심장판막 치환술 후 한의학적 치료에 대한 체계적 문헌고찰 및 메타분석 (East Asian Traditional Medicine Treatment for Patients after Heart Valve Replacements: A Systematic Review with Meta-Analysis)

  • Objectives: To compare the effectiveness and safety of East Asian traditional medicine treatments (EATMT) versus conventional management in patients following heart valve replacement surgery. Methods: We searched several databases, including the Korean Studies Information Service System, PubMed, China National Knowledge Infrastructure, and Citation Information by NII. The search range included randomized controlled trials from each first issue until June 27, 2021. Two review authors independently extracted the data. We assessed the risk of systematic errors by evaluating risk domains using the "Risk of bias" tool. Results: We included 5 trials in the review. In the EATMT, the investigators reported significant improvements in reshaping of the heart structure: left ventricular end diastolic diameter (MD -4.43, 95% CI -6.06 to -2.79; 130 participants; 2 studies; high evidence). Comparisons with usual care revealed a significant decrease in gastrointestinal complications rate (OR 0.30, 95% CI 0.20 to 0.47; 503 participants; 2 studies; high evidence). We assessed 4 studies as having a low risk of bias and 1 study as having a high risk of bias. Conclusion: This systematic review suggests that East Asian traditional medicine interventions may be effective in preventing and alleviating complications, but we found evidence of important trade-offs between known benefits and known adverse effects in cardiac dysfunction and inflammation following heart valve replacement. Consequently, additional high-quality studies should be conducted.

고관절 치환술 후 추나치료 효과: 체계적 문헌고찰 및 메타분석 (Effects of Chuna Manual Therapy after Hip Arthroplasty: A Systematic Review and Meta-Analysis of Randomized Controlled Trials)

  • Objectives This study was conducted to assess the effect of chuna manual therapy after hip arthroplasty. Methods We searched across 9 electronic databases (PubMed, Cochrane Library, Wangfang data, China National Knowledge Infrastructure [CNKI], Oriental Medicine Advanced Searching Integrated System [OASIS], National Digital Science Library [NDSL], Korean Medical Database [KMBASE], Koreanstudies Information Service System [KISS], Research Information Sharing Service [RISS]) to find randomized controlled clinical trials for chuna manual therapy after hip arthroplasty. Results On inclusion criteria, 11 appropriate studies were included and analyzed. The deep vein thrombosis incidence of the chuna manual therapy group was statistically lower than the conventional treatment group (p=0.0002). Chuna manual therapy significantly improved the Harris hip score compared with conventional treatment (p<0.00001). Also, chuna manual therapy combined with herbal fumigation therapy significantly elevated the hip joint function score of Harris hip score compared with the conventional therapy group (p<0.00001). Conclusions The systematic review showed that chuna manual therapy had significant effects on hip arthroplasty. Nonetheless, considering the high risk of bias and geographic bias, further research with well-designed studies is required to support the effectiveness of chuna manual therapy.

수면장애에 대한 인지행동 치료 효과에 대한 체계적 문헌 고찰 개관 (Effectiveness of Cognitive Behavioral Therapy for Sleep Disorder: An overview of Systematic Review)

  • Objectives: The purpose of this overview was to summarize the evidence regarding the effectiveness of Cognitive Behavioral Therapy (CBT) for sleep disorders through systematic reviews (SRs) and meta-analyses (MAs). Methods: An overview of systematic review was conducted according to the study protocol (reviewregistry1320). A comprehensive literature search was performed using three databases (Pubmed, Cochrane Central Register of Controlled Trials, and Web of Science) and three Korean databases (KoreaMed, KMbase, and ScienceON). Final studies were selected by three authors according to inclusion and exclusion criteria, and data needed for analysis were extracted by a pre-planned extraction framework. Methodological quality of systematic review was assessed using the 'Assessment of multiple systematic reviews 2 (AMSTAR2)'. Results: Fourteen SRs and MAs were included, of which eleven SRs were performed MAs. Twelve studies studied insomnia among sleep disorders, and the rest are nightmares and sleep disturbances with PTSD. Ten studies reported the effect of CBT on sleep disorders measured by insomnia severity index (ISI) and sleep onset latency (SOL), and all reported a significant improvement effect. Eight studies reported the effect of CBT on sleep disorders measured by wake time after sleep onset (WASO), and seven studies reported a significant improvement effect. The methodological quality of the studies evaluated with AMSTAR 2 was mainly low or very low because of omission of protocol registration and excluded study list. Conclusions: Practical guidelines and studies show that CBT is effective for sleep disorders, but access to CBT needs to be improved.

