Horticultural Therapy Programs Enhancing Quality of Life and Reducing Depression and Burden for Caregivers of Elderly with Dementia

  • Background and objective: The problem that follows the increase of dementia patients is the burden of caregivers caring for dementia patients. The purpose of this study was to examine the effects of horticultural therapy programs improving the quality of life and reducing the depression and burden of caregivers of the elderly with dementia. Methods: In this study, 19 caregivers of the elderly with dementia were selected, and the experiment was conducted by dividing the control group (n=9) and the experimental group (n=10) by random distribution. The experimental group was given eight horticultural therapy programs twice a week for a total of 4 weeks. Subjects were assessed using the depression(CES-D), quality of Life (WHOQOL-BREF), and care burden scales. The evaluation results were verified at a 95% significance level using descriptive statistics, the Mann-Whitney U test, and Wilcoxon signed-rank test. Results: In the case of depression, the control group's score tended to increase, and the experimental group's score appeared to decrease, but it was not a statistically significant change. In the quality of life, the control group was not statistically significant, but scores decreased overall. On the other hand, in the experimental group, the general quality of life increased significantly from 11.60 to 14.20 points (p = .02), and the total quality of life increased to a marginally significant level from 61.59 points to 68.85 points (p = .059). In the post-test of the total care burden score, a marginally significant difference was found between the control group (94.44 points) and the experimental group (82.50 points; p = .079). Conclusion: This study confirmed the applicability to reduce the burden of caregiving and improve the deterioration of quality of life of the caregivers. In particular, the results will serve as an opportunity to confirm accessibility in a new way to support the caregiver of dementia patients by demonstrating the applicability of horticultural therapy at a time when problems such as the burden of supporting the caregiver are emerging as social problems.

The Effects of Care Stress and Internal and External Control on Depression in Caregivers of Elderly with Severe Dementia (중증치매노인보호자의 돌봄스트레스와 내외통제성이 우울에 미치는 영향)

  • Objective : The purpose of this study is to identify and understand the internal and external control of the caregivers of elderly with severe dementia and the effects of care stress on depression, and to provide basic data of occupational therapy intervention. Methods : The internal and external control, care stress, and depression were examined in questionnaires for the caregivers of elderly who were diagnosed with moderate or higher degree of dementia through CDR by medical institutions. Internal and external control and depression by care stress was analyzed by using independent samples t-test and ANOVA, and the effects of internal and external control and care stress on depression were analyzed by performing multiple regression analysis. Results : The results were as follow; The first, there was a significant degree of depression according to difference of internal and external control. The second, there was a significant degree of classification in the care stress. The third, the effects of internal controls and care stress on depression were shown to be statistically significant. Conclusion : Based on the analyzed results, the caregivers of elderly with severe dementia needs to seek the role of a occupational therapist who can enhance internal control and lower care stress in intervention of occupational therapy because internal control and care stress can affect depression.

A Study on Family Caregivers' Preferences for Utilization of Community-based Welfare and Health-care Services (요보호 노인 부양가족의 재가복지 및 재가보건 서비스 유형별 이용의향에 관한 연구 - 2002년도 서울시 노인복지 기초수요조사 자료 분석 -)

  • The purposes of this study are, 1) to explore the factors related to the family caregivers' preferences for service utilization both of the community-based welfare and health-care services, and 2) to examine the reasons why not want to use services analyzing a survey data obtained from family caregivers(n=1,000). Anderson and Newman's Behavior model was employed to examine the factors related to the preferences for service utilization. The main results are as follows. 1) Logistic regression analyses demonstrated that predisposing factor(such as age and relations with frail elderly) and enabling factor(such as economic status, secondary caregiver, informal informational support provider, etc) were significant predictor for caregivers' preference for service utilization. Contrary to an expectation, needs factor was negatively related to the preference for service. More specifically, the more they have service needs, the less they show their willingness to use community-based service both in welfare and health-care services except for care education program. 2) Caregiver identified 'family caregiving consciousness'(family should take care of frail elderly, elderly dislike be taken care of) as an important reason not want to use community services next to financial factor. These findings have several implications for policy making especially for 'public long-term care insurance' which was planned to start in 2007.

