A STUDY ON THE POSTOPERATIVE STABILITY OF OCCLUSAL PLANE IN ORTHOGNATHIC SURGERY PATIENTS DEFENDING ON THE DIFFERENCE OF OCCLUSAL PLANE (악교정 수술시 교합평면의 차이에 따른 술후 안정성에 관한 연구)

  • Surgical-orthodontic treatment is performed for the skeletal Class III patients with no remaining growth and too big a skeletal discrepancy (or camouflage treatment, and two jaw surgery is needed in order to have maximum effect in such patients. In two jaw surgery cases, surgical alteration of the occlusal plane is necessary to establish optimal function, esthetics and postoperative sability, therefore the establishment of the occlusal plane is essential in diagnosis and treatment. The object of this study is to evaluate the stability of the indiviual ideal occlusal plane bsaed on the architectural and structural craniofacial analysis of Delaires. Thus, the subjects of this study were 48 patients who underwent two jaw surgery, and divided in two groups. Each group were composed of 24patients, A group were operated with ideal occlusal plane and B group were not. Two groups were compared at the preoperative, immediate postoperative (average 4.3days), and long-term postoperative (average 1.3years) lateral cephalometric radiographs. The following results were obtained: 1. There was no significance in occlusal plane angulation between $T_2\;and\;T_3$. Average long term follow-up changes of occlusal Plane angle were $0.24^{\circ}{\pm}2.43$, with FH plane and $0.15{\circ}{\pm}2.16{\circ}$ with SN plane in all 48 patients. These results demonstrated that the occlusal plane after two jaw surgery in skeletal Class III malocclusion was stable. 2. There was no significance in postoperative stability of occlusal plane between A and B group. 3. There was no significance in postoperative stability of occlusal plane depending on surgeon and operative method within each group. 4. The postoperative changes of occlusal plane were correlated to the postoperative changes of jaw rather than tooth position. 5. There was no correlation between the postoperative changes of occlusal plane and maxillary impaction and mandibular setback with surgery.

  • Purpose: To evaluate 2D and 3D of occulsal, mesial-occlusal and mesial-occlusal-distal cavity of composite resin inlay. Methods: Abutment tooth 16, 36 of FDI system was selected for the study. Inlay prostheses classified as occlusal cavity (OC group), mesial-occlusal (MOC) and mesial-occlusal-distal cavity (MODC) were prepared using composite resin. Composite resin was injected with composite resin in prepared tooth cavity and then photopolymerized with UV light. Additional thermal polymerization was performed. Marginal gap of composite resin inlays were measured by digital microscope(x160) with silicone replica technique. The data was analyzed from statistical software for Kruskal-Wallis test (${\alpha}=0.05$). 3-dimensional analysis was analyzed through superimposition method. Results: The smallest 2D marginal fit measure of the three groups was $47.0{\pm}21.6{\mu}m$ in the MOC group. The largest 2D marginal was $69.1{\pm}33.8{\mu}m$ in the MODC group. In the trueness of the three groups, the most accurate figure was $14.4{\pm}2.3{\mu}m$ for the MODC group. In Precision, the most accurate figure was $14.5{\pm}4.3{\mu}m$ for the MODC group. Conclusion : In this study, 2D marginal fit of OC, MOC, and MODC cavities fabricated with composite resin was applicable to all clinical applications. In the 3D inner surface accuracy evaluation, the MODC group showed the accuracy results.

Evaluation of bone quality in alveolar crest obscured by dental implants ; A pilot study by densitometric digital analysis in mandibular bone specimen (치과 임플란트 주변 협설측 치조골의 변화분석 - 하악골 시편에서의 디지털 농도분석법을 이용한 실험적 고찰 -)

  • Despite of technical difficulties, the combination of occlusal projection and densitometric digital analysis may ultimately provide a means of detection of subtle bone loss at the facial and lingual side of dental implant (Oblique occlusal view is more useful for $ITI^{(R)}$ dental implant due to its contour of shoulder as like tulip flower). In this study, conventional periapical projections of x-ray beam had shown more high sensitivity to detect the bony defects than oblique occlusal projections in alveolar crest obscured by dental implants or not, even if the difference was not statistically significant. Unlike conventional periapical projections. occusal projections combined with densitometric digital analysis technique may provide a means for detection of subtle bone change at the all around of implants without obscuring effect by implant itself. Although the results from this in vitro study were performed under limited circumstances, these results might afford more possibility and versatile modality of diagnosis options to clinician in the implant practice.

