Effect of Bee Venom Phamacopuncture Complex Therapy on Residual Symptom of Bell's Palsy After the Early Stage (봉약침 복합치료가 초기 회복지연에 따른 말초성안면마비 후유증에 미치는 영향)

  • Objectives : This study was to investigate the effectiveness of bee venom phamacopuncture complex therapy on the sequelae of peripheral facial palsy. Methods : We observed the effectiveness of bee venom pharmacopuncture complex therapy on three patients who have residual symptoms of Bell's palsy although early stage(0 to 3months) elapsed. H-B grade and Yanagiha's total score was used for evaluating the patient. Results : 1. Deviation of the bee venom therapy on the sequelae of peripheral facial palsy, One patient was improved from 19 to 39, another patient was improved from 25 to 40, the third patient was improved from 15 to 26 on Yanagiha's total score. 2. In the bee venom therapy on the sequelae of peripheral facial palsy, compared with baseline, at final, H-B grade and Yanagiha's total score was increased. Conclusions : Bee venom phamacopuncture can be available for relieving residual symptom of bell's palsy after the early stage.

Case Study of a Patient with Sequelae of Facial Palsy (안면신경마비 후유증 정안침 증례보고)

  • This study examines a clinical progress of treatment for the sequelae of facial palsy through Jung-ahn acupuncture. The patient in this case was diagnosed with facial paralysis a few years ago. The patient was treated with Korean medicine and Western medicine, but was given up without improvement. The paretic symptom was found out in left side of the face. Also facial spasm and epiphora caused by blepharoptosis were existed. The patient got 8 times Jung-ahn acupuncture treatment from September 18th, 2014 to September 26th, 2014. House-Brackmann facial nerve grading system(H-B scale) was implemented. On the first time of the treatment, H-B scale was Grade Ⅴ and facial nerve grading was 2/8. Facial spasm and epiphora caused by blepharoptosis in lower eyelid were appeared on facial expressions and conversation. After total 8 treatments(therapies), H-B scale was Grade Ⅲ and facial nerve grading was 5/8. The symptoms of facial paralysis and blepharoptosis were improved. Jung-ahn acupuncture is estimated to be effective in facial palsy sequela. More cases are required to develop treatment of facial palsy sequela.

Reviewing Recent Research on Facial Synkinesis of Bell's Palsy Sequelae - Pubmed Research - (Bell's palsy 후유증 중 facial synkinesis에 관한 최근 논문 경향 분석 - Pubmed 중심으로 -)

  • Objective : The purpose of this study is to analyze research trends on facial synkinesis after bell's palsy in order to build stepping stone in the hope of further related study in Korean medicine society. Methods : We searched Pubmed with the title "facial synkinesis" only developed after bell's palsy for the last 10 years and analyzed their impact factors, interventions, types of study, published countries, journals and years. Results : We found 12 papers and interventions included in each studies turned out to be effective and showed meaningful outcomes, but, the number of papers is still inadequate though, constantly published annually. And we were unable to find any studies with oriental treatments. Conclusions : Further attention and studies are needed on this topic in Korean medicine society in order to search appropriate oriental clinical care for the bell's palsy patients suffering facial synkinesis.

  • Objjectives : The purpose of this study is to investigate the correlation between surface electromyography(SEMG) and assessment scales for facial palsy. Methods : The subjects for this study were 21 patients who had peripheral facial nerve palsy. We analyzed about the correlation between the results of SEMG and facial nerve assessment scales that were carried out at the same time. The assessment scales used in this study were House-Brackmann scale, Yanagihara grading system, Sunnybrook facial grading system. Results : There are considerable correlation between the results of SEMG and the results of three kinds of assessment scales for facial nerve palsy. Conclusions : SEMG may be an objective examination for facial nerve palsy. Some research aimed at development of SEMG examination guideline for facial nerve palsy will be needed after this. And we expect that many researches for development of diagnostic equipments or treatment equipments that utilize SEMG will proceed.

