Proton pump inhibitors (PPIs) are widely used over 20 years for management of symptoms due to acid related diseases such as peptic ulcer and reflux esophagitis. Serious adverse events are extremely rare for short-term PPIs use. Recently, as long-term PPIs use increase, diverse reports have been reported on adverse event related with long-term PPIs use. Long-term PPIs use is generally referred as use of PPIs more than 1 year. Secondary hypergastrinemia after long-term PPIs use is associated with development of fundic gland polyps (FGP) and hyperplasia of enterochromaffin-like cell (ECL) that might be concerned with gastric carcinoid tumor. Furthermore, several studies have posed the relationship between the risk of gastric cancer and long-term PPIs use with co-existing H. pylori infection. The present review summarize the recent accumulated evidence on neoplasm associated with secondary hypergastrinemia after long-term PPIs use.

Proton-pump inhibitors (PPI) have important roles in the management of acid-related disorders, especially gastro-esophageal reflux disease and peptic ulcer disease. They are considered safe, but some side effects, such as oxyntic cell hyperplasia, glandular cysts, hypergastrinemia and fundic gland polyps, are also reported. Long-term PPI administration in Helicobacter pylori (H. pylori) positive subjects promotes a shift from antral to corpus-predominant gastritis. The shift leads to corpus atrophy eventually that is known predisposing factor of gastric adenocarcinoma. It is recommended that patients being considered for long-term PPI therapy should be tested for H. pylori infection. And if present, H. pylori eradication should be preceded to PPI administration. Also, long-term PPI administration can cause enterochromaffin-like cell hyperplasia. Although the underlying mechanism and pathogenesis are not yet fully understood, it is possible that long-term PPI administration can promote the development of gastric carcinoid tumor. Therefore, to minimize the side effects, it should be used in adequate dose for a precise duration.

Proton pump inhibitors (PPI) are very effective drug used frequently in acid-related disorders. Long-term use of PPI is becoming increasingly common, often without appropriate indications. The debate had focused on the adverse effects related to long-term use of PPI during the last years. This article is a detailed review of the evidence on this topic, focusing on the adverse effects of long-term PPI use that have developed the greatest concern; vitamin B12 deficiency, iron deficiency, impaired calcium absorption, bony fracture, hypomagnesemia, increased susceptibility to pneumonia and enteric infections. Although PPIs have been used with a high margin of safety, the clinicians should consider reducing the dose of PPIs and reassessing the treatment duration to prevent adverse effects of PPIs.

Clopidogrel prevent atherothrombotic complications following acute coronary syndromes by inhibiting platelet aggregation. However, in case of concomitant use of clopidogrel and proton pump inhibitor (PPI) for prophylaxis of gastrointestinal (GI) bleeding, the anti-clotting of clopidogrel will be decreased because PPI inhibit competitively cytochrome P450 enzymes, especially CYP2C19. Through several pharmacodynomic studies, omeprazole, but not other PPIs, worsens surrogate markers (e.g., platelet reactivity index) of clopidogrel efficacy. And many observational studies show concomitant use of clopidogrel and PPI have increased the risks of cardiovascular (CV) events (hazard/odds ratios = 1.25-1.5). However, only one prospective randomized trial (COGENT trial) of omeprazole vs. placebo in clopidogrel users show no difference in CV events (hazard ratio = 1.02) and decrease the risk of GI bleeding (hazard ratio = 0.13). Further well designed research will need to determine whether a pharmacodynamic effect of clopidogrel affected by PPI will be changed clinical CV events. Clinical decisions about concomitant use of PPI and clopidogrel must be balance overall risks and benefits, considering both CV and GI complications.

