Obesity and Pulmonary Function in Young Adult Women (20대 여성의 비만과 폐기능에 관한 연구)

  • Purpose: The purpose of this study was to identify Forced Vital Capacity (FVC), Forced Expiratory Volume in 1 Second ($FEV_1$), $FEV_1$/FVC according to obesity in young adult women. Method: Height, weight, body mass index (BMI), and percent body fat (PBF) were obtained by using bioelectrical impedance analysis. Neck circumference (NC), waist circumference (WC) and spirometric values (FVC, $FEV_1$, $FEV_1$/FVC) were obtained for 135 women college students who were healthy and non smokers. Results: Mean BMI and PBF were $21.8kg/m^2$ and 30.5%. Obesity prevalence according to BMI and PBF were respectively 13.3%, and 50.9%. Lean body mass (LBM) was positively correlated with FVC, $FEV_1$, and PBF was negatively correlated with FVC, $FEV_1$ and $FEV_1$/FVC. FVC and $FEV_1$ of the underweight or obese group were lower than those of normal weight group. Conclusion: PBF, but not BMI, is negatively associated with pulmonary function in women college students.

Relationship among Bone Mineral Density, Body Composition, and Metabolic Syndrome Risk Factors in Females

  • Osteoporosis is a disease that increases the fracture rates and a major cause of increased mortality and morbidity in the elderly people. This study is to determine which components of body composition and metabolic syndrome risk factors are important to bone health, we analysed the relationship among bone mineral density (BMD), body composition and metabolic syndrome risk factors in females. Totally 630 females participated in a medical check-up program (mean age 47 years) were selected for this study. Body composition analysis was performed by segmental bioelectrical impedance method, muscle mass, and percent body fat were measured. We also measured metabolic syndrome risk factors including abdominal obesity, HDL-cholesterol, triglyceride, blood pressure and fasting glucose level. Metabolic syndrome was defined by NCEP-ATP III criteria. The lumbar spine and femoral neck BMD were measured using the dual energy X-ray absorptiometry. Osteopenia and osteoporosis were observed in 180 and 51 persons, respectively. Muscle mass and HDL-cholesterol decreased in osteopenia and osteoporosis groups compared to the control group, and the grade was shown progressively by the symptoms. Significant positive correlation between BMD and muscle mass was observed. Multi variable regression analyses showed that % body fat and muscle mass were independent predictors of BMD after adjustment of age, height and weight. In conclusion, the BMD showed negative correlation with the metabolic and body composition was associated with BMD.

The Effect of Management Program of Oriental Medicine for Obesity Control on Overweight and Obese Pediatrics (과체중.비만 소아에 대한 한방비만치료프로그램 효과)

  • Objectives The purpose of this study was to investigate the effect of oriental treatment on pediatric overweight and obesity. Methods From the December 2006 to the May 2008, 19 children were treated of obesity control in J Korean Hospital, electrolipolysis, auricular acupuncture was done 2 times per week for 4 weeks with the consulting of diet and exercise. Nine children were treated for 9 weeks. We checked the change of weight at treatment time and the body composition measured by bioelectrical impedance analysis(BIA) per 2 weeks. Results After 4 weeks treatment, the weight loss was 2.67${\pm}$1.75kg. And after 9 weeks treatment, the weight loss of 9 children was 5.91${\pm}$2.73kg. Total fat mass measured by BIA significantly decreased 2.38${\pm}$1.19kg after 4 weeks. BMI and the weight loss of children were significantly correlated. After 4 weeks treatment the BMI significantly decreased 1.43${\pm}$0.89kg/m$^2$. After 8 weeks treatment the BMI of 9 children significantly decreased 3.31${\pm}$1.47kg/m$^2$. Conclusions The pediatric obesity could be effective treated by herdal medicine and electroacupuncture.

The Change of Weight Loss of Oriental Obesity Treatment (한방비만프로그램의 체중감량 경과 관찰)

  • Objectives The purpose of this study was to investigate the clinical effect of oriental medical therapy on obesity and the progress of weight loss through an oriental obesity program. Methods The subjects were treated from January 2008 to February 2009 in J Korean Medical Hospital. Herbal medical Hospital. Herbal medicine, electrolipolysis, auricular acupuncture were performed during the treatment period. We checked the change of weight every treatment time and measured the body composition by bioelectrical impedance analysis(BIA) every 2 weeks. Results The progress of weight loss was steady in the first 4 weeks except for the sixth weight check. After 4 weeks treatment, the weight loss was 3.52${\pm}$1.98Kg and 4.83${\pm}$2.68%. Total fat mass measured by BIA significantly decreased 2.58${\pm}$1.30Kg after 4 weeks. Age and the weight loss were significantly negative correlated. Conclusions Oriental obesity treatment could be effective on obesity, but further studies would be needed for the foundation of a clinical guideline reflecting various factors.

