Clinical and Radiological Results of Reverse Total Shoulder Arthroplasty Using a 25-mm Glenoid Baseplate

  • Background: The size of the baseplate used in reverse total shoulder arthroplasty (RTSA) tends to be larger than the average size of the glenoid in the Korean population. The mismatch between the sizes of the baseplate and the patient's glenoid may result in improper fixation of the glenoid baseplate. This in turn may lead to the premature loosening of the glenoid component. Thus, we evaluated the short-term results of using a 25-mm baseplate in RTSA. Methods: Seventeen patients with cuff tear arthropathy underwent RTSA with a 25-mm baseplate. The mean age of the patients was 70.1 years, and the mean follow-up period was 14.0 months. We evaluated clinical outcomes preoperatively and postoperatively: the range of shoulder motion, the American Shoulder and Elbow Surgeons (ASES) score, and the Korean Shoulder Society (KSS) score. Results: We found that the mean ASES score and KSS improved from 35.0 to 74.4 (p<0.001) and from 46.9 to 71.8 (p<0.001) with RTSA. The mean forward elevation and abduction, external rotation also improved from $78.6^{\circ}$ to $134.3^{\circ}$ (p<0.05) and from $66.6^{\circ}$ to $125.0^{\circ}$ (p<0.05), from $20.2^{\circ}$ to $28.4^{\circ}$ (p=0.43). Postoperative complications were seen in 12% of patients, but neither the loosening of the glenoid baseplate nor inferior scapular notching were observed. Conclusions: In sum, the results of using a 25-mm baseplate in RTSA were similar to those of previous reports. Even though the outcomes are those of a short-term follow-up, neither the loosening of the glenoid baseplate nor the scapular notching were observed.

  • 본 연구에서는 중심 축하중을 받는 H형강 기둥의 베이스플레이트의 지압응력의 분포와 설계에 대하여 조사하였다. 일반적으로 강구조 기둥의 베이스플레이트는 지압응력이 등분포하다고 가정하고 그 크기와 두께를 결정한다. 그러나 중심 축하중이 적으면 베이스플레이트의 크기도 작아지고 두께도 얇아지며 지압응력은 등분포하게 되지 않으며 기둥단면의 하부에 집중된다. 본 연구에서는 실험적 방법과 해석적 방법으로 지압응력의 분포에 대하여 조사하고 그 결과를 이용하여 설계법을 검토하였다. 7개의 H형강 기둥 베이스플레이트 시험체를 제작하여 실험을 수행하였다. 또한 유한요소해석프로그램인 ANSYS를 이용하여 베이스플레이트의 지압응력의 분포를 해석하였다. 연구결과 지압응력은 기둥단면의 하부에 집중되고 등분포하지 않아 강구조 한계상태 설계기준에서의 등분포한 지압응력의 가정은 적정치 않았다.

Analysis and design of demountable steel column-baseplate connections

  • This paper aims to investigate the demountability of steel column-baseplate connections subjected to monotonic and cyclic loading. This paper presents the finite element analysis of steel column-baseplate connections under monotonic and cyclic loading. The finite element model takes into account the effects of material and geometric nonlinearities. Bauschinger and pinching effects were also included in the developed model, through which degradation of steel yield strength in cyclic loading can be well simulated. The results obtained from the finite element model are compared with the existing experimental results. It is demonstrated that the finite element model accurately predicts the initial stiffness, ultimate bending moment strength of steel column-baseplate connections. The finite element model is utilised to examine the effects of axial load, baseplate thickness, anchor bolt diameter and position on the behaviour of steel column-baseplate connections. The effects of various parameters on the demountability of steel column-baseplate connections are investigated. To achieve a demountable and reusable structure, various design parameters need to be considered. Initial stiffness and moment capacity of steel columnbaseplate connections are compared with design strengths from Eurocode 3. The comparison between finite element analysis and Eurocode 3 indicates that predictions of initial stiffness for semi-rigid connections should be developed and improved design of the connections needs to be used in engineering practice.

Ten technical aspects of baseplate fixation in reverse total shoulder arthroplasty for patients without glenoid bone loss: a systematic review

  • The aim of this systematic review was to collect evidence on the following 10 technical aspects of glenoid baseplate fixation in reverse total shoulder arthroplasty (rTSA): screw insertion angles; screw orientation; screw quantity; screw length; screw type; baseplate tilt; baseplate position; baseplate version and rotation; baseplate design; and anatomical safe zones. Five literature libraries were searched for eligible clinical, cadaver, biomechanical, virtual planning, and finite element analysis studies. Studies including patients >16 years old in which at least one of the ten abovementioned technical aspects was assessed were suitable for analysis. We excluded studies of patients with: glenoid bone loss; bony increased offset-reversed shoulder arthroplasty; rTSA with bone grafts; and augmented baseplates. Quality assessment was performed for each included study. Sixty-two studies were included, of which 41 were experimental studies (13 cadaver, 10 virtual planning, 11 biomechanical, and 7 finite element studies) and 21 were clinical studies (12 retrospective cohorts and 9 case-control studies). Overall, the quality of included studies was moderate or high. The majority of studies agreed upon the use of a divergent screw fixation pattern, fixation with four screws (to reduce micromotions), and inferior positioning in neutral or anteversion. A general consensus was not reached on the other technical aspects. Most surgical aspects of baseplate fixation can be decided without affecting fixation strength. There is not a single strategy that provides the best outcome. Therefore, guidelines should cover multiple surgical options that can achieve adequate baseplate fixation.

