반치환 슬관절의 설계 평가를 위한 굴곡각도에 따른 곡률반경과 접촉응력의 상관도 분석 (The Correlation Analysis through the Contact Stress and the Curvature Radius according to Flexion Angle for the Design of Unicompartment Knee Replacement)

  • Conformity between the femoral component and tibial insert within the knee replacement may be measured in frontal or sagittal view, and shows differences in the curvature radius of the femoral component depending on the flexion angle, i.e., curvature radius has a complex effect on contact stress. Therefore, it is essential to confirm how the curvature radius effects contact stress, and provide an important variable to reduce contact stress. This study correlated contact stress with curvature radius measured in frontal and the sagittal views and confirmed the effect of curvature radius for assessment of the Newly Designed Unicompartment Knee Replacement (NDUKR). Finite element models were constructed for NDUKR and $Zimmer^{(R)}$ Unicompartment High Flex Knee Replacement system (ZUKR), incorporating the curvature radius as measured in either frontal or sagittal view. The femoral component had 1200N of compressive load applied approximately 1.65xbody weight. Contact stress was predicted at flexion angles $0^{\circ}C$, $30^{\circ}C$, $60^{\circ}C$, $90^{\circ}C$ and $120^{\circ}C$, for NDUKR: 42, 47.7, 47.7, 51.2, and 54.1 MPa, and ZUKR: 41.2, 49.5, 53.2, 54.3, and 57.4 MPa, respectively. Correlation analysis showed the influence of curvature radius measured from the sagittal view was larger than for frontal view.

  • This paper presents the three dimensional gait analysis of the patients with osteoarticular knee allograft reconstruction. The gait analysis has been performed in some medical fields such as orthopedics and neurosurgery for the purpose of the rehabilitation of patients. However, to the author's knowledge, the analysis of gait for the patients with osteoarticular knee allograft reconstruction caused by tumor has not been reported. In this work, In this work, we confirmed the validity of this method by analyzing 50 samples per one gait cycle obtained from each of 3 patients and 3 normal persons. The motion capture was performed using six infrared cameras. The symmetry and stability of the gait patterns are investigated (patients' r=0.39, p<0.05, normal persons' r=0.65, p<0.05) respectively using the correlation coefficients and the standard deviations of the joint angles of the left and right legs. It also would be applied to the comparison analysis where artificial knee joint is transplanted.

Effects of Artificial Leg Length Discrepancies on the Dynamic Joint Angles of the Hip, Knee, and Ankle During Gait

  • PURPOSE: This study examined the dynamic range of motion (ROM) of the hip, knee, and ankle joint when wearing different shoe sole lifts, as well as the limb asymmetry of the range according to the leg length discrepancy (LLD) during normal speed walking. METHODS: The participants were 40 healthy adults. A motion analysis system was used to collect kinematic ROM data. The participants had 40 markers attached to their lower extremities and were asked to walk on a 6 m walkway, under three different shoe lift conditions (without an insole, 1 cm insole, and 2 cm insole). Visual3D professional software was used to coordinate kinematic ROM data. RESULTS: Most of the ROM variables of the short limbs were similar under each insole lift condition (p>.05). In contrast, when wearing a shoe with a 2 cm insole lift, the long limbs showed significant increases in flexion and extension of the knee joint as well as; plantarflexion, dorsiflexion, pronation, eversion, and inversion of the ankle joint (p<.05). Of the shoes with the insole lifts, significant differences in all ROM variables were observed between the left and right knees, except for the knee internal rotation (p<.05). CONCLUSION: As the insole lift was increased, more ROM differences were observed between the left and right limbs, and the asymmetry of the bilateral lower limbs increased. Therefore, appropriate interventions for LLD are needed because an artificial mild LLD of less than 2.0 cm could lead to a range of musculoskeletal problems of the lower extremities, such as knee and ankle osteoarthritis.

