Trigger Wrist with Carpal Tunnel Syndrome Accompanied with Trifid Median Nerve: A Case Report and Literature Review

  • Trigger wrist, characterized by a clicking or snapping sensation around the wrist joint during finger or wrist motion, and bifid or trifid median nerve, which occurs in carpal tunnel syndrome along with anatomical variation of median nerve, are rare conditions. We report the case of a patient with a thickened tendon caused by severe tenosynovitis and flexor tendon subluxation to the hamate hook due to bowing of the flexor retinaculum, thereby resulting in trigger wrist as well as an anatomical median nerve variation (bifid median nerve in the right wrist and trifid median nerve in the left wrist). A 59-year-old housewife visited our hospital with bilateral fingertip numbness, tingling sensation, and aggravated severe night cramping that began 2 months ago. She also complained about trigger wrist during small finger flexion. Based on magnetic resonance imaging, ultrasonography, and nerve conduction study, trifid median nerve and bilateral severe median nerve neuropathy of the wrist were diagnosed; therefore, transverse carpal tunnel release and exploration under wide-awake anesthesia were planned. Intraoperative findings showed trifid and bifid median nerves in left and right wrists, respectively. Additionally, bowing of flexor retinaculum and severe flexor tendon tenosynovitis were observed. Tenosynovitis with thickened flexor sheath resulted in subluxation of the small finger flexor tendon above the hamate hook. After transverse carpal ligament release with antebrachial fascia release and tenosynovectomy, subluxation of the flexor tendon was resolved. At 6 months postoperatively, the tingling and dullness in fingertips also resolved, and no trigger wrist or any other complications were noted.

유비쿼터스 센서 네트워크를 이용한 실시간 악취유발물질 배출원의 모니터링 시스템 (Real-time Monitoring Systems for Malodor Compounds Sources using Ubiquitous Sensor Networks)

  • 실시간으로 악취 유발 물진의 배출원에 대한 데이터를 구축하여 악취 민원에 효율적으로 대처할 수 있는 악취 정보 관리 시스템이 필요하다. 본 논문에서는 이러한 문제를 해결하기 위하여 유비쿼터스 센서 네트워크(Ubiquitous Sensor Network : USN)기반의 악취 측정 시스템을 통하여 실시간적으로 수집된 악취 데이터를 데이터베이스에 축적 및 주기적으로 데이터를 조회하여 지도상에 표시함으로서 즉각적인 악취 배출상황을 확인할 수 있는 시스템을 구축하였다. 또한, 지도상의 센서노드 아이콘을 클릭함으로서 센서노드 주변의 업체 목록을 지도상 좌표 기준으로 조회, 강한 악취가 발생한 부근의 주변 업체 종류와 배출물질 종류를 파악할 수 있도록 하였다. 제안된 시스템은 이러한 정보로 부터 업체관리 방안을 계획하여 악취배출 업체에 대한 관리 효율을 높일 수 있을 것으로 사료된다.

엑셀 매크로기능을 이용한 베이즈 정리 교육도구 개발 (Development of Bayes' rule education tool with Excel Macro)

  • 본 논문에서는 엑셀매크로로 베이즈 정리 교육도구를 개발하여 사용의 예를 소개한다. 주어진 어느 한 사건이 발생했을 때 그 사건이 특정조건하에서 발생되었는지 여부에 관심이 있다. 이런 경우의 확률계산에 사용할 수 있는 것이 베이즈 정리이다. 베이즈 정리는 새롭게 얻어진 부가적인 정보를 기초로 통계적 의사결정을 하는데 매우 유용한 정리이다. 베이즈 정리를 중간과정과 설명을 통해 학습자 스스로 효율적으로 학습할 수 있도록 개발한 교육도구를 소개한다. 조건부확률, 곱셈법칙, 전확률 공식, 사전확률, 사후확률 등에 대한 설명과 활용 예를 단계적 학습을 통해 이해할 수 있도록 하였다. 결과가 나오기까지의 과정을 단계적인 개념설명과 그림으로 표현하여 단계적, 시각적인 학습이 되도록 하였다. 한 화면상에서 계산과정과 결과를 나타내도록 하기 위하여 분할 2개와 3개에 대하여 엑셀 자체에서 제공되는 분석기능과 비주얼베이직으로 작성된 프로그램을 연결하여 명령단추를 누르면 매크로가 실행되게 하였다.

