흉부 전·후방향 검사 시 고관전압 및 산란선 후처리 소프트웨어 적용이 화질과 선량에 미치는 영향 (Effect of High Tube Voltage and Scatter Ray Post-processing Software on Image Quality and Radiation Dose During Chest Anteroposterior Radiography)

  • This study aims to present new chest AP examination exposure conditions through a study on the effect on image quality and patient dose by applying high tube voltage and scatter ray post-processing software during chest AP examination in digital radiography equipment. This study was used a human body phantom and in the chest AP position, the dosimeter was placed horizontally at the thoracic spine 6. The experiment was conducted by dividing into a low tube voltage (70 kVp, 400 mA, 3.2 mAs) group and a high tube voltage (100 kVp, 400 mA, 1.2 mAs) group. The collimation size (14″× 17″) and the source to image receptor distance(110 cm) were same applied to both groups. Radiation dose was presented to dose area product and entrance surface dose. Image quality was compared and analyzed by comparing the difference between the signal-to-noise ratio and the contrast-to-noise ratio of the image according to the application of the scatter ray post-processing software under each condition. The average value of the entrance surface dose in the low and high tube voltage conditions was 93.04±0.45 µGy and 94.25±1.51 µGy, which was slightly higher in the high tube voltage condition, but the dose area product was 0.97±0.04 µGy and 0.93±0.01 µGy. There was a statistically significant difference in the group mean value(p<0.01). In terms of image quality, the values of the signal-to-noise ratio and the contrast noise ratio were higher in the high tube voltage than in the low tube voltage, and decreased when the scattering line post-processing function was used, but the contrast resolution was improved. If there is a scatter ray post-processing function during chest AP examination, it is helpful to actively utilize it to improve the image quality. However, when this function is not available, I thought that applying a higher tube voltage state than a low tube voltage state will help to realize images with a large amount of information without changing the dose.

  • 본 연구는 자동노출제어장치 사용 시 관전압과 감도 변화에 따른 dynamic step wedge와 히스토그램의 동적 범위 분석을 통해 검출기의 성능을 평가하는 방법을 제안하고자 하였다. 실험방법은 관전압 60, 70, 81, 90 kVp 별로 검출기 감도 S200, S400, S800, S1000에 해당하는 선질, 입사표면선량, 관전류량, 동적범위를 측정하여 검출기의 성능을 평가하고자 하였다. 그 결과 선질은 모두 합격 기준에 부합하였고, 입사표면선량과 관전류량은 감도가 높게 설정될수록 단계적으로 감소하였다. Dynamic step wedge는 관전압이 높아질수록 관찰 가능한 동적 범위도 함께 증가하였고, 또한 히스토그램은 관전압과 감도가 높게 설정될수록 양자화 이격 현상과 히스토그램 양단의 정보량이 상실되는 언더플로우와 오버플로우가 나타났다. 결론적으로 검출기가 성능 저하로 인하여 자동노출제어장치 사용 시 관전압과 감도 변화에 따른 동적 범위를 제대로 구현해내지 못한다는 것을 알 수 있었고, 검출기 성능평가 시 dynamic step wedge와 히스토그램을 통한 평가가 유용하다는 점을 알 수 있었다.

유방 촬영검사에서 사전조사 관전압과 실제조사 관전압 편차에 따른 원인 분석 (Analysis of the cause by Pre Exposure Tube Voltage and Actual Exposure Tube Voltage deviation in Mammography Examination)

  • 본 연구에서는 유방촬영검사에서 사전조사 관전압과 실제조사 관전압 편차에 따른 원인분석을 유방압박두께, 유방크기, 체질량지수와 연관하여 규명하고 개선책을 찾고자 하였다. 국민건강보험공단에서 실시하는 유방촬영 검진자 중 40세 이상 여자 377명을 대상으로 조사하였다. 유방촬영검사에서 상하방향촬영에 의한 영상을 참고하여 의료영상저장정보시스템으로 전송되어진 선량 보고서(dose report)의 파라메타 중 사전조사 관전압과 실제조사 관전압의 편차에 따른 유방압박두께, 유방크기, 체질량지수를 분석하였다. 결과로는 유방압박두께가 얇을수록, 유방크기가 작을수록, 체질량지수가 작을수록 관전압 편차가 크게 나타났다. 결론적으로 유방촬영검사에서 유방압박두께와 유방크기에 따른 관전압 설정을 하기 위해 우리나라 실정에 맞는 유방촬영기기의 최소 관전압이 재설정 되어야 할 것이며, 또한 유방압박두께가 얇은 환자나 유방크기가 작은 환자를 검사할 경우 정확한 조사조건 매뉴얼을 만들어 검사함으로서 촬영조건의 편차를 줄여 방사선피폭 경감과 좋은 영상의 화질을 만드는데 노력해야 할 것으로 사료된다.

