CASE REPORT : A NASOPALATINE DUCT CYST IN 9-YEAR-OLD CHILD (9세 소아에서 나타난 비구개낭의 치험례)

  • Nasopalatine duct cyst(NPDC) is the most common non-odontogenic cyst in the oral area and is thought to be originated from the epithelial remnants of the nasopalatine duct. Many etiologic factors have been proposed for the NPDC; trauma to the region during mastication or from ill fitting dentures, bacterial infection, and spontaneous proliferation of tissue. The majority of cases are seen between the ages of 40th and 60th and it is rare in children. Surgical enucleation is the recommended treatment for NPDC, usually under general anaesthesia. A 9-year-old boy was refered to the chosun university pedodontics clinic that this child who has anterior palatal swelling is in routine check. In this case, swelling was presented at the bottom of the anterior nasal cavity as well as in the labial aspect of the upper alveolar ridge. It was so large that we treated it by marsupialization and obturator was put. This patient is on the continuous observation. We treated the child who had NPDC by marsupialization, and got the successful results such as rapid bone regeneration and the consistency of incisor vitality.

The Effects of Milk Group Intake to Dietary Diversity Score and Nutrient Adequacy Ratio among Toddler (유아의 우유군의 섭취가 식이의 다양성과 영양소 섭취의 적정도에 미치는 영향)

  • This research was conduced to 1) examine the influence of milk group intake on the dietary, considering dietary diversity and nutrient adequacy, 2) identify which foods truly contribute to the intake of specific nutrients 3) point out the dietary problems of this age group. The subjects were 664(male 357, female 307) healthy toddlers of the ages of 12-36 months, who had neither been taking any medicine-type supplements nor had any diseases that affect the dietary intakes, chosen among the outpatients of university and private hospitals in Seoul and Kyonggi province during Mach-August, 1999. The 24 hour recall method and CAN Pro(food analysis program) were used to analyze the amount of nutrient intake. The grouping of the subjects for the analysis by monthly age was based on the 98 Korean Child growth standard. The results that the milk group intake showed significant negative correlations with the grain group(r=-0.194, p<0.01), the vegetable group(r=-0.115, p<0.01) and the fruit group(r=-0.125, p<0.01). It also showed that the more the milk group intake, the lower the Dietary Diversity Score(r=-0.131, p<0.01), and the more the intake of raw milk, the more dificient the intake of iron(r=-0.211, p<0.01), vitamin A(r=-0.169,p<0.01), vitamin B$_1$(r=-0.078, p<0.05) and vitamin C(r=-0.187, p<0.01). Foods contributory to nutrient intake for of 12-14 months of were mostly infant formula, and the contribution ratio was high. In the age of 15-17 months, the subjects obtained most of the nutrients from raw milk., liquid-type yoghurt, soybean milk, cooked rice, etc. As for iron intake, the age of 21-23 month obtained the most from ionic drinks, and 24-29 months from raw milk, chocolate cakes, ionic drinks, etc. Though the NAR(Nutrient Adequacy Ratio) and MAR(Mean Adequacy Ratio) generally tended to increase accordingly with the DDs, as for nutrients Ca, vitamin A, vitamin B$_1$, and vitamin C the NAR marked the highest when the DDS marked the lowest(due to infant formula). And for iron, even when the DDS was 5 the NAR value was low. Im conclusion, the more the milk group intake, the lower was the dietary diversity. High dependency on infant formula and others from milk group over the age of 12 months proved to result in a decrease of nutritious solid food intake, iron deficiency and a low dietary diversity score, which can lead to an unbalanced diet. Therefore, nutrition education towards mothers/caretakers of toddler is of urgent need, in order to reduce the fluid milk group intake of low nutritional density and to provide various solid foods the children.   • Animation as a public visuals media have been expanding increasingly its social and cultural influences beyond the ages and nations on the basis of global consumption. However, animation increases the negative impact in modern popular culture, and in regard to this, 'the recovery of ethics' should be considered in a reflexive and educational perspectives for the social role of animation. Thus, the research addresses the animation films of Brigham Young University students which contain a ethical values and receive attention by New York Times, etc. as a successful educational model. To do this, firstly, literature has reviewed by focusing on the negative impact of animation, 1) violence, 2) excessive sensationalism, 3) confusion of cultural identity, 4) gender discrimination, and 5) distorted view of history. Secondly, the education system of animation course at Brigham Young University will be analysed. Thirdly, based on this, the case study will be conducted by focusing on the 13 animation films of students to reveal the characteristics of the way of film direction. Through this research, firstly, most of animation films are comic genre, consisting of children and animal characters, family-friendly and lyrical story style and deployment of coincidental and allegoric incident. Thirdly, the religious spirit and multidisciplinary methods of education in Brigham Young University has influenced to the ethical expression and technical perfection in animation filmmaking. In the light of this, the research and suggests the new paradigm is for the practical disciplines of animation in the restoration of the ethical perspective and explores how the animation production adopts the moral significance.

