국내 의료기관의 급식위생 현황분석 (Hygienic status of Korean hospital foodservice)

  • 국내 의료기관 96개소를 대상으로 이들의 급식위생관리 실태를 파악하기 위하여 2000년 3월부터 한 달간 총 96개 의료기관에 대해 해당병원 영양사를 대상으로 일반 위생관리 및 교육 프로그램에 대하여, 그리고 이들 중 35개소에 대해서는 급식을 위한 생산단계별 위생, 개인 위생, 급식설비 위생 분야로 구분하여 설문조사를 실시하였다. 조사에 응한 의료기관들의 급식위생관리 상태는 전반적으로 양호하게 나타났으나, 총 참여기관 중 병원급식에 HACCP을 도입하였다고 응답한 곳은 4%에 지나지 않았으며 문서화된 위생작업기준을 소지한 곳도 1/3에 지나지 않았다. 세부 항목별로 살펴보면 전체적으로 기록이나 문서화가 필요한 부분이 취약하게 나타났다. 의료기관의 종류, 소재지, 병상규모, 급식유형, 그리고 영양사의 업무분장에 따라 위생수칙 준수 율을 비교하여 보았을 때, 경우에 따라 차이는 있었으나 예상과 달리 종합병원이 3차 병원보다 양호한 결과를 보인 경우가 다수 있었으며, 소재지별로는 대도시에서 읍면지역으로 갈수록 취약하게 나타났다 또한 위탁급식 형태가 직영급식보다 대체적으로 양호한 위생수칙 준수 율을 보였고, 영양사의 업무가 임상영양서비스와 급식서비스로 구분되어 있을 때 그렇지 않은 경우보다 위생수칙 준수 율이 높았다. 위 설문조사에서 파악된 구체적인 문제점들은 추후 국내 의료기관의 급식위생수준 향상을 위한 지침 또는 프로그램의 개발에 유용한 기초자료가 될 것이다.

영양사 직무기술서 작성 연구 (Analysis and Framing of Dietitian's of Description)

  • This study was conducted as a part of the project developing the standards of national board test for the dietitian's licence. The purpose of this study was to define the job description of dietitians, and to describe the task elements based on the detailed analysis of the dietitians' work. This study team established the research team which was composed of food and nutrition experts, 7 of registered dietitians and 3 of university professors. Draft job description form was made to fully explain the dietitians' task elements, process and sequence by the research team. Final job description form was confirmed after the validity of 576 task elements was reevaluated upon 4 point scale test based on the frequency, importance, and difficulty by 21 field registered dietitian by the study team. It was also confirmed by the university. This form was formated by following DACUM method which analyzed the job description containing duty, task and task elements. It was organized to include every dietitians' work and to describe the every detailed process of work. Duty was defined as the specific and independent work as a dietitian, and was composed of 17 parts, i.e 10 parts in food supply management area and 6 parts in nutritional care service area and self promotion area. Duties were also divided into 99 tasks containing the 576 detailed task elements to describe definite action, steps and decisions. Seventeen parts are following; menu planning, storage and inventory control, food production, meal service, waste management, sanitation, equipment and facility management, human resource management, financial management, nutrition assessment of life cycle, nutritional assessment of several disease, nutritional assessment in specific condition, medical nutrition therapy, nutritional education, public health nutrition and self promotion. If these protocols are properly performed and presented, they can provide the likelihood that dietetics professionals work as a primary contributors in promoting health care and preventing disease nationally.