Effects of Physical Factors on Computed Tomography Image Quality

  • The purpose of this study was to evaluate the effects of X-ray photon energy, tissue density, and the kernel essential for image reconstruction on the image quality by measuring HU and noise. Images were obtained by scanning the RMI density phantom within the CT device, and HU and noise were measured as follows: images were obtained by varying the tube voltages, the tube currents and eight different kernels. The greater the voltage-dependent change in the HU value but the noise was decreased. At all densities, changes in the tube current did not exert any significant influence on the HU value, whereas the noise value gradually decreased as the tube current increased. At all densities, changes in the kernel did not exert any significant influence on the HU value. The noise value gradually increased in the lower kernel range, but rapidly increased in the higher kernel range. HU is influenced by voltage and density, and noise is influenced by voltage, current, kernel, and density. This affects contrast resolution and spatial resolution.

Feasibility of Using the Clamp Meter in Measuring X-Ray Tube Current

  • The clamp meter maintains electric safety as a non-invasive method while measuring the absolute value of tube current with it has been recently developed for an X-ray high-tension cable. Especially this can show high accuracy at short X-ray exposure time. Considering such a condition, this study is to evaluate the feasibility of a clamp meter in measuring X-ray tube current by taking the measurements and comparing with those of the Dynalyzer III which has been considered as a standard measuring device. From measuring the tube current accuracy depending on changes in tube voltage and exposure time, the clamp meter showed higher accuracy rate which was -1.3~4.2% difference. Thus clamp meter can be used for clinical radiologists who are not familiar electric circuit to manage X-ray devices easily and correctly in the future.

정류방식에 따른 장치의 정확도와 출력 파형의 비교 (Comparison of Accuracy and Output Waveform of Devices According to Rectification Method)

  • This study examined the following: accuracy of the exposure conditions in the inverter device and three-phase device; output waveform over the exposure conditions; and average and standard deviation of the output waveform. After assessing whether the dose corresponding to the theoretical dose was presented, the following conclusions were obtained: 1. The accuracy of the tube voltage(kVp) and tube current(mA) exposure time(sec) was within the tolerable level prescribed in Korea's Safety Management Standards. In the error, Inverter device was large the tube voltage and exposure time, the three-phase device was large the tube current. 2. In terms of the output waveform of the exposure conditions and the average and standard deviation of the output waveform, the higher tube voltage and larger tube current resulted in greater standard deviation in pulsation. Moreover, the standard deviation of pulsation was shown to be greater in the inverter device than the three-phase device; there was also greater standard deviation in the inverter device considering the exposure time. 3. Regarding the exposure conditions over the output dose, all linearity showed the coefficient of variation which had an allowable limit of error within 0.05. Although the output dose ratio for the inverter device was 1.00~1.10 times no difference that of the three-phase device, there was almost no difference in dose ratio between the tube currents.

  • 본 연구는 자동노출제어장치 사용 시 파라미터 조합에 따른 영상 화질과 방사선 출력을 분석하여 임상에 적용할 수 있는 최적의 방법을 모색하고자 하였다. 실험방법은 관전압 70, 81 kVp와 자동노출제어장치(Automatic Exposure Control, AEC)의 감도 S200, S400, S800, S1000을 조합해서 복부와 골반부의 입사표면선량, 관전류량, 신호 대 잡음비(Signal to Noise Ratio, SNR), 대조도 대 잡음비(Contrast to Noise Ratio, CNR), 시간-방사선량 곡선을 구하였다. 그 후 영상 화질과 출력의 안정성을 평가하였다. 그 결과 입사표면선량, 관전류량, 신호 대 잡음비, 대조도 대 잡음비는 관전압과 감도가 높게 설정될수록 감소하였다. 또한 관전압과 감도가 높게 설정될수록 시간-방사선량 곡선은 출력의 안정성이 줄어드는 양상을 보였다. 결론적으로 복부와 골반부 검사 시 관전압과 감도를 높게 조합할수록 검출기는 영상 화질과 방사선 출력을 정상적으로 재현해내지 못하였다. 따라서 비교적 낮은 관전압과 감도를 조합하여 검출기가 파라미터의 조합을 인식할 때 발생하는 오차 범위를 최소화해야 영상 화질과 방사선 출력의 안정성을 최적화할 수 있다는 것을 알 수 있었다.