  • Complete facial nerve palsy is emotionally and physiologically devastating condition. Management of patients with the facial nerve palsy poses significant challenges to achieve the goal of returning patients to their premorbid state. We experienced a case of iatrogenic facial nerve palsy in a 66 year-old patient, who was diagnosed as facial nerve schwannoma with previous incisional biopsy. The author describes the management of this patient and reviews the literature.

Cryopreserved fascia lata allograft use in surgical facial reanimation: a retrospective study of seven cases

  • Background: Facial palsy treatment comprises static and dynamic techniques. Among dynamic techniques, local temporalis transposition represents a reliable solution to achieve facial reanimation. The present study describes a modification of the temporalis tendon transfer using a cryopreserved fascia allograft. Case presentation: Between March 2015 and September 2018, seven patients with facial palsy underwent facial reanimation with temporalis tendon transfer and fascia lata allograft. Patients with long-term palsy were considered, and both physical and social functions were evaluated. The mean follow-up time was 21.5 months. No immediate complications were observed. Patients reported improvement in facial symmetry both in static and dynamic. Improvement was noticed also in articulation, eating, drinking, and saliva control. The Facial Disability Index revealed an improvement both in physical function subscale and in the social/well-being function subscale. Conclusions: This modified orthodromic technique allows to reduce the operative time and the risk of complications connected to the use of autologous tissues. The use of the cryopreserved fascia allografts from cadaveric donors seems to provide promising and long-standing results in the treatment of facial palsy.

Clinical predictive diagnostic study on prognosis of Bell's palsy with the Digital Infrared Thermal Image (적외선 체열진단법을 이용한 Bell's palsy의 임상적 예후 진단 연구)

  • The Background and Purpose : Most diagnostic method for the facial palsy were invasive and complex. And we don't know very well prognosis for the recovery of facial palsy in the first stage after the onset. But the Digital Infrared Thermal Image(DITI) isn't invasive and complex diagnostic method for the facial palsy. So we should study on the clinical prognostic diagnosis of Bell's palsy among facial palsy with the DITI. Objective and Methods : This study researched into the clinical statistics for 89 case who are in Bell's palsy, and they are treated with oriental medical care at the Woosuk university during 2 years form November 1998 to October 2000. All objectives have the Grade 6(Zero state) of Bell's palsy in first week after the onset. It takes a patient's facial temperature after the onset. Group A is taken from 1 day to 4 days after the onset. Group B is taken from 5 day to 8 days after the onset. And group C is taken from 9 day to 12 days after the onset. Results and Conclusions : The Digital Infrared thermal image technique showed the more high temperature, the more rapid cure and short treatment period on TE23, B2, S3, S6 in abnormal site of Bell's palsy. But it showed the more low temperature, the more rapid cure and short treatment period on TE17 of abnormal site of Bell's palsy. As a conclusion, we could think that the prognostic diagnosis of Bell's palsy closely related with the thermal difference normal and abnormal site of Bell's palsy that were took picture after the onset.

A Clinical Study on Children and Adolescents Who Visited the Hospital of Korean Medicine for 284-Peripheral Facial Palsy (한방병원에 내원한 소아청소년기 말초성 안면마비 환자 284례를 통한 임상적 고찰)