충청북도 지역의 학교급식과 산업체급식 영양사의 직무 수행시 식품영양학전공 교과내용의 활용도 평가 (The Evaluation on the Extent of Dieticians' Application of the Curriculum of the Food and Nutrition Department to their Work)

  • The purposes of this study were to evaluate the extent of dieticians' application of the curriculum of the food and nutrition major to their work and to suggest improvement measures for more efficiently applicable curriculum. A questionnaire survey of 552 dieticians at school food service and industry food service operations in Korea was conducted and 178 completed questionnaires were available for the purpose of the statistical evaluation. Statistical analyses were made of raw data by way of using the SPSS/win 7.2 program. The main results of this study were summarized as follows : Dietitian’s job could be categorized into 11 duties and 92 task elements. The application level was ranked at a level between 3 point and 4 pont(out of 5 scale) in the most area of the 92 task elements. Out of 11 duties, duty of sanitation management was applied at the highest level, while duty of nutrition education and duty of consultation management were utilized at the lowest level. Compared with the industry food service dieticians, dieticians at school food service operations employed the curriculum at the higher level. The effectiveness of experiments and practicum curriculum was ranked at the lower level than 3 point(out of 5 scale).

초등학생 급식 만족과 조리종사자 직무 만족간의 관계 분석 (Relationship between Students' Foodservice Satisfaction and Foodservice Employees' Job Satisfaction at Elementary Schools)

  • The purpose of this study was to explore the relationship between student's foodservice satisfaction and foodservice employee's job satisfaction at elementary schools. The survey was conducted on 5th and 6th grade students and foodservice employees at 19 elementary schools in Gwangju, Gyeonggi. Statistical data analysis was completed using SPSS ver. 17.0 for descriptive analysis, frequency analysis, independent sample t-test, and ANOVA. Students were highly satisfied with 'variety of menu' (3.78) and 'food taste' (3.75). The healthy group and no plate waste group showed significantly higher satisfaction levels on seven items, except 'sanitary utensil', as compared to others. Foodservice employees had high levels of satisfaction with human relationships and their jobs, but they were dissatisfied with their wages. Older employees had a high level of satisfaction with 'relationship with a dietitian' (P<0.05), whereas employees with a low level of education exhibited higher 'respect and reflection of his/her opinion on the duty' (P<0.01) and 'current duties' (P<0.05) as compared to others. The group with higher student satisfaction showed significantly higher employee job satisfaction for 11 items, including 'cooperation with co-workers' (P<0.01), 'relationship with a dietitian' (P<0.05), and inversely, the group with higher job satisfaction exhibited significantly higher student foodservice satisfaction for all nine items. Therefore, foodservice satisfaction and job satisfaction have a mutually positive influence on each other.

영양교사의 자질 및 업무에 대한 인식도 분석 (The Perception of Nutrition Teacher's Competency and Duty)

  • The purposes of this study were to identify the significance of a nutrition teacher, to analyze the importance of competence as a nutrition teacher and to evaluate the importance and difficulty for duties of nutrition teachers. The questionnaire was distributed to 554 preliminary teachers and a total of 468 usable data were collected with 84.5% of response rate from August to October 2006. The statistical data analysis was completed by using SPSS Win (ver. 10.0) for descriptive analysis, t-test, ANOVA and ${\chi}^2-test$. The results were as follows: Preliminary nutrition teachers expected that conversion of a school dietitian into a nutrition teacher could lead to systematic and constant nutrition education and come into synergic effect by doing meal service with nutrition education. For a teacher's competence, all three paradigms were perceived as important. It was evaluated to be high for a knowledge factor in the empirical analytic paradigm, a personality factor in the practical interpretive paradigm and an attitude factor in the critical emancipatory paradigm. And 'a knowledge for nutrition and sanitation (4.59)' and 'an ability to raise a self-management capacity for right food habit and dietary life (4.59)' were showed as most important in a nutrition teacher. It appeared that both important and difficult duties of nutrition teachers were nutrition education, nutrition counseling and sanitation quality of nutrition education; the most necessary solution was to develop a reference and program for nutrition education (37.4%). In conclusion, the systematic supports of district offices of education and associated institutions as well as sustained efforts of preliminary nutrition teachers for themselves should lead to establish a successful system of nutrition teachers.

