저항성과 대사증후군에 대한 예측력이 낮은 지표였으며, WHtR은 인슐린저항성 및 대사증후군 위험 예측력이 가장 높은 지표임을 확인하였다.

한국인의 정상 폐활량 예측치 (Normal Predictive Values of Spirometry in Korean Population)

  • 연구배경 : 우리나라 국민을 대상으로 실시한 폐활량검사의 판정과 해석은 우리나라 국민을 대상으로 구한 폐활량 예측식이 사용되어야 한다. 그 동안 외국인을 대상으로 구한 폐활량 예측치가 사용되어 오류가 있었다. 본 연구에서는 전 국민을 대상으로 대표성 있고 신뢰할 수 있는 노력성 폐활량과 일초간 노력성 폐활량, 6초간 노력성 폐활량, 일초율에 대한 정상 예측식을 개발 하고자 하였다. 연구방법 : 전 국민을 대상으로 층화표본 추출법을 사용하여 조사대상을 선정하였으며, 폐활량검사기와 검사방법, 검사과정, 결과의 선택을 미국흉부학회에서 권고하는 기준에 따라 체계적인 정도관리와 질관리를 실시하였다. 폐활량 검사를 실시한 대상자 4,816명 중에서 비흡연자이면서 폐활량에 영향을 미칠 수 있는 호흡기 질환 및 증상이 없고, 흉부방사선학적 검사에서 심폐 이상 소견이 없으면서, 폐활량에 영향을 미치는 유해인자에의 노출력이 없는 대상자는 1,212명으로 남자 206명, 여자 1,006명이었다. 이들은 지역과 연령별로 우리나라 국민을 대표할 수 있었다. 통계분석에서 혼합효과모델을 적용하여 AIC 값이 가장 작은 모델로서 남자와 여자에 공통적으로 포함된 변수들을 일반선형회귀분석에 적용하여 폐활량 예측식을 구하였다. 결 과 : 노력성 폐활량의 예측식은 남자 -4.8434 - 0.00008633*연령$^2$(년) + 0.05292*신장(cm) + 0.01095*체중(kg)이었으며, 여자 -3.0006 - 0.0001273 *연령$^2$(년) + 0.03951*신장(cm) + 0.006892*체중(kg)이었다. 일초간 노력성 폐활량의 예측식은 남자 -3.4132 -0.0002484*연령$^2$(년) + 0.04578*신장(cm)이었으며, 여자 -2.4114 - 0.0001920*연령$^2$(년) + 0.03558*신장(cm)이었다. 6초간 노력성 폐활량의 예측식은 남자 -4.4244 -0.0001367*연령$^2$(년) + 0.05156*신장(cm) + 0.008246*체중(kg)이었으며, 여자 -3.1433 - 0.0001442*연령$^2$(년) + 0.04018*신장(cm) + 0.007077*체중(kg)이었다. 일초율의 예측식은 남자 119.9004 - 0.3902*연령(년) - 0.1268*신장(cm)이었으며, 여자 97.8567 - 0.2800*연령(년) - 0.01564*신장(cm)이었다. 결 론 : 본 예측식은 미국흉부학회에서 제시하고 있는 연령과 신장 변수에 체중이 포함되어 차이가 있었다. 이러한 이유는 연령효과와 젊은 연령에서 신장과 체중이 급격하게 변화하는 체격효과가 복합적으로 작용하기 때문이라고 해석된다. 본 예측식과 다른 국내 및 국외 예측식을 비교할 때 본 예측식이 노력성 폐활량과 일초간 노력성 폐활량, 일초율의 예측치를 높게 추정하였으나 대부분 그 차이가 10% 이내로 비슷하였다. 코카시안인 백인을 대상으로 구한 외국의 예측식보다 본 연구의 정상 예측치가 낮지 않았다. 이러한 이유로는 우리나라 젊은 사람들의 체격조건의 변화와 함께 엄격한 정상인의 선정기준, 검사방법과 검사결과의 정도관리 및 질관리에 기인하는 것으로 판단되었다.

Performance assessment of RC frame designed using force, displacement & energy based approach

  • Force based design (FBD) approach is prevalent in most of the national seismic design codes world over. Direct displacement based design (DDBD) and energy based design (EBD) approaches are relatively new methods of seismic design which claims to be more rational and predictive than the FBD. These three design approaches are conceptually distinct and imparts different strength, stiffness and ductility property to structural members for same plan configuration. In present study behavioural assessment of frame of six storey RC building designed using FBD, DDBD and EBD approaches has been performed. Lateral storey forces distribution, reinforcement design and results of nonlinear performance using static and dynamic methods have been compared. For the three approaches, considerable difference in lateral storey forces distribution and reinforcement design has been observed. Nonlinear pushover analysis and time history analysis results show that in FBD frame plastic deformation is concentrated in the lower storey, in EBD frame large plastic deformation is concentrated in the middle storeys though the inelastic hinges are well distributed over the height and, in DDBD frame plastic deformation is approximately uniform over the height. Overall the six storey frame designed using DDBD approach seems to be more rational than the other two methods.