Comparison of healing assessments of periapical endodontic surgery using conventional radiography and cone-beam computed tomography: A systematic review

  • Purpose: This systematic review aimed to compare assessments of the healing of periapical endodontic surgery using conventional radiography and cone-beam computed tomography (CBCT). Materials and Methods: This review of clinical studies was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) checklist. All articles published from 1990 to March 2020 pertaining to clinical and radiographic healing assessments after endodontic surgery using conventional radiography and CBCT were included. The question was "healing assessment of endodontic surgery using cone-beam computed tomography." The review was conducted by manual searching, as well as undertaking a review of electronic literature databases, including PubMed and Scopus. The studies included compared radiographic and CBCT assessments of periapical healing after periapical endodontic surgery. Results: The initial search retrieved 372 articles. The titles and abstracts of these articles were read, leading to the selection of 73 articles for full-text analysis. After the eligibility criteria were applied, 11 articles were selected for data extraction and qualitative analysis. The majority of studies found that CBCT enabled better assessments of healing than conventional radiography, suggesting higher efficacy of CBCT for correct diagnosis and treatment planning. A risk of bias assessment was done for 10 studies, which fell into the low to moderate risk categories. Conclusion: Three-dimensional radiography provides an overall better assessment of healing, which is imperative for correct diagnosis and treatment planning.

오픈 소스 기반의 거대 언어 모델 연구 동향: 서베이 (A Survey on Open Source based Large Language Models)

  • 최근 대규모 데이터 세트로 학습된 거대 언어 모델들의 뛰어난 성능이 공개되면서 큰 화제가 되고 있다. 하지만 거대 언어 모델을 학습하고 활용하기 위해서는 초대용량의 컴퓨팅 및 메모리 자원이 필요하므로, 대부분의 연구는 빅테크 기업들을 중심으로 폐쇄적인 환경에서 진행되고 있었다. 하지만, Meta의 거대 언어 모델 LLaMA가 공개되면서 거대 언어 모델 연구들은 기존의 폐쇄적인 환경에서 벗어나 오픈 소스화되었고, 관련 생태계가 급격히 확장되어 가고 있다. 이러한 배경하에 사전 학습된 거대 언어 모델을 추가 학습시켜 특정 작업에 특화되거나 가벼우면서도 성능이 뛰어난 모델들이 활발히 공유되고 있다. 한편, 사전 학습된 거대 언어 모델의 학습데이터는 영어가 큰 비중을 차지하기 때문에 한국어의 성능이 비교적 떨어지며, 이러한 한계를 극복하기 위해 한국어 데이터로 추가 학습을 시키는 한국어 특화 언어 모델 연구들이 이루어지고 있다. 본 논문에서는 오픈 소스 기반의 거대 언어 모델의 생태계 동향을 파악하고 영어 및 한국어 특화 거대 언어 모델에 관한 연구를 소개하며, 거대 언어 모델의 활용 방안과 한계점을 파악한다.

Influences of Aerobic and Resistance Exercise on Health-related Problems in Cancer Patients: A review of the literature

  • Purpose : Until recently, the number of cancer patients continues to increase, and these patients have many limitations in their activities of daily living. In the republic of Korea, cancer patients are showing an increasing trend every year. Cancer disease not only significantly reduces the quality of life in individuals, but also causes various side effects if not managed. The purpose of this study was to investigate the effects of aerobic exercise and resistance exercise on health-related problems in cancer patients. Methods : This study searched for studies that applied aerobic exercise and resistance exercise to cancer patients reported in search engines (google scala, dbpia, and pubmed) from 2017 to 2022. Six randomized controlled trials and two systematic reviews and meta-analyses were used for analysis in our study. "physical activity", "exercise", "aerobic exercise", "resistance exercise", and "cancer patient" were the main search terms. The data included aerobic exercise, resistance exercise, cancer patients' muscle strength, physical strength, quality of life, and physical activity. Results : According to the eight studies that met the criteria included in this review study, it was found that aerobic exercise and resistance exercise had an effect on the increase in physical fitness, muscular strength, quality of life, and range of motion in cancer survivors. Conclusion : It was confirmed that aerobic exercise and resistance exercise are safe and effective interventions that can be applied to cancer patients without side effects. A limitation of this study is that it did not examine cancer diseases in various population groups such as the elderly and children. Therefore, in future studies, studies that consider specific details such as age, gender, type of cancer, and physical differences are needed.

어깨관절 질환에 대한 관절가동술 유형의 연구 동향 : 주제범위 문헌고찰 (Research Trend of Joint Mobilization Type on Shoulder : A scoping review)

  • Purpose : This study sought to investigate research trends regarding joint mobilization type among patients with shoulder joint diseases. Methods : A scoping review was conducted according to the five steps outlined by Arskey and O'Malley and PRISMA-ScR. We searched six domestic databases (ScienceOn, DBpia, Riss, Kmbase, Kiss, KCI) and three international databases (CINAHL, Pubmed, Cochrane central) between 2013 and June 2023. The keyword terms used were 'joint mobilization', 'Kaltenborn', 'Maitland', 'Mulligan', and 'shoulder joint'. Results : There were a total of 44 studies that investigated the topic, and these were divided into quantitative analysis and topic analysis. In terms of publication year, the number of studies within the last five years has increased more than compared to the previous five years, with most of them being randomized clinical trials. In shoulder joint diseases, it was found that the majority of joint movement studies focused on adhesive joint cystitis and shoulder collision syndrome. The Mulligan concept was the most commonly studied type of joint motion. The dependent variables used included pain, joint function (disability), and muscle function. The visual analog scale was the most commonly used for the pain variable, followed by the numeric rating scale. For joint function and disability variables, range of motion was the most commonly used, followed by shoulder pain and disability index, and disabilities of the arm, shoulder, and hand. For muscle function, variables such as muscle tone, strength, and activity were used. Conclusion : We believe that findings of this scoping review can serve as valuable mapping data for joint mobilization research on shoulder joint diseases. Further studies including systematic reviews and meta-analyses based on these results are recommended.