Prediction of Suitable Site to Measure Abdominal Skin Fold Thickness and Correlation among Obesity Indicators in Patients with Type 2 Diabetes Mellitus (제2형 당뇨병 환자에서 피부두겹두께의 측정부위 예측 및 비만지표들 간의 관련성)

  • Purpose: The purpose of this study was to predict measuring site suited for abdominal skin fold thickness (ASFT) by measuring the distribution of abdominal subcutaneous fat thickness (AScFT) and ascertain the correlations among obesity indicators. Methods: The size of analysis materials was 124 secondary data measured by ultrasonic device, bioelectrical impedance analyzer and caliper. Data were analyzed using t-test, and Pearson's correlation. Results: The average of AScFT was 10.63± 6.79mm with its range 1.39-36.16 mm, and AScFT of female and of central parts were thicker than those of male and outer parts in the abdomen. The average of ASFT was 29.26±12.59 mm. Site 5 on Figure 1 was most similar to the average of AScFT in both sexes. Body mass index (BMI) and waist hip ratio (WHR) were 23.65±3.98 and 0.88±0.05 respectively. The body weight, BMI, WHR, visceral fat, ASFT vs AScFT revealed in significant correlation (r= .29, r= .55, r= .39, r= .33. r= .29). Conclusion: BMI and WHR seem more useful than other obesity indicators, when obesity control is necessary for Type 2 diabetes patients. Site 5 on Figure 1 is most suitable site to measure ASFT.

A Patient with Liver Fire Flaming Upward and Metabolic Syndrome Improved by Korean Medicine: A Case Report (대사증후군이 동반된 간화상염 변증 환자에 대한 한의정신요법 병행 한의치료 증례보고)

  • The purpose of this study is to report a case of a patient with pattern identification of liver fire flaming upward and metabolic syndrome improved by Korean medicine. The patient felt completely lost. She was treated for headache, insomnia, and metabolic syndrome with Korean medicine and psychotherapy. Pittsburgh Sleep Quality Index (PSQI), Insomnia Severity Index (ISI), Pattern Identification Tool for Insomnia (PIT-Insomnia), Minnesota multiphasic personality inventory-2 (MMPI-2), and bioelectrical impedance analysis (BIA) were used for assessment. During treatment for six months, the patient stopped taking headache pills. Her blood pressure was stabilized. Her body weight, body fat, body mass index (BMI), and visceral fat decreased significantly. The sum of ISI, PSQI, PIT-insomnia also decreased significantly. Her Ego strength increased in the MMPI-2 supplementary scale. We assume that Korean medicine treatment through acupuncture, herbal medicine, and psychotherapy is effective in treating pattern identification of liver fire flaming upward and metabolic syndrome.

The Suggestion of MF BIA Analyzer Method to Reflect the Resistance in the Extracelluar and Intracelluar Fluid at Acupoint & the Evaluation of Reflection Rate on the Tissue under Skin (MF-BIA Analyzer를 이용한 경혈의 세포 내액과 외액에 의한 저항성분 추정 방법 제시 및 반영률 평가)

  • Objectives : The aim of this study was to suggest a new method to estimate the resistance by acupoint compositions by using the multi-frequency bioelectrical impedance analysis(MF-BIA) of 5 kHz, 50 kHz and 200 kHz within 2 cm of acupoints as a local segment. Moreover, we try to confirm the relation to between measured resistance at skin surface and measured resistance by various manual acupuncture needle insertion depth. Methods : Ten male subjects participated in this study. We measured the resistance at left/right ST36 at skin surface and various manual acupuncture needle insertion depth(skin, 5 mm, 10 mm, 15 mm, 20 mm, 25 mm and 30 mm). Results : It was also observed that the all measured resistances were the highest at 5 kHz and the lowest at 200 kHz. There were significant differences at 5 kHz, 50 kHz and 200 kHz between measured resistance at skin surface and measured resistance by various manual acupuncture needle insertion depth(p<0.05). There was no significant difference in the left and right identical acupoints under the identical condition(p>0.05). Conclusions : We conclude that the measured resistance at skin surface has limitation as to reflect the information of tissue. However, the measured resistance at each frequency was changed as similar pattern by different insertion depth. Hence, we confirmed the possibility of assumption on information of tissue which was expected to locate an acupoint.

