항암화학요법을 받는 종양 환자의 영적 안녕, 희망이 피로에 미치는 영향 (Relationship of Spiritual Well-being, Hope on Fatigue in Cancer Patients on Chemotherapy)

  • Purpose: This was a correlation study to identify the relationship of spiritual well-being, hope on fatigue in cancer patients on chemotherapy. Methods: The subjects completed structured questionnaires: the 'Spiritual Well-being Scale', developed by Paloutzian & Ellison, 'Hope Scale', developed by Kim & Lee and 'Fatigue Scale', developed by Mendoza et al. Data were collected from 120 patientsat two general hospitals and were analyzed using t-test, ANOVA & Sheffe's test, Pearson's correlation coefficients and multiple stepwise regression. Results: Participants with higher fatigue had lower scores for hope (r=-.36, p<.001) and lower scores for spiritual well-being (r=-.23, p=.011). Participants with higher scores for hope had higher scores for spiritual well-being (r=.61, p<.001). The factors seen as contributing to fatigue were hope, financial burden of treatment, period of religious life, living with spouse, and reported pain. These variables explained 32.3% of the variance in fatigue. Hope with 13% was the most influential. Conclusion: The fatigue of the cancer patients on chemotherapy can be reduced if hope is improved, and hope can be improved if the spiritual well-being is improved. Therefore, we suggest developing a nursing intervention program that leads to improve hope and spiritual well-being of the cancer patients on chemotherapy for reducing fatigue.

  • Purpose: The purpose of this study was to explore factors affecting uncertainty and general well-being based on Uncertainty in Illness Theory. Methods: Data were collected from 125 outpatients who had received hemodialysis. The path model among four concepts, such as period of hemodialysis, social support, uncertainty, and general well-being, was tested. Tangible support, positive social interaction, affectionate, and emotional/informational support were measured as social support. Adaptation in the model was operationalized as general well-being which consisted of anxiety, depression, positive well-being, self-control, and general health. Results: All paths were statistically significant at the level of ${\alpha}$=.05. The significant paths were the path from period of hemodialysis to uncertainty (t=-2.86), social support to uncertainty (t=-2.01), uncertainty to general wellbeing (t=-2.85), and social support to general well-being (t=3.55). Conclusion: Patients who perceived low uncertainty and high social support were likely to feel well-being. Therefore, nurses should give patients appropriate information according to their needs and have meaningful interaction with patients to reduce their uncertainty and render social support.

  • Purpose: This study utilized a non-equivalent control group pre-post design to assess the effects of spiritual nursing care on loneliness and spiritual well-being of terminal cancer patients. Method: Forty-one terminal cancer patients in a general hospital, were divided into an experimental group of 20 patients and a control group of 21 patients. Those in the experimental group received four weeks of treatment three times a week for about 45 minutes each session. The treatment included nurses' spiritual care involving five instruments of spiritual nursing intervention, each of which was used according to the six types of spiritual need assessment. Data was analyzed with descriptive statistics including real number, percentage, $X^2$-test, t-test, and ANCOVA. Result: There were significant differences between the experimental and control groups in the level of loneliness and spiritual well-being. Conclusion: Spiritual nursing care was verified as an effective program that can lessen the loneliness and improve the spiritual well-being of patients with terminal cancer.

  • Purpose: The purpose of this study was to investigate the factors that affect the psychological well-being in family caregivers of stroke patients. Method: The General Health Perception, short form 36, Health Survey Questionaire was used to measure health perception. The Caregiving Mastery Scale was used to assess the mastery, while the Psychological General Well-Being Index was used to examine the level of well-being. Result: Subjective health, caregiving mastery, patient's ADL and caregiving duration influenced on caregiver's psychological well-being. Subjective health had effect on psychological well-being both directly and indirectly. Caregiving duration and patient's ADL had indirect effect on psychological well-being through caregiving mastery. Conclusion: It is need to develop a health program for the caregivers of stroke patient's and to provide nursing intervention to improve the caregiver's ability, thereby improving the well-being of the family caregivers.

  • Purpose: The aim of this study was examine effects of fatigue, anxiety, depression, social support, and spiritual well-being of supportive education program for hospice patients's family. Method: Using a non-equivalent control group pre-post quasi-experimental design, 70 study subjects were assigned into two groups, experimental group (n=35) and the control group (n=35). Measures were fatigue, state-anxiety, depression, social support, and spiritual well-being to test for the effects of supportive education program for hospice patients's family. Data analyzed using $x^2$ test, t-test with SPSS/WIN 19.0 version. Results: The experimental group receiving supportive education program for hospice patients's family had a significant changes of fatigue, state-anxiety, depression, social support, and spiritual well-being. Conclusion: The supportive education program for hospice patients's family is an effective intervention to enhance social support and spiritual well-being and to decrease fatigue, anxiety and depression.

