Convergence awareness of basic emergency treatment by private ambulance drivers (민간구급차 운전자의 기본 응급처치 지식에 관한 융복합적 의식조사)

  • The purpose of the study was to investigate the awareness of basic emergency care by private ambulance drivers. A self-reported questionnaire was completed by 300 private ambulance drivers in Seoul, Gyeonggi, and Incheon from February 20 to April 1, 2013. The questionnaire consisted of basic life support related airway management(3 items), respiratory management(9 items), circulation(4 items), consciousness(4 items), and infection control(3 items). The awareness of infection control accounted for 87% and emergency care including airway, breathing, consciousness, and respiration was 60%. The knowledge of emergency care was 15.20(maximum 23.0 points). Consciousness was 2.55(maximum 4.0), airway management was 2.08(maximum 3.0), respiration was 5.35(maximum 9.0), circulation was 2.56(maximum 4.0), and infection control was 2.64(maximum 4.0). In order to improve the quality of private ambulance drivers, it is important to develop the emergency care program and expansion.

The Developmental Program of Emergency Medical System in Correctional Facilities (교정시설 내 응급의료체계 발전방안)

  • The purpose of Emergency Medical System(EMS) is what a patient returns to society with recovering mental tone in the shortest time as giving prompt and proper medical treatment to patient in emergency situation, and the Correctional Administration(CA) is purposed for the convict to return and settle down to society after release from prison in success as executing schooling, enlightenment activity, vocational training to convict who was quarantined from the society for the term of imprisonment. The EMS and CA will coincide each other which is reverting people to society in safety. This study aims to suggest the developmental program of EMS in correctional facilities through the cause of emergency situation, system, human resources, establishment, medical equipments, state of budget and the point issue for safety of victim who has many chances that is exposed to physical damage and disease because of particularity of lower culture in correctional facilities and the staff who works there. First, in the view of the correctional facility security system, a proper number of the emergency rescuers should be employed. Second, the effective transportation system along with some emergency medical equipment needs to be established. Third, the correctional officers and the prisoners should be learned the first-aid training which is realistic, practical and systematic. Fourth, the cooperative system should be established such as 1339 emergency medical information center in society. Fifth, the Ministry of Health and Welfare must increase EMS budget for correctional facilities.

Current status of traffic accident victims who were transported by 119 ambulances in Chungcheongnam-do (충청남도 지역의 119 구급으로 이송된 교통사고 환자의 발생 특성)

  • Purpose: The study aimed to analyze the current status of traffic accident victims who were transported by 119 ambulances within the administrative district of Chungnam province and provide essential data for accident prevention. Methods: The pre-hospital care records of patients who called the 119 emergency service in 2019 were obtained from the Chungnam Fire Department. Data pertaining to 13,663 traffic accident victims who were transported to hospitals were analyzed. Results: Patients in those aged ≥60 years accounted for 49.8% of the total cases. In patients aged ≥80 years(n=2,154), motor cycle accidents were highest as 28.3%. In addition, cultivator (n=135) and buggy car (n=79) accident victims were the highest in aged ≥80 years as 66.7% and 67.1%, respectively. Traffic accident victims-population ratio in Chungnam was 0.65%, wherein 2.03% included population aged ≥80 years. Conclusion: It was clear that accidents varied across administrative districts depending on the age group of population distribution. Thus, safety measures for preventing motorcycle, cultivator, and buggy car accidents are necessary for areas with many older people aged ≥80 years.

The Analysis of the Flight Nurses Activities in Helicopter Emergency Medical Services (HEMS) (응급의료전용헬기 항공간호사의 항공이송업무분석)

  • Purpose: This study was aimed to describe Helicopter Emergency Medical Services (HEMS) and analyze the flight nurses' activities in HEMS. Methods: Data were collected retrospectively from the air transportation reports that contained data of 168 patients transported by aircraft to G University medical center in Incheon since June, 2012 to March, 2013. Data were analyzed using descriptive statistics, and Mann-Whitney U test. Results: Average distance of flights was 44.0 km, duration of field treatment took 13.6 minutes, and duration of a flight from scene to hospital was 14.5 minutes. Nursing activities were categorized into 12 direct nursing activities and 5 nursing management activities, and a total number of 7806 nursing activities were occurred in HEMS. The most frequently performed nursing activity was measurement and monitoring (27.9%) followed by medication (11.5%) and respiratory management (8.7%). The most frequent nursing management were information management (11.0%). Nursing activities performed were significantly different depending on the patient's level of consciousness, cause of illness, crew configuration, and type of transportation. Conclusion: This study described HEMS nursing activities performed by flight nurses. Difference in nursing activities according to patient characteristics, crew configuration and type of transportation requires flight nurses to be prepared through educational programs to improve nursing activities and nursing management during air transportation.

  • Glucose is the most importantenergy source in the central nervous system. Because the central nervous system cannot synthesize or store glucose. low blood glucose even for a short period results in no functionally normal activity of the brain. In addition, continuous low blood glucosecan cause irreversible brain damage and brain death. Therefore, the current status of emergency care by first aid staffs during transfer of patients with hypoglycemia has been investigated in this study, 114 patients who have visited the hospital using 119 Emergency Medical Service from January 1, 2004 till December, 2004 and shown blood glucose of 2.8 mmol/L and less on arrival have been involved. In case of nurses, assistant nurses, first aid staffs receiving first aid training and second-class emergency medical technicians(EMTs), they provided basic life support (BLS). In case of first-class EMTs, they measured blood sugar and then they orally administered glucose to conscious patients or provided simple first aids and prompt transfer for unconscious patients. As a result of analysis, it is suggested that first aid retraining is required.

