Demonstration Project on Utilization of Telephone Consulting and Telemedicine System for Home Health Care of the Elderly (노인건강상담전화 운용과 가정간호사업 활성화를 위한 원격의료 시범사업)

  • Advanced countries such as the USA and Japan are eagerly seeking ways to improve health and welfare of the elderly. One of the services is home health care service using the telephone. Various types of services using the telephone have been developed, improved and are being utilized ranging from the basic consulting to emergency response systems in the area of health care for the elderly. A demonstration project was launched to study the feasibility of a consulting system and telemedicine for the elderly using the public phone system in Korea. For this project, a gathering site for the elderly was selected and those who visited this place were interviewed to find out what kinds of services they wanted and what kind of system they needed to provide the required services. Based on the users' requests and the surrounding environment, a telephone consulting facility was established at the Research Institute of Nursing Science at Seoul National University and consulting personnel was recruited, trained and posted at the center. An Application program for home health care nurses to use when they visited the patients at their homes was developed. This system operates on a notebook Computer and allows nurses to communicate with a doctor at a local hospital through a modem and telecommunication line. These systems were implemented for three months and problems which developed during operation of the systems were identified and progressively modified. Through system evaluation, it was found that a consulting system using phone service will be an invaluable system for the welfare of the elderly in the future. But in order to meet the elderly's need, more services than mere consultation are needed. That is, communication with physicians and hospitals are needed. Thus, when there is any need for physicians' attention, physicians or hospitals should be contacted directly. Similarly for telemedicine, when the home health care nurse visits elderly patients she can assess the patient's problem and provide nursing care, access a physician or hospital to refer her patient to or consult directly using the telecommunication the system. The above mentioned system is a basic form of futuristic telemedicine for the elderly and those who have chronic disease problems. This kind of system will be of great value when it is used on the national information super-highways in the future. In order to get to that stage, of course, this project needs great improvement in the technical, academic, and legal aspects.

Factors Influencing Life Satisfaction the Elderly living alone (독거 노인의 생활만족도 영향요인 분석)

  • Purpose: This study was to identify factors that influence the life satisfaction of solitude elderly. Method: The subject were 100 homebound the elderly living alone(age=76.58) who were live in M city located in Jeollanamdo. Data were collected from June 1 to September 30, 2002. The instrument used for this study were a survey of general characteristics, residential environment, MUNSH, Health Self-rating scale, ADL, loneliness and POMS. The data was analyzed using descriptive statistics, t-test, and stepwise Multiple Regression. Result: In general the perceived health status was poor, but ADL was moderate., loneliness was high but mood and life satisfaction were low. In regression analysis, life satisfaction was significantly influenced by mood(51%), ADL(4%), loneliness(3%), and perceived health status(1%). These variables explained 59% of the variances in the life satisfaction. Conclusion: The result suggests that health care professionals should give more attention to helping the elderly raise their life-satisfaction. A further study is necessary to find out an effective nursing intervention for a better those in a comfortable residential environment, decreasing the loneliness and to promoting the mood for those elderly who live alone.

Middle-Class Baby-Boomers' Preferred Characteristics of Continuing Care Retirement Communities (중산층 베이비부머의 연속보호체계형 은퇴주거단지에 대한 선호특성)

  • The purpose of this study was to identify some residential characteristics of Post-retirement and characteristics of the Continuing Care Retirement Communities (CCRCs) favored by those baby-boomer generations soon retiring. A web-based online survey was conducted with middle-class baby-boomers living in the Seoul-Incheon-Gyengki capital area and the five metropolitan cities of Korea between 9th and 20th of February in 2017. A total 0f 507 responses were valid for statistical analysis. The findings obtained that they preferred living Urban outskirts after retirement; Desired area of post-retirement was differentiated depending on asset and educational attainment; 63.4% of respondents had no intentions to live with adult child; a combination of independent living and assisted living was the most preferred form of the CCRCs; Over 80% of respondents had intention to live in independent living, assisted living and nursing home; and Intention to live in assisted living and nursing home was differentiated depending on Intention to live with adult child. Among the four Community-linked types, the Hospital-based one was most popular. The University-based and Religion-based ones were then preferred by females than males. The results obtained here can be used for reflecting the residents needs when planning the CCRCs in Korea.

