기능성 자궁출혈에 대한 한약치료 : 체계적 문헌 고찰 (Traditional Herbal Medicine for Dysfunctional Uterine Bleeding versus Western Medicine: A Systematic Review of Randomized Controlled Trials)

  • Objectives: To evaluate the effectiveness and safety of traditional herbal medicine (THM) in the treatment of dysfunctional uterine bleeding (DUB) versus conventional western medicine. Methods: Randomized Controlled Trials (RCTs) comparing THM vs. conventional western medicine for DUB, were obtained from PubMed, Cochrane Library, Embase, CNKI, RISS, NDSL, KISS and OASIS. The risk of bias was assessed by using Cochrane's risk of bias tool. Results: 16 RCTs with 1,659 patients were identified and reviewed. 10 RCTs reported THM was statistically effective than control group in effective rate. Also recurrent rate was estimated in 6 RCTs and was lower than control group. 7 studies observed adverse events (AEs) and severe AEs were not reported. Conclusions: Despite several limitations, this review suggested that THM was safe and effective in the treatment of DUB. THM may also decrease the recurrence rate. However, this could not be proven conclusively. To ensure evidence-based clinical practice, more sternly designed trials are warranted.

  • Objectives The goal of this study is to investigate childhood epilepsy based on the recent literature of Eastern and Western medicine and look for better treatment. Methods This study reviewed definition, etiology and therapy method of childhood epilepsy based on 24 studies about the treatment of epilepsy in children Results and Conclusions Based on the comparison studies, not only herbal medication has shown better efficacy than anti-epileptics, but also as a monotherapy, or as combination therapies, herbal medications were superior in childhood epilepsy.

  • Objective : The aim of this study was to investigate treatment of childhood myopia in current Traditional Chinese Medical Journals. Material and Method : We searched for treatment of childhood myopia in all Traditional Chinese Medical Journals published from January 2001 to October 2005. Results : Thirteen articles were selected and 7 of them showed total improvement rate was higher than 90% in childhood myopia. Treatment methods were acupuncture, auricular acupuncture, moxibustion, herbal medicine, adhesive plaster. Conclusion : Oriental medical treatments for childhood myopia were presumed to be effective in most trials. But there remains a debate about diagnosis and outcome criteria. Further studies are needed.

대상포진 후 신경통의 중의치료 최신 연구 동향 - CNKI 검색을 중심으로 (A Review of Clinical Studies of Chinese Medicine Treatment for Postherpetic Neuralgia - Research using CNKI Database)

  • Postherpetic neuralgia(PHN) is one of the most common intractable chronic pain syndromes. This review was conducted to investigates the trends in recent clinical studies of PHN in China and to find out the efficacy of Chinese medicine treatment for PHN. Based on CNKI databases, nineteen randomized controlled trials(RCTs) were analyzed. The search focused on the authors, publication year, sample size, age, duration of illness, treatment period, lesion sites, methods, outcome measures, and results of the selected articles. Four studies used herbal medicine. The most frequently used herbs were Angelicae Gigantis Radix(當歸) and Corydalis Tube(玄胡索). Eighteen studies used acupuncture treatment. The most frequently used acupoints were ST36, LR3, LI4. Three studies used moxibustion and 5 studies used cupping treatment. In all nineteen studies, the efficacy of Chinese medicine treatments was confirmed. More varied and scientifically designed clinical studies are required to develop treatments for PHN. The results of current study could be used as guidance for further studies of treatment on PHN.

뇌졸중 후 우울증에 대한 침과 한약의 결합 치료 효과: 체계적 문헌 고찰 및 메타분석 연구 (Clinical Effect of Acupuncture Combined with Herbal Medicine in Treatment of PSD: A Systematic Review and Meta-Analysis)

