Usage and Dosage of Ginseng Radix (panax ginseng C. A. Meyer) Based upon Traditional and Recent Scientific Clinical Applications (인삼의 한의학적 및 현대임상적 측면에서의 복용량 검토)

  • Ginsenf Radix (Panax ginseng C. A. Meyer) has been traditionally used as a herbal medicine for many therapeutic or prophylactic purposes in the oriental countries such as Korea, Japan and China for at least two thousand years and also extensively studied in the modern scientific field of chemistry, biochemistry and pharmacology. The herb is now also indicated for use as tonic or a prophylactic and restorative agent for enhancement of mental and physical capacities, in case of weahess exhaustion tiredness loss of concentration, impotence, cold limbs, during illiness anuor convalescence. Ginseng is commonly used in the form of detections, extract and powderl and ginseng products, in the form of capsules tablets and drinks. And also ginseng radix has been widely traditionally prescribed as an important comuonents of manny Chinese prescriptions or alone in various diseases and for health with its different dosages. Nowadays since rinsenf can be generally classified into food or medicine in many nations, it is very difficult to give any exact desnition on the dosage, which may be of particular importance in clinical applications. In addition, the establishment of the reasonable dosage is currently of great significance to meet the demand for such wide applications. Accordingly in this review paper we summarized the dosage of ginseng on the basis of oriental medical books oriental and western pharmacopeias and modern scientific clinical data. The recent survey demonstrated that the averare dosare of finsenf is considered to be three to four grams per day unless prescribed apart, while one to two grams per day in western countries from the western viewpoint of classification of ginseng as a medicine, surrorted by the dosage of not more than one gram per day in most clinical studies. For that reason, it seems likely that the dosage in western countries is ascribed to the safety of ginseng considering side or unwanted effects. Consequently whether the differences of dosage between oriental and western countries depend on dietary habits and races should be closely investigated. Besides, further studies on the pharmacokinetics and bioavailability of ginseng components in clinical trials need to be done to decide optimum dosage of ginseng.

  • Herbal medicine has been traditionally used in Asian countries for a long time. Many pharmacological effects are identified in the herbs and these herbs are believed to be safe for human. However, the safety or adverse effect of some traditional herbal medicines has not been established. We have chosen Cyperi rhizoma based on the Korea Herbal Pharmacopoeia and which have been widely used for an anti-inflammatory effect in Korea. The object of the study was to evaluate safety of Cyperi rhizoma in rats. The aqueous extract of Cyperi rhizoma was prepared according to the standard hot water extraction method of the Korea Pharmacopoeia. In the sub-chronic study, the aqueous extract of Cyperi rhizoma was orally administered once daily as 0, 125, 250, 500, 1000 and 2000 mg/kg/day to male and female F344 rats for 13 weeks. There were no treatment related abnormalities in mortality, clinical signs, food consumption, ophthalmologic examination, hematology, serum chemistry, urinalysis, gross observation, organ weight and histopathologic examination. In conclusion, The NOAEL (No Observed Adverse Effect Level) for Cyperi rhizoma aqueous extract was determined as more than 2000 mg/kg/day in the present experimental condition.

  • An analysis of patients and fatalities due to exposure to polyhexamethylene guanidine (PHMG) shows that PHMG causes mainly lung diseases such as pulmonary fibrosis. However, no research on the other organs has been conducted on this matter yet. So, an in-depth discussion on toxicological techniques is needed to determine whether or not PHMG is toxic to organs other than just the lungs. For the test of target organ toxicity by PHMG exposure, a toxicokinetic study must first be conducted. However, measurement method for PHMG injected into the body has not yet been established because it is not easy to analyze polymer PHMG, so related base studies on analytical technique for PHMG including radio-labeling chemistry must come first. Moreover, research on exposure-biomarker and effect-biomarker must also be conducted, primarily related to clinical application. Several limitations seem to be expected to apply the biomarker study to the patient because much time has passed after exposure to the humidifier disinfectant. It is why a more comprehensive toxicological researches must be introduced to the causality for the victims.