경상북도 지역 국민학교 공동관리 급식 실태 및 영양사 직무만족도 평가 (Evaluation of the Common-management Foodservice System of the Elementary Schools in Kyungbook Region)

  • This study is to seek methods for optimum management of foodservice system for elementary schools in Korea through surveying and analyzing the current status of the "common-management foodservice system" and certain important factors relating to its operations. For the survey of the current status of the "common-management foodservice system", questionnaires are prepared as against schools adopting "common-management system"("Common Schools") as well as schools adopting "conventional management system" ("Conventional Schools") in Kyungsangbookdo ("Kyungbook") area. The survey items are composed of general items, items concerning tasks of dietitians, items concerning purchasing activities and items concerning level of recognition and job-satisfaction of the dietitians. Total 46 dietitians from Common Schools and 65 dietitians from Conventional Schools in Kyungbook region responded the questionnaires. The results of this study are as follows: 1. Information Concerning Dietitians Average age of the dietitians: 26.6 yrs Ratio of two years or less of experiences as school dietitian: Dietitians from Common Schools: 59.1% Dietitians from Conventional Schools: 50.8% 2. Types of the Foodservice In both schools, rural type are prevalent; however, between urban types and islands types, the ratio of urban types are larger than the ratio of islands types in Conventional Schools, and the opposite is true in Common Schools. 3. Decision of Suppliers The ratio of dietitian's participaion in the decision making reaches only 38.3%, indicating the possibility of problems in quality control and hygienic aspects of the food supply. 4. Factor Analysis of the Foodservice Duties Food supply and general affairs administration work was carried out very well (4.2), while personnel management and operational improvement work (2.8), nutritional education work (2.3), and information related work were poorly carried out. No significant differences were found between the two types of the foodservice management systems. 5. Job Satisfaction scores of Dietitians Dietitians were more satisfied with the works (50.0), supervision (50.5) and co-workers (46.9) than pay (18.0) and promotion (22.3).

학교급식 영양사의 식행태, 업무소요시간 및 피로도에 과한 실태조사 -대전.지역충남의 급식방식별 비교 - (Dietary Behaviors, Consumed Time for Job Duties and Fatigue Levels of Dietitians by School Foodservice Type in Taejon and Chungnam)

  • The school foodservice(FS) dietitians of conventional, co-management and central kitchen systems were compared for their levels of fatigue degrees in order to examine their work specification but no to diminish their educational roles. One hundred and thirtheen primary schools in Taejon and Chungcheongnam-Do participated in the survey, of which foodservice were consisted of 99 conventional types, 7 comanagement types and central kitchen types. Information was collected about the various working hours dietary behavior and fatigue symptoms of the dietitians. The results of this study were as follows : Among 3 school foodservice types, dietitians spent 8 hours and 16 minutes daily for their for their work on the average. However, the central kitchen type dietitans in cities spent 3 hours more compared to the others. The data from the dietitians' work schedule sheets showed that the 5 parts of dietitian's duties set in the school foodservice laws were not well performed. In the central kitchen type they spent more time for food production whereas in the conventional they spent more time for menu formation and hygiene control. It was found that fatigue levels of school foodservice dietitians were chronic with no difference among foodservice systems or among school locations. Our results indicated that nutrition education in primary schools was almost neglected. It is suggested that the school foodservice should meet all the objectives of school-lunch program even though they saved dietitians' work load more or less. 학교급식소와 사업체급식소 영양사 직무의 중요도와 수행도 및 직무만족도 비교연구 (A Comparative Study of Job Importance, Performance Level, and Job Satisfaction of School and Office Foodservice Dietitians)

