Objective: We describe the case of an 84-year-old man with a midbrain infarction causing pupil sparing oculomotor nerve palsy, with ipsilateral cerebellar ataxia diagnosed as Claude's syndrome. Methods: The patient was treated with Korean medicine, including herbal medicine, acupuncture, and electro-acupuncture, during 45 days of hospitalization. Improvements in the patient's symptoms and changes in ptosis were evaluated using the Korean version of the Scale for the Assessment and Rating of Ataxia (SARA). Results: After 45 days of treatment with the Korean medicine, the patient's symptoms, including ataxia and ptosis, improved. Conclusions: This study suggests that the treatment with Korean medicine was effective in aiding the patient's recovery.

Objective: To describe the effects of traditional Korean medicine (TKM) on a patient diagnosed with Parkinson's disease. Methods: The patient was treated with Korean medicine, including pharmacopuncture, acupuncture, and Chungpajunsin-bang. The Unified Parkinson's Disease Rating Scale (UPDRS) and modified Hoehn and Yahr staging (H-Y stage) were used to measure changes in amplitude and improvements in the patient's symptoms. Results: After treatment with the Korean medicine, according to the UPDRS and H-Y stage assessments, the patient's tremor improved, with the tremor amplitude decreasing from 30 cm to 5 cm. Conclusions: We suggest that TKM could be effective in reducing tremor in Parkinson's disease.

Objective: This study was undertaken to examine the effect of Cortex Phellodendri on prostatic urethral pressure and mean arterial blood pressure of rabbits. Methods: To measure prostatic urethral pressure and mean arterial blood pressure, a Mikro-Tip catheter transducer was inserted and positioned in the prostatic urethra and left carotid artery. After a stabilizing period, phenylephrine ($1{\mu}/kg$) was intravenously administered two or three times to increase the urethral pressure and mean arterial blood pressure. Cortex Phellodendri (2.5 mg/kg and 5 mg/kg doses of Cortex Phellodendri extracted from 80% Ethanol) was administered intravenously, followed by phenylephrine, with no time interval between the doses. The urethral pressure and mean arterial blood pressure were then measured to determine whether they had stabilized. Results and Conclusion: Cortex Phellodendri appeared to inhibit phenylephrine-induced increases in prostatic urethral pressure and mean arterial blood pressure.

Objective: This study describes the case of a patient with a left basal ganglia (BG) infarct who had made almost a complete recovery upon discharge from the hospital in 2014 but who was re-admitted after 31 months with a worsened hemiplegic gait. Methods: The patient had undergone no rehabilitation treatment in the 31 months since his discharge. When the patient was re-admitted to our hospital in 2017, stance and gait data were collected on the patient using a treadmill gait analysis system. In addition, the patient underwent a manual muscle test (MMT) evaluation, and his Motricity index (MI) and modified Barthel index (MBI) scores were recorded. After rehabilitation for one month, the patient was reassessed, and the results were compared to those on the day of re-admission and those recorded in 2014. Results: Compared to the 2014 evaluation results, the patient's stance parameters and gait parameters had worsened at re-admission. However, there was no significant change in the patient's MMT grade or MI and MBI scores in comparison to the results of the 2014 evaluation. After one month of rehabilitation, the patient was re-evaluated again, but there was no significant change in comparison to the evaluation results at re-admission. Conclusion: Some of the stroke patients who have passed six months since the onset of their stroke may require ongoing rehabilitation although the functions of them is almost recovered. Because there is a possibility that recovered functions get worse again without any rehabilitation for a long time. And once the recovered functions get worse, re-recovery of them is not easy.

Objective: The purpose of this case report is to describe the efficacy of treatment with Korean medicine of a patient with nonalcoholic steatohepatitis (NASH) and metabolic syndrome. Methods: The patient was treated with Korean medicine (Saenggangunbi-tang) from April 27, 2016 to October 22, 2016. Results: Post-treatment, the patient's AST and ALT levels were significantly decreased. Triglyceride levels were within the normal range during the treatment. Abdominal ultrasonography revealed a significant improvement in hepatic steatosis. Conclusions: The results suggest that Saenggangunbi-tang could be an effective treatment for NASH with metabolic syndrome.

