A Qualitative Single Case Study on Change in Interactive Bibliotherapy Experience with A Middle-aged Widowed Spouse (배우자 사별 중년여성의 상호작용적 독서치료 경험에서 나타나는 변화에 대한 질적 단일 사례연구)

  • This paper is a qualitative single case study and it was conducted for the purpose to explore change in interactive bibliotherapy experience with a middle-aged widowed spouse. A Middle-aged Widowed Spouse was taken as a research partaker. Through this research, we explored partitively the partaker's experience changes by each session during the commencing of interactive bibliotherapy and the partaker's inner/external experience change after the end of session. The result, interactive bibliotherapy facilitated and expressed the reaction of a partaker by its writing skills and reading materials including partaker's own poems. Furthermore, facilitated emotion from material and self-awareness made a partaker understand and accept itself through therapeutical interaction. In addition, those kind of changes appeared inner/external experience in life, and lasted after the end of the session. In this regard, the inner/external change appeared from a partaker means the effect of bibliotherapy, and we can notice that it is a result that facilitated through truthful reaction of counselor and texts.

Effects of Interaction of Social Support with Multiple Losses on Depressive Symptoms (노년기 사별로 인한 우울증상에 대한 사회적 지지의 조절 효과 분석)

  • The current study examines the association between multiple losses and depressive symptoms and the role of social support in multiple losses. Using a prospective designed dataset(Changing Lives of Older Couples), this study found a significant difference on the depressive symptom levels between multiple losses and single loss. In addition, there was a significant buffering effect of social support in bereavement, as oppose to previous literature that social support does not buffer the initial bereavement reaction in comparisons between the bereaved with multiple losses and the bereaved with a single loss. The author discusses the importance of monitoring elderly people with multiple losses and availability of social support before and after the loss.

Gender Differences in the Mediating Effects of Social Participation on the Association between Widowhood and Depressive Symptoms: Widowhood, Social Participation, and Depressive Symptoms (배우자 사별과 우울감과의 관계에 있어 사회참여의 매개효과: 성차를 중심으로)

  • The study examined whether informal and formal social participations may mediate the association between widowhood and depressive symptoms, and whether gender may moderate the mediated associations. The data consisted of men and women 65+ who participated in the $3^{rd}$ wave of the Koran Longitudinal Study of Aging (KLoSA ; N = 4,146). Findings suggest that first, widowed older adults experienced significantly higer levels of depressive symptoms compared to their married counterparts. At the same time, the widowed individuals also reported more frequent contacts with family, friends, and neighbors (informal social participation), which contributed to narrowing the depression gap between widowed and married. Second, gender moderated the mediated association such that the mediating effect of informal social participation was significant only for older widowed women. Further, no mediating effect was found of formal social participation (participation in social, leisure, religious groups) for widowed and married regardless of gender. Previous studies indicated that social participation may be an important mediating factor that attenuates the effect of widowhood on depressive symptoms. Building on existing research, the present study highlighted theoretical rationales for potential gender differences in the way social participation may mediate the widowhood-depression linkage and evaluated statistically whether gender may moderate the mediated association.

Factors Influencing Life Satisfaction in Widowhood among Older Women: A Comparison of Three Age Groups (배우자 사별노인의 생활만족도에 영향을 미치는 요인: 연령집단 비교를 중심으로)

  • The purpose of this study was to analyze the factors influencing life satisfaction in widowhood among older females. Using the sixth Korean Retirement and Income Study (KReIS) supplementary survey data, a total sample of 1,471 women aged 65 or older whose spouse had died were selected, divided into 508 young-old (65-74), 745 middle-old (75-84), and 218 oldest-old (85+). Then, factors influencing the life satisfaction of these three groups were analyzed, based on a combination of perceived health and interpersonal satisfaction levels. It was found that among the young-old group, a higher level of subjective health perception indicated by monthly income and material support and interpersonal satisfaction linked to more frequent participation in community gathering combined for an overall higher level of life satisfaction. For the middle-old group, higher life satisfaction was provided by monthly income, informational support, and material support for subjective health perception and no diseases for interpersonal satisfaction. The oldest-old group with higher life satisfaction referred to monthly income for subjective health perception and no experience in donation activities for interpersonal satisfaction. Based on these findings, this study provides implications for policy and practice to improve the life satisfaction of elderly women who have experienced their spouse's death.