운전 숙련도에 따른 자동차 페달시스템 유형이 운전자세에 미치는 영향 (Effect of Driver's Posture with Different Car Pedal Systems and Skilled Levels)

  • The purpose of this study was to compare drivers' postures with different car pedal systems and skilled levels. Fourteen subjects participated in this experiment and for three-dimensional analyses, six cameras (Proreflex MCU-240, Qualisys) were used to acquire raw data. The parameters were calculated and analyzed with Visual-3D. In conclusion, the patterns of pelvic, hip, knee and ankle joint angles were different as to pedal systems and skilled levels, and distal joints ROA pelvis have large angles. ROA(range of angle) of a double pedal system was small, but ROA of a single pedal system was large. These findings suggested that we should improve a present single pedal system.

새로운 수술적응증에 의한 골 미성숙 전방십자인대 손상 환자에서의 성장판을 통과하는 전방십자인대 재건술의 결과 (Clinical Results of ACL Reconstruction in the Immature Adolescent via Transphyseal Approach in Tibia Based on a New Indication Paradigm)

  • 목적: 새로운 수술 적응증으로 선별한 성장판이 열려있는 전방십자인대 손상 환자에서 성장판을 통과하는 전방십자인대 재건술 후 임상적 및 방사선적 결과를 분석하고자 하였다. 대상 및 방법: 2005년 1월부터 2007년 1월까지 성장판이 열린 환자에서 성장판을 통과하는 십자인대 재건술을 시행한 환자 15명을 대상으로 평균 4.3년(4-6) 추시하였다. 수술 적응증은 역연령이 남자는 16세, 여자는 14세 이상이고, 성장판이 2 mm 이하로 열려있는 경우와 Risser sign과 성 성숙도(Tanner stage) 3 이상으로 설정하였다. 사용된 이식건은 모두 동종 전경골건를 사용하였으며, 대퇴골과 경골 부위 고정은 각각 endobutton과 bioscrew로 시행하였다. 기능적 평가는 Lysholm knee score scale과 Tegner activity level scale, International Knee Documentation Committee (IKDC) 2000 subjective score를 조사하였다. 이학적 검사상 안정성은 Lachman과 pivot shift 검사로 평가하였다. 방사선적 평가는 stress 영상에서 건측과의 전방 전위의 차이와, 최종 추시 시 scannogram상 양측간 하지부동정도, 대퇴경골각, 해부학적 및 역학적 외측 원위 대퇴각, 역학적 내측 근위 경골각의 차이로 하였다. 결과: Lysholm knee score scale은 술 전 평균 51점에서 술 후 최종 추시 상 97점으로, Tegner activity level scale은 술 전 평균 2.6점에서 술 후 7.1점으로, IKDC score는 술 전 평균 32.6점에서 술 후 88.3점으로 호전되었다. 건측과의 전방 전위의 차이는 술 전 평균 6.7(${\pm}1.0$) mm에서 술 후 평균 1.9(${\pm}0.9$) mm로 호전되었다. 골 변형을 평가하기 위한 모든 영상 지표들은 건측과 통계적으로 유의한 차이를 보이지 않았으며, Scannogram 상 하지부동은 평균 1.5 mm로 5 mm를 초과하는 하지부동은 관찰되지 않았다. 결론: 저자들의 새로운 수술 적응증으로 선별할 경우, 성장판이 열려있는 환자에서도 성장판을 통과하는 전방십자인대 재건술로 유의한 성장장애 없이 좋은 임상적 결과를 얻을 수 있을 것으로 사료된다.

자가 중력 지압에 의한 내반슬 교정 효과: 후향적 관찰 연구 (Effect of Self-gravity Acupressure on Varus Knee Correction: Retrospective Observational Study)