Clopidogrel is a prodrug that is converted in the liver to an active thiol metabolite, which irreversibly inhibits the platelet $P2Y_{12}$ adenosine diphosphate receptor. This mechanism requires cytochrome P450 2C19 (CYP 2C19) enzyme. Proton pump inhibitors (PPIs) competes against CYP 2C19 and inhibits the conversion of clopidogrel into its active metabolite, therefore, clopidogrel-PPIs drugs interaction may exist. These interactions could result in competitive inhibition decreasing the conversion of the clopidogrel pro-drug to the active metabolite and could potentially translate into an increased risk for cardiovascular events by inadequate platelet $P2Y_{12}$ receptor inhibition. Many studies including retrospective cohort studies and studies using platelet function tests demonstrated the possible interactions between clopidogrel and PPIs leading to a decrease in the antiplatelet efficacy of clopidogrel and worse cardiovascular clinical outcomes than clopidogrel alone. In contrast, few comparative trials using clinical outcomes found no serious drug interactions between them. In this review, we introduce possible harmful effects of combined use of clopidogrel and PPIs on platelet function. In addition, we suggest how to overcome clopidogrel-PPIs interactions.

Endoscopic resection has become accepted as a standard treatment in selected patients with early gastric cancer (EGC) with negligible risk of lymph node metastasis. Endoscopic resection preserves the stomach and therefore improves quality of life compared with surgery. And it allows accurate histological staging of the tumor, which is critical in deciding whether additional treatment is necessary. The endoscopic submucosal dissection (ESD) method has been widely used with higher en bloc resection and complete resection rates than conventional endoscopic mucosal resection (EMR) with acceptable complication rates. Long-term clinical outcomes of these techniques are promising in terms of disease-free and overall survival. Recently, the expanded indication of endoscopic resection for early gastric cancer has been proposed because of technical advances of ESD. Long-term outcome data of the expanded indication are needed for the clinical application of the expanded criteria of ESD.

Liver injury due to prescription and nonprescription medications is a growing medical, scientific, and public health problem. Worldwide, the estimated annual incidence rate of Drug-induced liver injury (DILI) is 13.9-24.0 per 100,000 inhabitants. And there is increasing concern about the potential risk for hepatotoxicity from complementary and alternative medicines (CAM) including herbal products because they are unregulated and therefore not standardized with regard to their contents. CAM, including herbal drugs and dietary supplements seem to be major causes of DILI in Asian countries, in contrast to lower incidences in Western countries. However, with increasing use of CAM, DILI from CAM seems to now be a problem all over the world. The classification and definition of the causative agents can be difficult. For example, in Korea, herbs might sub-categorize into herbal medications (medications prescribed and compounded by a doctor of oriental medicine), herbal preparations (preparations compounded by an oriental pharmacist), and medicinal herbs or plants (preparation compounded by an unauthorized lay person). Also the division of herbs and folk remedies is unclear at times. Determining hepatotoxicity remains a major challenge in clinical practice due to lack of reliable markers. Standards are lacking for validation of drug causality assessment methods. The RUCAM/CIOMS scale have been proposed to establish a causal relationship between offending drug and liver injury. Although, there is a need to validate a new instrument with an abridged scale that would provide a better approximation to the truth. It would be feasible to develop some refinements to make the RUCAM/CIOMS scale more realistic.

목적: 독성간염에 관한 연구는 대부분 증례보고 형식이며 비교분석 연구 또는 조직소견에 대한 보고는 적다. 이에 저자들은 독성간염의 임상양상 및 조직소견과 예후와의 연관성에 대해 알아보기로 하였다. 방법: 2004년 1월부터 2010년 1월까지 독성간염으로 입원 치료 받은 환자 184명을 대상으로 후향적으로 분석하였다. 이중에서 간 조직검사를 시행 받은 환자는 62명이었다. 독성간염의 진단은 RUCAM 점수를 이용하여 4점 이상인 경우를 독성간염으로 간주하였다. 결과: 독성간염은 남자(n = 59)보다 여자(n = 125)에서 더 많이 발생하였다. 환자들의 평균나이는 $45.9\;{\pm}\;14.4$세이었고 입원 당시 가장 흔한 증상은 황달이었다. 원인약제는 한약이 80예(43.5%), 민간요법 62예(33.7%), 의사에 의한 처방 약물 40예(21.7%)로 한약이 가장 많았다. 조직소견은 간세포괴사(n = 62), 담즙정체(n = 43), 지방변성(n = 23), 호산구 침윤(n = 22) 그리고 풍선양 변성(n = 20) 순으로 관찰되었다. 조직학적으로 담즙정체와 간세포괴사가 있는 경우에 입원기간과 회복기간이 길었다. 입원 당시의 혈청 총 빌리루빈만이 입원기간 및 회복기간과 독립적 연관성이 있었다. 결론: 독성간염의 가장 흔한 조직소견은 간세포괴사와 담즙정체이다. 그러나 독성간염의 특이적인 조직소견은 아니다. 독성간염의 임상경과에 영향을 미치는 인자로는 입원 당시 혈청 총 빌리루빈수치가 높으면 입원이 길어지고 회복이 늦어질 수 있다. 하지만 연령, 성별, 약물 복용기간, 음주력 등은 유의한 연관성은 없었다.

