Effects of Robotic Gait Training with Lower Extremity Restraint on Static Balance, Lower Extremity Function, Gait Ability in Subacute Stroke Patients

  • Objective: The purpose of this study is to compare the effect of static balance, lower extremity function, and gait ability between a lower extremity restrain robot gait training and a general robot gait training in subacute stroke subjects. Design: Two-group pretest-posttest design. Methods: A total of 12 subacute stroke patients were randomly divided into an Experimental group (n=6) and a control group (n=6). Both groups were performed for four weeks, three times a week, for 20 minutes. To compare the Static balance function, the center of pressure (COP) path-length and COP velocity were measured. The Fugl-Meyer assessment lower extremity (FMA-LE) were evaluated to compare the Lower Extremity function. 2D Dartfish Program and 10 Meter Walking Test (10 MWT) on Gait ability were evaluated to compare the gait function. Results: In the intra-group comparison, Experimental groups showed significant improvement in COP path-length, velocity, Lower Extremity Function, 10 MWT, Cadence, by comparing the parameters before and after the intervention (p<0.05). Comparison of the amount of change between groups revealed significant improvement for parameters in the COP path-length, velocity, Lower extremity function, 10 MWT by comparing the parameters before and after the intervention (p<0.01). Conclusions: The Experimental group showed enhanced efficacy for variables such as COP path-length, velocity, Lower extremity function, 10 MWT as compared to the control group.

Accuracy Verification of Heart Rate and Energy Consumption Tracking Devices to Develop Forest-Based Customized Health Care Service Programs

  • This study was carried out to verify the accuracy of fitness tracking devices in monitoring heart rate and energy consumption and to contribute to the development of a forest exercise program that can recommend the intensity and amount of forest exercises based on personal health-related data and provide monitoring and feedback on forest exercises. Among several commercially available wearable devices, Fitbit was selected for the research, as it provides Open API and data collected by Fitbit can be utilized by third parties to develop programs. Fitbit provides users with various information collected during forest exercises including exercise time and distance, heart rate, energy consumption, as well as the altitude and slope of forests collected by GPS. However, in order to verify the usability of the heart rate and energy consumption data collected by Fitbit in forest, the accuracy of heart rate and energy consumption were verified by comparing the data collected by Fitbit and reference. In this study, 13 middle-aged women were participated, and it was found that the heart rate measured by Fitbit showed a very low error rate and high correlation with that measured by the reference. The energy consumption measured by Fitbit was not significantly different from that measured in the reference, but the error rate was slightly higher. However, there was high correlation between the results measured by Fibit and the reference, therefore, it can be concluded that Fitbit can be utilized in developing actual forest exercise programs.

여성 독거노인의 삶의 질 영향요인: 2021년 국민건강영양조사 자료활용 (Factors Affecting the Quality of Life of Female Seniors Living Alone: Using Data from the 2021 Korea National Health and Nutrition Examination Survey)

  • Purpose: This study sought to identify factors affecting the health-related quality of life (HINT-8) of female seniors aged 65 years or older living alone after the COVID-19 pandemic. Methods: Raw data from the eighth third year (2021) National Health and Nutrition Examination Survey was used, and the total number of subjects was 379. Data analysis was performed using SPSS 22.0 program, employing complex sample frequency and percentages, t-tests, analyses of variance, and regression analyses, while a post-hoc test (Bonferroni correction) was performed to determine differences in health-related quality of life. Results: The results of the study showed that the study subjects' health-related quality of life differed significantly depending on activity restrictions, number of walking days, age at menopause, perceived stress, depression, generalized anxiety disorder, and subjective health. Most of the influencing factors were emotional factors such as perceived stress level, generalized anxiety disorder, and subjective health status carrying an explanatory power of 46%. Conclusion: The results of this study can be used as basic data to establish intervention strategies based on factors that affect the health-related quality of life for female seniors living alone.

Finite element analysis of the behavior of elliptical cracks emanating from the orthopedic cement interface in total hip prostheses

  • This study examines crack behavior within orthopedic cement utilized in total hip replacements through the finite element method. Its main goal is to compute stress intensity factors (SIF) near the crack tip. The analysis encompasses two load types, static and dynamic, applied to a crack starting from the interface between the cement and bone. Specifically, it investigates SIFs under mixed mode conditions during three activities: normal walking, climbing upstairs, and downstairs. The results highlight that a crack originating from a micro-interface under substantial loading can cause cement damage, leading to prosthetic loosening. Stress intensity factors in modes I, II, and III are influenced by the crack tip's orientation and location in the bone cement, with a 90° orientation yielding notably higher values across all three modes.