  • Objectives The purpose of this study is to investigate the clinical characteristics in children and adolescents with peripheral facial palsy that was treated with Korean medicine. Methods The study was conducted based on 284 cases that consisted of 261 children (less than 19 years old) who visited the hospital of Korean medicine from August 2010 to August 2014. Then, this study was analyzed by reviewing the patients' charts. Results The incidence of facial palsy in pediatric patients was more common in boys than in girls and the mean age was 11.2 years. The prevalence of facial palsy was more common in winter and most were diagnosed with Bell's palsy. Recurrence was identified in 12.6% of the patients. The average age of the first attack was 9.5 years. It took about 3.2 years for recurrence to occur from the first onset. It took about 5 days to visit the hospital of Korean medicine from the onset. 81% of the patients came within 1 week from the onset. 50.7% of the cases did not receive any treatment before and 49.3% were previously treated at other hospitals. More than half of the patients (65.1%) received Korean medicine first. 54.6% of the patients were hospitalized and 45.4% received outpatient treatments in the hospital of Korean medicine. Outpatient treatments were administered about 11.5 times while hospitalized patients were treated for an average of 16.6 days. In general, hospitalized patients were treated more often than the outpatient group. An overall average duration of treatment was 70.6 days from the onset of facial palsy until the last day of treatment, and 53.0 days from the first day of treatment until the last day of treatment. 41.5% of the cases were treated only with Korean medicine and 49.0% were treated with a combination of Korean and Western medicine. The types of Korean medicine treatment used for facial palsy in descending order of frequency were acupuncture, Infra red, herbal medicine, SSP therapy, electro-acupuncture, carbone, hot pack, cupping, and moxibustion. The treatments that were most frequently used from Western medicines were steroid as monotherapy (61.7%) and a combination of steroids and antiviral therapy (37.6%). Conclusions This result showed that the risk of facial palsy recurrence among children is relatively high and that facial palsy requires a long-term treatment. It's helpful to explain about prognosis, treatment duration, and recurrence potential in children. Facial palsy in children is treated in various ways but more studies about the therapeutic effects of Korean medicine, Western medicine, and combination of Korean-Western medicine in children are needed.

Study of the Relation of Idiopathic Facial Palsy and Imbalance of Autonomic Nerve System by the Heart Rate Variability Analysis (심박변이도(Heart Rate Variability) 분석을 통한 특발성 안면신경마비와 자율신경실조의 상관성 연구)

  • Objectives : This study investigated the effect of imbalance of autonomic nerve system on the idiopathic facial palsy by the comparison Heart Rate Variability results of Facial palsy group and healthy control group, and to clarify correlation between House-Brackmann Grade and Heart Rate Variability results. Methods : 119 idiopathic facial palsy patients and 88 health subjects who underwent HRV test were retrospectively reviewed based on medical records. We compared between the HRV results of facial palsy group and that of normal control group, and also compared the HRV results of facial palsy subgroup classified by House-Brackmann Grade. Results 1. All HRV results-Mean Heart Rat(MHRT), Standard Deviation of all the Normal RR intervals(SDNN), Total Power(TP), Very Low Frequency(VLF), Low Frequency(LF), High Frequency(HF), ratio betwween the Low Frequency and High Frequency power(LF/HF ratio) of facial palsy group are decreased compared to that of normal control group, especially SDNN, TP, VLF, LF, LF/HF ratio showed significant difference(p<0.05). 2. HRV results showed no significant correlation in House-Brackmann Grade. Conclusions : This study showed that lower HRV results of facial palsy group than normal control group and suggests that imbalance of autonomic nerve system related with facial palsy. HRV could be a objective tool to reflect condition of idiopathic facial palsy patients.

The Comparative Study on Oriental Medicine Treatments of Facial Paralysis Due to Mastoiditis and Bells Palsy

  • Objectives : The purpose of this study is to compare the improvement of facial paralysis due to mastoiditis and bells palsy. Methods : Fifteen patients with facial paralysis due to mastoiditis and Fifteen patients with bells palsy who visited the Daejeon Oriental Medical Hospital of Daejeon University were evaluated. Evaluation was carried out by Yanagihara's unweighted grading system. Results : 1. According to bells palsy and facial paralysis due to mastoiditis, in grade comparison between two groups according to cure period, there is no difference between the averages of both groups. 2. According to bells palsy and facial paralysis due to mastoiditis, theraputic effects are similarly increased between two groups. 3. According to bells palsy and facial paralysis due to mastoiditis, In cure score change according to cure period, there is grade difference according to cure period. Conclusion : With this result, when the same treatment is done to bells palsy and facial paralysis due to mastoiditis, there is the same theraputic significance.
