말티즈견에서 괴사성 뇌막뇌염 증례 (A Case of Necrotizing Meningoencephalitis in a Maltese Dog)

  • A 18-month-old, male Maltese dog with acute seizure was referred to Veterinary Medical Teaching Hospital, Kyungpook National University. Abnormal neurologic findings included a tendency to fall to the left, turn and circle to the right, negative bilateral menace responses and pupillary light reflexes, and diminished responses to facial sensation testing on both sides. Radiographic signs were normal, except for occipital dysplasia. Magnetic resonance imaging of the brain revealed bilateral asymmetric ventriculomegaly, which is more on left side. Cerebral edema was also suspected. The dog died after 9 days from the onset of seizure, and a necropsy was performed. Histopathologically, the brain lesions in the meninges and the cerebral cortex of this dog was presented. There was severe diffuse nonsuppurative inflammation in perivascular area of the meninges and the cerebral white matter. Based on the results, this case was diagnosed as necrotizing meningoencephalitis in Maltese dog.

Neuroprotective effects of Korean White ginseng and Red ginseng in an ischemic stroke mouse model

  • Background: Stroke is a neurological disorder characterized by brain tissue damage following a decrease in oxygen supply to brain due to blocked blood vessels. Reportedly, 80% of all stroke cases are classified as cerebral infarction, and the incidence rate of this condition increases with age. Herein, we compared the efficacies of Korean White ginseng (WG) and Korean Red Ginseng (RG) extracts (WGex and RGex, respectively) in an ischemic stroke mouse model and confirmed the underlying mechanisms of action. Methods: Mice were orally administered WGex or RGex 1 h before middle cerebral artery occlusion (MCAO), for 2 h; the size of the infarct area was measured 24 h after MCAO induction. Then, the neurological deficit score was evaluated and the efficacies of the two extracts were compared. Finally, their mechanisms of action were confirmed with tissue staining and protein quantification. Results: In the MCAO-induced ischemic stroke mouse model, WGex and RGex showed neuroprotective effects in the cortical region, with RGex demonstrating superior efficacy than WGex. Ginsenoside Rg1, a representative indicator substance, was not involved in mediating the effects of WGex and RGex. Conclusion: WGex and RGex could alleviate the brain injury caused by ischemia/reperfusion, with RGex showing a more potent effect. At 1,000 mg/kg body weight, only RGex reduced cerebral infarction and edema, and both anti-inflammatory and anti-apoptotic pathways were involved in mediating these effects.

Mongolian Gerbil의 Reversible Forebrain Ischemia 모델에 미치는 보양환오탕의 효과 (The Effect of Boyanghwano-tang Extracts on Reversible Forebrain Ischemia in Mongolian Gerbil)

  • Objectives : This study was carried out to evaluate the effect of Boyanghwano-tang Extracts on Reversible Forebrain Ischemia in Mongolian Gerbils. Methods : The water content of cerebral edema and the morphologicalㆍneurocytochemistrical change of neuron in MG were implemented to conduct this study. Results : The change rate of water content by ischemia in the normal group was 78.90% on the third day and 79.12% on the seventh day after an attack of ischemia. The rate in the control group was 80.34% and 81.72%. In the sample group the rate was 79.85% and 80.97%. This is a significant result. According to the result of the optical microscopic examination, the cells seen to be continuous and systematic in the pyramidal cells of hippocampus were changed into discontinuous and unsystematic forms in the control group. But compared with the control group, the cells were less damaged in the sample group. Conclusions : The data shows that the effect of Boyanghwano-tang Extracts on Reversible Forebrain Ischemia in MG is a significant result.

Intracerebral Hemorrhage Following Evacuation of a Chronic Subdural Hematoma

  • Burr hole drainage has been widely used to treat chronic subdural hematomas (SDH), and most of them are easily treated by simple trephination and drainage. However, various complications, such as, hematoma recurrence, infection, seizure, cerebral edema, tension pneumocephalus and failure of the brain to expand due to cerebro-cranial disproportion may develop after chronic SDH drainage. Among them, intracerebral hemorrhage after evacuation of a recurrent chronic SDH is very rare. Here, we report a fatal case of delayed intracerebral hemorrhage caused by coagulopathy following evacuation of a chronic SDH. Possible pathogenic mechanisms of this unfavorable complication are discussed and a review of pertinent literature is included.

Osteoma of the Frontal Sinus with Secondary Subdural Empyema Formation

  • Osteomas of the paranasal sinuses rarely lead to intracranial manifestations. The authors report an unusual case of a frontal sinus osteoma leading to subdural emyema formation. A 19-year-old man presented with headache and fever one month after minor facial trauma. Neuroradiological studies revealed subdural empyema in left frontal lobe with moderate cerebral edema and a osteoma in the left frontal sinus with sinusitis of maxillary sinus. The patient was surgically treated in one stage operation of decompressive craniectomy, removal of subdural empyema with frontal sinus osteoma, and endoscopic sinus surgery via cranial and nasal route. The patient recovered very well after surgery and postoperative antibiotic therapy. The etiology of intracranial infection and the treatment strategy are to be discussed.