1년간 일개 장애 어린이 전문병원에 내원한 치과 외래 환자에 대한 고찰 (A YEAR REVIEW OF OUTPATIENTS IN DENTAL CLINIC OF THE MUNICIPAL HOSPITAL FOR THE DISABLED CHILDREN)

  • Objectives: The goal of this study is to establish data baseline to ameliorate oral health policies for patient. The research was conducted in dental clinic at Seoul Metropolitan Children's Hospital - the major municipal hospital for the disabled. The outpatients' visiting patterns to the dental clinic were analyzed, compared to other medical departments. For further research related to TMD, the questionnaires were given to the subjects. Material and methods: Data for visiting patterns/frequencies was collected from patient records of 1419 outpatients who visited the dental clinic at Seoul Municipal Children's Hospital in 2011. Then TMD-related questionnaires were given to 127 outpatients who were over 13 years old. Results: 1. 219 out of 1419 patients (15.4%) who visited the dental clinic at Seoul Municipal Children's Hospital in 2011 were the disabled. 2. Compared to normal patients, disabled patients visited the dental clinic and rehabilitation medicine department more frequently.(p<0.05) However, there was no apparent difference in visiting frequencies between the disabled and the normal patients in the psychiatric department.(p=0.3237) 3. 44% of the TMD-related questionnaire subjects responded that they often consumed relatively hard or tough food, 53% answered they had unilateral chewing habit, 3% had bruxism, and 12% had clenching habit. Conclusion: In the dental clinic at Seoul Municipal Children's Hospital, 19% of outpatients over 13 years old had TMD such as clicking and pain. Also, the disabled patients' visits to the dental clinic were considerably more frequent, compared to the normal patients and to other medical departments. Accordingly, the result may suggest that the proper systematic supports from the government are essential to the dental clinic at municipal hospital for the disabled.

악관절원판 절제술 후 이개연골 이식 (AUTOGENOUS AURICULAR CARTILAGE GRAFT FOLLOWED BY DISCECTOMY OF THE TEMPOROMANDIBULAR JOINT)

  • Arthrosis of the temporomandibular joint is defined as a disease of a joint with chief complaint of pain, clicking, limited jaw movements. Generally, most patients with the temporomandibular arthrosis can be treated conservatively with muscle relaxation therapy combined with mandibular repositioning prostheses, followed by occlusal equilibration, restorative dentistry and/or orthodontics, and many other forms of treatment. In case prior nonsurgical treatment proved to be ineffective or the disease is chronic and severe, surgical operation is recommended. For patients with arthrosis of the temporomandibular joint, only discectomy as therapeutic method of the surgical treatment should not be applied and the removed articular disc of the temporomandibular joint should be replaced. Allograft such as Proplast-Teflon, Silastic, etc have been used as replacements of removed articular disc. However, these allograft materials have caused complications such as inflammatory changes, foreign body reactions. As a result, a replacement material which is autogenous, space occupying, easy to harvest and less inflammatory change has been developed. Auricular cartilage with perichondrium satisfies many of these requirements. The apparent advantages of autogenous auricular cartilage as an interpositional graft after a discectomy are as follows, (1) the form of the external ear corresponds to joint morphology, (2) a graft of adequate size can be harvested, (3) the form of the external ear remains unchanged after surgery, (4) the graft can be obtained adjacent to the surgical site, (5) biologically acceptable material is used, (6) the additional expense of allogenic graft is avoided. Because we considered autogenous auricular cartilage as a good replacement material, removed articular disc has been replaced with fresh autogenous auricular cartilage in the case of three patients. The result of the treatment is favorable, and the cases being presented here.