오리피스 형상에 따른 발사관 내 부가추력 특성 연구 (Analysis of the Thrust Augmentation in the Canister with Baseplate Orifices)

  • If the flow of booster gas which is exhausted to the rear part of a canister is properly restricted in the canister of a hot-launch system, the resultant pressure built up in the canister provides additional force to accelerate the missile to a required launch velocity. These thrust augmentation performances can be controlled through the configuration design of baseplate orifices. In this paper, the simple technique to analyze the thrust augmentation performances of baseplate orifices is suggested and the thrust augmentation characteristics by its various configurations are compared. According to the initial displacement of a missile, the inner pressure of a canister is measured from scaled cold flow tests, and the discharge coefficient of baseplate orifices is calculated. Then the thrust augmentation in a canister is simulated by applying these discharge characteristics to the AMESIM software for launch dynamics.

  • 본 연구에서는 중심축하중을 받는 각형강관기둥의 베이스플레이트의 지압응력의 분포와 설계에 대하여 조사하였다. 일반적으로 강구조 기둥의 베이스플레이트는 지압응력이 등분포하다고 가정하고 그 크기와 두께를 결정한다. 그러나 축하중이 적으면 베이스플레이트의 크기도 작아지고 두께도 얇아지며 지압응력은 등분포하게 되지 않으며 기둥단면의 하부에 집중된다. 본 연구에서는 실험적 방법과 해석적 방법으로 지압응력의 분포에 대하여 조사하고 그 결과를 이용하여 설계법을 검토하였다. 4개의 각형강관기둥 베이스플레이트 시험체를 제작하여 실험을 수행하였다. 또한 유한요소해석프로그램인 ANSYS를 이용하여 베이스플레이트의 지압응력의 분포를 해석하였다. 연구결과 축하중이 적은 경우 지압응력은 기둥단면의 하부에 집중되고 등분포하지 않으므로 유효폭의 개념을 이용하여 베이스플레이트를 설계하는 방법이 적절함을 나타내었다.

  • Free vibration analysis was performed for a spinning disk/spindle system mounted on a flexible baseplate. A simplified model was presented considering the effects of the baseplate flexibility on a disk/spindle system, and the equations of motion were derived by the assumed mode method and Lagrange's equation. From the results of the tree vibration analysis, the variations of the natural frequencies were investigated by changing rotating speed, baseplate thickness. They were attributed to the coupling between the flexible modes of the spinning disk/spindle system and the baseplate. This simplified model was used to predict the dynamic characteristics of a small size disk drive. The validity of the simplified model was verified by experiments and FE analysis.

  • 본 연구에서는 현재 신교통 수단으로 연구 중인 바이모달 트램의 전용 교량을 개발하는데 있어 바이모달 트램 운행 특성을 고려한 실물크기의 중공화된 프리캐스트 바닥판을 새롭게 설계 제작하여, 신뢰성 있는 비선형 유한요소해석 프로그램(RCAHEST)을 통한 해석 결과와의 비교 분석을 바탕으로 적합성과 타당성을 검증하였다. 하중 재하 위치를 변수로 실험을 수행하여 설계 하중에 비하여 약 8~9배의 충분한 안전율을 확보하고 있음을 확인하였으며, 본 연구에서 적용한 재료적 비선형성을 고려한 비선형 유한요소해석 프로그램을 통한 해석 결과 역시 콘크리트의 균열 발생 및 철근의 항복을 포함한 파괴시까지의 극한 거동 특성을 비교적 적절히 평가하고 있음을 알 수 있다.

HDD 액추에이터의 스웨이징성능향상을 위한 베이스플레이트 최적설계 (Baseplate Design to Improve Swaging Performance of Actuator in a HDD)

  • In the manufacturing process of HDD, ball swaging method is commonly used to joint the Head Gimbal Assembly(HGA) with the arm of the actuator. The hub on the HGA is placed into the hole of the actuator arm, and the hub and arm is bonded by the pressure of steel ball. The pressure for plastic deformation on the baseplate causes the undesirable deformation on HGA, such as tilting, flying height change of head. After obtaining the key parameters that have large sensitivity on the swaging process, the optimal shape of baseplate is proposed to increase the static performance during swaging process. Contribution of the proposed design for the swaging performance is verified by contact simulation with elasto-plastic deformation.

Whole brain radiotherapy using four-field box technique with tilting baseplate for parotid gland sparing

  • Purpose: The aim of this study is to evaluate the efficacy and feasibility of four-field box whole brain radiotherapy (FB-WBRT) with tilting baseplate by comparing bilateral WBRT (B-WBRT). Methods and Materials: Between March 2016 and September 2018, 20 patients with brain metastases underwent WBRT using the four-field box technique. WBRT is performed with a dose of 30 Gy in 10 fractions daily. Two computed tomography simulations per person were performed. One was in the traditional supine position for B-WBRT and the other by applying the tilting acrylic supine baseplate to elevate the head by 40° for FB-WBRT. The B-WBRT used the field-in-field technique, which is the most commonly used method in our institution. The FB-WBRT comprised anterior, posterior, and bilateral beams. A wedge was applied in anterior and posterior fields to compensate for skull convexity. Results: The average of Dmean of both parotid glands was 10.2 Gy (range, 3.8 to 17.8 Gy) in B-WBRT and 5.4 Gy (range, 2.0 to 11.7 Gy) in FB-WBRT (p < 0.05). Compared to B-WBRT, FB-WBRT reduced the mean dose of the right and left parotid glands from 10.1 Gy to 4.9 Gy and from 10.4 Gy to 5.8 Gy, respectively (p < 0.05). Further, V5, V10, V15, V20, and V25 for the parotid gland decreased significantly in FB-WBRT (p < 0.05). The Dmax and Dmean of lens decreased according to the dose-volume histogram. Conclusion: Compared to B-WBRT, FB-WBRT with a tilting baseplate is a simple and effective method that takes feature of noncoplanar beam to protect the parotid gland.