Analysis of Occlusal Contacts Using Add-picture Method (Add-picture 방법을 이용한 교합접촉점 분석)

  • The purpose of this study was to analyze the area of occlusal contact points using visual method. One subject was selected who had Angle Class I, normal dentition, without dental caries, periodontal disease and temporomandibular disorders. Forty times PVS impressions were taken and 10 pairs casts were fabricated using dental super hard stone. After mounting the casts with customized loading apparatus, 78.9kg/f force was loaded as a maximum biting force. In T-Scan method, occlusal contact points measurement was repeated twice. Then, using Photoshop program (Adobe photoshop CS3, Adobe. San Jose, USA), the pixels which indicated occlusal contact points by color was recognized, and the distribution of recognized pixels were calculated to area. In Add picture method, polyether bite material applied to the occlusal surface of the casts. Then, the image of the translucent areas was recorded and classified $0{\sim}10{\mu}m$, $0{\sim}30{\mu}m$, $0{\sim}60{\mu}m$ area by the amount of transmitted light. To acquire occlusal surface, the numbers of pixels from the photograph of the contact area indicated cast converted to $mm^2$. The mean occlusal contact area by two methods was statistically analyzed (paired t-test). Part of the red and pink area in T-Scan image were almost equivalent to the $0{\sim}10{\mu}m$, $0{\sim}30{\mu}m$, $0{\sim}60{\mu}m$ area in Add picture image. The distribution of occlusal contact points were similar, but the average area of occlusal contact points was wider in T-scan image (P<.05). Pink and red area in T-scan image was wider than $0{\sim}10{\mu}m$, $0{\sim}30{\mu}m$ area in Add picture image (P<.05), but similar to $0{\sim}60{\mu}m$area in Add picture image (P>.05). Occlusal contact points in T-scan image did not indicate real occlusal contact points. Occlusal contact areas in T-scan method were enlarged results comparing with those in Add picture method.

Three-Dimensional Finite element analysis of Canine and Carnassial for Observation of Dog Bite forces (개(犬)의 교합력 관찰을 위한 견치와 열육치의 3차원 유한요소 분석)

  • Purpose: This study is for the prosthesis of dogs. Observe the occlusal relation between the dog's canine and carnassial teeth. The strength and the direction of the occlusal by 3D FEM analysis. Methods: The mandibular canine and carnassial of dogs were tested. The dog's skull was contact point confirmed by dental CAD. The skull of the dog was 3D modeled by CT. The 3D model was analyzed by ABAQUS. Opening and closing movement has been a force of 100N, 200N, 300N, 500N, 1000N, 1,500N. The peak von Mises stress distribution was confirmed. Results: As occlusal force increased, stress appeared to 1.34 MPa, 3.32 MPa, 5.00 MPa, 6.19 MPa, 5.58 MPa, 5.47 MPa in left canine. and Stress was seen at 2.10 MPa, 3.08 MPa, 3.89 MPa, 5.50 MPa, 7.04 MPa, 7.18 MPa in the right canine. Stress appeared at 2.41 MPa, 3.53 MPa, 5.15 MPa, 7.28 MPa, 31.26 MPa, 67.22 MPa in the left carnassial. and Stress was seen at 1.57 MPa, 2.96 MPa, 3.76 MPa, 6.01 MPa, 20.94 MPa, 64.38 MPa in the right carnassial. Conclusion: Peak von Mises stress values were found at the peak of the canine, the buccal of the central cusp of the carnassial, and the occlusal surface of the distal cusp.

Generation of virtual mandibular first molar teeth and accuracy analysis using deep convolutional generative adversarial network (심층 합성곱 생성적 적대 신경망을 활용한 하악 제1대구치 가상 치아 생성 및 정확도 분석)

  • Purpose: This study aimed to generate virtual mandibular left first molar teeth using deep convolutional generative adversarial networks (DCGANs) and analyze their matching accuracy with actual tooth morphology to propose a new paradigm for using medical data. Methods: Occlusal surface images of the mandibular left first molar scanned using a dental model scanner were analyzed using DCGANs. Overall, 100 training sets comprising 50 original and 50 background-removed images were created, thus generating 1,000 virtual teeth. These virtual teeth were classified based on the number of cusps and occlusal surface ratio, and subsequently, were analyzed for consistency by expert dental technicians over three rounds of examination. Statistical analysis was conducted using IBM SPSS Statistics ver. 23.0 (IBM), including intraclass correlation coefficient for intrarater reliability, one-way ANOVA, and Tukey's post-hoc analysis. Results: Virtual mandibular left first molars exhibited high consistency in the occlusal surface ratio but varied in other criteria. Moreover, consistency was the highest in the occlusal buccal lingual criteria at 91.9%, whereas discrepancies were observed most in the occusal buccal cusp criteria at 85.5%. Significant differences were observed among all groups (p<0.05). Conclusion: Based on the classification of the virtually generated left mandibular first molar according to several criteria, DCGANs can generate virtual data highly similar to real data. Thus, subsequent research in the dental field, including the development of improved neural network structures, is necessary.

