Obesity is a chronic disorder that is a significant risk factor for diabetes, cardiovascular diseases, malignancy, and other chronic diseases. Lifestyle modifications form the basis of most treatments for obesity, but it has become clear that such modifications alone are not enough for many obese patients. When a behavioral approach is insufficient, pharmacological treatment may be recommended. In recent years, the US Food and Drug Administration (FDA) has withdrawn several therapeutic options for obesity due to their side effects, but has approved four novel anti-obesity agents. Until recently, orlistat was the only drug approved for the management of long-term obesity, but the US FDA approved the novel anti-obesity drugs lorcaserin and phentermine/topiramate in 2012, and naltrexone/bupropion and liraglutide in 2014. The present review discusses the different pharmacotherapeutic options for the treatment of obesity.

A small number of viable tuberculosis bacilli can reside in an individual with latent tuberculosis infection (LTBI) without obvious clinical symptoms or abnormal chest radiographs. Diagnosis and treatment of LTBI are important for tuberculosis (TB) control in public and private healthcare facilities, particularly in high-risk populations. The updated 2017 Korean guidelines for TB recommend that tuberculin skin tests, interferon-gamma release assays, or a combination of them can be used for the diagnosis of LTBI, depending on the age and immune status of the patient as well as their TB contact history. For diagnosis of LTBI, exclusion of active TB is essential, and the possibility of healed TB in those without a history of treatment for TB but at risk of its development must be considered. The treatment options for LTBI include isoniazid, rifampicin, isoniazid/rifampicin, and isoniazid/rifapentine. The benefits and risks of these agents based on the age of the patient and their hepatotoxicity must be considered when selecting the appropriate drug. Standardized diagnosis and treatment of LTBI based on the updated 2017 guidelines will contribute to the control of TB in Korea as well as to further revisions of the guidelines.

Hypertriglyceridemia a major cause of acute pancreatitis, accounting for up to 10% of all cases. The pathophysiological mechanism of hypertriglyceridemia-induced acute pancreatitis (HTGP) is presumed to involve the hydrolysis of triglycerides by pancreatic lipase resulting in an excess of free fatty acids and elevated chylomicrons, which are thought to increase plasma viscosity and induce ischemia and inflammation in pancreatic tissue. Although the clinical course of HTGP is similar to other forms of acute pancreatitis, the clinical severity and associated complications are significantly higher in patients with HTGP. Therefore, an accurate diagnosis is essential for treatment and prevention of disease recurrence. At present, there are no approved guidelines for the management of HTGP. Different treatment modalities such as apheresis/plasmapheresis, insulin, heparin, fibric acids, and omega-3 fatty acids have been successfully implemented to reduce serum triglycerides. Following acute phase management, lifestyle modifications including dietary adjustments and drug therapy are important for the long-term management of HTGP and the prevention of relapse. Additional studies are required to produce generalized and efficient treatment guidelines for HTGP.

목적: 급성 심근경색증 환자에서 좌심실 이완 기능과 좌심실 충만압은 환자의 예후와 관련되어 있다고 알려져 있다. 이 연구의 목적은 젊은 급성 심근경색증 환자에서 좌심실 이완 기능 저하 및 충만압 상승에 따른 임상 경과의 차이를 파악하고자 하였다. 방법: 2011년 1월부터 2015년 8월까지 Korea Acute Myocardial Infarction Registry-National Institute of Health (KAMIR-NIH)에 등록된 환자 중 젊은 급성 심근경색증(남 < 45세, 여 < 55세)으로 전남대학교병원에 입원한 환자 240명중에서 관상동맥중재술을 시술받고 2년 동안 임상 추적 관찰이 되어 분석 가능한 환자 200명(남 : 여 = 160명 : 40명)을 대상으로 하였다. 200명의 환자를 좌심실 이완 기능이 정상인 군(46명, $39.5{\pm}5.3$세)과 비정상군(154명, $43.5{\pm}5.1$세)으로 나누었다. 좌심실 충만압을 측정하였으며, 좌심실 충만압 상승을 E/e' ratio 15 이상으로 정의하였다. 좌심실 이완 기능 정상군과 비정상군으로 나누었고 정상 좌심실 충만압군과 좌심실 충만압 상승군으로 나누어 각각 주요 심장 사건을 분석하였다. 주요 심장 사건은 사망, 심근경색증의 재발 및 재관류술로 정의하였다. 결과: 연구 대상자를 평균 $40.9{\pm}11.6$개월 동안 임상적으로 추적 관찰한 결과, 이 중 26명(13%)에서 사망, 심근경색증재발 및 재관류등의 주요 심장 사건이 발생하였고, 좌심실이완 기능 정상군과 비정상군 간의 유의한 차이는 없었다(p = 0.810). 또한 좌심실 이완 기능을 정상군과 1도 이완기기능 장애군과 2도 이완기 기능 장애군으로 나누어서 추가 분석하였다. 2도 이완기 기능 장애군에서 심근경색증 재발은 유의한 차이를 보였지만(p = 0.006), 주요 심장 사건과 유의한 차이는 없었다(p = 0.081). 그러나 좌심실 충만압 상승군에서 정상 좌심실 충만압을 가진 환자군에 비하여 주요 심장 사건의 발생률은 유의한 차이가 있었다(p < 0.001). 다변량 분석 결과 주요 심장 사건 발생의 독립적인 인자는 E/e' ratio 15 이상 환자로 파악되었다(p = 0.019). Kaplan-Meier 생존 곡선을 이용한 사망률을 추적한 결과 좌심실 수축 기능 40% 미만인 환자(p < 0.001)와 E/e' ratio 15 이상 환자(p = 0.004)에서 유의하게 생존율이 낮았다. 결론: 높은 좌심실 충만압은 젊은 급성 심근경색증 환자의 예후에 대한 독립적 예측 인자였으며, 좌심실 충만압의 측정은 젊은 급성 심근경색증 환자에서 심근경색증 후 고위험 환자군 분류에 유용할 것으로 기대된다.