  • Purpose: The purpose of this study was to elucidate the effects of spiritual nursing intervention on spiritual well-being and depression level of the hospice patients. Method: The subjects for this study were collected from 62 patients who were admitted in the hospice care unit from July 28, 2002 to October 31, 2002 in D city K hospital. Subjects were 31 members of the experimental group and 31 members of the control group. It was devised with a nonequivalent control group pretest-posttest design. The spiritual nursing intervention was given by using the therapeutic use of self, Scripture, prayer, Hymn and music, use of church community involvement and refer to pastors according to assessment of patients' spiritual need for 3 weeks(total 12 times and 1 hour per each intervention). Sangsoon Choi(1990) and Jungho Kang(1996)'s spiritual well-being scale, which was modified from Palautzian and Ellison(1982)'s spiritual well-being scale, was used to investigate patients' spiritual well-being. To investigate level of depression, OkHyun Song(1977)'s Depression Scale, which was modified from Zung(1965)'s Depression Inventory, was used. Data were analyzed by x2-test, t-test, Repeated measures ANOVA with SPSS/Win 10.0 program. Results: 1.The 1st hypothesis, 'total spiritual well-being score in the experimental group, who received the spiritual nursing intervention, will be higher than the control group who did not receive the spiritual nursing intervention' was supported(F=6.28, p=0.015, Interaction: p=0.000). 2.The 1-1st sub-hypothesis, 'religious well-being score in the experimental group, who received the spiritual nursing intervention, will be higher than the control group who did not receive the spiritual nursing intervention' was supported(F=12.75, p=0.001, Interaction: p=0.000). 3.The 1-2nd sub-hypothesis, 'existential well-being score in the experimental group, who received the spiritual nursing intervention, will be higher than the control group who did not receive the spiritual nursing intervention' was supported(F=6.87, p=0.016, Interaction: p=0.000). 4.The 2nd hypothesis, 'depression level in the experimental group, who received the spiritual nursing intervention, will be lower than the control group who did not receive the spiritual nursing intervention' was supported(F=10.45, p=0.002, Interaction: p=0.000). Conclusion: From the above results, spiritual nursing intervention was an effective program to improve spiritual well-being state and decrease depression level for the hospice patients. In the future, when the spiritual intervention, which the researcher developed, applied on nursing field, the hospice patients can have comprehensive well being including spiritual well being and peaceful dying life.

뇌졸중 환자를 돌보는 주간호제공자의 부담감과 안녕감에 관한 연구 (A Study on Burden and Well-being of Primary Caregivers of Patients with a Stroke)

  • Purposes: The purposes of this study were to identify factors related to burden and well-being in primary caregivers of patients who have had a stroke and examine the correlation between burden and well-being of the caregivers. Methods: Between April 2006 to June 2007 data were collected using self-report questionnaires and interviews with 85 primary caregivers of stroke patients in C University Hospital. Data were analyzed using t-test, ANOVA, Duncan test and Pearson's correlation with SPSSWIN 15.0. Results: Factors related to burden of primary caregivers were sex and activities of daily living of the patients, and age, education level and satisfaction with income of the caregivers. The factors related to well-being of primary caregivers were sex of patients, and age and education level of caregivers as well as cohabitation with the patient. A negative correlation was found between burden and well-being of the caregivers (r= -.393, p<.001). Conclusion: These results indicate a need to develop a nursing intervention program for caregivers and education program for other family members to reduce caregiver burden. These programs should lead to improvements in the well-being of the caregiver.

암 환자들의 심리적 안녕을 위한 디지털 중재: 통합적 고찰 (Digital Interventions for the Psychological Well-being of Patients with Cancer: An Integrated Review)

  • Objectives: This study aimed to evaluate digital interventions on psychological well-being in patients with cancer through an integrated review. Methods: Fifteen studies were selected using PubMed, CINAHL, Scopus, DBpia, and RISS. The Scottish Intercollegiate Guidelines Network was used by two reviewers to evaluate independently the quality of the selected articles. Results: Digital interventions for improving psychological well-being in cancer patients were most often based on cognitive behavioral therapy alone or a combination of interventions with professionals and other patients. In addition, a number of interventions were established on internet-based mindfulness therapy. Conclusions: Providing cognitive behavioral intervention alone did not significantly improve the level of depression or anxiety of patients with cancer. Meanwhile, interventions, including cognitive behavior therapy and interactions with professionals and other patients, were found to reduce significantly the level of depression or anxiety in the experimental group.

아로마 손 마사지가 호스피스 환자의 안녕감에 미치는 효과 (The Effect of Aroma Therapy on Well-being in Hospice Patients)

  • Purpose: The purpose of this study is to compare the effect of aroma hand massage with hand massage without aroma on well-being of hospice patients. Methods: The design of this study is a quasi-experimental design. Data collection was conducted from May to December, 2012. Sixteen subjects were participated in the experimental group (aroma hand massage) and thirteen were paticipated in the control group (hand massage without aroma). The essential oil for aroma therapy was composed with 1% of Lavender and Bergamot. The aroma hand massage for the experimental group and hand massage for the control group were provided at 9 pm once a day for five days in a row. Subjects' characteristics and well-being were measured. To test the effect of aroma therapy a t-test was used with SPSS WIN 18.0. Results: There was no difference on well-being between aroma hand massage only group and hand massage group (t=1.90, p=.068). Conclusion: To develop aroma therapy to improve well-being for hospice patients, does not show that hand massage with aroma oil is superior them hand massage only for hospice patients. we recommend further studies to consider patients' preferences to aroma essential oil, aroma concentration and the time to provide aroma therapy need to be assessed.