  • 배경: 자가 중력 지압 프로그램 참여 후 내반슬이 개선되는 사례가 개별적으로 확인되고 있으나 체계적인 관찰이 진행되지 못한 상태이다. 목적: 자가 중력 지압 프로그램을 실시 후 무릎 내측 각도의 변화를 후향적으로 관찰하여 자가 중력 지압 프로그램이 내반슬 교정에 미치는 효과를 평가하기 위함이다. 방법: 내반슬이 관찰되는 50명을 대상으로, 지압 도구 위에 누워서 75분간 지압 프로그램을 수행한 후 프로그램 실시 전후 무릎 내측 각도의 변화를 비교했다. 결과: 연구대상자들의 무릎 내측의 평균 각도는 지압프로그램 실시 후 좌측 7.29±3.85° (p < .000), 우측 7.08±3.86° (p < .000) 증가하여 연구 대상자의 내반슬이 유의하게 개선되는 것이 확인됐다. 결론: 본 연구의 결과는 자가 중력 지압 프로그램이 내반슬 교정에 활용될 수 있다는 가능성을 제시한다. 본 연구의 결과를 임상적으로 활용하기 위해, 자가 중력 지압 프로그램이 내반슬을 개선하는 효과에 대한 기전 및 임상적 효과를 최적화하는 추후 연구가 요구된다.

폴리우레탄 바닥재에서 방향 전환 각도에 따른 하지 관절의 움직임과 마찰력에 관한 연구 (The Study of Joint Motion and Friction on the Floor of Poly Urethane for the Cutting Movement of Various Angles)

  • Objective : The purpose of this study was to give the basic data for the cutting movement with the various angels on the poly urethane. Method : Ten healthy men voluntarily participated in this study. A three-dimensional motion analysis system (VICON) and force plates were used to analyze the movements of the joints for the lower extremities. For the statistical analysis the IBM SPSS 21.0 was used to perform repeated measured ANOVA and post-hoc comparison result was used to perform the Scheffe and the level of significance was set up at ${\alpha}=.05$. Results : There were significant differences for the time required for the increasing angles of the cutting movement(p<.05). In addition, there were significant differences for the maximum dorsiflexion, plantarflexion of ankle joint, maximum flexion of knee joint and hip joint with the increasing the angles of cutting movement(p<.05). Also, there were significant differences for the maximum adduction and abduction angle of the hip joint with the increasing of the angles of cutting movement. There was signigicant difference for the resultant utilized coefficient of friction(RuCOF) for the increasing angles of cutting movement(p<.05). Conclusion : There was a pattern to increase the coefficient friction with the angle of cutting direction. Also, it would be possible to use the poly urethane for the outdoor floor with the results of this study.

정상 성인과 편마비 환자의 보행분석 연구 (A Study on Gait Analysis of Normal Adult and Hemiplegia Patients)

  • The aim of this study is to present the basic reference data of age and specipic gait parameters for Hemiplegia Patients. The basic gait parameters were extracted from 30 Adult Hemiplegia Patients and 30 normal adult, 50 to 60 years of age using VICON 512 Motion Analyzer. The results were as follows; 1) The mean Cadence of the adult to the hemiplegia were $108.50\pm11.67$ steps/min, to $77.57\pm22.71$ steps/min. 2) The mean Walking Speed of the adult to the hemiplegia were $1.07\pm0.18m/s$, to $0.44\pm0.14m/s.$. 3) The mean Stride Length of the adult to the hemiplegia were $1.17\pm0.12m$, to $0.69\pm0.21m.$ 4) The mean maximal angles of joint on the pelvic tilt for different adult or hemiplegia Were $7.60\pm3.91.,\;to\;9.63\pm4.94.\;(P<0.05)$ 5) The mean maximal angles of joint on the hip flexion motion for different adult or hemiplegia were $29.53\pm5.03.,\;to\;25.30\pm9.94.\;(p<0.05)$ 6) The mean maximal angles of joint on the knee flexion motion for different adult or hemiplegia were $56.36\pm5.81.,\;to\; 41.64\pm17.21.(P<0.05)$ 7) The mean maximal angles of joint on the ankle dorsiflexion motion for different adult or hemiplegia were $16.65\pm2.72.,\;to\;16.53\pm7.45$(P>0.05) 8) The mean maximal angles of joint on the ankle plantarflexion motion for different adult or hemiplegia were $7.11\pm5.42.,\;to\;2.81\pm6.14.$(p<0.05)

좌.우측 편마비 환자의 3차원적 보행분석 (3-Dimensional Gait Analysis of Left or Right Hemiplegia Patients)