대동맥류의 외과적 요법 (Surgical treatment of the aortic aneurysm)

  • Ten patients underwent operation for aortic aneurysms from Jan. 1983 to April 1988 at the Department of Thoracic and Cardiovascular Surgery, Keimyung University Hospital. There were 7 males and 3 females in this series. The age ranged from 16 to 70 years with the mean age of 45 years. The cause of the aneurysm was atherosclerosis in 8 patients, mycosis in 1 patient and unknown in 1 patient. There were two patients with ascending aortic aneurysm treated by Dacron graft replacement, with no hospital death. One patient with aortic arch aneurysm was received Dacron graft replacement under cardiopulmonary bypass and died on the 21st postoperative day because of cerebral edema. There were three patients with descending aortic aneurysm. The aneurysm in two patients was successfully repaired by Dacron graft. One additional patient with ruptured aneurysm died at operation because of ventricular fibrillation. Four patients with abdominal aortic aneurysm were underwent Dacron graft replacement and the results were good.

Role of Diffusion-weighted MR Imaging in Children with Various Brain Pathologies

  • To exhibit our clinical experience of diffusion-weighted (DW) MR imaging for various brain pathologies and to determine its role in characterizing brain pathologies in children. DW images in 177 children (M:F=96:81, mean age, 4.7 years) with various brain pathologies were retrospectively collected over past 3 years. DW images (b value: 1000 s/mm) were reviewed along with corresponding apparent diffusion coefficient (ADC) maps. Brain pathologies included cystic or solid brain tumor (n = 55), cerebral infarct (n = 32), cerebritis with or without brain abscess (n = 21), metabolic or toxic brain disorder (n = 19), demyelinating disease (n = 16), hypoxic-ischemic encephalopathy (n = 16), intracerebral hemorrhage including traumatic brain lesion (n = 15), and posterior reversible leukoencephalopathy (n = 3). We reviewed whether DW images and ADCmaps contribute to further characterization of brain pathologies by defining a chronological age of lesions, the presence of cytotoxic edema in lesions, and the nature of cystic lesions.

제주지역 재래흑돼지 모돈에서 발생한 소금중독증 (Salt poisoning in a Jeju native black sow)

  • A 3-year-old Jeju native black sow with sudden death was requested to the Jeju Self-Governing Provincial Veterinary Research Institute for diagnosis in July 2009. Because the pig died suddenly, we could not find any clinical signs. Gross lesions such as pulmonary edema, redness of heart, liver, kidney and lymph nodes were observed at necropsy. Histopathologically, there were severe eosinophilic meningoencephalitis, atrophy, degeneration and necrosis of laminar neurons, and neovascularization in cerebral cortex. Based on histopathological findings and history taking about temporal suspension of water supply, this case was diagnosed as salt poisoning of a Jeju native sow. In our best knowledge, this is the first report for salt poisoning of sow in Korea, especially in Jeju native pig.

고려홍삼 복합방이 실험적 뇌경색에 미치는 영향 (Effects of Complex formula including Korea Red Ginseng (CKRG) on Brain Ischemia Induced by Occlusion of Middle Cerebral Artery)

  • This study was performed to investigate the effect of complex formula(CKRG) consisting of Panax ginseng Radix rubra Koreana. Ganoderma, Cinnamomi Cortex, Glycyrrhizae Radix and Laminariae Thallus on brain ischemia and injury such as KCN-induced brain injury, forced brain ischemia, pulmonary thrombosis. The results were summarized as follows: 1. CKRG extracts showed a decrease of the duration of KCN-induced coma and showcd an increase in life expectancy. 2. CKRG extracts showed a decrease of neurologic grade in hind limb but did not affect neurologic grades in fore limb. Also. CKRG extracts showed a significant decrease of brain ischemic area and edema in MCA occlusion, 3. CKRG extracts showed a protective effect on pulmonary thrombosis induced by collagen and epinephrine. These data suggested that CKRG extracts could be applied to the protection of brain ischemia and injury.

뇌졸중(腦卒中) 급성기(急性期)에 응용되는 하법(下法)에 대한 연구 (Research on the Xiafa(下法) used at the acute stages of Stroke)

  • Stroke is badly demaged for patient, family, society and country. To reduce a sequelae and return rapidly to society, treatment at acute stages is very important. In the research on xiafa(下法;diarrhea technique) used at the acute stages of stroke, the results were as follows. On the treatment of stroke, we are to control qi(氣) For it, sweeting technique(汗法), vomitting technigue(吐法) and diarrhea technique(xiafa ;下法) are used. For treatment on biaoshizheng(標實 reyufushi, 熱鬱腑實) at acute stages, tongfuxiexiafa(通腑瀉下法) is in general use. At acute stages, mental state is very important. Therefore, Xiafa(下法) have to conduct by zangfu(臟腑)'s law. At the acute stages, xiafa(下法) improved a whole body problems by treatment on a constipation and dysuria. We think that xiafa(下法) clinically effect on the cerebral edema and increased intracranial pressure. In the future, experimental study is needed.