A Study on The Conditions of The Department Stores in Seoul -Emphasis on the Layout of the Fashion Zone and Brands- (국내 백화점의 패션매장 구성과 브랜드 전개현황 분석)

  • This study analyzed the trends of fashion market in 15 branch stores of 3 major department stores in Seoul. The purposes of this study were to classify fashion zone and brands in each floor of the department stores, and to analyze the rate which a fashion brand was located department store. The results were as follows : 1. There were sundry goods on main floor, women\`s clothings on 2-4th floor, sports & golf wears and infants & children\`s clothings from 6th floor to the top in most department stores. 2. Lotte Chamshil branch had the largest number of fashion brands in it, the nest was Lotte Main store and followed by Hyundai Chunhoe branch, Shinsegae Gangnam branch, Hunndai Shinchun branch, Lotte Gangnam branch, Hyundai Main store, Hyundai Muyeuk-Center branch, and Lotte Youngdeungpo branch, etc. 3. The fashion categories of the Lotte Department stores were segmented as the Casuals (character, young, young basic, career, town, jean, city), Young worlds, Imported beautique, Madams, Designers(beautique), Intelligences, Unisex, Ladies formal wears, the Seasonables, and the Formals, Missy Careers. This was the most various fashion market segments among 3 major department stores. This store had 667 Women\`s fashion and Casual brands. The Chamshil branch and Main store were intensified the Casual & sundry goods on 5th floor. 4. The fashion categories of the Hundai Department stores were segmented as Women\`s wears, Women\`s casuals, Young-Adult, Young live, Women\`s former wears, Royal beautique and Young characters. It was less segmented than other Department stores. Total number of Women\`s fashion and casual brands were 471 brands. The market segmentation of fashion zone was well done at Chunho branch and Shinchun branch. It was intensified that Fashion sundry goods at Muyeuk-Center branch and The Women and Young fashion zone at Chunho branch. 5. The fashion categories of the Shinsaegae Department stores were segmented as Casuals (young, young character, X-, missy, career, character), Imported beautique, Designer\`s characters, Young basics, Elegance, Missy, Young weave, Original brands. This store had 304 Women\`s fashion and Casual brands. Shinsaegae has also developed it\`s own brand(PB items) and classified as the Original zone which differentiated it from other Departments. 6. The Deco was the most popular brand in the department stores, the next were Micha/Botticelli, and followed by Darks/System/lzzat Baba, Givy/Obzee/Lee won jae/Kim yeon Joo, and so on. The target of 6 out of 10 brands which were included in here were career women of age 20 to 30 ages. The price rate were from 200,000 won to 300,000 won.

The Development of Health Promotion Programs for Middle Aged Women (중년여성의 건강증진 프로그램 개발에 관한 연구)

  • The purpose of this study was to develop health promotion programs for middle aged women and to identify the adaptability and the effectiveness of the program in order to provide a model for health promotion programs as a basis for nursing intervention. The research design was a quasi-experimental, nonequivalent control-group pretest -posttest design. The data were collected from October 30 to December 11, 1996. The study subjects were middle aged women residing in Chonju city, with ages from 40 to 59. The experimental group consisted of 42 subjects who were recruited through announcements of the local newspaper. The control group consisted of 49 subjects who were mothers of nursing college students. The health promotion program for middle aged women was based on the Bandura's self efficacy theory and Pender's heath promotion behavior theory consisting of exercise and heath diaries as performance accomplishments as well as education and group sessions as verbal persuasion and vicarious experiences. The study program was provided for 6 weeks, 3 hours a day per week. There was a pretest before the program and a posttest after the 6 week program. The instruments used for the study were a Self Efficacy Scale and a Health Promotion Behavior Scale developed by Park(1995). The data analysis was done by the use of a SPSS/PC. The study results were as follows: 1. In the analysis of the homogeneity between the experimental and control groups, there were significant differences in the socio-demographic characteristics, self efficacy and health promotion behavior. There were significant differences between the experimental and control groups in occupation, the number of children, and the status of involvement in social activities. 2. The first hypothesis, 'The level of self efficacy of the experimental group will be higher than that of the control group.' was supported(F=10.154, p=.002). The second hypothesis, 'The degree of health promotive behaviors in the experimental group will be higher than that of the control group.' was supported(F=17.349, p=.000). 3. There was a significant positive correlation between the self efficacy and the health promotion behaviors in pretest and posttests (pretest: r=.732, p=.000 ; posttest : r=.754, p=.000). 4. The significant variables for health promotion behaviors were religion(t= -1.97, p=.05), family income(F=4.85, p=.00), education level (F=6.38, p=.00) and involvement in social activities(t= -3.06, p=.00) in socio-demographic characteristics. In summary, a heath promotion program based on self efficacy theory has made an improvement on health promotion behaviors. Also, the results show that the higher the level of self efficacy, the better the health promotion is in middle aged women. The study has proved that nurses can provide nursing intervention for the improvement of health promotion in middle aged women through the adaptation of a program increasing the subject's self efficacy level.