  • Objectives: The purpose of this study was to provide clinical evidence to support the use of acupuncture combined herbal medicine for post-stroke depression. Methods: Studies were identified by searching Pubmed, EMBASE, Cochrane Library, NDSL, RISS, KMbase, KISS, and CNKI. Randomized controlled trials that verified effects of the acupuncture combined with the herbal medicine for PSD were included in this study. Literature searches of English, Chinese, and Korean databases were performed. Two authors independently extracted the data and assembled the study quality. Results: Of 698 RCTs screened, 17 RCTs were selected. Most of the studies had unclear risk of bias. Among the 17 RCTs, 10 were meta-analyzed. As for the outcome measurement, the effective rate and the HAMD were most commonly used. The meta-analysis of the studies revealed that the PSD cure effective rate in the acupuncture combined with herbal medicine group was higher than that in the Western medicine group (RR: 1.23, 95% CI: 1.14 to 1.33, p<0.00001, I2=0%). Also, the HAMD in the acupuncture combined with herbal medicine group was significantly different compared to the Western medicine group (MD -5.54, 95% CI -9.80 to -1.29, p=0.01, I2=99%). The quality of the selected RCTs was low. Conclusions: Acupuncture combined with herbal medicine is effective for treating post-stroke depression. However, It is worth noting that the included studies were of relatively poor quality. The sample sizes were also small. Based on the findings of this study, further investigations into the diagnosis and treatment of PSD are warranted.

사상체질의학(四象體質醫學)과 증치의학(證治醫學)의 비교(比較) 및 상호(相互) 보완(補完) 방안(方案) 연구(硏究) (Study on Comparison and Mutual Complementary Development between Sasang Constitutional Medicine and Syndrome Identification and Treatment Medicine)

  • Objectives : Through a comparative study between Sasang constitutional medicine and Syndrome identification and treatment medicine about the theory of diagnosis, the theory of herbal medicine, the theory of prescription, the theory of acupuncture and constitutional medicine to put forward a complementary perfect solution which has good clinical utility. Methods : We compared and sorted out the textbook of Korean Sasang constitutional medicine which named Sasang medicine, and also the textbook of Chinese TCM which was including Basic theory of TCM, Diagnostics of TCM, Science of Chinese materia medica, Pharmacology of traditional Chinese medical formulae, Science of acupuncture and moxibustion, Constitution of TCM, and also related papers. Results : (1) Through a comparative study between Sasang constitutional medicine and Syndrome identification and treatment medicine about diagnosis, herbal medicine, prescription and acupuncture, Sasang constitutional medicine is a medical system which gives priority to balance of organs, when Syndrome identification and treatment medicine is a medical system that gives priority to improve symptom. (2) The prescriptions of Sasang constitutional medicine are used for the treatment of basic pathogenesis in the chronic stage of the disease, which can recover vital qi, when Syndrome identification and treatment medicine are used for the treatment of stage pathogenesis in the acute stage of the disease, which can get rid of pathogenic factors. (3) There are two kind of complementary perfect solution between Sasang constitutional medicine and Syndrome identification and treatment medicine. One perfect complementary medicine is that Sasang constitutional medicine plays a major role when Syndrome identification and treatment medicine plays a supporting role. The other one is that Syndrome identification and treatment medicine plays a major role when Sasang constitutional medicine plays a supporting role. It determined by the usual symptoms and the symptoms now, vital qi and pathogenic factors, symptom and the root cause, acute stage and chronic stage, and the state of pathogenesis. Conclusion : A new perfect complementary medicine can be created which is based on the setting that Sasang constitutional medicine primarily treat the usual symptoms, vital qi, the root cause, chronic stage and basic pathogenesis when Syndrome identification and treatment medicine primarily treat the symptoms now, pathogenic factors, symptom, acute stage and stage pathogenesis.

  • Acanthopanax cortex (Acanthopanax sessiliflorum, Araliaceae, KP VIII), an important herbal drug, has been used as tonic, antistress and immuno-enhancing drugs in Korea. And Peiplocae cortex(Periploca sepium, Asclepiadaceae, CP 2000) has been used as cardiotonic, anti-inflammatory, and sedative effect in china. These are called “Namogapi” of Acanthopanax cortex and “Bukogapi” of Periplocae cortex in Chinese herbal market. These herbal medicines are sometimes circulated as same herbal medicine “Ogapi”. 현대 중의학 관점의 고혈압 연구동향 분석 (Recent Research Trends on Hypertension in Traditional Chinese Medicine)