Factors Affecting the Job Performance of Occupational Health Manager at the Hospital (의료기관 보건관리자의 직무 수행도에 영향을 미치는 요인)

  • Purpose: To identify the importance and performance levels of health management duties, work-related and general attributes of health managers in medical institutions and analyze their impact on the performance of managing health-care related tasks. Methods: This research identified the performance levels of 150 health managers who have been executing industrial health-related duties for more than six months as nurses in medical institutions with more than 30 hospital beds. The variables which affect their performances were then analyzed. Results: The average importance of health care duties was 8.1 out of 10 and 5.5 for performance levels. Multi-regression analysis on the variables affecting performance levels of health managers in medical institutions showed that health managers exhibit higher numbers under the following conditions: over 300 full-time employees, more than 1 year but less than 3 years of experience, positions above section chief level, affiliation to the safety and health department, and high perception of duty importance. Conclusion: Improved cognizance of health manager importance should occur initially; then, health-care center setup, assigning of exclusive occupational health managers, and organizational efforts to improve the working environment in tandem with the provision of educational training programs to improve work quality are necessary.

Koreans' Experience at Health Facilities in Thailand (한국인의 태국 의료기관 이용 경험)

  • Purpose: The purpose of this study was to identify experiences when Koreans who reside in Thailand visit Thai health facilities. Methods: The data were collected via direct interview from 36 Korean residents of Thailand. Collected data were analyzed by content analysis. Result: 1) 162 content analysis data were collected from the participants' statements. These data were categorized into 70 attributes and 9 higher attributes. 2) Out of the 70 attributes, the 3 attributes which were most commonly mentioned by the participants were the following: medical expense for foreigners were expensive(66.7%), difficulties in communication with health care providers(36.1%), kindness of nurses(33.3%). 3) The 9 higher attributes were 1) high medical expense to foreigner, 2) difficulties in communication, 3) distrust about medical service, 4) excessive prescription in oral medicine, 5) kindness, 6) good service, 7) difference in medical environment according hospital type, 8) irrational process in medical treatment for out patients, 9) cultural difference in postpartum care. Conclusion: When Korean residents in Thailand use Thailand health facilities, they experience high medical expense, difficulties in communication, low quality in medical service and kindness of the medical staff.

Characteristics, Motivation of Choice and Childbirth Experience of Women Who Selected Delivery at Midwifery Clinic (조산원 분만을 선택한 여성의 특성, 선택동기 및 출산경험 조사)

  • Purpose: This study is descriptive research to identify characteristics and childbirth experience of women who selected delivery with midwives. Methods: The research methods included structured questionnaires, open questions and charts and data were collected from March 2009 to May 2010 at one midwifery clinic. The 108 data of primipara and multipara were analyzed using descriptive statistics and grouping in same meaning. Results: The general characteristics of participants were age of 31~35, housewife, college or university graduate, religion 'yes', and economic state 'middle'. The obstetric characteristics of participants were abortion rate of 23.1%, none complication to pregnancy and delivery of 97.2%, and none postpartum complication. This study was analyzed using positive experiences of 3 categories, that is 'comfortable and natural childbirth', 'satisfaction and trust to personal care of midwives', and 'experience of baby-and-family-centered childbirth' and negative experiences of 3 categories, that is 'improvement of healthcare environment', 'insufficient facilities and nursing care', and'burden of cost'. Conclusion: This study is significant, since it investigates in the absence of domestic research on the characteristics of women who delivered at midwifery clinic. Thus, this study provided basic data on the characteristics of women who delivered at midwifery clinic.

A Study of the influence of stimulus on newborn infant생s response -Specifically in heart rate change- (감각적 자극에 따른 신생아 반응에 관한 연구 -심박동수를 중심으로-)

  • Early extra-uterine development of the newborn infant has the potential for clarifying important questions of nature and care taking environment. In this study, tactile and auditory stimulation is used on .a group of infants to determine the effects of their response. Infants were tested by EKG at 24∼48 hrs after birth ana all they are free from complication. Stimuli presented were-touch and sound (40dB, 70dB and 40∼70dB). The result of the study were as follows: 1) The heart rate was seen to depend on their sleep-wake state. (light sleep ; 125/min, eyes: open & motor activity increase ; 151/min crying ; 185/min). 2) The heart rate affected by tactile & auditory stimulus. The heart rate response was acceleration above the prestimulus level. (tactile, 231min increase, auditory ; 16∼21/min increase), and heart rate change was seen to depend on prestimulus rate. (γ=.73) 3) There was a decrease in heart rate when they were soothe. (touch ; 24.8sec, 21/min decrease, pacifier ; 14.5sec, 26/min decrease) and significant difference in time between touch and pacifier (t= 1.76, p<0.05). This result support the evaluation of newborn infant's sensory abilitics, and has even begin to influence changes in nursing care. But systemic monitoring of newborn infants response would. further define.