비뇨기계 CT 검사 시 체질량 지수 적용의 유용성 평가 (Evaluation on Usefulness of BMI Application to Urological CT Examination)

  • 본 연구에서는 체질량 지수에 따라 각기 다른 관전압을 적용하여 비뇨기계 CT 검사 시 체질량 지수 적용의 유용성을 평가하고자 하였다. A그룹(n=38)은 체질량 지수가 25 이하이며 관전압 100 kVp로 검사하였고 B그룹(n=45)은 체질량 지수가 25 이상이며 관전압 120kVp로 검사하였다. C그룹(n=37)은 체질량 지수가 25 이하이며 관전압 120kVp로 검사하였다. 체질량 지수가 25 이하로 낮은 A그룹(100 kVp)과 C그룹(120kVp)의 두 그룹간 평균 선량의 차이는 $214.8mGy{\cdot}cm$의 차이를 나타내었으나 정성적 평가에서는 큰 차이가 없음을 확인할 수 있었으며 체질량 지수가 25이상인 환자군과 비교하여서는 오히려 좋은 결과를 확인할 수 있었다. 따라서 체질량 지수가 25 이하로 낮은 환자는 관전압을 100 kVp 로 낮추어 촬영하여도 영상의 질에는 부정적인 영향을 주지 않음을 알 수 있다.

  • This study was measured the radiation-induced current - X-ray dose, dose rate, X-ray quality, time, temperature, electric field characteristics and the dependence of gap length in insulating oil under of D.C. Voltage before, during and after X-ray irradiation. The obtained results can be summarized as following. 1. The radiation - induced current is more the dependence of X-ray quality (tube voltage) than quantity (tube current), the dependence of quantity is appeared at the high than low X-.ay tube voltage. 2. The dependence of dose rate is appeared at the more dose rate, and ${\triangle}\;=\;0.64{\sim}0.74$. 3. The higher temperature of insulating oil and X-ray tube voltage (X-ray quality) is increased, at the low electric field, the more radiation-induced current. 4. $G_{eq}-G_{o}(={\triangle}G)$ is increased at the low than high temperature, high than low X-ray quality. 5. The dependence of temperature is appeared before than during X-ray irradiation. 6. The RIC saturation region is appeared at the high than low insulating oil temperature during (1000 V/cm above) than before (4000 V/cm above) X-ray irradiation.

Tube Voltage, DNA Double-Strand Breaks, and Image Quality in Coronary CT Angiography

  • Objective: To evaluate the effects of tube voltage on image quality in coronary CT angiography (CCTA), the estimated radiation dose, and DNA double-strand breaks (DSBs) in peripheral blood lymphocytes to optimize the use of CCTA in the era of low radiation doses. Materials and Methods: This study included 240 patients who were divided into 2 groups according to the DNA DSB analysis methods, i.e., immunofluorescence microscopy and flow cytometry. Each group was subdivided into 4 subgroups: those receiving CCTA only with different tube voltages of 120, 100, 80, or 70 kVp. Objective and subjective image quality was evaluated by analysis of variance. Radiation dosages were also recorded and compared. Results: There was no significant difference in demographic characteristics between the 2 groups and 4 subgroups in each group (all p > 0.05). As tube voltage decreased, both image quality and radiation dose decreased gradually and significantly. After CCTA, γ-H2AX foci and mean fluorescence intensity in the 120-, 100-, 80-, and 70-kVp groups increased by 0.14, 0.09, 0.07, and 0.06 foci per cell and 21.26, 9.13, 8.10, and 7.13 (all p < 0.05), respectively. The increase in the DNA DSB level in the 120-kVp group was higher than those in the other 3 groups (all p < 0.05), while there was no significant difference in the DSBs levels among these latter groups (all p > 0.05). Conclusion: The 100-kVp tube voltage may be optimal for CCTA when weighing DNA DSBs against the estimated radiation dose and image quality, with further reductions in tube voltage being unnecessary for CCTA.