목적: 스트레스 유발성 심근병증은 최근까지 그 빈도가 증가하고 있다. 그러나 아직까지 다기관 등록 자료를 통한 연구가 없었고, 또한 원내 사망률과 좌심실 수축기능의 회복과 관련된 예후인자에 대한 연구는 없었다. 본 연구는 한국내 다기관 등록자료를 통하여 스트레스 유발성 심근병증 환자에 있어서 원내 사망률과 좌심실 수축기능의 회복과 관련된 예측인자에는 어떠한 것이 있는지 알아보고자 하였다. 방법: 2004년 11월부터 2010년 11월까지 한국 내 8개 병원에서 후향적으로 수집하여, Mayo Clinic에서 제안하는 네가지 임상적인 진단 요건을 만족하는 총 155명의 환자들을 대상으로 하였다. 기본적인 임상자료들과 검사실 결과들을 분석하였고, 발병 당시와 일주일 후 추적 심초음파 검사를 시행하여 좌심실 구혈률 및 WMSI 등을 비교하였으며, 원내사망 및 재발 등을 측정하였다. 통계분석은 t-검정(독립 표본), 교차분석, 로지스틱 회귀분석 등을 사용하였다. 결과: 총 8명(5.2%)의 원내 사망이 있었고, 원내 사망과 관련된 의미 있는 예측인자는 초기의 높은 혈소판 수치였다. 좌심실 구혈률의 회복과 관련된 의미 있는 예측인자는 없었고, WMSI의 회복과 관련된 의미 있는 예측인자는 발병 당시 부정맥의 부재와 초기 좌심실 수축기 말 직경이 큰 경우였다. 결론: 발병 당시 부정맥이 발생하지 않고 좌심실 수축기말 직경이 큰 경우에 시기 적절한 좌심실 수축기능의 회복을 좀 더 기대해 볼 수 있었다.

목적: 제2형 당뇨병 환자에서 알부민뇨는 신장 손상의 지표일 뿐만 아니라 심혈관 질환의 이환과 사망을 증가시키는 위험요인으로 알려져 있다. 알부민뇨는 동맥경화증에 의해 발생하는 말초동맥 질환의 중요한 위험인자로 보고된다. 이 연구는 제2형 당뇨병 환자를 대상으로 알부민뇨와 ankle-brachial index (ABI)와 toe-brachial index (TBI)와의 관련성을 알아보고 알부민뇨가 말초동맥 질환에 미치는 영향을 분석하고자 하였다. 방법: 제2형 당뇨병 환자를 대상으로 신체계측, 생화학검사, 알부민뇨 측정을 하였으며 말초동맥 질환 검사를 위해 ABI 및 TBI를 측정하였다. 결과: 총 390명의 환자를 대상으로 하였으며 평균 나이는 $60.9\;{\pm}\;12.2$세, 당뇨병 이환기간의 중앙값은 5년이었고 당화 혈색소는 7.9%였다. ABI 0.9 미만인 군에서 0.9 이상인 군에 비해 당뇨병 이환기간, 혈청 크레아티닌, 사구체 여과율 및 알부민뇨의 정도가 유의하게 높았다. 또한 ABI 0.9 미만인 군에서 미세 알부민뇨 또는 현성 단백뇨의 빈도가 유의하게 더 높았다. TBI 0.6 미만인 군에서 0.6 이상인 군에 비해 연령, 당뇨병 이환기간, 알부민뇨의 정도가 유의하게 높았고 고밀도 지단백 콜레스테롤은 유의하게 낮았다. 또한 TBI 0.6미만인 군에서 미세 알부민뇨 또는 현성 단백뇨가 빈도가 유의하게 더 높았다. 발목 근위 동맥(between the aorta and the ankle joint)에서의 말초동맥 질환과의 연관성을 알아보기 위해 다중회귀분석을 하였을 때 알부민뇨는 발목 근위 동맥의 말초동맥 질환과 밀접한 연관성이 있었다(OR = 1.980, 95% CI = 1.001-3.918). 발목 원위 동맥(distal to the ankle joint)에서의 말초동맥 질환과의 관계를 알아보기 위해 다중회귀분석을 하였을 때 알부민뇨는 발목 원위 동맥의 말초동맥 질환과 유의한 관계를 보였다(OR = 3.149, 95% CI = 1.260-7.871). 결론: 본 연구에서 알부민뇨는 제2형 당뇨병 환자에서 발목 근위 동맥뿐만 아니라 발목 원위 동맥의 협착성 병변과 밀접한 관련이 있으며 이는 알부민뇨가 혈관의 전반적인 동맥경화성 병변과 관련이 있음을 보였다.