Foreign Immigrant Mothers' Experiences of Pregnancy, Childbirth, and Child Rearing (결혼이민자 어머니의 임신, 출산 및 영유아 양육 경험)

  • Purpose: This study was conducted to explore foreign immigrant mothers' experiences of pregnancy, childbirth, and child rearing. Methods: Ten married female migrants were selected by a purposive and snowball sampling method. Data were collected during the period from September to November, 2009 by individual in-depth interviews and all interviews were audio-taped and transcribed verbatim. Results: Ten categories were induced: In pregnancy and childbirth domain, social support-request help to their own parents, social support-depending on husband and mother's in law, Enduring emesis-gravidarum in strange environment were extracted. In Chid-rearing domain, child-rearing overburden, lack of self-confidence on maternal role, child-rearing difficulties due to language barrier, lack of information and education on child-rearing, lack of accessibility to public health care services, expectation of child's assimilation in Korea, financial strain in child-rearing. Conclusion: This study provides basic knowledge regarding married female migrants' experiences related to pregnancy, childbirth, and child rearing. The findings of the study will help health professionals designing effective interventions for married female migrants with difficulties related to child care, parenting, rearing.

Relationships between Resilience, Job Stress, and Organizational Commitment in ICU Nurses (중환자실 간호사의 회복탄력성, 직무스트레스 및 조직몰입)

  • Purpose: This study explored the relationships among resilience, job stress, and organizational commitment in ICU nurses. Further, the possibility of the application of resilience to the hospital environment was evaluated to provide basic data for program development to improve resilience and to promote nurses' commitment to their organizations. Methods: Data were collected from 174 ICU nurses at A University Hospital. The cross-sectional descriptive survey was conducted using a structured questionnaire. Results: The mean resilience score was $2.41{\pm}0.47$ out of 4. The mean score for job stress was $3.60{\pm}0.36$ out of 5, and that for organizational commitment was $3.25{\pm}0.55$ out of 5. Analysis of the relationships between the participants' resilience and organizational commitment indicated a significantly positive correlation (r=.45, p<.001). According to the general characteristics, organizational commitment levels showed a significant difference according to the ICU type (F=1.38, p<.001). Conclusion: It may beconcluded that the important individual factor of resilience had a positive influence on organizational commitment. Thus, resilience should be actively promoted, and a program should be developed to enhance the resilience of ICU nurses.

The Influence of Emotional Labor, Job Stress, and Burnout on Turnover Intention of Care Worker's at Long-Term Care Hospitals (요양병원 요양보호사의 감정노동, 직무스트레스 및 소진이 이직의도에 미치는 영향)

  • The purpose of this study was to identify the extent of and relationships among emotional labor, job stress, burnout and turnover intention and the factors influencing the turnover intention of care workers at long-term care hospitals. The study was approved by KUIRS, and the data were collected from August 12 to September 30, 2015, and analyzed using the SPSS WIN 18.0 program. The participants of this study were 200 care workers at ten different long-term care hospitals in B city and G province. Data were collected through self-reported questionnaires and analyzed by descriptive statistics, the t-test, ANOVA, Pearson's correlation coefficients, and Stepwise multiple regression. The turnover intention was significantly correlated with emotional labor (r=.372, p<.001), job stress (r=.471, p<.001), and burnout (r=.411, p<.001). In the multiple regression, job stress (${\beta}=.290$, p<.001), burnout (${\beta}=.239$, p<.001), type of employment (${\beta}=.185$, p<.001), and emotional labor (${\beta}=.157$, p=.017) were identified as factors influencing the turnover intention. These factors explained 32.6% of the total variance in the turnover intention. Based on the results, in order to reduce the turnover intention, the improvement of work environment and clear job-descriptions are needed to reduce the job stress, and the development and implementation of proactive psycho-social interventions are necessary to reduce burnout and emotional labor in care workers at long-term care hospitals.
