Background : The purpose of the present study was to evaluate the direct and indirect composite restorations which had been placed for 1 year Methods : The composite restorations which had been placed between 1999. Mar and 1999, Dec was evaluated after 1 year For direct restorations. Spectrum (Dentsply, USA) and Z100 (3M, USA) were used in the anterior teeth and Surefil (Dentsply, USA) were used. For class V restorations of anterior and posterior teeth. Spectrum was used. For indirect restorations, Targis/Vectris system (Vivadent/Ivoclar, Liechtenstein) was used 2 examiners evaluated marginal quality, proximal contact. discoloration, presence of 2$^{nd}$ caries, loss of filling and hypersensitivity of restorations. The restorations was clinically evaluated by modified methods based on USPHS. Results : 60 teeth were evaluated. 59 were clinically acceptable and 1 restoration which was placed in class v cavity in the posterior tooth was fallen out. In most cases, the restorations were clinically accept-able. For restorations which had been directly placed in the class II cavities, loose proximal contact was indicated as the main complaints. Conclusions : Most of Anterior and posterior restorations which bad been directly or indirectly placed for 1 year were clinically acceptable. For posterior teeth, loose proximal contact was indicated as the main problem in the directly placed Class II restorations. Long term clinical study is needed.
The anterior guidance is one of the important factors in prosthodontic treatment of anterior teeth. The lingual surface shape of anterior restorations is so critical that small errors of treatment procedure can cause discomfort of the patient and disharmony of the dentition. If the anterior restorations are achieved harmonious anterior guidance through the fabrication and adjustment of provisional restorations, it is important to accurately reproduce the lingual surface shape of provisional restorations as the final prosthesis. In this case report, it was used the modified Dahl principle and copy-milled technique of computer-aided design/computer-aided manufacturing (CAD/CAM) system for systematic diagnosis and treatment. Therefore, we tried to reconstruct the restoration shape more precisely by setting the appropriate anterior guidance and superimposing the digital image of the abutment teeth and the provisional restorations. Thus, by promoting functional and esthetic recovery, this case report demonstrates satisfying results to both the patients and dentist.

The purpose of this study is to investigate the effect of dental restorations on the periodontal and periapical tissues. The author examined 620 cases of amalgam, 390 cases of gold inlay, 422 cases of crown and 644 cases of bridge through the standard intraoral films being appended in the charts that had been kept at the Dept. of Oral Diagnosis in Seoul National University Hospital. This study obtained the following results; 1. The restorations of amalgam, gold inlay crown and bridge were found more frequently in female than in male. 2. The restorations of amalgam, gold inlay and crown were found more numerously in mandibular teeth than in maxillary teeth in both sexes. But in the case of crown, the fact is quite the reverse especially in anterior teeth. 3. On the contrary, in the case of bridge, the restorations of bridge were much more distributed in the maxillary teeth than in the mandibular teeth. 4. Roentgenographic changes of periodontal tissues whose teeth were treated with any type of four restorations were periodontal space widening, lamina dura discontinuity and periapical lesion in the order described in both sexes. 5. On the occasion of between amalgam and gold inlay or between crown and bridge, the differences of periodontal changes were of no consequence. On the other hand, the differences of periodontal changes were apparant between the group of amalgam & gold inlay and the group of crown & bridge.

STATEMENT OF PROBLEM. Little information is available about the buccal gingival level of multiple implant restorations. PURPOSE. This study was aimed to evaluate the relationship between width and height of buccal soft tissue around single and 2 adjacent implant restorations. MATERIAL AND METHODS. Four implant restoration groups (first and second molars, single second molars, posterior single restorations between teeth, and anterior single restorations between teeth) were randomly chosen from one dental institute. Each group comprised of 6 patients. After 6 months of function, silicone impressions were taken and stone models were fabricated for each restoration group. The stone models were cut in bucco-lingual direction at the most apical point of buccal gingival margin. The height and width of buccal supra-implant soft tissue were measured. One way ANOVA and Tukey HSD post hoc tests were performed to analyze the data obtained (P < .05). RESULTS. The most unfavorable width-height ratio was noted for the group, which was comprised of the second molar in the multiple adjacent (first and second molar) implant-supported restorations. The group also resulted in the shorter height of buccal supra-implant mucosa rather than that of anterior single implant restorations between natural teeth. CONCLUSION. To achieve a favorable level of buccal gingival margin, greater thickness of buccal supra-implant mucosa is required for the implant restorations without a neighboring natural tooth compared to the implant restorations next to a natural tooth.

