설태의 자외선 형광 반응을 이용한 설태 영역 추출 (Coated Tongue Region Extraction using the Fluorescence Response of the Tongue Coating by Ultraviolet Light Source)

  • 본 논문에서는 한방 의료의 설진에서 진단 지표로 활용될 수 있는 효과적인 설태 영역 추출 방법을 제안한다. 제안한 방법은 설태의 자외선 광원에 의한 형광 반응 특성을 이용하여 기존의 설태 추출 방법의 단점으로 지적되었던 진료 환경의 제약성 및 진료 결과의 객관성 부족에 대한 문제점을 해결할 수 있다. 처리 과정으로는 자외선 광원을 사용하여 설진 영상을 획득하고, 설질(Tongue body)과 설태(Tongue coating) 영역의 색차 크기에 상응하는 히스토그램(Histogram) 상의 골-포인트(Valley-points)를 임계 처리하여 이진화(Binarization)를 수행한다. 최종적으로 설진을 위하여 한의사에게 제공되는 진단 영상은 이진 영상에 케니-에지(Canny-Edge) 알고리즘을 사용하여 설태 윤곽 정보를 추출한 후에 환자의 원 혀 영상에 부과하여 제시한다. 제안한 방법의 성능 평가를 위해서는 다양한 혀 영상을 수집하고, 한의사가 수작업으로 설정한 설태 영역을 참영상(True image)으로 하여 제안한 방법으로 추출한 설태 영역과 비교하였다. 그 결과 제안한 방법은 87.87%의 추출률을 나타냈으며, 추출된 설태 영역의 형태 유사도도 높게 나타났다.

성인의 구취 자각도 실태와 관련요인 (Relationship self-perceived halitosis and related factor among adults)

  • Objectives: The purpose of this study is to analyze the factors affecting the self-perceived halitosis of adults and to utilize them as basic data for the development of prevention and management program to reduce of self-perceived halitosis. Methods: A questionnaire survey of 301 adults in their 20s and 50s living in Gwangju and Jeonnam selected by convenience sampling from July 1, 2017 to August 31, 2017 was conducted to investigate oral health status and behavior, fatigue and perceived stress. Statistical analysis was performed using frequency analysis, independent t-test, one way ANOVA, Pearson's correlation analysis, and multiple regression analysis. Results: The degree of self-perceived halitosis was higher in married (2.51) and unemployed (2.71), with tongue brushing (2.68), intention to participate in halitosis education (2.57), taking medication (2.73), coated tongue (2.82) and there was both otolaryngology and gastrointestinal disease (2.72) and periodontal disease (2.89) (p<0.05). Fatigue (${\beta}=0.237$), periodontal disease (${\beta}=0.196$), and coated tongue (${\beta}=0.237$) affect the self-perceived halitosis (p<0.001). Conclusions: The self-perceived halitosis in adults was correlated with fatigue and perceived stress. Also fatigue, periodontal disease, coated tongue factors were analyzed as factors influencing self-perceived halitosis. Additional studies such as prevention and management of periodontal disease and coated tongue, which is a factor of self-perceived halitosis in adults, and development of programs to reduce self-perceived halitosis for fatigue management, which is an external factor, are necessary.

설태의 형광특성 - 설태 형광현상의 발현기전 소개 및 제안 - (Fluorescence of Tongue Coat - Mechanism of Tongue Coat Fluorescence and Suggestions on Development of Digital Tongue Diagnosis System -)

  • In traditional Korean medicine, inspection of the tongue is an important method of making medical diagnoses and determining prognosis. We surveyed the fluorescence characteristics of the tongue coat in the ultraviolet light. The tongue coat comprises micro-organisms, blood metabolites, leukocytes from periodontal pockets, large amounts of desquamated epithelial cells released from the oral mucosa and different nutrients. In the ultraviolet light tissues of the oral cavity generally emit weak red or green fluorescence, which is not easily seen by the human eye, but is readily detected. This fluorescence has been proved to be due to the production of porphyrins by oral micro-organisms. While the composition of motile micro-organisms on the dorsum of the tongue is not constant, variations also occur persistingly in the fluorescence characteristics of the tongue coat. But because live bacteria contain a variety of intracellular biomolecules that have specific excitation and emission wavelength spectra characterizing their intrinsic fluorescence, the tongue coat emits fluorescence. the tongue itself, on the other hand, emits very weak or not fluorescence. In conclusion, we suggests that the uncoated tongue area be eliminated from the coated tongue area with the difference between the fluorescence characteristics of the tongue and that of the tongue coat.