Prevalence and Characteristics of Informal Caregiving in Korea (한국 성인 남녀의 부양지원 제공 실태와 특성)

  • The purpose of this study was to describe 1) the prevalence of caregiving among adult men and women in Korea and 2) the characteristics of caregiving context. The main results were as follows: 1) About 1 in 6 adults reported that they provided care for the relatives and friends during the previous year. 2) The proportion of male to female caregivers was similar, indicating a higher prevalence of male involvement than previous studies indicated. Yet, the relationships to care recipients and types of care were found to be different by gender of caregiver 3) Adult caregivers are most likely to provide care for the elderly, but continue to provide care for younger persons as well. 4) A nontrivial proportion of the respondents reported providing care for more than one person, suggesting the need to pay special attention to these multiple caregivers.

Patient Safety Awareness and Emergency Response Ability Perceived by Nursing Homes and Home Visiting Caregivers (요양시설과 재가방문 요양보호사가 인식하는 환자안전관리의식과 응급상황대처능력)

  • Purpose: The purpose of this study was to identify patient safety awareness and emergency response ability and affecting factors perceived by nursing homes and home visiting caregivers. Methods: This study was a descriptive study that conveniently extracts nursing caregivers who care for elderly patients in S and G provinces, Korea. Data collection was done by structural questionnaires from April to May 2018. A total of 204 responses consisting of 103 nursing homes and 101 home visiting caregivers were used for data analysis in SPSS Win 22.0. Results: Patient safety awareness and emergency response ability of nursing homes caregivers with each $4.24{\pm}0.50$, $74.26{\pm}09.57$ was each higher than that of the home visiting caregivers with $3.68{\pm}0.49$, $68.02{\pm}12.12$ (p<.001). The affecting factors of the patient safety awareness were working place, safety education, and daily average working hours with 12 or more (F = 27.30, p<.001) and that of emergency response ability were number of patients per personnel with 9 or more and emergency situation experience (F=14.00, p<.001). Conclusion: These results suggest that it is necessary to develop a safety education program that can share indirectly experience emergency situations that occur on the job site.

Impact of an Integrated Infection Control Education Program on Caregivers in Long-Term Care Facilities (장기요양시설 요양보호사를 위한 통합적 감염관리 교육프로그램의 효과)

  • Purpose: This study aimed to develop and assess the impact of an integrated infection control education program on the awareness, attitudes, and performance of infection control among caregivers in long-term care facilities. Methods: Participants were recruited from two long-term care facilities with 25 caregivers in both the experimental group and the control group. This study used non-equivalent control group quasi-experimental pre-post design. The effectiveness of a developed Integrated Infection Control Education Program was evaluated based on infection control awareness, attitudes, and performance. Data were analyzed using SPSS/WIN 28.0 through descriptive statistics, chi-square tests, Fisher's exact tests, and independent t-tests. Results: There was a statistically significant difference in the degree of awareness (t=-5.00, p<.001), attitude (t=-4.91, p<.001), and performance (t=-6.66, p<.001) of infection control between the two groups. Conclusion: Given these results, the integrated infection control education program significantly improved infection control awareness, attitudes, and performance among caregivers in long-term care facilities. This study is noteworthy because it provided comprehensive education on infection control practices to caregivers in environments that are particularly susceptible to infections, especially following COVID-19. This educational program is actively utilized and validated in practice, it would enhance the infection control performance of caregivers, thereby reducing infection rates within facilities, shortening the length of stay for elderly residents in long term care facilities, and contributing to the reduction of healthcare costs.

An Analysis of the Difference between Importance and Satisfaction of Selection Attributes and Reuse Intention in Long Term Care Hospital for Elderly Patient Caregivers (노인요양병원 환자보호자의 병원 선택속성의 중요도와 만족도 차이 분석 및 재이용 의도에 관한 연구)

  • Advances in healthcare technology and rapid economic growth lead to the increased life expectancy and consequently the size of elderly population. Korea is one of the countries that are rapidly aging. Thus, it is particularly important to prepare for the aging society. Recently, the number of healthcare institutions for the elderly citizens has increased. The purpose of selecting a hospital for the elderly is, in general, maintenance of health rather than improvement of health receiving proper treatment. Unlike choosing a hospital for treatment, customers of a long term care hospital have a different set of factors to consider. Especially, when choosing a long term care hospital, the influence of patient's family is greater than the patient. This study examines the factors they consider for long term care hospital. A total of 198 questionnaires were collected from the families of actual patients of long term care hospitals. Twelve questionnaires were found to be non-usable because of missing and unsatisfactory responses. Consequently, 186 questionnaires were used for the analyses. Findings of this study are as follows. First, seven factors have been identified to consider when choosing a long term care hospital for the elderly. They include convenience of facilities, costs variety of facility programs, service hours, reputation, accessibility, quality of medical staff, medical facilities, and facility size. Second, This study measured both importance and satisfaction with these attributes and analyzed the difference between them. Satisfaction was lower than importance in the categories of convenience of facilities, costs, and programs, and accessibility. On the other hand, satisfaction was higher in terms of service hours, reputation, and quality of medical staff. Finally, the current study found positive impact of accessibility and quality of medical staff on reuse intention of a long term care hospital.