One of the important things in the full mouth reconstruction is the determination of therapeutic position. Centric jaw relation is used as a therapeutic position for the full mouth reconstruction. There are several techniques associated with recording this position. Five clinically acceptable techniques are as follows: 1) Swallowing or free closure, 2) Chin point guidance, 3) Bimanual method 4) Myo-monitor technique, 5) Anterior deprogrammer. Centric relation obtained utilizing the anterior acrylic resin platform in this case. Another important thing in full mouth reconstruction is provisional restoration. Provisional restorations are an excellent diagnostic instrument, especially in full remain esthetics, phonetics, function, parafunction, and dysfunction after evaluation and acceptance through clinical trial with the provisional restorations should be accurately transferred to the final restorations to ensure the same clinical success. Especially, anterior guidance should be accurately transferred to the final restorations. An accurate anterior guidance is critical for optimal esthetics, phonetics, comfort, function, stress minimization, and longevity of teeth and restorations. To record optimum anterior guidance, customized anterior guide table is used in this case. Considering previously mentioned points, we did successive treatment. And it resulted in a better situation esthetically and functionally. Followings are what we cared in treating a patient in this case. 1) Accurate centric relation recording 2) Accurate transference of anterior guidance to the final restorations.

A technique is presented through clinical cases for restoring anterior teeth to enhance dental esthetics using bonded partial ceramic restorations (BPCRs) without or with minimum reduction of tooth structures. These restorations offer the advantages of the superior properties of ceramic materials and the conservative nature of a direct composite restoration at the same time.
Increased anterior teeth mastication following posterior teeth loss leads to greater anterior occlusal force. It may cause greater attrition of anterior teeth, traumatic force occlusion (TFO), also often followed by antagonist extrusion and occlusal disharmony. This clinical report describes the treatment for a 67-year-old female patient diagnosed with loss of both maxillary and left mandibular posterior teeth, severe attrition of maxillary and mandibular anterior teeth and extrusion of multiple teeth. A diagnostic cast was mounted on articular in centric relation (CR) position to evaluate vertical dimension (VD) and interspace. To provide adequate space for the prosthetic reconstructions, VD was increased by 3 mm on the anterior pin. And then diagnostic wax-up was completed upon that VD. Wax-up was converted to provisional restorations and verified in the patient's mouth and the final restorations were delivered. Clinical follow up examination held 3 months after temporary restoration owing to changes in vertical dimension revealed proper function in mastication without evidence of temporo-mandibular joint (TMJ) disorders. This clinical report presents successfully restoring severe attrition case with increasing vertical dimension resulting in satisfaction in esthetics and function.

Some patients with generalized attrition and teeth discoloration may want their anterior teeth to be treated just for esthetic improvement. Ameologenesis imperfecta, however, should be considered for such patients prior to any treatment with thorough clinical and radiographic examination. If a patient is diagnosed with amelogenesis imperfecta, the treatment on anterior teeth just for esthetic purpose is not advisable. In this case, a young man with amelogenesis imperfecta was treated with metal-ceramic restorations. The patient had generalized attrition, teeth discoloration, crown fracture, and cross-bite on the left teeth. The ultimate objective of this treatment was to enhance esthetics and masticatory function. The cross-bite on the left anterior teeth was treated with restorations, whereas the reverse horizontal overlap was maintained on the posterior. The patient was satisfied with the result esthetically and functionally, and the third month recall examination revealed no pathologic changes associated with the treatment.
Maxillary anterior crown fractures are a common form of injury that mainly affects children and adolescents. The position of maxillary incisors and their eruptive pattern carries a significant risk for trauma. In the pre-adhesive era, fractured teeth needed to be restored either with pin-retained inlays or cast restorations that sacrificed healthy tooth structure and were a challenge for dentists to match with adjacent teeth. But. recently the development of adhesive dentistry has allowed dentists to use the patient's own fragment to restore the fractured tooth. The purpose of this paper is to present 2 cases of reattachment of fractured teeth which had different fracture mode, and to evaluate prognosis of cases.
An endodontically treated tooth is likely to be brittle than a vital tooth. Internal structure of the tooth has been weakened due to a significant removal of dentin by coronal access, canal preparation. There are many controversies concerning with various methods of reinforcing an intact anterior tooth that has endodontic treatment. In this experiment, 128 extracted maxillary anterior teeth were endodontically treated, and prepared with 4 methods of restorations; Composite resin filling with zinc phosphate cement, composite resin filling without zinc phosphate cement, composite resin filling with post, and metal crown with post. An Instron testing machine was used to measure the fracture loads of the specimens. The means of the failure loads for the 4 groups were compared by F-test statistically and the failure modes were observed. The results were as follows; 1. There were no statistically significant difference between the failure loads of the four methods of restoration. 2. Teeth without post were fractured in a horizontal or oblique plane through upper or middle third of the root. 3. In the posted teeth, fractures were occurred around the post. 4. In the metal crowned teeth with post, the fracture were occurred around the post or coronal area.