Analysis of the patient who were rejected by 119 emergency requests transferred (119 구급요청 거절 대상 환자의 이송 현황 분석)

  • Purpose: The study aimed to collect data from patients who were rejected by emergency requests for transfer to a tertiary hospital through 119 EMT and to analyze the data in the hospital to improve the plan. Methods: We analyzed 4,702 cases of emergency requests made by patients who were rejected by 119 emergency assistance out of the 22,568 patients who visited the emergency medical center in the C area of G metropolitan city from January 2018 through December 2020. The collected data were analyzed using IBM SPSS Statistics Version XX (IBM Corp., Armonk, N.Y., USA). Results: The major medical department with the largest number of such cases was the department of emergency medicine, with 2,519 cases (53.6%). Simple bruises were the most common diagnosis, with 2,819 cases (61.2%). KTAS classification was the highest with 3,562 patients (75.8%) in grade 4. As for the results, 4,084 patients (86.9%) were discharged from the hospital. Conclusion: Most of the patients who were rejected by emergency requests were non-emergency patients and were discharged from the hospital. emergency requests must be rejected at public relations and sites. In addition, the law should be amended to specifically present the reasons for refusal of emergency requests.

Evaluation of patient transportation and response intervals among emergency medical squads (구급대 환자이송과 반응시간의 평가)

  • Purpose: The purpose of this study was to present evidence for quality management based on analysis of patient transportation and response intervals among emergency medical squads. Methods: The chi-square test was used to determine whether mental status and patient assessment affected direct medical control and hospital destination. One way analysis of variance was used to compare response intervals depending on mental status and patient assessment using data drawn from 1172 prehospital care reports. Results: There was a statistically significant relationship between mental status and direct medical control (p<.001); there was a statistically significant relationship between patient assessment and hospital destination (p=.011). However, there was no statistically significant relationship between mental status and hospital destination. The interval from arrival at the patient's side to departure from the scene showed a statistically significant difference (p<.001, p<.001), however, it took the longest time (16.8 minutes) in unresponsive patients. It showed a statistically significant difference (p<.001) in the interval from arrival at patient's side to departure from the scene depending on patient assessment; however, it took the longest time (9.6 minutes) in emergency patients. Conclusion: There was call for direct medical control based on patient assessment; however, patient transportation and response intervals were not appropriate.

A study on the occurrence and transfer status of emergency patients at Incheon International Airport (인천국제공항의 응급환자 발생 및 이송 현황에 관한 연구)

  • Purpose: The study aimed to analyze the occurrence and transfer status of emergency patients at Incheon International Airport in Korea. Methods: This study design included 810 patients for eight years, from July 1, 2014 to June 31, 2022. The collected data were analyzed with SPSS statistics version 25.0. Results: For the demographics, 531 (65.6%) were males, 151 (19.5%) were in the highest age group (in their 50s), and 289 (35.9%) were foreigners. The most common place of occurrence was in protected areas in the passenger terminal, with 341 (42.1%) of cases occurring here. The time of arrival at the site (z=-3.444, p=.001), stay duration at the site (z=-8.145, p=. 001), and transfer time (z=-3.623, p=.000) were all significantly longer. Conclusion: It is necessary to consider a rapid emergency transportation plan, such as developing a system that simplifies immigration procedures when emergency patients move from protected areas to general areas. Moreover, in order to quickly respond to foreign emergency patients, detailed characteristics will need to be identified. Lastly, considering the long transfer time, an instructing doctor should accompany an ambulance and actively perform advanced resuscitation.

Legal Reviews and Countermeasures against Violence to 119 Emergency Medical Technicians (EMT) (119구급대원의 폭행피해에 대한 법적인 고찰 및 대응방안)

  • There have been various discussions to prevent violence to 119 emergency medical technicians, and the aims of the were as follow ; first, the staff dispatched to the scene should be reinforced so that the patients committing violent acts can be subdued. Secondly, self-defense devices should be provided to 119 emergency medical technicians so that they can use them in a dangerous situation and escape from it. And finally, 119 emergency medical technicians should be allowed to refuse to transport the patients when they or their guardians do violence to them in order to prevent potential violent cases. When countermeasures after violence is committed, it is important to secure evidence and determine strongly to bring charges to the offenders rather than agree to overlook the violence in order to correct them right. It is necessary to introduce a support program to help the victim technicians concentrate on their physical and mental treatment. Violence to 119 EMT should be punished so that the society will think it is natural to punish such offenders. In a safer environment, 119 EMT can perform their duties and it is very important to draw this consensus.

Perceived Usefulness and Attitude toward Smart-glass for First-aid Remote Support among Coast Guards in Korea (응급처치 원격지도용 스마트글래스 사용에 대한 한국 해양경찰의 인지된 유용성 및 태도)

  • This study was to investigate the types of emergencies transported by the Southwestern Coast Guard, the need for telemedicine guidance, and the perception and attitude of smart glasses as a communication method targeting 31 coast guards. A relatively high frequency and training requirement were confirmed for bleeding, abrasion, and abdominal pain. The demand for telemedicine guidance on medication and triage was higher, and the perceived usefulness and attitude scores for the use of smart glasses were 3.76±0.61 and 3.64±0.45, respectively. A moderate correlation between perceived usefulness and attitude toward smart glasses was confirmed (r=.630, p<.01). With the development of technology, it is time to actively introduce new devices such as smart glasses.