Black hairy tongue is a benign medical condition, which is characterized by elongated filiform lingual papillae, with a typical carpet-like appearance on the dorsum of the tongue. We describe the case of a 70-year-old Korean female with black hairy tongue, who complained of black hairy tongue accompanied by xerostomia. The patient was treated with Jibaekjihwang-tang-gagambang and electric acupuncture for 13 days. Black hairy tongue was evaluated by its visual appearance and by calculating hue-saturation-brightness (HSB) and red-green-blue (RGB) values of images of the tongue using a photo-edit program. Salivary flow rate (SFR) was evaluated to evaluate oral dryness. After 13 days of treatment, the black fur disappeared, and the brightness and RGB values of the black fur increased. In addition, the SFR increased. We conclude that Korean medicine could be the treatment choice for black hairy tongue.

Objectives: The purpose of this case study is to describe the effect of Korean medicine on a patient with delirium caused by a left middle cerebral artery infarct. Methods: The patient was treated with Sopungbosimdodam-tang, acupuncture, and moxibustion. The Korean version of the Delirium Rating Scale (K-DRS), Neelon and Champagne (NEECHAM) Confusion Scale, and Korean Version of the Mini-Mental State Examination (MMSE-K) were used to evaluate the effect of the treatment. Results: After 50 days of treatment with Sopungbosimdodam-tang, the patient's K-DRS score decreased from 16 to 8. The NEECHAM Confusion Scale also improved from 17 to 23, and the MMSE-K score improved from 1 to 7. Conclusion: This clinical study suggested that Korean medicine could contribute greatly to the treatment of delirium caused by a left middle cerebral artery infarct.

The patient was a 69-year-old Korean male with Lewy's body dementia and insomnia. He was treated with acupuncture and a herbal medicine, Ukan-san. To evaluate insomnia, the patient's total sleeping time was recorded. The patient's sleeping time increased after the administration of the Korean medicine treatments. Treatment with Korean medicine may be effective in treating insomnia in cases of Lewy's body dementia.

Objectives: The aim of this review was to compare the findings of 14 studies of tongue diagnosis in Korean stroke patients. Methods: The purpose, subjects, observation methods, statistical methods, classification of tongue diagnosis, significance, and limitations were analyzed and compared. Results: In Korean stroke patients, tongue diagnosis is significant in pattern identifications, prognosis evaluation, degree of neurological deficit, past history, blood tests, and pulse diagnosis. Interobserver reliability of tongue diagnosis is relatively high.

Objectives: The purpose of this case report is to describe the efficacy of Saenggan-tang-gagam treatment in a patient with immune-tolerant chronic hepatitis B who converted to the immune active phase during hospitalization for Intracerebral hemorrhage. Methods: A 61-year-old male patient with chronic hepatitis B was hospitalized from July 21, 2016 to October 24, 2016. On September 5, the patient showed fatigue and dyspepsia, and his urine was dark. These symptoms were accompanied by a dramatic elevation of aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), and gamma-glutamyl transferase (GGT) levels. Saenggan-tang-gagam was administered three times a day until the patient was discharged from the hospital. Results: After the Saenggan-tang-gagam treatment, AST and ALP levels decreased significantly, below the upper limit level. In addition, the levels of ALT and GG were significantly decreased. In addition, the fatigue, dyspepsia, and dark urine induced by hepatitis improved after the Saenggan-tang-gagam treatment. Conclusion: This case report suggests that traditional Korean medicine has a beneficial effect on the immune active phase of chronic hepatitis B.

Objectives: The aim of this clinical study is to describe the case of a patient with antibiotic-associated diarrhea, which was improved by treatment with herbal medicine. Methods: The antibiotic-associated diarrhea improved following treatment with Boikyangwi-tang and Opae-san for two months. During this period, western medicine, including antibiotics, was also administered. To evaluate the effect of the treatment, the Bristol Stool Form Scale was used. Results: During the treatment, the Bristol Stool Form Scale changed from type 7 to type 5. Conclusion: The results suggest that traditional Korean medicine may be an effective treatment for antibiotic-associated diarrhea.