  • This study was conducted to analyze job importance, performance level, and job satisfaction of school and office foodservice dietitians. Self-administered questionnaires were collected from 181 dietitians in the Chungnam area. Statistical data analysis was completed using SPSS v. 14.0. School dietitians were 55% and office dietitians were 45%. Of both, age of 20s and 30s scored 85%; the age of 30s of schools dietians scored 70% while those in office dietians 20s scored 79%. More than 84% of school and office dietitians were university graduates. Total average of job importance of school dietitians was 4.24 and job performance was 3.97 while that of office dietitians was 4.15 and job performances was 3.69. A duty that had a big difference in job importance and performances as a dietitian was 'education of nutrition' (p<.001) and 'self-improvement' (p<.001). According to the meal service except, importance and performances of duties showed a significantly high level (p<.001) for office dietitians, but most measures of job importance and performances showed a high level at school. Therefore, in order to improve job performances and satisfaction for school dietitians, it is necessary to develop systematic and professional education programs for qualification improvement and to grow pride and improve treatment as a nutrition teacher through expansion of appointment. Meanwhile, for office dietitians, it is required to strengthen duty education of 'human resource management' and 'meal service management' through education and supplementary training in companies, on which personal characteristics are reflected.

영양교사의 영양교육 직무설정을 위한 초등학교 영양사와 학생의 영양교육에 관한 인식도 조사 -대구.경북 지역을 중심으로- (Perception of the Elementary School Dietitians and Students on Nutrition Education to Set up the Roles of Nutrition Teacher -Centered on Daegu City and Gyeongbuk Province-)

  • The purpose of this study was getting information to set up the new roles focused on nutrition education for school dietitians as nutrition teachers. One hundred thirty nine school dietitians and 1169 elementary school children residing in Daegu city and Gyeongbuk province were surveyed for this study. Sixty eight percent of the school dietitians perceived 1st-3rd year of the elementary school is the most proper time to start nutrition education, and 59.0% of them wanted to practice nutrition education as a discretion teaching time. The largest proportion(79.1%) of the school dietitians expected that nutrition education is helpful to get good dietary habit. School dietitians responded that major contents that should be included in the nutrition education was balanced diet, diet and habit, managing healthy weight, dining etiquette, food safety and problems of environmental contamination. Fairly large proportion of the students(64.4%) responded nutrition education is urgently needed. The contents of the nutrition education students wanted most were cooking and healthy diet. Forty six percent of the students perceived school dietitians are responsible for nutrition education and they wanted to have nutrition education as a part of special activity class. The most preferred frequency of nutrition education was 1 hour/week and 46.2% of the students responded they wanted to participate cooking camp.

초등학교 영양교사 직무설정을 위한 직무중요도 및 난이도 조사 (A Survey on the Perceived Importance and Difficulty to Set up the Job Duties of Nutrition Teachers in Elementary School)

  • The purpose of this study was to investigate the needs of educators, learners and parents in elementary schools for implementing a systematic nutrition education program by nutrition teachers to begin in 2006. The subjects were 833 dietitians, 808 principals, 3,141 teachers, 7,577 students and 6,003 parents in elementary schools. A fairly large proportion of the principals (98.4%), teachers (95.5%), parents (96.6%) and students (62.8%) responded that nutrition education is urgently needed. Every dietitian, principal and teacher has recognized that for the enforcement of a nutrition program in the future, the placement of one nutrition teacher in every school is needed without a moment's delay. Many subjects responded that elementary low grades are the most proper time for nutrition education for students and it is also needed to extend this not only to students, but also parents and teachers who have a direct influence on them. In the education hour, they responded that it is proper one hour a month for teachers and one hour a week for students. Fifty-six percent of dietitians, 58.3% of principals and 67.0% of teachers responded that the best educational way to heighten the effect of nutrition education was education through discretional activities. It was found that 46.4% of students have wanted nutrition education to be enforced during special activities. Education through their school homepage or in-school broadcasting system, while it has been most utilized, was low in a degree of preference with 10.9%. In regards to a nutrition program, principals and parents have attached much importance to the dining etiquette, dietary attitudes, and relations of foodstuffs with health, while dietitians and teachers have made much of a balanced eating habit, and the relations of dietary life with health. Finally, for a nutrition program to be enforced towards a direction that the educators and students want, it is judged that first of all the role of nutrition teachers is the most important, furthermore the support of manpower and the budget should be made.