The Relationship between Existence of Spouses and Obesity for Korean Adults (우리나라 성인의 배우자 유무와 비만과의 관련성)

  • The population with massive waist circumference is increasing in comparison with that with obesity in entire bodies in Korea. It is assumed that prevalence of obesity is closely related with changes in marriage status such as divorce or separation by death, and age, educational background, diet habits and living environment depending on presence or absence of spouses. To identify whether there are relations between spouse, BMI and waist circumference under the conditions such as life cycle age, educational level, income, smoking, drinking and walking controlled, this study used data of national health and nutrition research organized by Korea Center for Disease Control and Prevention. The subjects of the study were a total of 7,178: 3,043 men and 4,135 women whose ages were above 19 and married. As a result of controlling independent variables except a factor of spouse, there were no statistically significant relations between presence of spouse, waist circumference and BMI in men, but odds rate of waist circumference was lower, 0.78(95% CI 0.636-0.954) when the female subjects had no spouse than when they had spouse.

Development and Validation of the Post-bereavement Growth Inventory in Later Life (노년기 사별 후 성장척도의 개발 및 타당화)

  • The purpose of this study was to develop the Post-bereavement Growth Inventory(PBGI) for the elderly and to evaluate its validity. The items for developing PBGI were compiled from a literature search, content validity review, and a preliminary survey. A main survey of 308 widowed persons(aged 60+) was conducted. The results were as follows. First, an exploratory factor analysis revealed six factors of the PBGI. The results of the confirmatory factor analysis supported a five-factor structure for the final 24 scale items of scale. Second, in terms of concurrent validity, the PGBI compared favorably with the "Personal Growth" scale from The Hogan Grief Reaction Checklist (HGRC: Hogan and Schimdt, 2001). Third, the results of contrast group validity tests demonstrated that the group scoring in the upper 30% on the scales of Psychological Well-being (PWB, Ryff and Keyes, 1995) and the Grief Reaction Inventory (GRI: Remondet and Hansson, 1987) showed a significantly higher degree of post-bereavement growth (PBG) than the group scoring in the lower 30%. Fourth, Cronbach's ${\alpha}$ values for the five factors were between .649-.856 and .907 overall, indicating high internal consistency. Finally, education had the most significant influence on PBG. All results indicated the high validity of the PBGI. The findings from this study could provide the rationale for practical interventions from a growth-oriented perspective for widowed persons, as well as an objective assessment tool for practitioners in clinical areas.

Effects of a problem drinking monitoring program for bereaved older men: A pilot study (사별 남성 노인의 문제성 음주 경감 프로그램 효과성에 관한 파일럿 연구)

  • Purpose: The aim of this study is to test a brief modified version of intervention for problem drinking monitoring program for bereaved men who lost their loved one. Methods: Inclusion criteria were experiencing the recent loss of a spouse and providing informed consent for participation. Of the 32 referred individuals who met the inclusion criteria, 26 participants were recruited. Study participants were randomly assigned into Alcohol problem monitoring(APM) group (N=13) or comparison group (N=13). Results: Older men receiving APM substantial decreased in problem drinking during three months of intervention. Improvement over time was observed on the outcomes, particularly in the Short Michigan Alcoholism Screening Test-Geriatric Version(SMAST-G). Conclusion: The findings that a modified version of APM for bereavement helped improve complicated grief symptoms and social impairments and reduce levels of problem drinking, provide important insights into the interventions for psychological and alcohol-related difficulties in bereavement.

  • Death of spouse is the most heartbreaking stressful and inevitable tragic life event. In middle aged men who belong to the social middle class and accomplished their occupational success, experiences of spousal bereavement are great shock. The aim of this study was to find out how they overcome their mental and physical pain and to obtain the basic materials to develop suitable nursing care programs for them. The methodological approach of this study is Giorgi's phenomenological analysis meaning unit. This method also makes theme focal meaning, situated structural description and create general structural description grasped by participator's experience through situated structure description. This study performed from November 2002 to May 2004, and participators were four men. Data collected through in-depth personal interviews. which had been tapped and analysed the Giorgi's method. Finally, the five focal meaning below have been abstracted. Theme 1. Physical symptom loss of appetite, fatigue, insomnia. outbreak of illness, weight loss. Theme 2. life of spiritless lack of desire, sense of emptiness, unstableness, prosaic life. wandering. indifference of appearances, avoidance of meeting people. Theme 3. life of retrospction reflection for his wife, yearning, grief, muttering to himself, never-to-be forgotten wife, leading a lonely life. Theme 4. negative emotion reproaching, feeling hurt, marriage of daughter, feeling heavy, getting angry, sexual desire, awareness of his sinfulness. loneliness Theme 5. social support and adjustment getting his wife off his mind, curring favor with children, support and consolation by his daughter-in-law, appreciation for hospice nurse, considering remarriage, taking care of himself, good relation with his children. The result of this study showed that middle aged men bereaved of their wife by cancer need other's concern. And we have to study further to understand their experience. Until now hospice nursing is concentrated on cancer patients But from now we have to provide their families suitable nursing care programs to adjust themselves to social life before and after death of patients.