악관절내장증의 진단에 있어 Dynamic MRI의 효용 (DIAGNOSTIC RELIABILITY OF THE DYNAMIC MRI FOR THE INTERNAL DERANGEMENT OF TEMPORO-MANDIBULAR JOINTS)

  • The Magnetic Resonance Imaging has been used widely to evaluate the disk position without any interruption of the TMJ structures, and the Dynamic MRI presenting computed serial imaging or the video-recorded simulation images is thought to be very effective to evaluate the disk position under function. This is to study the correlation between the clinical diagnosis and the findings of Dynamic MRI for the diagnosis of internal derangement of the temporomandibular joints. 30 joints(15 patients) were examined clinically, and the movement of TMJ meniscus was reviewed in the dynamic MRI. The comparative results are as follows : 1. All internal derangements of TMJ disk displacement without reduction were consistent with MRI findings. 2. 5 joints (50%) of disk displacements with reduction could not be confirmed by MRI findings. 3. The disk displacements in MRI were found in 55% of painful joints, 50% of clicking joints, and 70% of the joints with restricted movement. 4. The reliability of MRI for the diagnosis of TMD was evaluated as 77% ; 24 of 30 joints who presented with clinical diagnosis of TMD. 5. MRI is very reliable to diagnose the disk displacement without reduction, but it is rather not so effective to diagnose the early derangement or muscle disorders.

Karnaugh Map 간략화 과정의 학습을 위한 교육용 자바 애플릿의 설계와 해석 (Design and Analysis of Educational Java Applets for Learning Simplification Procedure Using Karnaugh Map)

  • 본 논문에서는 디지털논리회로의 설계에 있어 필수적인 카르노 맵 간략화 과정을 교육용 자바 애플릿의 형태로 구현하였다. 학습자는 구현된 자바 애플릿으로부터 흥미로운 학습을 경험할 수 있으며, 자바 애플릿 설계과정에서 교육공학적인 요소를 단계별로 고려하였기 때문에 학습 효율의 극대화가 가능하다. 학습자는 구현된 자바 애플릿으로부터 디지털 논리회로의 간략화 과정을 마우스로 버튼을 클릭하거나 텍스트를 채워가면서 웹상에서 가상실험을 진행한다. 또한, 간략화 과정에서 발생되는 논리식과 논리회로도는 학습자가 효율적으로 학습할 수 있도록 서로 다른 프레임으로 구성하였으며, 학습자가 구성한 논리회로도가 올바르게 구성되었는지 확인할 수 있도록 하였다. 마지막으로 본 논문에서는 수정된 Quine-McCluskey 간략화 기법에 기초하여 자바 애플릿을 구현하였기 때문에 오프라인 교육의 보조도구로서 사용된다면 학습효율의 향상에 기여할 수 있다는 것을 입증하였다.

『동의보감(東醫寶鑑)』의 정기신(精氣神) 양생법(養生法)과 그 특징 (The Jing-Qi-Shen Cultivation Method and its characteristic of Donguibogam)

  • Objectives : Various cultivation methods can be found suggested in Donguibogam(東醫寶鑑) Naegyeongpyeon(內景篇) ranging from Juchenhwahu(周天火候), massage(按摩), physical and breathing exercise(導引), fetal breathing(胎息), clicking teeth(叩齒), saliva swallowing (嚥液), sounding the celestial drum(鳴天鼓), Six words(六字訣), Seven step(七星步), God pillow method(神枕法), and Taoism. Methods : The cultivation methods in Donguibogam are mainly found in the first volume of Naegyeongpyeon, and some can be discovered in Oehyeongpyeon(外形篇) and Japbyeongpyeon(雜病篇). This paper aims to study the system and special features of the cultivation methods that unfold within the first volume of Naegyeongpyeon. Results : Donguibogam's cultivation methods are Jing-Qi-Shen cultivation method(精氣神養生法), and Naegyeongpyeon's Sinhyeong(身形) can be described as a collection of general remarks, and Jing(精), Qi(氣), and Shen(神) is dealt individually. The condition for Jing culvation is not to spill sperm, Qi cultivation is to control the breathing, and Shen culviation is having a calm mind. Conclusions : The Jing-Qi-Shen cultivation method of Donguibogam revolves around Taoism integrated with Buddhism. It places importance in the cultivation of the Inner Center(內丹), and emphasizes Shen in particular among Jing, Qi, and Shen.