Using machine learning to forecast and assess the uncertainty in the response of a typical PWR undergoing a steam generator tube rupture accident

  • In this work, a multivariate time-series machine learning meta-model is developed to predict the transient response of a typical nuclear power plant (NPP) undergoing a steam generator tube rupture (SGTR). The model employs Recurrent Neural Networks (RNNs), including the Long Short-Term Memory (LSTM), Gated Recurrent Unit (GRU), and a hybrid CNN-LSTM model. To address the uncertainty inherent in such predictions, a Bayesian Neural Network (BNN) was implemented. The models were trained using a database generated by the Best Estimate Plus Uncertainty (BEPU) methodology; coupling the thermal hydraulics code, RELAP5/SCDAP/MOD3.4 to the statistical tool, DAKOTA, to predict the variation in system response under various operational and phenomenological uncertainties. The RNN models successfully captures the underlying characteristics of the data with reasonable accuracy, and the BNN-LSTM approach offers an additional layer of insight into the level of uncertainty associated with the predictions. The results demonstrate that LSTM outperforms GRU, while the hybrid CNN-LSTM model is computationally the most efficient. This study aims to gain a better understanding of the capabilities and limitations of machine learning models in the context of nuclear safety. By expanding the application of ML models to more severe accident scenarios, where operators are under extreme stress and prone to errors, ML models can provide valuable support and act as expert systems to assist in decision-making while minimizing the chances of human error.

청소년의 비자살적 자해에 관한 연구동향 -국내학술지 중심- (Research Trends on Non-suicidal Self-Injury in Adolescents -Focusing on Domestic Academic Journals-)

  • 본 연구의 목적은 비자살적자해(Non-Suicidal Self-Injury: NSSI)와 관련한 논문을 분석하여 연구적 성과와 한계점을 검토하고 향후 연구 과제를 제시하는 데 있다. 연구에 필요한 분석 대상 논문은 자해 관련 문헌 연구가 시작된 2010 년부터 현재까지 국내학술지를 중심으로 분류절차를 거쳐 수집하였다. 분류범주(일반적 특성, 연구대상, 연구변인, 프로그램 활용)에 따라 분석한 결과 첫째, 비자살적자해와 관련한 국내연구는 2017년(3편) 증가하기 시작한 이후로 2019년과 2022년을 거쳐 꾸준한 증가 양상을 보이고 있다. 둘째, 연구대상의 특성에 변화가 나타나기 시작했다. 이전의 선행 연구들이 자해를 경험한 청소년이 주 대상이었다면 초기성인기(대학생)까지 확대되었고 자해 중단경험을 가진 대상과 상담자, 부모로 연구대상이 다양해졌다. 셋째, 다양한 연구방법들이 나타나기 시작했다. 양적연구가 활발하던 2017년에 비하여 다양한 질적 연구방법들을(내러티브, 현상학, 근거이론, 메타분석, 사례연구) 적용한 연구가 진행되었다. 마지막으로 연구 결과에 대한 논의와 후속 연구를 위한 제언을 추가하였다.

관점지향 프로그래밍 및 리플렉션 기반의 동적 웹 서비스 조합 및 실행 기법 (A Dynamic Web Service Orchestration and Invocation Scheme based on Aspect-Oriented Programming and Reflection)

  • 웹 서비스 조합 분야는 단일 서비스를 재사용하여 가치 있는 서비스를 생성하기 위해 등장했으며, 최근에는 차세대 웹 서비스인 시멘틱 웹을 구현하기 위해 IOPE를 기반으로 단순 검색 및 조합 대신에 규칙이나 AI를 통한 검색 및 조합 방법이 제안되고 있다. 또한 보다 효율적인 모듈화를 위해 기존의 객체지향 프로그래밍 방식보다는 관점지향 프로그래밍 방식이 도입되고 있다. 본 논문에서는 시멘틱 웹을 위해 관점지향 프로그래밍(Aspect-Oriented Programming, AOP) 및 리플렉션(Reflection)을 적용한 동적 웹 서비스 조합 및 실행 기법을 설계한다. 제안하는 기법은 웹 서비스의 메타 데이터를 동적으로 획득하기 위해 리플렉션 기법을 사용하고 아울러 동적으로 웹 서비스를 조합하기 위해 AOP 기반 접근방식을 통해 바이트 코드를 생성한다. 또한 리플렉션을 이용한 동적 프록시 객체를 통해 조합된 웹 서비스를 실행하는 방식을 제안한다. 제안하는 기법의 성능 평가를 위해 비즈니스 로직 계층과 사용자 뷰 계층 측면에서 조합된 웹 서비스를 검색하는 것에 대한 실험을 수행한다.