Concept Development of Grief-Focusing on the Process of Spousal Bereavement (애도 개념개발 - 배우자 사별과정을 중심으로 -)

  • Purpose: This study was done to develop the concept of grief focusing on the process of spousal bereavement in Korea. Methods: The Hybrid model was used for analysis according to the 3 phases. An extensive literature review was done for the Theoretical phase. In-depth interviews were conducted with 15 participants whose spouses died within the past 3 years in the Field phase. In the Final analytic phase, the results in the Theoretical and the Field phases were compared, analyzed, and integrated according to the process of grief. Results: The antecedent of the concept of spousal grief was spousal death. The dimensions of grief were classified to inner dimensions related to oneself, relational dimensions related to family and others, and existential dimensions related to the meaning of being. The attributes of grief were physical suffering, decline of cognitive ability, heartbreaking sorrow, expectations and conflicts of a new life, social stigma, dependence on or resentment towards God, etc. The empirical referent of grief was physical, psychological, social, and spiritual health status. The grieving progressed through 3 phases-shock-emancipation, suffering, and integration. Conclusion: Nurses should recognize the importance of their unique position as supporters for grievers, and try to assess individual characteristics and to provide tailored nursing interventions.

A Study on the Relationship between married existence of spouse and drinking behavior (결혼 후 배우자 유무와 음주 행태의 관련성 연구)

  • 서론: 최근 음주가 사회적 문제로 더욱 심각하게 인식되고 있는 가운데 우리나라의 음주 소비량은 점점 증가하고 있는 추세이다. 또한 빠른 사회경제적 변화는 새로운 도덕과 가치의 변화를 가져왔으며 알코올 소비량의 증가와 관계가 있다.(양정남, 1998) 특히 결혼 및 배우자 유무는 음주에 중요한 관련 요인이라는 주장이 있음에도(김태수 등, 2003) 실증적인 연구는 미흡한 실정이다. 따라서 본 연구는 결혼 후 배우자 유무와 음주 형태의 관련성을 파악하고 음주 관련 교육 자료로 활용하고자 한다. 연구방법: 연구 대상자는 2005년 국민건강영양조사에서 40대 이상의 남녀 중 결혼 경험이 있는 14,970명 중 음주행태에 관한 설문에 응답한 3,190명이었으며, 결혼 후 배우자의 유무에 따른 고위험 음주 경험 및 다빈도 음주, 음주자제빈도, 고위험음주빈도와의 관련성을 통계 분석하였다. 연구결과: 결혼 후 배우자 유무에 따른 음주 행태의 연구 결과는 다음과 같다. 연구대상자의 일반적 특성은 남자 1,678명(52.6%)이며, 여자 1,512명(47.4%)였다. 연령별로는 65세 미만 2,359명(73.9%), 65세 이상 831명(26.1%)이었다. 결혼상태는 유배우 2,621명(82.2%), 사별 344명(10.8%), 이혼 및 별거 225명(7.1%)이었다. 고위험 음주경험은 한달에 한번 미만 1,883명(59.0%), 한달에 한번 정도 379명(11.9%), 일주일에 한번 이상 928명(29.1%)으로 한달에 한번 미만 고위험 음주를 경험한다가 가장 많았으며, 일주일에 한번이상. 일주일에 한번정도 순으로 고위험 음주를 한다고 응답하였다. 월평균 음주 횟 수에 있어서는 5회 이하 1,149명(36.0), 6회-10회 395명(12.4%), 11회이상 593명(27.7%)이었다. 고위험 음주 경험을 종속변수로 한 로지스틱 회귀분석에서 여자보다 남자가 고위험 음주 경험을 많아 하며, 연령이 증가하면 고위험 음주음 경험이 낮아지며, 교육수준이 높아지면 고위험 음주 경험은 낮아지는 것으로 나타났다. 음주자제 경험 종속변수로 한 로지스틱 회귀분석에서 여자보다 남자가 음주자제 경험을 많아 하며, 연령이 증가하면 음주자제 경험이 낮아지며, 교육수준이 높아지면 음주자제 경험은 낮아지는 것으로 나타났다. 다빈도 음주 경험 종속변수로 한 로지스틱 회귀분석에서 여자보다 남자가 다빈도음주 경험을 많아 하며, 연령이 증가하면 다빈도 음주 경험 높아지며, 가구 평균 소득이 증가 하거나, 교육수준이 높아지면 다빈도 음주 경험은 낮아지는 것으로 나타났다. 논의 및 결론: 결혼 후 배우자 유무와 고위험 음주는 남녀를 합친 전체와 남자에게서 통계적으로 유의하였으며, 음주자제빈도에서는 남녀를 합친 전체에서 통계적으로 유의하였다. 또한 다빈도 음주에서 여자에게서 통계적으로 유의하였다. 이러한 결과는 절주사업 기초자료로 활용될 수 있는 가능성을 확인하였으며, 추가적인 연구의 필요성을 시사하였다.