균열설을 보인 구강작열감증후군 환자의 임상적 특징 : 환자군 연구 (Clinical Characteristics of Patients Showing Fissured Tongue with Burning Mouth Syndrome: Case Series)

  • Objective: The aim of this study was to investigate the clinical characteristics of patients showing fissured tongue (FT) with burning mouth syndrome (BMS). Methods: We reviewed the clinical records of 16 FT patients with BMS who visited the Oral Diseases Clinic of Kyung Hee University Korean Medicine Hospital from March 1, 2017 to February 28, 2018. The subjects were graded by FT classification and compared with each group on a numeral rating scale (NRS) of tongue pain, proportion of coated tongue, unstimulated salivary flow rate (USFR), Ryodoraku, and heart rate variability (HRV) results. Results: FT grade had no significant correlation with NRS of tongue pain and USFR. FT grade had a substantially significant positive correlation with BMI and LF/HF ratio in HRV. FT grade had a substantially significant negative correlation with proportion of coated tongue. The most frequent accompanying symptoms were xerostomia and insomnia. Conclusions: The results of this study suggest that FT has no correlation with tongue pain in BMS. Rather, FT seems to correlate with sympathetic function acceleration.

다차원 컬러벡터 기반 백태 및 황태 분류 판별함수 설계 (Design of Discriminant Function for White and Yellow Coating with Multi-dimensional Color Vectors)

  • In Oriental medicine, the status of tongue is the important indicator to diagnose one's health, because it represents physiological and clinicopathological changes of inner parts of the body. The method of tongue diagnosis is not only convenient but also non-invasive, therefore, tongue diagnosis is one of the most widely used in Oriental medicine. But tongue diagnosis is affected by examination circumstances a lot. It depends on a light source, degrees of an angle, doctor's condition and so on. So it is not easy to make an objective and standardized tongue diagnosis. As part of way to solve this problem, in this study, we tried to design a discriminant function for white and yellow coating with multi-dimensional color vectors. There were 62 subjects involved in this study, among them 48 subjects diagnosed as white-coated tongue and 14 subjects diagnosed as yellow-coated tongue by oriental doctors. And their tongue images were acquired by a well-made Digital Tongue Diagnosis System. From those acquired tongue images, each coating section were extracted by oriental doctors, and then mean values of multi -dimensional color vectors in each coating section were calculated. By statistical analysis, two significant vectors, R in RGB space and H in HSV space, were found that they were able to describe the difference between white coating section and yellow coating section very well. Using these two values, we designed the discriminant function for coating classification and examined how good it works. As a result, the overall accuracy of coating classification was 98.4%. We can expect that the discriminant function for other coatings can be obtained in a similar way. Furthermore, if an automated segmentation algorithm of tongue coating is combined with these discriminant functions, an automated tongue coating diagnosis can be accomplished.

보건계열 학생들의 구강건강상태와 주관적인 구취자각과의 관련성 (Relationship between oral health status and subjective oral malodor in public health students)

  • Objectives: The purpose of this study is to analyze relationship between oral health status and subjective oral malodor awareness of public health students. Furthermore, this study aimed to recognize the importance of prevention and treatment of internal and external factors and to contribute to the correct oral health management behavior. Methods: The study subjects were 500 students from 5 health departments of 2 universities located in Jeollanamdo who participated in self-administered survey from September 1 to 15, 2014. For statistical analysis, SPSS 21.0 for Windows was used. Descriptive analysis and a Chi-square test were conducted to investigate the effects of general characteristics, health behavior, oral health behavior, and oral health status on subjective oral malodor awareness. Finally, to investigate the relationship between oral health status and subjective oral malodor awareness logistic regression analysis was performed. Results: Subjective oral malodor awareness was significantly higher in the group requiring dental care with the score of 1.63 (95% CI 1.00-2.65) compared to the group not requiring dental care. Subjective oral malodor awareness was significantly higher in the severe coated tongue group with the score of 5.31 (95% CI 1.45-19.40) and significantly higher in the moderate coated tongue group with the score of 2.56 (95% CI 1.61-4.08). Subjective oral malodor awareness was significantly higher in the often mouth breathing group with the score of 2.13 (95% CI 1.02-4.47) and significantly higher in the sometimes mouth breathing group with the score of 2.66 (95% CI 1.65-4.29). Conclusions: In order to prevent oral malodor, it is emphasized that regular dental checkups, proper brushing after the meal, and brushing of the tongue are necessary to remove the coated tongue. In addition, the use of supplementary oral care products is considered to be a meaningful oral health behavior.

타액분비저하에 따른 구강작열감증후군 환자의 임상적 특징 비교 (Comparison of the Clinical Characteristics of Patients with Burning Mouth Syndrome due to Hyposalivation)

  • Objectives: The aim of this study was to investigate the clinical characteristics of patients with Burning Mouth Syndrome (BMS) due to hyposalivation (HS). Methods: We reviewed the clinical records of 39 BMS patients who visited the Department of Digestive Diseases of Kyunghee Korean Medicine Hospital from March 1st, 2020 to July 31st, 2020. The subjects were classified according to the presence or absence of hyposalivation and both groups were compared for the proportion of coated tongue, heart rate variability (HRV), Ryodoraku, and the numeral rating scale (NRS) score of tongue pain results. Results: The BMS with Hyposalivation (HS group) and the BMS without Hyposalivation (Non-HS group) showed a significant difference in the proportion of coated tongue and the NRS score for tongue pain. The NRS score was significantly higher in the Non-HS group and the proportion of coated tongue was lower. However, no significant differences were noted in several HRV parameters between the two groups. The most frequent accompanying symptoms were xerostomia and dyspepsia. Conclusions: The results of this study suggest that hyposalivation might be one of the main causes of tongue pain, the key complaint in BMS patients. Sympathetic/parasympathetic imbalance might not be a main contribution of hyposalivation in BMS. Instead, factors such as the number of medications taken seem to correlate with hyposalivation in BMS. This results could be useful in the management of BMS patients with hyposalivation in clinical practice.