목적: 사구체 신염 등의 환자에서 단회뇨를 이용한 단백/크레아티닌 농도비(P/C ratio)가 24시간 요단백량과 밀접한 상관관계가 있음은 수차례 보고된 바 있다. 서구의 신이식 환자에서도 이러한 상관관계가 관찰되었으나 동양인 신이식 환자에 대해서는 연구가 부족하다. 이에 저자들은 한국의 신이식 환자에서 24시간 요단백량과 단회뇨 P/C ratio를 비교하고, 두 값의 상관관계에 영향을 미치는 요인들에 대하여 알아보고자 하였다. 방법: 2005년 1월부터 2010년 7월까지 본원 신장내과에 내원한 66명의 신이식 환자를 대상으로 소변 채집을 하였다. 24시간 요 수집 단백량과 수집을 시작하는 날 아침 채집한 단회뇨 P/C ratio를 비교하였다. 24시간 요단백량과 단회뇨 P/C ratio 사이의 상관관계에 영향을 끼칠 수 있는 인자로 성별, 면역억제제 종류, 기저질환, 이식 기간, 사구체 여과율, 일일 요단백량, 나이에 대해 분석하였다. 결과: 대상 환자의 평균 연령은 $47\;{\pm}\;14.1$세였으며, 평균 혈청 크레아티닌은 $1.53\;{\pm}\;0.58$ mg/dL, 사구체 여과율은 $55.75\;{\pm}\;22\;mL/min/1.73\;m^2$, 평균 이식 기간은 $74\;{\pm}\;80$개월이었다. 24시간 평균 요단백량은 $1.31\;{\pm}\;1.69$ g/day이었고, 단회뇨의 P/C ratio $1.29\;{\pm}\;1.70$이었으며 높은 상관계수를 보였다(r = 0.95). 성별이 단회뇨 PCR를 이용한 24시간 요단백 예측에 영향을 끼치는 요인으로 고려된다(p = 0.003). 단회뇨 P/C ratio와 24시간 요단백량은 남성에선 각각 $1.05\;{\pm}\;1.51,\;1.26\;{pm}\;1.68$ g/day이었고(p = 0.005), 여성에선 $1.57\;{\pm}\;1.88,\;1.36\;{\pm}\;1.72$ g/day이었다(p = 0.047). 결론: 본 연구에서 단회뇨 P/C ratio는 24시간 요단백량을 가늠할 수 있는 방법으로 사용될 수 있을 것으로 생각한다.

저자들은 복강 내 종양 의심 환자에서 위궤양 천공에 의한 복강 내 농양을 진단하고 약물치료만으로 완치된 예를 경험하여 이를 문헌고찰과 함께 보고하는 바이다.

