  • The stability and retention of mandibular complete denture have been a continuing problem for dentists and patients. Magnets have been investigated or used in clinical dentistry as retentive aids. The obvious advantages are that these attachments do not need spacial equipments or require frequent adjustment of replacement with use, and they likely transmit less lateral forces to the abutment.   • Fiber-reinforced materials have highly favorable mechanical properties. and their strength-to-weight ratios are superior to those of most alloys. When compared to metals they offer many other advantages as well. including non-corrosiveness. translucency. good bonding properties. and ease ofrepair. Fiber-reinforced materials can be categorized to pre-impregnated. impregnation required. dental laboratory products. chairside products and prefabricated posts. so it is not suprising that fiber-reinforced composites have potential for use in many applications in dentistry. Manufacturing of metal-framework for maxillary removable partial dentures using milling wax-blocks (Wax-block milling을 이용한 상악 가철성 국소의치 금속구조물 제작 증례)

  • This research introduces the manufacturing process of the metal-framework of one of the maxillary partial dentures, the "obturator", using milling wax-blocks, for patients with palate loss due to oral cancer. It explains the protocol of taking the patient's oral impression, preparation of a working cast, scanning, designing using a computer-aided design program, investing the milling wax-blocks, and completing the obturator. This method does not follow the traditional wax and agar process thereby reducing the errors arising during the manufacturing process and decreasing the time, material, and labor required. Moreover, the retention, stability, and compatibility of the metal framework were observed to be high in both the working cast and oral cavity.

  • Maxillofacial defect comes from congenital defect, trauma and surgical resection. Patients with intraoral defect are commonly related to maxillary defect and they need prosthetic rehabilitation. Functional reconstruction of partially edentulous mandible has many limitations. However, if both condyles are intact, maxillofacial prosthesis using partial denture give competent results. In this case, a patient of 58 year-old male has a defect on palate and left mandibular posterior teeth from gunshot. The maxillary defect of this patient is Class IV according to Aramany classification and the mandibular one is Type V according to Cantor and Curtis classification. For retention of the obturator, remaining teeth are fully utilized and artificial teeth are arranged harmoniously to provide stable occlusion. Mandibular RPD covered limited range of deformed soft tissue derived from mandibular resection surgery. With these treatments, the patient in this case showed improvements in mastication, swallowing and speech.

  • Adoption of CAD/CAM milling in dentistry has allowed production of more analytic and precise prosthesis. Such precision enables definite path of insertion and undercut to be provided in removable partial denture especially when designing a surveyed crown on an abutment tooth. This case is about the production of surveyed crown with proper path of insertion utilizing CAD/CAM electronic surveying method on a patient with edentulous maxilla. Resulting removable partial denture fit well and showed acceptable stability and retention with no clinical problem.

A STDUY ON THE SURFACE MORPHOLOGY AND BOND STRENGTH OF DENTURE BASE RESIN TO COBALT-CHROMIUM ALLOY AFTER VARYING MODES OF SURFACE TREATMENT (COBALT-CHROMIUM 합금의 표면처리방법에 따른 레진 의치상의 접착강도 및 표면상태에 대한 연구)

  • The purpose of this experiment was to determine the effects of various treatments on denture base resin to metal bond for cobalt-chromium alloy. The metal surface was treated as follows. Group 1 : Sandblasted with $50{\mu}m$ aluminum oxide. Group 2 : Sandblasted with $250{\mu}m$ aluminum oxide. Group 3 : Sandblasted with $250{\mu}m$ aluminum oxide and followed by silicoating. Group 4 : Electrochemically etched. Group 5 : treated with oxidizing solution. Group 6 : Beaded with $200{\mu}m$ retention structure and followed by silicoating. All specimens were applied with 4-META resin and were thermocycled 1000 times at temperature of $5^{\circ}C$ to $55^{\circ}C$. The effects of various surface treatments on the bond strength between 4-META resin and metal interface were measured by using the universal testing machine. All specimens were observed with SEM. The results were as follows 1. The bond strength of 4-META resin were significantly higher to Co-Cr alloy. 2. The bond strength decreased in the following orders : group 6, group 3, groups 1 and 2, group 4, group 5 and there was no statistically significant difference in bond strength among groups 1 and 2.(p>0.05) 3. The bond strength of cobalt-chromium alloy to 4-META resin were not significantly different.(p>0.05) 4. The treated surface of groups 1, 2 and 3 has more fine undercut than that of groups 4 and 5 with SEM. 5. Stable adhesion can be achieved when mechanically roughened metal surface by sandblasting than treating in an electrochemical etching and an oxidizing solution with potassium manganate.

  • In the case of failed fixed implant prosthesis accompanied by abutment screw fracture, fractured screw fragment must be removed to use the existing implant fixtures. A 61-year-old male patient, who had a failed maxillary fixed implant prosthesis accompanied by three abutment screw fracture, hoped to reconstruct the maxillary implant prosthesis, while maintaining the existing implant fixtures. To use the existing implant fixtures, fractured screw fragments were removed. A maxillary implant overdenture using available existing implants was planned. Bar-attachment with Locator was used for implant splinting, denture stability, and retention. Final impression was taken after treatment of peri-implantitis. Jaw relation registration was taken to evaluate available interarch space for bar-attachment. After fabricating bar-attachment, centric relation was taken. Implant overdenture using bar-attachment with Locator was delivered after wax-denture evaluation. This case report showed that a satisfactory clinical result was achieved by implant overdenture using existing implant fixtures in a maxillary edentulous patient.

  • Ectodermal dysplasia is a hereditary disease characterized by congenital dysplasia of one or more ectodermal structures. Intraorally, common findings are anodontia or oligodontia, conical teeth, and, consequently, generalized spacing. This case presented the oral rehabilitation of a child with hypohidrotic ectodermal dysplasia. Oral rehabilitation is important from functional, esthetic, and psychologic perspectives. Due to the absence of teeth, the volume of alveolar bone and its growth are decreased, resulting in a loss of vertical dimension and protuberant lips. The treatment involved increasing the patient's vertical dimension of occlusion, fabricating a maxillary partial denture, and using magnets to help retain the mandibular partial denture. A 5-year 7-month old Korean boy was referred to the pediatric department for examination, evaluation and treatment of his disorder. we used magnets on '73 and '83 for enhanced retention of a mandibular overdenture. The magnet used in this case was the Magfit system(GC Co., Japan).

  • Unlike class I patients, skeletal class II patients have unstable occlusion thus leading to instability of mandibular complete denture. Therefore, mandibular implant overdenture has been the standard of care due to its advantages in stability and retention. The types of attachments can be divided into two categories: solitary and bar type. The indications vary between two categories. In this clinical report, digital technology was utilized from the implant planning to the choice of appropriate attachment. Implants were placed at the desired location as previously planned in terms of angle and depth. Maxillary removable partial denture and mandibular implant overdenture are expected to have fair prognosis.

  • In patients with myasthenia gravis, it is difficult to manufacture and maintain dentures because the muscles and nervous system in the oral cavity are not properly adjusted. In addition, excessively extended or thick dentures may cause muscle weakness by stimulating the muscles, so dentures should be manufactured in consideration of this. In this case, a denture was fabricated using the closed mouth technique in a patient with myasthenia gravis with mouth opening limitation. Using the closed mouth technique, the patient's masticatory pressure and muscle movements were reproduced and denture with good retention were manufactured.