A Study on the Korean Costume Pattern Design Using CAD System(I) -With Concentration on the Girl's Color-Strip Blouse & Skirt- (CAD SYSTEM을 이용한 한복의 기성복 설계에 관한 연구(I) -여아 색\ulcorner 저고리 및 치마를 중심으로-)

  • The research, aiming an automatic pattern design of Korean Costume by utilizing CAD system, tried Grading, Seaming and Marking with a theme of girl's color-strip blouse & skirt. The content & conclusion of the research can be outlined as follows; 1) As we reviewed the current situation of Korean costume which becomes more likely celebrational or festival costume, while the demand of it is getting increase, it's being dealt mostly as ready-made dresses with different qualities & designs. Especially childrens ready-made ones were highly demanded to be picked up as the theme. 2) For the original drawing of children's color-strip blouse & skirt, Kyung-Ja Park's drawing method was used here, and for the substitute of particular body parts absolute size, Joo-Won Lee's standard size chart from a size-study by ages. 3) To work with CAD system we had input master pattern, drawn for age 5 as basic size, then graded six step-sizes for 1-11 years old. For add-subtract of particular body parts size, we graded through computing the standard variation among items to get the pattern developed into ready-made standard size, we can make precise plotting by grading wanted size very rapidly if we correct the rule of changed items different from standard size to make utilization possible enough with easy order method of ready-made Korean Dress. 4) We produced Marker after attaching a margin to seam accordingly by parts for each pattern using P/D/S to mark, In mass gament-cutting, the loss of time and material can minimized. In this research the apparel CAD system which has been utilized and only be western fashion industry was introduced for the design of ready-made Korean costume and utilized it in Grading, Marking which are critical steps to improve productivity and have reported the result in the research. Thus we expect that less cost, improved productivity and better quality with minimized loss of material from marking as well as from prompt and precise size-drawing. Furthermore the utilization of CAD system is considered as an effective one in terms of the research & the development to remove effective one in terms of the research & the development to remove irrational elements in the design and production process of Korean costume as well as in terms of the study of Korean Costume development through creative works of Korean Costume.

Hand-Foot syndrome induced by sorafenib, a multitargeted tyrosine kinase inhibitor, in a patient with advanced renal cell carcinoma (진행된 신세포암 환아에게 sorafenib 투여시 발생한 Hand-Foot syndrome 1예)

  • Renal cell carcinoma (RCC) arising from epithelial cells of the renal tubules is a highly aggressive and malignant tumor in all ages; however, it rarely occurs in children. the standard treatment for RCC is radical nephrectomy with lymph node dissection when the tumor is localized and can be completely resected. Adjuvant chemotherapy, radiotherapy, and immunotherapy are used for pediatric patients with advanced RCC involving lymph nodes or metastatic lesions. Sorafenib is an oral, multikinase inhibitor that has recently been approved for use in metastatic RCC. Common toxicities that have been reported include dermatologic changes such as rash or desquamation and hand-foot skin reaction, diarrhea, fatigue, alopecia, and hypertension. In particular, hand-foot syndrome (HFS) an erythematous skin lesion of the palms and solesis most often caused by cytostatic chemotherapeutic agents. In this report, we have studied a 14-year-old female patient with hand-foot syndrome that occurred in association with sorafenib for the treatment of metastatic RCC. Furthermore, this case demonstrates that reversal of complications can be achieved by discontinuing the drug and intervention with topical steroids, vitamin E, and high-dose pyridoxine.