  • Objective : The purpose of this study was to know the clinical effects of hydrotherapy based Ultrasonic spa using SoPung-Tang on pulse wave velocity in patients with sequelae of CVA. Methods : PWV(Pulse wave velocity), Bio Chemistry, Hematology, Blood Pressure were investigated in 20 patients(Experimental group 13, Control group 7) who have been in Semyung Oriental medicine hospital from December 2005 to February 2006. Results : 1. In experimental group, Rt PWV diminished 4.8%, Lt PWV 5.1%. 2. In control group, Rt PWV diminished 3.3%, Lt PWV 1.8%. Conclusion : Hydrotherapy based Ultrasonic spa using SoPung-Tang showed useful effects in patients with sequelae of CVA.

  • A ten-year-old, male cat presented with recent loss of body weight, depression, vomiting, anorexia, polydipsia, and polyuria. General physical findings included depression, weakness, severe dehydration and a strong acetone odor on the breath. A complete blood count and serum biochemical profiles were leukocytosis, hyperglycemia (286 mg/dl), hypokalemia (2.6 mEq/L), hyponatremia, and high serum fructosamine (600 $\mu$mol/L). In blood gas analysis the cat had acidosis (pH 7.127, p$CO_2$26.7 mmHg). In urinalysis glycosuria and ketouria were appeared. On the basis of clinical signs, serum chemistry, blood gas analysis and urinalysis, diabetic ketoacidosis was diagnosed. Treatment included subcutaneous administration of protamine zinc insulin (0.75 U/head) and intravenous administration of 0.9% saline. Potassium phosphate and sodium bicarbonate was added to the fluids. Serum fructosamine for assessment of glycemic control was measured on occasion calls. On day 296, the patient improved clinically and did not experience any problems resulting from diabetic ketoacidosis.

  • An association study with Korean alcoholic patients(n=50) and normal controls(n=53) was performed to find the relationship between catechol-O-methyltransferase(COMT) gene polymorphism and alcoholism using polymerase chain reaction-restriction fragment length polymorphism. When we compared the allele and genotype frequencies of Nla III COMT gene polymorphism in alcoholism and normal controls, there was no significant difference between two groups. Our results do not support an association between the Nla III polymorphism of COMT gene and alcoholism.

  • Clofibrate, a peroxisome proliferator, is hepatocarcinogenic in rats in a dose-dependent manner. A total of 70 male and female F344 rats, 5-week-old, were divided into three groups. Rats were fed clofibrate at 0, 0.25, or 0.5% in diet for 30 days. All rats were anesthetized with $CO_2$, blood samples were taken by cardiac puncture for hematology and clinical chemistry, and the rats were killed by exsanguination. Livers, kidenys, pancreas, adrenal glands, spleen, heart, lungs, thyroid gland, reproductive organs, and digestive organs were removed, weighed, later processed, and embedded with paraplast for histological examination. The relative liver and kidney weights with respect to final body weight in the clofibrate-treated group were significantly increased compared with those of control group at all dose levels (p<0.01). It has been suggested that clofibrate may influence on hepatotoxicity by increases in peroxisomal proliferation.

  • To determine Pb level in blood, we usually .used to pull out about 5ml blood from venous vessel and this sample was digested with acids to decompose organic matter and then determined the Pb contents by Atomic Absorption Spectrophotometer with flame. But recent trend in quan titating Pb in small amount of sample is very much recommended in clinical chemistry specially pediatrics, and industrial hygiene and occupational health area. Authors tried to determine Pb contents in small amount blood of $25{\mu}l$ by using capillary tube method and got the possibility of determination of ng amount of Pb in $25{\mu}l$ whole blood sample without any pretreatment of sample.

Clinical Case Studies on Hepatoma Patients with Dietary Therapy (식이요법에 의한 간암환자의 일부증상의 호전)

  • This study was performed to evaluate the effects of high supplementary diet of vitamins, minerals, and polysaccharides on enhancing immunity and preventing from cancer in patients with hepatocellular carcinoma(hepatoma). Dietary therapy for patients was performed with dried yeast, wheat germ. Gonoderma extract, and vegetable juice. The vegetable juice was made of fresh green edible vegetables. Ten patients of total 30 patients were considered as improved state in tumor sizes, AFP, or symptoms. Most of patients complained of weight loss, fatigue, indigestion, anorexia before dietary therapy, but most of symptoms disappeared after dietary therapy. There was no significant side effects because of dietary therapy except for occasional slight diarrhea.