만성 폐질환 환자에서 재택 호흡재활치료방법 개발 연구 (Development of the Home-Based Pulmonary Rehabilitation Program for Patients with Chronic Lung Disease)

  • Background : Even though it is well known that pulmonary rehabilitation (PR) improves exercise capacity, and the quality of life, in patients with chronic lung diseases, not many patients can attend hospital based intensive PR in Korea. The purpose of this study was to develop a method for a home-based PR program, and study its effectiveness. Materials and Methods : Twenty patients with chronic lung diseases were randomly divided into two groups : a home PR group comprising of 10 male patients, with a mean age of 70 years, and a control group comprising of 10 male patients, with a mean age of 65 years. We developed exercise programs, depending on the exercise capacity of each patient, which were easy to do at home. The PR program consisted of a 12 week period of enforced aerobic (mostly walking) and muscle strengthening exercises, as prescribed by the exercise specialist, in accordance with the functional capacity of the patient. In addition to the education, nutritional and psychiatric consultation was undertaken, and respiratory muscle training arranged. Patients visited hospital every 2 weeks for evaluation and exercise prescription. Results : All patients finished the 12 week course of therapy. Following the home PR, the endurance times and work capacity of the upper and lower extremities were significantly increased in the treatment group in comparison to the controls. The six minute working (Eds note:should) 'working' read "walking"?) distance was increased from $465{\pm}60m$ to $508{\pm}37m$ and the maximal inspiratory pressure from $72.8{\pm}27.2cmH_2O$ to $91.4{\pm}30.9cmH_2O$. The quality of life, as assessed by St Georges Respiratory Questionnaire (SGRQ), was also improved following PR. Conclusion : The home PR program we developed seemed to be applicable, and effective, to most of the patients with chronic lung diseases in this study.

팬데믹 시기 비대면 비만관리 프로그램의 개발 및 평가 (Development and Assessment of a Non-face-to-face Obesity-Management Program During the Pandemic)

  • 본 연구는 팬데믹 시기 비대면 비만관리 프로그램의 개발하여 적용한 후 그 효과를 평가하고자 하였다. 근거기반 비대면 비만관리 프로그램 개발은 Intervention Mapping Protocol (IMP)를 이용하였다. 프로그램의 운영은 대구광역시 시민건강 놀이터 온라인 채널을 통해 참여를 신청한 과체중 및 비만 성인 48명을 대상으로 2020년 9월 14일부터 11월 13일까지 8주 동안 실시하였다. 효과평가는 프로그램 전·후 자가 신체 계측 측정, 건강행태 설문 및 만족도 설문 분석을 통해 평가하였다. IMP를 통한 중재 프로그램의 개발은 IMP 6단계의 과정을 적용하였고 1단계 요구사정은 간호·영양·운동 교육자 면담을 통해 대상자 선정 기준, 프로그램 운영 방법 등을 결정하였다. 2단계 목표 설정은 비만관리를 위한 행동 변화목표 3가지를 설정하였고 3단계에서는 건강·영양·운동 영역별 근거기반으로 건강행태 변화를 위한 중재 방법 및 수행 전략을 선택하였다. 4단계 프로그램 설계는 8주 동안 대상자의 수행 과제 및 인증방법 등 구체적으로 프로그램을 설계하였고 5단계에서는 개발된 프로그램의 실행 계획을 작성하고 실제 적용하였으며 6단계에서는 적용한 프로그램의 효과를 평가하였다. 비대면 비만관리 프로그램 참여 전·후 신체 계측 값을 통한 효과 평가 결과 평균 체중은 1.2kg이 감량되었고, 허리둘레는 3cm가 감소하였으며, 체질량지수(BMI)는 0.8kg/m2 감소하였다(p<0.05). 대상자의 건강행태 설문을 통한 효과 평가 결과는 프로그램 참여 전·후 1일 평균 섭취 열량, 과일 섭취, 걷기 운동 실천 등의 생활습관이 긍정적으로 개선되었고 통계적으로 유의한 차이가 있었다(p<0.05). 프로그램 과정 평가를 위한 만족도는 4.57±0.63점으로 높게 나타났다. 본 연구 결과에 근거하여 비대면 비만관리 프로그램은 팬데믹 시기에 시간과 장소의 제약 없이 전문가의 개별상담과 자가 신체 계측 측정 및 기록을 통한 대상자의 적극적인 참여가 가능한 프로그램으로 지역사회 성인 대상 비대면 비만 관리에 유용하게 활용될 수 있을 것이다. 그러나 비대면 비만관리 프로그램은 앱 사용 등 대상자의 나이에 따라 비대면 프로그램 참여에 일부 제약이 있다. 따라서 본 연구로 개발된 비대면 프로그램과 함께 필요시 대면 프로그램의 병행을 제언하며, 추후 장기적인 프로그램의 운영에 따른 효과 평가를 검증하기 위한 지속적인 연구들이 필요할 것으로 사료된다.