목적: 혈액투석 환자에서 체수분 상태는 중요한데 혈액투석 이후의 과수분 상태는 고혈압, 부종, 심혈관계 합병증을 유발할 수 있다. 혈액투석 환자의 근육량 평가 및 체수분 상태를 측정할 수 있는 BIA 검사를 이용하여 혈액투석 환자에서 이환율, 생존율과 관련된 인자를 알아보고자 하였다. 방법: 본 연구는 후향적 연구로 2010년 1월부터 2015년 5월 30일까지 BIA를 측정한 말기 신부전으로 진단받고 투석을 3개월 이상 시행한 환자를 대상으로 하였으며, 2017년 6월 30일까지 추적 관찰하였다. 혈액투석 환자에서 시행하는 기본 혈액 검사와 BIA data (ECW, ICW, TBW, SLM, FFM, SMM 등)를 참조하여 환자의 이환율, 생존율 등을 분석하였다. 결과: 사망군과 생존군에서 SLM, SMM, FFM의 차이는 없었다. 생존군에서 사망군보다 나이, C-반응단백 수치가 낮았고, 혈청 알부민, 부갑상샘호르몬, 인이 높았으며, 총 체액량(TBW) 중 세포외액량(ECW)의 비율은 사망군에서 좀 더 높은 결과를 보였다($0.39{\pm}0.01$ vs. $0.40{\pm}0.01$; p < 0.05). 과수분군을 ECW/TBW ratio > 0.4로 정의하였을 때 Kaplan-Meier 생존분석에서 과수분군에서 통계적으로 유의하게 사망률이 높았다. 결론: 혈액투석 환자에서 BIA로 측정한 근육량 감소과 과수분 상태는 환자의 사망률과 이환율과 관련된 중요한 인자로 생각되며, 근육량 감소보다는 과수분 상태가 사망률과 좀 더 밀접한 관련이 있을 것으로 생각된다.

Intestinal tuberculosis is an infection of the gastrointestinal tract by the Mycobacterium tuberculosis complex. To the best of our knowledge, solitary intestinal tuberculosis accompanied by intestinal obstruction, particularly in the middle of the small intestine, is extremely rare. We report a case of solitary jejunal tuberculosis in a 49-year-old man with no underlying disease. He was admitted a few days after the onset of diffuse abdominal discomfort. Upon evaluation, we initially considered a malignancy of the distal jejunum with ileus due to the presence of a mass. Therefore, he underwent laparoscopic resection of the small bowel. Unexpectedly, the histologic specimen showed a chronic caseating granulomatous lesion with acid-fast bacilli. Ultimately, he was diagnosed with solitary jejunal tuberculosis. He was successfully treated with anti-tuberculosis drugs without any complications.

자가면역 췌장염 환자에서 드물지만 가성낭종이 동반될 수 있다. 자가면역 췌장염에 동반된 가성낭종은 대부분 스테로이드 투여만으로 소실된다고 알려져 있다. 하지만 본 증례에서는 스테로이드 투여에도 불구하고 가성낭종의 크기가 증가하였고, 이차적으로 낭종내 감염이 발생하였다. 이에 초음파내시경을 이용하여 낭종 배액술을 시행하였으며, 이를 통하여 가성낭종이 완전히 소실되었다.

PCIS는 심장 수술, 급성 심근경색 또는 경피적 관상동맥중재술 등에 의하여 발생한 심장 손상 후 발생하는 합병증이다. 본 저자는 경피적 관상동맥중재술 이후 급성으로 발생한 PCIS를 스테로이드를 사용한 약물로 치료하였기에 이를 문헌고찰과 함께 보고하는 바이다. 비스테로이드항염제에 반응하지 않는 PCIS 환자에게 스테로이드의 조기 투약이 도움이 될 수 있고 장기적인 유지 요법이 도움이 될 수 있음을 말해주고 있다.

니볼루맙을 사용하는 도중 폐렴이 발생할 수 있고, 폐렴이 의심되면 치료하기 이전에 바이러스를 포함한 감염 여부와 림프관성 암의 진행 상태 및 울혈성 심장 기능 상실 여부 등의 감별이 필요하며 폐렴의 등급과 중증도를 평가하여 스테로이드를 경험적으로 쓰는 것이 중요하다. 본 증례에서는 감염을 배제하기 위하여 기관지 폐포세척을 시행한 후 폐포자충 폐렴이 진단된 사례로 드물기는 하지만 페포자충 폐렴의 감별이 필요함을 보여주어 증례로 보고하는 바이다.

저자들은 최초 복부 CT 소견상 후복막강섬유화증으로 오인되었으나 적극적인 조직 검사를 통하여 후복막강에만 국한된 원발 부위 불명 전이암으로 진단된 증례를 경험하였다. 최근 후복막강섬유화증의 진단에 있어서 PET-CT 등 영상 검사가 기술적으로 많은 발전을 이루었지만 후복막강섬유화증의 진단시 악성종양의 배제는 필수적이기 때문에 후복막강섬유화증의 확진은 반드시 조직학적인 검사에 기초하여 이루어져야 함을 강조하는 바이다.