Objectives: The aim of this study is to describe the effects of traditional Korean medicine treatment on a patient with an intracerebral hemorrhage (ICH) at the left basal ganglia, thalamus, and posterior limb of the internal capsule. Methods: The patient was treated with acupuncture, moxibustion, Hyangsayukgunja-tang, and Samul-tang-gami. The effect of these treatments were evaluated using the manual muscle test (MMT), modified Barthel index (MBI), and National Institutes of Health Stroke Scale (NIHSS). Results: After the treatments, the patient's status improved, as shown by his MMT grade, which increased from 1/1 to 4/4. In addition, the patient's MBI score improved from 0 to 77, and his NIHSS score declined from 16 to 2. In addition, the patient's Levin tube and Foley catheter were removed. Conclusion: The results suggest that traditional Korean medicine may be effective in the treatment of patients with ICH.

A 70-year-old Korean female with diplopia and left ptosis due to oculomotor nerve palsy presented to the hospital. The patient was treated with electroacupuncture, sweet bee venom pharmacopuncture, and moxibustion. She also received acupuncture and herbal medicines Siryeng-tang and Boikyangwi-tang. The change in length (mm) from the medial canthus to the lateral iris was measured to evaluate limitation of eye movement, and Numeric Rating Scale (NRS) for diplopia. Limitation of eye movement and diplopia was used to determine diplopia. The limitation of eye movement and diplopia were improved after the administration of the Korean medicines. Korean medicine may be effective in treating limitation of eye movement and diplopia due to oculomotor nerve palsy.

Objectives: The purpose of this case report is to describe the effect of Korean medicine therapy on patients with tension-type headaches. Methods: This study consisted of four patients with tension-type headaches. The patients were treated with Korean medicine therapies, such as herbal medicine (Gunoe-tang), acupuncture, and cupping, in addition to physical therapy. None of the patients took any analgesics during the admission period. To evaluate the effects of the treatment, the Numeric Rating Scale (NRS) for headaches was used. Results: After the treatment, the patients' complaints decreased, as shown by the results of the NRS. Conclusion: Treatment with Korean medicine may ameliorate tension-type headaches.

Objective: This research aimed to understand patients' perceptions of Korean medicine treatment for osteoporosis in a clinical trial setting. Methods: Ten patients participating in a clinical trial of a herbal medicine drug for osteoporosis were invited to an interview. Data were collected by in-depth interviews and patient observations. This qualitative study adopted the case study research method, and within-case and cross-case analyses were conducted. Results: A model of the patients' osteoporosis management planning was derived from the study. The results showed that the patients' perceptions of osteoporosis were derived from their knowledge and experience during diagnosis and treatment of the condition. Two groups of patients were recognized: those who overlooked the importance of osteoporosis and those who recognized the importance of osteoporosis. Before making treatment decisions about osteoporosis, the patients evaluated the treatment options and weighed the advantages and disadvantages of each option. When evaluating their treatment, the patients combined their knowledge and experience of Korean and western medicine treatments. Their experience of participating in the clinical trial influenced the management planning of osteoporosis. Two major reasons for low compliance with osteoporosis treatment were ignorance of the disease and insufficient information provided by doctors. Conclusion: The results of this qualitative study pointed to four strategies that could be employed to improve accessibility to Korean medicine treatment for osteoporosis: the dissemination of comprehensive information on osteoporosis in the clinic and mass media, promotion of Korean medicine therapies for osteoporosis, management of clinical trial participants, and insurance coverage for Korean medicine.