  • Objectives : This study was performed to investigate the research trends of hypertension in Traditional Chinese Medicine(TCM), and it aims to promote hypertension research in Korean Medicine. Methods : We first collected more than 1,900 papers about hypertension research, and finally selected 108 papers related to this study. They were analyzed by the annul situation, the subject of the study, the perspective of TCM, the study stream, the research fields, the diagnostic patterns, and the distinguishing treatments of TCM. Results : The first review about hypertension in TCM was performed in 1963, and the number of studies has increased since 2000s. Doctors and researchers in China tried various diagnostic patterns to treat the patient because the diagnostic patterns were not unified. For this reason, most researches were reported to the diagnostic patterns and the treatments. Also we have discovered the diversity in treatments methods such as not only the typical herbal medicine, acupuncture, and Qigong but also specific treatments like Chuna, herbal acupoints stimulation, footbath, and pillow. Conclusions : Based on the results of this study, it can be proposed as follows : First, the various approach about hypertension in Korean Medicine is required. Second, the domestic research is needed to be extended to the external treatments as the distinguishing treatments of TCM.

소아 1형 당뇨의 한약과 양약 병용 치료에 대한 체계적 문헌 고찰 및 메타분석 (Efficacy of Combination Treatment of Herbal Medicine and Western Medicine for the Treatment of Type 1 Diabetes in children : Systemic Review and Meta-analysis)

  • Objectives The purpose of this study is to investigate the clinical studies on the effect of herbal medicine in type 1 diabetes in children and to seek better approach of herbal medicine to treat type 1 diabetes in children. Methods This study researched randomized controlled trials through various databases in the world about herbal medicine treatments in type 1 diabetes in children. Results 10 out of 337 studies were selected and analyzed. All studies were conducted in China. All studies were using herbal medicines, as an adjunctive treatment to the main regimen. As a result, the integrated Chinese medicine and western medicine lowered FPG by -1.56 mmol/L and 2hPG by -1.94 mmol/L on average, respectively. The HbA1c also decreased by -1.11% in the treatment group compared to the control group. Total efficacy of the treatment was 1.21 times more effective in the treatment group than in the control group. Conclusions Based on the results of the studies, it seems that the herbal medicine for the treatment of type 1 diabetes in children will be effective as a combination with conventional medicines. Further research is needed to prove the findings of this observatoional studies.

Microbiota, co-metabolites, and network pharmacology reveal the alteration of the ginsenoside fraction on inflammatory bowel disease

  • Background: Panax ginseng Meyer (P. ginseng) is a traditional natural/herbal medicine. The amelioration on inflammatory bowel disease (IBD) activity rely mainly on its main active ingredients that are referred to as ginsenosides. However, the current literature on gut microbiota, gut microbiota-host co-metabolites, and systems pharmacology has no studies investigating the effects of ginsenoside on IBD. Methods: The present study was aimed to investigate the role of ginsenosides and the possible underlying mechanisms in the treatment of IBD in an acetic acid-induced rat model by integrating metagenomics, metabolomics, and complex biological networks analysis. In the study ten ginsenosides in the ginsenoside fraction (GS) were identified using Q-Orbitrap LC-MS. Results: The results demonstrated the improvement effect of GS on IBD and the regulation effect of ginsenosides on gut microbiota and its co-metabolites. It was revealed that 7 endogenous metabolites, including acetic acid, butyric acid, citric acid, tryptophan, histidine, alanine, and glutathione, could be utilized as significant biomarkers of GS in the treatment of IBD. Furthermore, the biological network studies revealed EGFR, STAT3, and AKT1, which belong mainly to the glycolysis and pentose phosphate pathways, as the potential targets for GS for intervening in IBD. Conclusion: These findings indicated that the combination of genomics, metabolomics, and biological network analysis could assist in elucidating the possible mechanism underlying the role of ginsenosides in alleviating inflammatory bowel disease and thereby reveal the pathological process of ginsenosides in IBD treatment through the regulation of the disordered host-flora co-metabolism pathway.