합성태풍과 인공지능을 활용한 폭풍해일고 예측 (Prediction of Storm Surge Height Using Synthesized Typhoons and Artificial Intelligence)

  • 태풍 내습 시 신속하고 정확한 해일고 예측은, 연안재해 대응에 필수적인 요소이다. 이러한 해일고의 예측을 위해서 기존에는 태풍예측정보를 수치모델에 적용하여 예측자료를 생산하는 것이 대부분 이였다. 이러한 방법은 대용량의 컴퓨팅 자원과 시간이 소요된다는 단점이 있다. 최근에는 인공지능 기반으로 신속하게 예측자료를 생산하는 연구가 다양한 분야에서 진행되고 있으며, 본 연구에서는 인공지능 기반 해일고 예측을 수행하였다. 인공지능 적용을 위해서는 많은 수의 학습자료가 필요하게 되며, 기왕 발생태풍은 개수가 한정되어 있어 본 연구에서는 TCRM(Tropical Cyclone Risk Model)을 통하여 합성태풍을 생성하고, 이를 폭풍해일 모델에 적용하여 해일고 자료를 생성한 후, 학습자료로 활용하였다. 인공지능으로 예측한 해일고와 실제 발생 태풍에 대한 비교 결과, RMSE(Root Mean Square Error)는 0.09 ~ 0.30 m, CC(Correlation Coefficient)는 0.65 ~ 0.94, 최대 해일고의 ARE(Absolute Relative Error)는 1.0 ~ 52.5 %로 분석되었다. 특정 태풍/지점에서는 다소 오차가 크게 나타나고 있으나, 향후 학습자료의 최적화 등을 통하여 정확도를 개선할 수 있을 것으로 기대된다.

한국인 대상 대사증후군 진단 예측인자로서 허리-신장비 (The Study on Availability as a Predictor of Waist-to-Height Ratio for Metabolic Syndrome Diagnosis in Koreans)

  • Objectives: Recently, many studies have demonstrated that the waist-to-height ratio (WHtR) is more correlated with metabolic syndrome than the waist-hip ratio (WHR), body mass index (BMI). The aim of this study was to investigate and compare WHtR as a predictor of metabolic syndrome with other anthropometric indices. Methods: Approximately 3,800 data of subjects were used for the analysis, and data from Korean Medicine Data Centar). Receiver operating characteristic curve was performed and area under the curve (AUC) was calculated to find the usefulness of WHtR, WHR, and BMI. Results: WHtR was the most predictive factor with the highest diagnostic value for metabolic syndrome than WHR, BMI. AUC of WHtR was significantly higher in total subjects, men and women. Conclusions: WHtR may be the simple and effective anthropometric index for metabolic syndrome.

Diagnosis of Pulmonary Arterial Hypertension in Children by Using Cardiac Computed Tomography

  • Objective: To establish diagnostic criteria for pulmonary arterial hypertension (PAH) in children by using parameters obtained through noninvasive cardiac computed tomography (CCT). Materials and Methods: We retrospectively measured parameters from CCT images of children from a single institution in a multiple stepwise process. A total of 208 children with mean age of 10.5 years (range: 4 days-18.9 years) were assessed. The variables were classified into three groups: the great arteries; the ventricular walls; and the bilateral ventricular cavities. The relationship between the parameters obtained from the CCT images and mean pulmonary arterial pressure (mPAP) was tested and adjusted by the children's body size. Reference curves for the pulmonary trunk diameter (PTD) and ratio of diameter of pulmonary trunk to ascending aorta (rPTAo) of children with CCT images of normal hearts, adjusted for height, were plotted. Threshold lines were established on the reference curves. Results: PTD and rPTAo on the CCT images were significantly positively correlated with mPAP (r > 0.85, p < 0.01). Height was the body size parameter most correlated with PTD (r = 0.91, p < 0.01) and rPTAo (r = -0.69, p < 0.01). On the basis of the threshold lines on the reference curves, PTD and rPTAo both showed 88.9% sensitivity for PAH diagnosis, with negative predictive values of 93.3% and 92.9%, respectively. Conclusion: PTD and rPTAo measured from CCT images were significantly correlated with mPAP in children. Reference curves and the formula of PTD and rPTAo adjusted for height could be practical for diagnosing PAH in children.

Distribution Dynamics and Proposed Determinants: Exploring Morphological, Clinical Laboratory, and Lifestyle Factors in the Coexistence of Age-Related Skeletal Muscle Mass Loss and Obesity among Young Men: A Nationwide Cross-Sectional Study

  • PURPOSE: This study examined the distribution dynamics and proposed determinants, including morphological measurements, clinical laboratory tests, and lifestyle factors among young Korean men aged 20 to 29 years with the coexistence of age-related loss of skeletal muscle mass and obesity (CALSMO). METHODS: Six hundred and sixty-six participants were divided into two groups based on their skeletal muscle mass index, with 12 individuals categorized in the CALSMO group and the remaining 654 in the normal group. The proposed determinants variables consisted of three main components: morphological measurements, clinical laboratory tests, and lifestyle factors. The morphological measurement variables were height, weight, body mass index, waist circumference, and skeletal muscle mass index. The clinical laboratory tests were fasting glucose, triglyceride, total cholesterol levels, and systolic and diastolic blood pressure. The lifestyle factors considered were alcohol consumption and tobacco use. Complex sampling analysis was performed for the evaluation. RESULTS: The distribution dynamics were determined to be 1.81(1.02-3.18) %. Morphological factors, such as height, weight, body mass index, waist circumference, and skeletal muscle mass index, showed significant differences (p < .05). The clinical laboratory test variables, specifically the fasting glucose, triglyceride, and total cholesterol levels, also exhibited significant differences (p < .05). The lifestyle factor, alcohol consumption, also showed a significance (p < .05). CONCLUSION: This study provides insights into the distribution dynamics. The proposed determinants in young Korean individuals with CALSMO are height, weight, body mass index, waist circumference, skeletal muscle mass index, fasting glucose, triglyceride, total cholesterol levels, and alcohol consumption.