Reliability and Data Integration of Duplicated Test Results Using Two Bioelectrical Impedence Analysis Machines in the Korean Genome and Epidemiology Study

  • Objectives: The Korean Genome and Epidemiology Study (KoGES), a multicenter-based multi-cohort study, has collected information on body composition using two different bioelectrical impedence analysis (BIA) machines. The aim of the study was to evaluate the possibility of whether the test values measured from different BIA machines can be integrated through statistical adjustment algorithm under excellent inter-rater reliability. Methods: We selected two centers to measure inter-rater reliability of the two BIA machines. We set up the two machines side by side and measured subjects' body compositions between October and December 2007. Duplicated test values of 848 subjects were collected. Pearson and intra-class correlation coefficients for inter-rater reliability were estimated using results from the two machines. To detect the feasibility for data integration, we constructed statistical compensation models using linear regression models with residual analysis and R-square values. Results: All correlation coefficients indicated excellent reliability except mineral mass. However, models using only duplicated body composition values for data integration were not feasible due to relatively low $R^2$ values of 0.8 for mineral mass and target weight. To integrate body composition data, models adjusted for four empirical variables that were age, sex, weight and height were most ideal (all $R^2$ > 0.9). Conclusions: The test values measured with the two BIA machines in the KoGES have excellent reliability for the nine body composition values. Based on reliability, values can be integrated through algorithmic statistical adjustment using regression equations that includes age, sex, weight, and height.

Body Composition Factor Comparisons of the Intracellular Fluid(ICW), Extracellular Fluid(ECW) and Cell Membrane at Acupuncture Points and Non-Acupuncture Points by Inducing Multiple Ionic Changes (생체이온 변화 유발 후 경혈과 비경혈에서의 생체 구조 성분 분석 및 비교를 통한 경혈 특이성 고찰)

  • Objectives : The specificity of acupuncture point has been a highly controversial subject. Existing researches said that ion-distribution differences are observed on the acupuncture point. This study was conducted under the assumption that multiple ionic changes induced by muscle fatigue would be different between the acupuncture point with non-acupuncture point. Methods : To induce the identical fatigue, twenty subjects performed the knee extension/flexion exercise using the Biodex System 3. ST32 and ST33 as well as adjacent non-acupuncture points were selected. We measured blood lactate and analyzed the median frequency(MF) and peak torque. To obtain the information on the extracellular fluid(ECW), intracellular fluid(ICW) and cell membrane indirectly, we used the multi-frequency bioelectrical impedance analysis(MF-BIA) method. Results : MF, peak torque and blood lactate level of all measurement sites were gradually returned to normal. Re resistance of ST32 had a stronger response, but a non-acupuncture point adjacent to ST33 had a larger response up to 20 minutes post exercise. Ri resistances were similar for both acupoints and non-acupoints. The $C_m$ capacitance of ST32 had a stronger response after inducing fatigue, but ST33 had a smaller response than a non-acupuncture point adjacent to it. Conclusions : In comparison with before and after inducing fatigue, the specificity of acupuncture points was not clearly observed. Hence, we concluded that the body composition factors extraction method had the limitation as a method of finding the specificity of acupuncture points by inducing fatigue.

Correlation between Measured Resting Energy Expenditure and Predicted Basal Energy Expenditure in Female College Students (여대생을 대상으로 한 실측 휴식대사량과 예측 기초대사량의 상관관계에 관한 연구)

  • The aim of this study was to confirm the validity of predictive equations for the calculation of basal energy expenditure (BEE). One hundred twenty female college students were participated in this study. The resting energy expenditure (REE) was measured by indirect calorimetry for 30 minutes following an 12 hour overnight fasting. Among the available equations for predict BEE, Harris-Benedict, WHO/FAO/UNU and Cunningham methods were selected. Body composition was estimated by bioelectrical impedance analysis (BIA) for the equation of predicted BEE. The mean of measured REE was 1257.2$\pm$147.9 kcal/day, while the predicted value by Harris-Benedict, WHO/FAO/UNU and Cunningham were 1373.3$\pm$45.4 kcal/day, 1290.0$\pm$61.7 kcal/day and 1187.6$\pm$49.2 kcal/day, respectively. The Cunningham equation was more closed to measured values than Harris-Benedict and WHO/FAO/UNU equation by the correlation coefficient. Comparing Pearson's correlation coefficients, fat-free mass (FFM), body surface area (BSA) and body weight were higher than others such as height, body mass index (BMI), fat and fat%. The FFM's correlation coefficient was the highest as 0.74. Thus, the conclusion of this study suggested that the main determinant of BEE was FFM, and we derived a prediction equation as follows: BEE=-569.86+48.27 (FFM).