Current Trends and Future Directions of Research in the Area of Gifted Education in Korea (최근 국내 영재교육 연구의 흐름: $2000{\sim}2006$년도 연구물 분석)

  • The purpose of this study is to investigate current trends and future directions of research in the area of gifted education through the analysis of published manuscripts on giftedness and gifted education between $2000{\sim}2006$. About 521 articles among 35 journals and 49 dissertations listed in the Korea Education and Research Information Service, including the journal of gifted/talented education and the journal of giftedness and gifted education, were mainly analyzed in the present study. The articles were examined by topics, domains, ages, and research methods both yearly and synthetically. The most widely researched topic was curriculum and program issues in gifted education, and the topic related to factors and development of giftedness was the second. Most studies have continuously focused on the mathematically and scientifically gifted students, and studies on gifted students in the areas of art, language, and other domains were scant. Issues on underachieving gifted students and underachievement were researched actively in 2005. More research has utilized elementary students as samples rather than middle or high school students. Young children under 7 have attracted much attention by researchers after 2004. Related to research methods, literature review was the most widely used, survey was the second, and experimental and correlational studies were the next. Implications related to results were discussed in depth.

EXECUTIVE FUNCTIONS OF ATTENTION DEFICIT/HYPERACITIVITY DISORDER (주의력 결핍/과잉행동장애 아동의 실행 능력)

  • Objective:This study was designed to investigate the difference between executive function of Attention Deficit/Hyperactivity Disorder(ADHD) group and that of neurotics, and to investigate the developmental aspects of ADHD group's executive function. Method:Executive function between ADHD(N=87) and Neurotics(N=19) was evaluated through their performance on the Wisconsin Card Sorting Test. The results were analyzed by 2-way ANOVA and t-test. Results:The results revealed group difference between ADHD and neurotics in total correct reponses, total error responses, nonperseverative errors, number of categories completed, conceptual level responses. There was no significant difference between the performance of 8-12 aged group and 13-15 aged group. But 7-8 aged group showed significantly poor performance than 8-12 aged in total responses, total error responses, perseverative responses, perseverative error responses, nonperseverative error responses. Conclusions:In comparison to the neurotics group, the children of ADHD group are suggested to be lacking the ability to correct their responses according to the external feedback and they probably respond randomly without self-control. However, as there is no difference between perseverative errors and perseverative responses, the interpretation of this finding warrants caution. It also suggests that the developmental aspects should be considered in the studies of executive functions because there are differences in the performance of executive functions by ages.

Early and Mid-Term Results after Operations for Pulmonary Atresia with Intact Ventricular Septum (온전한 심실중격을 갖고 있는 폐동맥폐쇄증의 수술 후 조기 및 중기 성적)

  • Background: Pulmonary atresia with intact ventricular septum(PA/IVS) is an anatomically heterogenous anomaly with a variety of surgical strategies possible. The purpose of the study is to evaluate the influence of right ventricular size on the early and midterm results of surgical repair of PA/IVS. Material and method: Medical records of 20 consecutive patients with PA/IVS operated on between January 1993 and August 1999 were retrospectively reviewed. There were 12 boys and 8 girls whose ages ranged from 2 days to 14.5 months (median 6 days). Their body weight ranged from 2.52kg to 9.35 kg(median 3.18kg). The preoperative Z-value of the diameter of the tricuspid valve(T-valve) was less than or -4 in 5 patients, between -4 and -2 in 1, between -2 and 0 in 7, between 0 and 2 in 6, and greater than or 2 in 1. All patients who had z-value of tricuspid valve greater than -2.05 were attempted biventricular repair(n=15) and all patients who had it smaller than -4.4 underwent systemic-pulmonary shunt operation only(n=3) or bidirectinal cavopulmonary shunt with right ventricular reconstruction(n=2). Result: Two early deaths(2/20, 10%) occurred. Both were infants who underwent transannular patch with shunt. One of these two had huge right ventricle(Z-value of tricuspid valve = 5). There were 2 late non-cardiac deaths 3 and 7 months after operations respectively. Follow-up was completed in all children at a mean of 35.3 months(range, 5 to 54 months). 10 of 11 survivors who underwent transannular patch or valvotomy with or without shunt procedure were in NYHA functional class I even though some of them had small interatrial communication or patent shunt. All three patients who had shunt procedure only at initial palliation completed Fontan procedures with no death. Two patients who underwent right ventricular outflow reconstruction with bidirectional cavopulmonary shunt were also in good condition. Conclusion: The transanular RVOT patch or valvotomy with or without systemic-pumonary shunt as an initial palliative procedure to achieve biventricular repair for the patients who had neither too small nor too large right ventricle(-2.05$\leq$Z-value of T-value of T-valve$\leq$2) could be performed at low operative risk(1/14 7.1%). Systemic-pulmonary shunt procedure and bidirectional cavopulmonary shunt procedure for the patients who had small right ventricle(Z-value of T-valve$\leq$4.4) could be also performed with low risk. But a patient with huge right atrium and ventricle(Z-value of t-valve=5) had poor operative result.
