A Study on Greater yin Disease in Gangpyeong-Shanghanlun by Analyzing Four Mental Illness Cases (정신과질환 4례를 통한 강평본(康平本) 『상한론(傷寒論)』의 태음병(太陰病) 진단 고찰)

  • Objectives : This study aimed to suggest distinct signs of Greater yin disease (tàiyīnbìng, 太陰病) by researching the etymology of three terms: zi-tong (自痛), jie-ying (結硬), and yin-er (因爾), which are exclusive found only in Greater yin disease (太陰病) in Gangpyeong-shanghanlun, but not in other categories. Methods : We studied the etymologies of the three terms that are unique in Greater yin disease, then found some pathological signs that are related with them through the four mental illness cases, which include somatization, obsessive-compulsive behavior, delusion, and panic disorder. Results : Based on the definitions of each term, we diagnosed the four patients who had mental illnesses as having Greater yin disease, and we observed meaningful improvements after administering herbal medication. After Gyejigajakyak-tang administration, the Insomnia Severity Index (ISI) score decreased from 18 to 7 and the Beck's Anxiety Index (BAI) score decreased from 36 to 18 in the first case, the ISI score decreased from 27 to 16 and the BAI score decreased from 50 to 33 in the second case, and the ISI score decreased from 23 to 4 and the BAI score decreased from 34 to 5 in the third case. In the last case, the ISI and BAI scores were 16 and 22, respectively, at the first visit, and it was found that the scores had changed to 6 and 22, respectively, at the last visit. Conclusions : We found that the unique terms in Greater yin disease, including zi-tong (自痛), jie-ying (結硬), and yin-er (因爾), can be interpreted as groundless arbitrary assumption, resting strong tension, and psychological projection based on the etymological hypothesis. Therefore, we suggest these as specific signs of Greater yin disease.

Case Series Reporting 7 Somatic Symptom Disorder Cases Diagnosed as Greater Yin Disease by Shanghanlun Provisions (『상한론(傷寒論)』 태음병(太陰病)으로 진단된 신체증상장애 환자 7례 증례 보고)

  • Objectives: This study was conducted to confirm the reproducibility of the diagnosis of greater yin disease (太陰病) and the efficacy of herbal treatment using Gyejigajakyak-tang or Gyejigadaehwang-tang in somatic symptom disorder (SSD). We also further refined the characteristics of greater yin disease in SSD. Methods: We analyzed the demographic and treatment data of seven patients with SSD from four Korean medical clinics. The reliability of diagnosis and the efficacy of the treatments were evaluated by the Beck Anxiety Inventory (BAI), a numerical rating scale (NRS), and the patients' statements. Finally, we analyzed patients according to the diagnostic points of Shanghanlun provision 273. Results: Seven SSD patients took Gyejigajakyak-tang or Gyejigadaehwang-tang. The mean age of the patients was 52.1±15.8 years old, and the herbal medication period was 82.2±38.2 days. At the first visit, the patients showed hypochondriasis, obvious stress, and a severe BAI score (40.4±10.9). The NRS significantly decreased from 8.9±1.6 to 0.7±0.7 after the treatments. There were no side effects reported. As a result of analyzing these 7 cases, hypochondriasis, obvious stress, and anxiety were identified, which had been previously suggested as 自痛, 因爾, and 結硬. Conclusions: We confirmed the reproducibility of greater yin disease in 7 SSD cases from 4 medical institutions. These results imply that there could be a considerable relation between SSD and greater yin disease as defined by the Shanghanlun provisions.

A Case Report of Post-COVID-19 Syndrome with Fatigue Treated with Saengkangsasim-tang based on Disease Pattern Identification Diagnostic System by Shanghanlun Provisions (『상한론(傷寒論)』 변병(辨病) 진단체계(診斷體系)에 근거하여 생강사심탕(生薑瀉心湯) 투여 후 호전된 피로를 주소로 하는 Post-COVID-19 Syndrome 1례)

  • Objective : Fatigue is the most common symptom in post-COVID-19 syndrome. We report the case of a patient with post-COVID-19 syndrome with fatigue treated using herbal medicine (Saengkangsasim-tang). Methods : A 64-year-old man had severe fatigue for 4 months after a severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) infection. Saengkangsasim-tang was administered to this patient based on the disease pattern identification diagnostic system by Shanghanlun provisions (DPIDS). We used the Numeric Rating Scale (NRS) and Beck Depression Inventory (BDI) to evaluate the effectiveness of Saengkangsasim-tang for fatigue in the patient with post-COVID-19 syndrome. Results : According to DPIDS, the patient was diagnosed with the provision 157. After administration of Saengkangsasim-tang for 30days, the fatigue based on NRS and depression based on BDI were improved. Conclusions : The administration of Saengkangsasim-tang to patients with post-COVID-19 syndrome with fatigue, diagnosed according to the Shanghanlun provision 157, may be effective.