Objectives: This case study evaluated the effectiveness of Chengsimyeonja-tang-gamibang (Gamicheongsim-tang and Cheongsimsunhwal-tang) in a patient with a pontine hemorrhage and quadriparesis, dysarthria, and dysphagia. Methods: A patient diagnosed with a pontine hemorrhage was treated with Chengsimyeonja-tang-gamibang (Gamicheongsim-tang and Cheongsimsunhwal-tang) acupuncture, and moxibustion. The manual muscle test (MMT), modified Barthel index (MBI), National Institutes of Health Stroke Scale (NIHSS), modified Rankin Scale (mRS), Mini Mental State Examination-Korea (MMSE-K), and Articulatory Functional Ability of Achievement Scale were administered. Results: Improvements in the MMT, MBI, NIHSS, mRS, and K-MMSE were observed after the treatment. The MMT grade increased from Rt. 3/3- and Lt. 3/3- pretreatment to Rt. 4/4 and Lt. 4+/4+ post-treatment. The MBI increased from 10 to 50 post-treatment. The NIHSS decreased from 24 to 6 post-treatment, and the mRS fell from 5 to 4 post-treatment. Finally, the MMSE-K increased from 0 to 24 post-treatment. The Articulatory Functional Ability of Achievement Scale also improved. Conclusion: This study shows that Chengsimyeonja-tang-gamibang can be used to treat the symptoms of patients with a pontine hemorrhage.

The case of a 73-year-old Korean female with dysgeusia due to an acute left thalamic infarction is presented. The patient was treated with the herbal medicine Yanggyuksanhwa-tang, in addition to acupuncture and electronic acupuncture treatment. The filter-paper test and Oral Health Impact Profile-14, (OHIP-14) test were administered to evaluate dysgeusia. Post-treatment, the patient's dysgeusia improved. Korean medicine may be an effective treatment for dysgeusia caused by a thalamic infarction.

This case study describes the effect of Bojungikki-tang on chemotherapy-induced alopecia. Alopecia is a well-documented cause of distress to patients undergoing cancer treatment, but no approved pharmacological treatment exists for chemotherapy-induced hair loss. A 70-year-old female diagnosed with a cholangiocarcinoma and liver metastasis received chemotherapy, including gemcitabine and cisplatin, every three weeks. As a result of the continuous chemotherapy, she suffered various toxicity-related side effects, including bone marrow suppression, general weakness, nausea, peripheral numbness, and hair loss. Bojungikki-tang was initially administered to improve the patient's general weakness and fatigue. After three months of treatment, the patient's hair loss and general condition improved, and the color of the new hair was dark, despite the chemotherapy. The treatment did not improve other symptoms, such as bone marrow suppression and peripheral numbness. This case suggests that Bojungikki-tang could have a beneficial effect on chemotherapy-induced alopecia.

Objectives: The purpose of this clinical case report is to describe the efficacy of Korean medicine in the treatment of vasovagal syncope. Methods: The patient was treated with acupuncture, pharmacopuncture, and herbal medicine. The symptoms (headache, palpitation, sweating, flushing, weakness, coldness) of the patient were observed. Results: After the treatment, the patient's symptoms improved, but more treatments were required. Conclusion: This study can add to understanding of the effect of Korean medicine on vasovagal syncope.

In this study, a 54-year-old female woman diagnosed as Multiple System Atrophy (MSA) presented diplopia with other symptoms - gait disturbance, ataxia, sleep apnea, dysuria. She had been cared with Korean medical treatment - Herbal medicine, acupuncture, bee venom acupuncture, electroacupuncture, cupping, moxa. Notable improvement was observed in diplopia expressing time and Unified Multiple System Atrophy Rating Scale (UMSARS). For MSA patient with no typical treatment indispensable, Korean medical treatment may be effective.

Objectives: The purpose of this case report is to describe the clinical effectiveness of Korean medicine, especially Jungcheonwhadam-tang and Bopyeoyangyeong-jun, in a patient with chronic thoracic empyema while receiving treatment for this condition. Methods: The patient who had been diagnosed with empyema complained of a cough, with sputum. The patient was diagnosed with pe-ong (肺癰) based on his symptoms and x-ray findings. He was treated with a range of Korean medicines, including a herbal decoction, acupuncture, moxibustion, and cupping. A visual analogue scale and percentage pain reduction scale were administered after treatment. Results: After 20 days of the treatment, the patient's cough decreased by 80%, and sputum decreased by 50%. Conclusion: According to this study, Korean medicine, including Jungcheonwhadam-tang and Bopyeoyangyeong-jun, is effective in the treatment of chronic thoracic empyema.