만성 소화불량증 환자에서 설 색상과 심박변이도의 경향성 파악 (Trends in Tongue Color and Heart Rate Variability in Chronic Dyspepsia Patients)

  • Objectives From observing the tongue of a patient, one can assess the health status; this method has been frequently used in traditional Korean Medicine (KM) clinics. In particular, KM posits that the color of the tongue is highly related to digestive functions. In this study, the color of tongue and heart rate variability (HRV) were compared between chronic dyspepsia (CD) patients and healthy subjects. Methods Healthy subjects and CD patients with functional dyspepsia (FD), gastroesophageal reflux disease (GERD), or chronic gastritis (CG) were enrolled for the study. Profile view images of the tongue were acquired by using a computerized tongue image acquisition system (CTIS). The color of the tongue body was extracted from the non-coated region on the tongue images. Results Color differences in CIE L*a*b* color space between the three sub-types of CD patients and healthy subjects were analyzed by using multiple linear regression analysis with age and sex as the factors. The variable b* was significantly lower in GERD patients than in the controls (p=0.017). Variable a* was significantly lower in CG than in the controls (p=0.03). No significant difference was seen between FD and controls. In GERD, the tongue body seems to be intense red in color; in CG, pale red. Frequency domain analysis showed that HF was significantly lower in GERD patients than in the controls (p=0.041). Conclusions The color of the tongue body and HF of HRV can be used for diagnosing digestive functions in health care.

한국형 중풍 변증 표준시안의 습담 변증 지표에 대한 연구 (Study of the Indicators of Dampness-Phlegm Pattern Identification Based on Tentative Korean Standard Differentiation of the Symptoms and Signs for Stroke)

  • Objectives : This study was done to investigate clinical frequency and correlation among the indicators of dampness-phlegm pattern identification settled by tentative Korean Standard Differentiation of the Symptoms and Signs for Stroke. Methods : The subjects were 147 hospitalized patients with stroke, and a list of registry was made for each of them. Among the five types of pattern identification, fire-heat, dampness-phlegm, blood-stasis and deficiency of Qi and of Yin, those that have shown a high frequency in dampness-phlegm type were categorized as the dampness-phlegm pattern group. Frequency of dampness-phlegm indicators was compared with those from the non-dampness-phlegm pattern group. Correlations among dampness-phlegm indicators were also studied. Results : 1. Dampness-phlegm pattern group included 26 patients out of 147. 2. Among the indicators of dampness-phlegm pattern. those, in order of highest frequency, were 'tiredness or sluggishness', 'white Coated tongue' and 'sputum'. 3. In comparing dampness-phlegm pattern group with non-dampness-phlegml group, the indicators such as 'lightheadedness', 'nigrescence', 'sputum', 'dermatic dysaesthesia' showed significantly high frequency. 4. Among the indicators, 'sputum' and 'tiredness or sluggishness', 'sputum' and 'yellow coated tongue', and 'white coated tongue' and 'yellow coated tongue' showed significant correlation. 5. In investigation of the correlation of scale in symptoms, various results such as positive correlation and negative correlation were obtained. Conclusion : In this study, more sensitive indicators of dampness-phlegm pattern identification were found. Based on these results, it is suggested that a more practical Korean Standard Differentiation of the Symptoms and Signs of Stroke would be established through continuous clinical studies by giving weight on each specific type of pattern identification.

체질량지수와 구강건강지표 사이의 상관성에 대한 연구 (A Correlation Study of the Body Mass Index and the Indicators of Oral Health)

  • Objectives: We aim to observe the relation of body mass index (BMI) and the indicators of oral health. Methods: 400 subjects participated in the study. The BMI values are calculated from the height and weight. For the tongue diagnosis, we used the tongue imaging device to analyze the color, tongue coating, and tooth marks. We measured the concentration of hydrogen sulfide (H2S) and methyl mercaptan (CH3SH) to evaluate the halitosis. The dry mouth was evaluated through the measurement of saliva secretion and with the questionnaire asking the frequency of dry mouth. Results: The BMI values were significantly higher in the group with light-white and blue-purple colored tongue, and significantly lower for lightly-coated tongue. However, the correlation of BMI and the amounts of saliva secretion was not significant as well as in the correlation of BMI and the concentration of H2S, CH3SH. In tongue diagnosis, the subjects who had blue-purple colored tongue also had significantly higher H2S and CH3SH, but tendency of lower saliva secretion. Conclusion: We obtain data showing that BMI value and the indicators of oral health including tongue diagnosis have meaningful correlation.