Two cases of Atopic Dermatitis Treated with Huini-san (회역산(回逆散)으로 호전을 보인 아토피피부염의 치험 2례)

  • Background and Objective : Atopic dermatitis is a type of dermatitis, an inflammatory, relapsing, non-contagious and itchy skin disorder and affecting up to 30% of children in the Korea. In this study, Huini-san, which is mentioned in Shanghanlun(傷寒論), and acupunture treatment on Atopic Disease was investigated. Methods : We have classified the patients according to the Shanghanlun six meridian patterns diagnostic system and the patients are treated with Huini-san. The severity of Atopic dermatitis was evaluated by SCORAD index, visual analogue scale(VAS) and pictures. Results : After the treatment the SCORAD index and VAS of 2 patients dicreased significantly. Conclusions : Huini-san and acupunture treatment has improved the symptoms of Atopic dermatitis in this study.

The Pathologic Study on Difference between Prescriptions of 『Wenbingtiaobian』 and their Sources (『온병조변(溫病條辨)』 처방의 기원과 처방 변화의 병리학적 고찰)

  • Ojectives : To indicate source of prescriptions in "Wenbingtiaobian" and draw pathologic consideration for differences between prescription and source. Methods : Analysed cause and mechanism of disease, pattern identification, treatment, prescriptions and medicinal herbs based on "Translation of Wenbingtiaobian", "Modern Shanhanlun", "Jinkuiyaolueyishi", "Medical collection of Yetianshi" and "Herbal Formula Science". Results : 64.5% of prescriptions in "Wenbingtiaobian" are derived from "Linzhengzhinanyian", "shanghanlun" or "Jinkuiyaolue". Prescriptions from "shanghanlun" or "Jinkuiyaolue" have been modified to fit for heat pattern differentiations, to expand or reduce their medicinal scope, to build up efficacy by adding cold herbs, herbs of nourishing yin, engendering fluid or outthrusting through the exterior, to diffuse water-dampness or warm yang by adding warm herbs. Prescriptions from "Linzhengzhinanyian" have been modified to eliminate cold-dampness, disperse and outthrust with lightness, tonify yin. Conclusions : Wenbingtiaobian" inherited "Linzhengzhinanyian", "shanghanlun" nd "Jinkuiyaolue" andchanged and developed them to cure the febrile disease in the aspect of prescription, mechanism of disease, pattern differentiation and treatment.

A study on proper dosage of Ma-huang(麻黃) (마황(麻黃)의 적정 용량에 대한 고찰(考察))

  • Objective : The purpose of this study is to find out the proper dosage of Ma-huang for clinical use of Shanghanlun(傷寒論) Methods : To achive the purpose of this study, web-databases(pubmed, naver, google) were searched with the keywords including 'dose of Ma-huang Ephedra Ephedrine','dosage of Ma-huang Ephedra Ephedrine', and 'water extract of Ma-huang Ephedra'. The searched 30 papers and articles were reviewed. Results & Conclusions : 1. Proper dosage of Ma-huang 1) Adult: up to 9-12g/day 2) Adolescent: up to 6g/day 3) Hypertension disorder patient: up to 6g/day 4) lactating women: up to 6g/day 5) child: <2 years 0.7-2.5g/day, $${\geq_-}2$$ years 2.6-6g/day (Different from body weight) Although administration of Ma-huang to hypertension disorder patient, lactating women, child is safe on the paper, It is not recommended to these people because Ma-huang is one of toxic herbs. 2. Dosage form of Ma-huang There's no safety paper about pill or powdered Ma-huang(麻黃). There's not pill or powdered prescription of Ma-hunag in Shanghanlun(傷寒論), either. So it is recommended to administrate water exetract of Ma-huang.