협업 환경에서의 인터랙션을 위한 적외선 LED 포인터 (Infrared LED Pointer for Interactions in Collaborative Environments)

  • 본 연구는 Tiled Display 기반의 협업 환경에서 인터랙션을 위한 적외선 포인터를 구현하는 목적으로 진행되었다. 적외선 발광 다이오드에서 나오는 빛을 인지하여 포인터의 궤적을 인식하는 기술과 이를 응용하여 여러 분야에 접목시키는 것에 주안점을 두었다. 기본적인 마우스 클릭 기능과 단순 포인팅 기능을 제공하는 기존의 포인터와는 달리 적외선 발광 다이오드가 탑재된 포인터를 조립, 이용하여 기존의 기능뿐만 아니라 컴퓨터와 사용자 간의 새로운 커뮤니케이션 수단을 제공하고 있다. 장점으로는 책상, 테이블과 같은 제한된 공간을 벗어나서 적외선 투과 필터가 장착된 카메라 하나만 설치할 수 있는 곳이면 구현이 가능하며 적외선을 이용하므로 가시광선의 영향을 적게 받아 효율적으로 영상 처리를 할 수 있다는 데에 있다. 그리고 적외선을 이용한 사용자의 모션을 추적, 인식하여 복합적이 여러 가지 기능을 실행할 수 있으며, 보다 더 사용자와 인터랙티브한 작업이 가능하다. 구현된 하드웨어와 소프트웨어 시스템은 프레젠테이션 및 게임 컨트롤 등에 사용되었다.

악관절증의 측방두부계측 단층방사선학적 연구 (A STUDY ON THE LATERAL CEPHALOMETRIC TOMOGRAPHY OF TMJ ARTHROSIS)

  • The author obtained individualized lateral cephalometric tomograms from 23 young adults (46 of left and right normal TMJ) with normal occlusion and 20 patients (14 of patient asymptomatic TMJ and 26 of patient symptomatic TMJ) with clicking and painful TMJ after the analysis of submental vertex view. Individualized lateral cephlometric tomogram analysis and TMJ space analysis were performed after tracing each film. All data from these analysis was recorded and statistically processed with CYBER computer system. 1. The results were obtained as follows. In submental vertex view, the mean condylar angulation of Rt. side in normal group was 20.348°±6.358°, Lt. side was 18.870°±7.777° and Rt. side in patient group was 19.350°±7.576° Lt. side was 17.750°±6.146° respectively. The mean condylar angulation of Rt. side was larger than Lt. side in normal and patient group. 2. When the mandible was moved from centric occlusion to centric relation, condylar position relating to the glenoid fossa was placed posteriorly and superiorly in normal TMJ group and patient symptomatic TMJ group. 3. In centric relation position, the proportion of anterior space to posterior space was 1.593 for normal TMJ group, 1.604 for patient asymptomatic TMJ group and 1.671 for patient symptomatic TMJ group. In centric occlusion position, 1.390 for normal TMJ group, 1.539 for patient asymptomatic TMJ group and 1.196 for patient symptomatic group. Normal TMJ group, patient asymptomatic TMJ group and patient symptomatic TMJ group and patient symptomatic TMJ group revealed significant difference in ∠C₂ measurement. (ANOVA-test, p<0.05) 5. Normal group and patient group revealed significant difference in Fh, ∠C₁and ∠C₂ measurement. (T-test, p<0.05) 6. There were strong positive correlation (0.8771) between Fp and Fm, and strong negative correlation (-0.9039) between ∠C₂ and ∠C₁ from the lateral cephalometric tomogram analysis.