Fracture Strength Analysis of Monolithic Zirconia Ceramic by Abutment Shape (지르코니아 단일구조 전부도재관의 지대치 형태에 따른 파절 강도)

  • Purpose: This study was performed fracture strength test by conducted change of abutment and coping shape for suggesting monolithic all ceramic crown which has thin thickness and superior strength of the occlusal surface. Methods: The specimens on the four kinds abutment was made according to thickness of occlusal surface and angle of axis surface. And All ceramic coping specimens of 6 different kinds was made by the CAD/CAM Method. Compression strength test using the UTM and the verification of compression-stress situation using the 3D finite element method were conducted under optimum conditions. Results: 516C specimen was showed the strongest compression-fracture strength, followed by 516FR, 516F45, specimens. Did not show significant differences between 516FR and 516F45. 516C of the universal testing machine the specimen's surface that are within the vertical load is small, finite element method of a uniformly distributed load, so the value received suggests otherwise. Conclusion: In conclusion, abutments of monolithic ziconia ceramic when having a same thickness of the occlusal, as the angle of occlusal edge is small, the stress is well dispersed and it can endure well in the fracture.

A study of mandibular canal angle and location of mental foramen on the panoramic radiograph (파노라마방사선사진에서의 하악관의 각도와 이공의 위치에 관한 연구)

  • Purpose: To assess the angle between mandibular canal and occlusal plane at each posterior tooth region and location of mental foramen on the panoramic radiographs. Materials and Methods: This study analysed 46 half-mandibles of panoramic radiographs. Inferior border of mandibular canal was traced. Occlusal plane was drawn from lingual cusp tip of the first premolar to distolingual cusp tip of the second molar. Perpendicular line from occlusal plane was drawn at each tooth region and then tangential lines were drawn from the crossing points at canal. the angle between occlusal plane and tangential line was measured. The location of mental foramen was also studied. According to the location of mental foramen, radiographs were divided into M (mesial) group and D (distal) group on the basis of the second premolar. and then inter-group analysis about mandibular canal angle was done. Results: The angles of mandibular canals were -17.7$^{\circ}$, -9.5$^{\circ}$, 8.2$^{\circ}$, 22.3$^{\circ}$, and 39.2$^{\circ}$at first premolar, second premolar, first molar, second molar, and third molar, respectively. The commonest position of the mental foramen was distal to the second premolar. Inter-group comparison showed statistically significant difference at the second premolar and the first molar(p<0.001). Conclusion: The knowledge of mandibular canal angle and location of mental foramen can help understanding the course of mandibular canal. (Korean J Oral Maxillofac Radiol 2009; 39: 89-92)

Changes of bite force and occlusal contacts after stabilization splint therapy (교합안정장치 사용후 교합력 및 교합접촉의 변화양상에 관한 연구)

  • This study was done to identify the changes of bite force and occlusal contact between before and after stabilization splint therapy. 16 female patients, accepted for orthodontic treatment at the Department of Orthodontics, College of Dentistry, Chosun University, were selected as the sample of this investigation. For comparisons, the samples of 16 were divided into just before stabilization splint, 1 month after stabilization splint, 2 months after stabilization splint, and 3 months after stabilization splint and used the T-scan system to identify the bite force and occlusal contact changes for each group. Statistical analysis of the data was carried out ANOVA tests, and Turkey tests using $SPSS/PC^+$. The results were as follows : 1. Bite force change from just before treatment to 1 month after stabilization splint therapy was statistical significantly decreased (P<0.05). 2. Bite force changes from 1 to 3 months after stabilization splint therapy showed no statistical significance (p>0.05). 3. The changes of anterior occlusal contacts showed no statistical significance regardless of the wearing periods of stabilization splint (P>0.05). 4. The changes of posterior occlusal contacts from just before treatment to 1 month after stabilization splint therapy was statistical significantly decreased (P<0.05). 5. The changes of posterior occlusal contacts Outing 1 to 3 months after stabilization splint therapy showed no statistical significance (P>0.05). 6. Posterior teeth rather than anterior teeth were more influenced by the changes of the number of occlusal contacts. To sum up above results, we may respect to capturing and stabilizing centric relation Position just 1 month after stabilization splint therapy.

Effect of Increase in Occlusal Vertical Dimension on Appendage Muscle Strength (수직적 교합고경의 증가가 사지 근력에 미치는 영향에 관한 연구)

  • Objective. This study was conducted to observe the effect on appendage muscle strength according to increase in occlusal vertical dimension. Materials and methods. Ten males with a mean age of 21 were selected. The tested occlusal splints were made at the position of increased occlusal vertical dimension of 2mm,3.5mm and 5mm from the intercuspal position. Before and after wearing occlusal splints, the appendage muscle strength were tested by Cybex II dynamometer (Lumex Inc., Ronkonkoma, NY, USA). Results. Statistical analysis using the paired t-test revealed significant differences for flexion and extension of the hip, pronation of the forearm, internal rotation of the shoulder, external and internal rotation of the knee, and dorsiflexion and plantarflexion of the ankle (p<0.05). Conclusions. As the result of this study, we conclude that when occlusal vertical dimension was increased, most of mean muscular strength values were increased. Especially at the position of 3.5mm increased vertical dimension displayed the highest mean muscular strength value than other positions.