A Case Report of Menopausal Syndrome treated by Herbal medicine based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 준거하여 한약 투여 후 호전된 갱년기 증후군 1례 임상 보고)

  • Objective : This case report aims to demonstrate the effect of Herbal medicine on menopausal syndrome. Methods : A 54 years woman suffered from menopausal syndrome with hot flush, insomnia and dyspepsia. According to Diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Taeyang-byung and treated by Sosiho-tang decoction at first. But the next time the patient visited, she was treated by Oryeong-san decoction. The result of administrations were evaluated by Kupperman's index(KI), Menopause rating scale(MRS). Results : After administration of Sosiho-tang decoction for 35 days, KI was decreased from 50 to 19, MRS was decreased from 9 to 4. But her chronic dyspepsia wasn't getting better. So the KMD prescribed Oryeong-san decoction. After administration of Oryeong-san decoction for 14 days, KI was decreased from 19 to 0, MRS was decreased from 4 to 1. And her chronic dyspepsia was functionally improved. Conclusions : The patient completely recovered from menopausal syndrome and dyspepsia as treated by Sosiho-tang decoction and Oryeong-san decoction according to Diagnostic system based on Shanghanlun provisions.

2 Cases of Baekhogainsam-tang Treatment on Attention Deficit/Hyperactivity Disorder (ADHD) children. (주의력결핍 과잉행동장애(ADHD)아동에게 백호가인삼탕(白虎加人蔘湯)을 투여하여 호전된 2례)

  • Objective: The purpose of this study was to report the improvement of two children with attention deficit hyperactivity disorder (ADHD) following herbal treatment according to the provision of Shanghanlun. Methods: We diagnosed patients on methylphenidate for ADHD as no. 26 provision in Taeyangbyung, and analyzed the improvement from the medical records, teachers' statements, The Korean Version of Inattentive/overactive with Aggression-Conners Rating Scale(K-IOWA CRS). Results: We found a change in the symptom after Baekhogainsam-tang administration. In the first case, the K-IOWA-CRS score changed from 12 to 8 during the previous 70 days of treatment, however, surged to 21 without Baekhogainsam-tang treatment. The score stabilized to 8 after 49 days later. In the second case, the score changed from 9 to 3 following Baekhogainsam-tang treatment. Conclusions: These cases show behavioral improvements with Baekhogainsam-tang treatment in ADHD children, who were diagnosed with no. 26 provision according to Shanghanlun.

  • Objectives : Zhang Zhong-jing's Shanghanlun is based on Six-channels system to classified a disease. This paper is planning to describe the Transmutation among Six-channels system. Six-channels change not fixed in either direction and each is relative to each other. Methods : I will try to describe the Transmutation among Six channels system, through the letter of the Shanghanlun. First, I will find letters related to a disease transmission. Second, It will be described through the analysis of the past medical scientists. Results : Six-Meridian Pattern is the categorization of syndromes according to the theory of the six meridians, applied to the diagnosis of acute febrile diseases at different stages, but also useful for the pattern syndrome differentiation of other diseases. Transmutation among Six channels system is not fixed in a certain order and it each affects one another. Conclusions : We can see that Zhong-jing's medical treatment from syndrome differentiation is associated with a Mutual transmission(相互傳變).

A Study on the I-Ching Perspective in Shanghansuyuanji (『상한소원집(傷寒溯源集)』에 나타난 역학적(易學的) 관점 고찰)

  • Objectives : This paper aims to study the I-Ching perspective in Shanghansuyuanji. Methods : Key words associated with I-Ching was searched in Shanghansuyuanji and analyzed its meaning and categorized thematically. Results & Conclusions : 1. Qianhuang symbolized Shaoyang, The Power of the Great hexagrams, Jueyin Peace hexagrams, Taiyin Gen Earth, Yangming Kun Earth, Shaoyin Return hexagrams, but didn't symbolize Taiyang, making it not uniform. 2. Qianhuang explained physiology focusing the change of YangQi with I-Ching perspective. This is possible because Shanghanlun thought that much of YangQi and I-Ching set a high value on Yang. 3. Qianhuang explained pathology such as insomnia, epigastric fullness, splenic constipation syndrome, Taiyinbing syndrome, somnolence, thirst, YinYang exchange with I-Ching perspective, it is meaningful because it enriches medical YinYanglun. 4. Qianhuang explained prescriptions such as Daqinglongtang, Zhenwutang, Shizaotang, Fuzixiexintang, Dachaihutang, Baihutang, a comparison between Daqinglongtang and Xiaoqinglongtang with I-Ching perspective. This is helpful to grasp the image of prescriptions. 5. Qianhuang explained nature of drugs such as Guizhi, Fuling, Fuzi, Qiandan, Rougui with I-Ching perspective, and it combines well with the features of I-Ching with drugs efficacy. 6. Qianhuang explained diseases of the six Meridians curing time with I-Ching perspective. This shows diseases of the six Meridians recover when Corresponding Qi is vigorous or Conflict Qi appears, and it is persuasive. For reasons mentioned above; Shanghansuyuanji is meaningful as Shanghanlun commentary.