A Convergence Study on Depression of Main Caregiver for Elderly in Korean Nursing Home (우리나라 요양시설 입소노인 주 돌봄자의 우울에 대한 융복합 연구)

  • The purpose of this study is to investigate the depression of the main caregivers for the elderly in Korea by using a descriptive survey study. A total of two hundred eight participants who admitted at five nursing homes located in G city were analyzed using SPSS 25.0. There was a significant difference in depression according to general characteristics: higher level in extended family in family type. Participants showed significantly higher level of depression in cases of less than five years' hospitalization periods or Decision maker of elderly's hospitalization high level burden for expenses among caring related characteristics. They also had higher level of depression in cases of high level burden for expenses. less than 60 minute in commuting time, or non-satisfaction for nursing home service among caring related characteristics. Although we need more evidences through future studies, the findings from this study will be useful in the development of nursing intervention program for reducing the depression of the main caregivers for elderly population.

The Effect of Primary Caregivers' Guilt Feelings on their Request Behaviors for Help with Caring (부양자의 죄책감이 수발도움 요청행위에 미치는 영향)

  • Guilt feelings are dysfunctional feelings that the primary caregiver of the frail elder are apt to have and those feelings increase a burden of caring, while there is lack of empirical study on the effect of guilt feelings on caring behaviors. In light of this, this study lays its purpose on examining the effect of the primary caregivers' guilt feelings on their burden of caring and request behaviors for help with caring, paying attention to their guilt feelings in our society where Confucian values toward supporting the elderly have remained. The questionnaire survey was conducted for 220 primary caregivers caring frail elders over 60 years of age by visiting. As a tool for measuring guilt feelings, a self-designed measure for caregivers was used (${\alpha}=.949$), and factors of guilt feelings were classified into four namely, the factors of lack of self-control, lack of resources, burnout, and the normative factor As a result, the following findings were derived. First, it was revealed that the guilt feelings of caregivers as family members have a positive correlation with a feeling of burden of caring and the feeling of burden have even effects on the four factors of guilt feelings. Second, when primary caregivers request help with caring, they feel guilty toward cohabiting family members and neighbors, and also they show no guilt feelings when using day-care services for the elderly. Especially, guilt feeling factors affecting primary caregivers were found to be the normative factor to cohabiting family members, the factor of lack of resources to neighbors, and the factor of burnout to using day-care services for the elderly. This result tells that the dysfunctional feelings of primary caregivers namely guilt feelings arising when asking help with caring not only increase their burden of caring but also can cause difficulties in sharing the role of the caregiver. Accordingly for the mental health of caregivers, we should develop programs with which we could understand and cope with their guilt feelings.

Investigate the Factors that Affect the Safety Performance Ability of Patients by Caregivers (노인요양시설 요양보호사의 환자안전관리에 대한 안전수행능력)

  • The purpose of this research is to confirm the degree of awareness, knowledge and attitude for the safety management of patients in elderly care facilities, to investigate the factors that affect the safety performance of patients by nursing caregivers, and to provide basic material for the development of educational programs for strengthening performance. The research target audience was 142 nursing caregivers working in the G metropolitan city and S city elderly care facilities, and the data collected from 7/10/2020 to 7/28/2020, were analyzed with the SPSS/WIN 21.0 program. The ability of a nursing caregiver to perform patient safety is increasing as a result of higher education, higher patient safety awareness, and also more positive attitude towards patient safety. The major factors that affect the safety performance of patients by nursing caregivers were attitude toward patient safety, patient safety awareness, and the amount of participation in patient safety education. It's explanatory power was 26.9%. Based on the results of this research, we propose a study to confirm the positive effects of patient safety consciousness and attitude by confirming the education program completed by nursing caregivers and systematizing the education method and design in the education program of nursing caregivers.