We describe the cases of two patients with chronic subdural hematomas who were treated with Oreong-san. The patients' symptoms improved, as verified by brain computed tomography imaging. Oreong-san may control membrane permeability by inhibiting the aquaporin channel of the outer membrane of a hematoma. We speculate that hydrostatic modulation is a key mechanism underlying the effectiveness of Oreong-san in the treatment of subdural hematomas.

CREST syndrome is a form of limited cutaneous scleroderma that occurs only in certain parts of the body, such as the skin of the hands and face. CREST refers to the five main features of the syndrome: calcinosis, Raynaud's phenomenon, esophageal dysmotility, sclerodactyly, and telangiectasia. Currently, there is no standard treatment for CREST syndrome, and there have been no studies of the use of traditional Korean medicine (TKM) for this disease. This study describes the effects of Keumsuyukun-jeon on CREST syndrome. The patient in this case had typical clinical symptoms of CREST syndrome. These symptoms improved within a relatively short period of receiving the TKM treatment. The results of Anti-centromere antibody (AI) and high sensitivity C-reactive protein (mg/L) also improved.

Objective: We describe the case of a patient with a cerebellar infarction, with dizziness and ataxia who was treated with traditional Korean medicine. Methods: The patient was treated with the herbal medicine Jaeumgeonbi-tang. Acupuncture and moxibustion therapy were also administered on a daily basis. Dizziness was evaluated using the Numeric Rating Scale, and spontaneous movement was observed daily. Results: After the administration of Jaeumgeonbi-tang, the patient's dizziness and ataxia improved. Conclusion: This case shows that traditional Korean medicine appears to be effective in improving side effects of a cerebellar infarction.

Objective: This case study describes the use of traditional Korean medicine to relieve chest pain and headaches in a patient with ischemic heart disease. Methods: The patient was treated with a range of traditional Korean medicine, including acupuncture, moxibustion, and herbal medicines (Gamiondam-tang and Cheonmabanhwa-tang-gagambang). The Numerical Rating Scale (NRS) was used to measure the patient's status and improvements in the frequency of symptoms. Results: After the treatment, the NRS score for chest pain and headaches decreased from 6 to 0. In addition, chest discomfort and palpitations almost subsided. Conclusion: This clinical case study suggested that Gamiondam-tang and Cheonmabanhwa-tang-gagambang could be effective in relieving chest discomfort and headaches.

The patient presented to the clinic with the complaint of severe fatigue. The patient had been diagnosed with chronic hepatitis B a number of years earlier. Although he had used entecavir, an antiviral agent, for over two years, his HBV-DNA level had not dropped below undetectable levels. The fatigue seemed to be associated with chronic hepatitis B. Traditional Korean medicine (TKM) therapy for chronic hepatitis B was administered in conjunction with entecavir and at the same dose. The excessive fatigue gradually decreased following the treatment. On the 28th day, laboratory tests revealed that the patient's bilirubin level was slightly lower and that his HBV-DNA level had dropped below undetectable levels. The addition of TKM therapy may have contributed to the HBV-DNA clearance. No similar cases have been reported in Korea. Herein, we summarize the patient's progress.

Objectives: This case report describes the effect of Hominis placenta pharmacopuncture (HPP) on multiple sclerosis. Methods: A multiple sclerosis patient with gait disturbances, tremors, spasms of both lower limbs, and diplopia was treated with only HPP for two weeks. To evaluate the effects of HPP on the balance ability of the patient, the Functional System (FS) scale, Expanded Disability Statue Scale (EDSS), visual analog scale (VAS), manual muscle test (MMT), and Berg Balance Scale (BBS) were used. Results: The HPP treatment reduced the patient's gait disturbance, tremors, spasms of the lower limbs, and diplopia. According to the FS scale, the patient's cerebellar and optic functions were enhanced. In addition, the patient's performance on the EDSS improved from 4.0 to 3.0.3. As shown by the MMT, the lower extremities showed a mild improvement (from F+ to G.4). Furthermore, the VAS showed a considerable improvement, decreasing from 9 to 6.5 post-treatment. The patient's performance on the BBS, which denotes balance ability, showed a considerable improvement, increasing from 38 to 49. Conclusion: Treatment with only HPP could help to ameliorate the symptoms of multiple sclerosis.