담관 내 점액성 유두 종양이 다른 장기와 누공을 형성한 증례보고는 저자들이 문헌고찰한 것에 의하면 없으며, 누공형성 기전도 확립된 것이 없어 더 많은 보고와 연구가 필요하다. 저자들은 황달과 가려움증으로 내원하여 총담관-십이지장 누공을 동반한 담관 내 점액성 유두 종양을 진단하였으며, 간좌엽 절제술 및 담낭절제술로 치료한 증례를 경험하여 보고한다.

본 증례는 ST 분절 상승의 심근경색으로 약물 용출성 스텐트를 삽입한 환자에서 4년 후 발견된 스텐트 내강 안쪽을 둘러싼 원형의 두꺼운 석회침착소견에 대하여 신생내막과 관련된 새로운 죽상동맥경화증으로 추정하여 보고하는 바이다.

저자 등은 폐렴으로 치료받은 65세 여자 환자에서 흡연력이 전혀 없음에도 자발성 기흉 기왕력과 양측성 다발성 폐낭종 소견이 관찰되어 BHD 증후군을 의심하였고 folliculin 유전자 변이 검사를 통하여 이를 확인하였다. 특징적인 피부병변 없이도 BHD 증후군의 발생 가능성이 있으므로 폐질환이 임상적으로 이 질환에 합당한 경우에는 folliculin 유전자변이 검사를 고려해 보아야 할 것으로 생각한다.

저자들은 20년 전 정상지질혈증성 편평 황색종을 진단받고 특별한 치료 없이 지내온 환자에서 20년 후 다발성 골수종이 진단된 1예를 경험하여 정상지질혈증성 편평 황색종 환자의 지속적인 추적관찰이 필요함을 강조하면서 보고하는 바이다.

Nutcracker 증후군이란 좌측 신정맥이 대동맥과 상장간막동맥 사이에서 압박을 받아 발생하는 임상적인 증후군으로 무증상의 혈뇨, 좌측 요통이나 복통의 형태로 나타나고 심한 단백뇨는 드물다고 알려져 있다. 저자들은 신증후군과 현미경적 혈뇨로 내원하여 Nutcracker syndrome과 함께 특발성 막증식성 사구체신염 제1형으로 확진된 1예를 경험하였다. Nutcracker 증후군으로 진단되더라도 심한 단백뇨를 보일 때는 사구체신염의 동반 가능성도 배제해야 하므로 신생검을 고려해야 하겠다.

농흉과 연관된 림프종은 만성적인 농흉이 발생한 환자들에서 수십 년의 경과 이후에 발생하는 드문 B 세포 림프종으로 엡스타인 바 바이러스와 강한 연관성을 보이는 질환이다. 저자들은 흉부의 종양을 주소로 내원한 66세 남자 환자에서 농흉과 연관된 림프종을 경험하였는데, 환자에서 37년전 결핵에 의한 늑막염으로 치료받은 이후에 만성적인 농흉의 과거력이 있음을 확인할 수 있었고, 원발 부위의 조직학적 생검으로 미만성 거대 B세포 림프종을 진단할 수 있었다. 농흉과 연관된 림프종은 드문 질환으로 알려져 있으나 만성적인 농흉이 있는 환자에서 늑막을 침범하는 새롭게 발생하는 종양에 대해서는 조직학적 검사를 통한 감별 진단이 필요할 것으로 판단된다.

M. kansasii는 천천히 자라는 비결핵성 항산균(slow-growing mycobacteria)으로 대부분 천천히 진행하는 폐질환을 일으키고, 피부와 파종성 감염은 드물게 일어나는데 면역저하자에서 주로 발생한다. 아직까지 우리나라에서는 심장이식 환자에서 M. kansasii 피부감염증 보고가 없었다. 본 저자들은 심장 이식 후 발생한 M. kansasii의 피부 감염증의 1예를 경험 하였기에 문헌고찰과 함께 보고하는 바이다.

베체트병을 진단 받았거나, 의심되는 환자에서 왼쪽 옆구리 통증이 동반될 경우, 혈전성 혈관염에 동반된 비장 경색을 감별해야 하겠다.

저자들은 신 증후군 환자에서 $Visipaque^{(R)}$ 사용 후 발생한 후기 지연 부작용을 경험하였고, 이를 문헌고찰과 함께 보고하는 바이다.
