Expansion and Remodeling - The Case of Vocational Rehabilitation Center (증축공사에 대한 리모델링 - ○○○직업재활센터의 사례)

  • The Vocational Rehabilitation Center Expansion Project, with a budget of 15.3 billion KRW, transformed an existing facility into a more eco-friendly, user-friendly, and publicly accessible space. The project involved expanding the building from a basement and two above-ground floors to a basement and six above-ground floors, addressing the shortage of facilities for people with disabilities while promoting eco-friendliness. Design concepts included eco-friendly MASS, improvements to the existing building, social adaptability, and user-centric evacuation measures. Value Engineering (VE) and technical reviews led to proposals for cost reduction and functionality enhancement. Completed in September 2019, the project, renamed "Se-um Center," now hosts seven disability welfare facilities, serving as a vital infrastructure improving the lives of individuals with disabilities in the local community.

  • Under the Ministry of Health and Welfare of the Republic of Korea, the National Autism and Developmental Disorder Centers for people with developmental disabilities are gradually expanding. The headquarters of the National Autism and Developmental Disorder Center provides support for education, training, and research, and several centers have been effectively operating since 2020. This study aimed to provide practical recommendations and guidelines for specialists such as clinical psychologists, child psychiatrists, allied professionals, community workers, and related administrators. It was developed as a guideline to promote early diagnosis, provide important information on integrated treatment, and assist people with developmental disabilities in Korea to make the best decisions for their quality of life.

A Study on Development of the Job Functional Assessment Scale for people with Disabilities (장애인 근로능력평가 척도 개발에 관한 연구 -국민연금장애연금수급자를 중심으로)

  • The purpose of this study was to develop the Job Functional Assessment Scale. This study was involved took third steps. First, 93 preliminary items were developed based on theoretical literature and International Classification of Functioning, Disability and Health (ICF) Model. To verify the content validity, 3 rehabilitation experts rated those items. Second, the scale consisting of 73 items was administered to sample of 880 people with disabilities. Finally, items of the scale were reduced to 58 items. Factor analysis showed evidence of construct validity of the scale. Internal consistency (Cronbach's ${\alpha}$) of total score showed .94 and Cronbach's ${\alpha}$ of 7 subfactor demonstrated .74~.90. Thus, Job Functional Assessment Scale demonstrated satisfactory convergent validity and discriminant validity. In addition, the practical use of the scale was discussed.

Case Studies and Interviews to Review Public Library Services for People with Intellectual Disability (사례와 면접을 통해 본 국내 공공도서관의 지적장애인서비스)

  • This study focused on current status of Korean public library services for people with intellectual disabilities, as well as problems associated with managing the services, in order to suggest future directions for implementation. To analyze the current status, the researcher conducted a case study of current Korean public library services for people with intellectual disabilities. This research also conducted a focus group interview with librarians experienced in providing such services. The results show that the services are provided with the support of a community welfare center, in the form of a program over short periods of time. The librarians agreed on the appropriateness of the services but emphasized various difficulties associated with service management. Based on the results, we suggest possible points of consideration, including support for increased funding, more qualified personnel, adequate policies, and development of a model service for people with intellectual disabilities.

The effects of intensive gait training with body weight support treadmill training on gait and balance in stroke disability patients: a randomized controlled trial

  • Objective: The purpose of this study was to investigate the effects of intensive gait training with body weight support treadmill training on gait and balance in stroke disability patients. Design: Randomized controlled trial. Methods: Twenty-six stroke patients (20 men and 6 women) participated in this study. All subjects were hospitalized patients. They were randomly divided into two groups: the experimental group (body weight supported treadmill training group, n=14) and control group (treadmill group, n=12). The mean ages were 52.07 years (experimental group) and 53.83 years (control group). Subjects in both groups received conventional training 10 times/wk. Subjects in the experimental group practiced body weight supported treadmill training for 30 minutes a day, 3 day/wk. Subjects in the control group practiced treadmill training for 30 minutes. The Berg Balance Scale (BBS) and GAITRite were used to evaluate balance and gait parameters (step length, cadence and gait speed) before and after the intervention. Results: BBS scores in the experimental group showed significantly greater improvement ($4.33{\pm}1.54$), compared with the control group (p<0.05). Significantly greater improvement in the gait speed ($24.13{\pm}4.53$ cm/s), affected side step length ($10.40{\pm}3.42$ cm), sound side step length ($11.97{\pm}3.29$ cm), and cadence ($23.88{\pm}5.52$ step/min), compared with the control group (p<0.05). Conclusions: Intensive gait training with Body Weight Support Treadmill Training may improve gait and balance in subacute stroke.

The Medical Use of the Disabled Among Overusers of Medical Aid in Korea (의료급여 과다 이용 수급권자 중 장애인의 의료이용)

  • Objectives: In Korea, the top 10% of Medical Aid recipients represent nearly 60% of total payment, with the costs for those disabled for over 365 days representing approximately 30% of total payment. The purpose of this study was to compare Medical Aid use of the disabled with non-disabled recipients, and to identify contributing factors to the total payment in the top 2% of recipients identified as Medical Aid overusers. Methods: Subjects (n=2,211) selected were ${\geq}18$-years-of-age and received >1000 days of co-payment-free type I Medical Aid. Case managers (n=200) conducted interviews in December 2006, and collected data from Health Insurance Review & Assessment Service. Amounts over the 9 months from January September 2006 were analyzed descriptively and using Chi-square, ANCOVA, and robust multiple linear regression. Results: Disabled individuals (mean age 61.3 years) composed 36.6% of subjects; 44.8% of the disabled were male. On a monthly basis per capita, the disabled group averaged 10.5 outpatient days, total payment of 523,000 Korean Won(₩), inpatient payment of ₩359,000, and outpatient payment of ₩183,000. All values exceeded the monthly average for non-disabled individuals. Contributing factors were identified as male gender (₩82,000), elementary school or lower educational level (₩64,000), residence in a small city (₩82,000), lack of family support (₩61,000), kidney disability (₩673,000), intellectual disability (₩151,000), and multiple disabilities (₩119,000). Conclusions: The identification of contributing factors to Medical Aid use by those defined as disabled supports the adoption of comprehensive alternative policies such as strengthening of education and consultation services, provision of alternative facilities, and promotion of self-care.

Factors Affecting Medical Service Utilization of Disabled (장애인의 의료이용에 영향을 미치는 요인)

  • This article examined the condition of medical service utilization and the usage degree for the disabled in Korea, and analyzed factors affecting medical service utilization. This paper offers data for improving the health of disabled Korea residents and for enhancing medical service utilization. We analyzed data for disabled residents aged 19 from the 6th Korea National Health and Nutrition Examination Survey. Results found significant effects depending on gender, age, average monthly income, types of disorders, disability rating, and status of smoking. Concerning gender, men had a higher probability of lacking medical treatment compared with women, had increased probability of having a disability, and smokers had a higher probability of lacking treatment compared with nonsmokers. Therefore, for resolving medically untreated disabled Korean residents, government needs to improve the policy system and to managethe inequality of handicapped welfare work.

  • The purpose of this study is to figure out the stroller for baby with physical disability and to suggest improvement by using the usability test. Usability evaluation is separated by the user and the expert evaluation was conducted. Strollers for disabled users to analyze the evaluation of this experience using a guardian of a child with a disability was conducted targeting 30 people. The function and structure of the stroller easy to evaluate whether the five-point scale. Professional 10 strollers for disabled users with the problems of the design and structure were looking to make a suggestion. In this study, to obtain foreign products and differentiation, functionality, and ease of analyzing the conditions of satisfaction and competitiveness of domestic products will be able to contribute.

THE STATUS OF DENTAL CARIES EXPERIENCES IN KOREAN DISABLED PEOPLE (장애인의 치아우식 경험 상태)

  • The aim of the present study was to investigate the dental caries experience in disabled people according to the handicapped types in Korea. This study subjects based on the national disabled survey 2004 were collected by designed cluster sampling and interviewed with structured questionnaire in order to examine dental caries and sociodemographic status. The subjects were attending the special schools and private or public welfare institutions for crippled disorder, encephalopathy disorder, mental retardation, visual disturbance, and hearing defect. Oral examination was conducted by 13 trained dentists. 1,307 of handicapped people were finally analysed. ANOVA and Cochran-Mantel-Hanzel analysis were adapted for statistical analysis. The results were as follows; Dental caries prevalence, DMFT(Decayed, missing, filled teeth), and DMFS(Decayed, missing, filled surfaces) of disabled people were higher in crippled disorder and mental retardation than the others. There were significant differences in dental caries prevalence, DMFT, and DMFS according to the disabled. DMFT was significantly higher than national population in every age group except in 12-14 year-old and more than 35 group. It was likely that the experience level of dental caries was significantly different according to the handicapped types in Korea and higher than the people without handicap.

The length of hospital stay of the industrial workers with back injury (산업재해 요통근로자의 재원기간에 관한 연구)

  • Back injury is frequent in industry workers and is a common cause of productivity loss. It has been reported that the insured of industrial accident insurance tend to stay in hospital longer than that of other types of insurance. The purpose of this study was to identify factors affecting the length of hospital stay for the treatment of back injury in the workers under industrial accident insurance. The results of this study help insurers develop reasonable industrial accident insurance policy for back injury claims and prevention strategies of work-related back injury. A total of 2,949 patients whose industrial accident insurance claim has been approved for the treatment of work-related back injury from January to December 1999 were included in this study. Relationship between the length of hospital stay and characteristics of patient, work place, back injury, and hospital were assessed using ANOVA, t-test, simple linear regression and multiple resgression. The major findings of this study are as follows : 1. The average length of hospital stay(LOS) was 91.82 days, respectively. 2. Characteristics of Patient LOS of male patients was longer than that of female patients, there was positive correlation between age and LOS and between average wage and LOS. Working period was negatively correlated with LOS. Distance from resident to hospital was positively correlated with LOS and LOS was significantly different dependign on type of duty. 3. Characteristics of Work Place LOS was significantly different depending on types of industry and geographical region of work place. Size of work place was positively correlated with LOS. 4. Characteristics of Back Injury Occupational back pain required shorter LOS compared with back injury due to electric shock. Number of concomitant illnesses and severity of disability were positively correlated with LOS. 5. Characteristics of Hospital Patients treated in community hospitals required significantly longer LOS. Treatment in hospitals with rehabilitation program required decreased LOS. This was more prominent as number of physicians specialized in rehabilitation. 6. Multiple regression analysis revealed that distance form resident to hospital, geographical region of work place, size of work place, number of concomitant illnesses, severity of disability, and type of hospital were factors affecting LOS.