범이론모형(Transtheoretical Model) 기반의 운동행위강화 프로그램이 고혈압 노인의 혈압 및 신체활동량에 미치는 효과 (Effects of a Transtheoretical Model Based Exercise Behavior Improving Program on Blood Pressure and Physical Activity for Older Adults with Hypertension)

  • 본 연구는 범이론모형에 기반하여 운동행위강화 프로그램을 개발하고 이를 고혈압 노인에게 적용하여 혈압 및 신체활동량에 미치는 효과를 규명하고자 하였다. 연구대상자는 서울시 소재 노인복지관에 내원하며 연구에 참여를 허락한 65세 이상 고혈압 노인 41명으로 실험군 20명, 대조군 21명이었다. 실험군에게는 범이론모형에 기반한 운동행위강화 프로그램을 8주 동안 적용하였다. 연구결과 실험군은 대조군에 비하여 운동행위 변화단계, 전체 변화과정 점수, 행위적 변화과정 점수, 운동 자기효능감, 신체활동량이 유의하게 증가하였으나, 경험적 변화과정 점수, 의사결정균형, 혈압 등은 유의한 차이가 없었다. 이러한 결과를 바탕으로 운동행위강화 중재는 비활동 고혈압 노인의 운동행위로의 이행과 신체활동량을 증진시키는데 효과가 있음을 확인하였다. 본 프로그램이 지역사회에 거주하는 고혈압 노인에게 적용된다면 운동습관을 형성하게 하여 고혈압 합병증을 예방하는데 기여할 것으로 기대된다.

재가운동 프로그램이 여성 고령자의 생활기능 체력과 건강관련 삶의 질에 미치는 영향 (Effect of HSEP on elderly Women's Life Function Fitness and Health-related QOL)

  • 본 연구의 목적은 70세 이상의 여성고령자들에게 재가운동 프로그램(home support exercise program)을 16주간 주3회씩 실시하여 생활기능 체력과 건강관련 삶의 질(SF-36)에 대한 주관적 평가에서의 변화를 살펴보고자 하였다. 실험군(총17명, 평균나이:79.4)과 통제군(총17명, 평균나이:80.7)은 부산시 연제구에 거주하며 신체적 외상이나 거동에 불편함이 없고, 연구 참여에 동의한 여성 고령자로 구성하였다. SAS 9.2 통계 패키지를 활용하여 집단간 기술통계와 동일성 검증 및 사전측정을 공변량으로 한 공분산 분석을 실시하였다. 그 결과는 다음과 같다. 첫째, 실험군이 통제군에 비해 덤벨 들기, 일어섰다 앉기, 2.24m 왕복달리기, 2분간 제자리 걷기 항목에서 통계적으로 유의하게(p<.001) 증가한 것으로 나타났다. 둘째, 유연성을 측정하는 등 뒤 손잡기 항목에서는 실험군에서 유의할 만한 변화가 없는 것으로 제시되었다. 이러한 점은 재가운동프로그램에 유연성을 향상시킬 수 있는 운동항목을 추가하여 보완할 필요성이 있음을 시사하는 것이었다. 셋째, SF-36을 통한 건강관련 삶의 질에 대한 주관적 평가에서는 8개의 모든 하위요인에서 실험군과 통제군간에 유의할 만한 차이가 없는 것으로 제시되었다. 다만 통증과 활력 하위요인에서 운동을 처방받은 실험군이 통제군에 비해 p<.10 수준에서 향상된 것으로 나타나 긍정적인 변화를 미세하게 시사하였다.

오타고 운동프로그램이 낙상 경험 노인의 신체기능 및 낙상 위험도에 미치는 영향 (Effects of Otago Exercise Program on Physical Function and Risk of Falling in the Falls Experienced Elderly)

  • 본 연구는 오타고 운동프로그램이 낙상 경험 노인의 신체기능 및 낙상 위험도에 미치는 영향을 알아보고자 실시하였다. 낙상을 1회 이상 경험한 노인 36명을 대상으로 각각 오타고 운동프로그램 집단(실험군)과 일반적인 균형훈련집단(대조군)으로 무작위 분류하였다. 각 집단은 30분씩 주 3회, 총 8주 동안 운동을 실시하였다. 낙상의 위험성을 평가하기 위해 Tetrax를 사용하여 측정하였다. 또한, 신체기능의 측정을 위해 정적균형은 Biorescue, 동적균형은 TUG, 보행능력은 10MWT, 하지근력은 FTSST를 이용하여 측정하였다. 각 그룹 내 운동 전·후 차이를 검증하기 위하여 대응표본 t-검정(Paired T-test)을 실시하였고, 그룹 간의 비교를 위하여 독립표본 t-검정(Independent Sample T-test)을 실시하였다. 실험결과, 실험군, 대조군 모두 정적균형능력 및 동적균형능력은 운동 후 유의하게 증가하였으며, 낙상의 위험성은 감소하였다(p<.05). 하지근력 및 보행능력은 실험군에서만 운동 후 유의하게 증가하였다(p<.05). 본 연구의 결과를 종합해보면, 오타고 운동프로그램은 낙상 경험 노인의 신체기능 증진 및 낙상의 위험성이 감소에 효과적이라는 것을 알 수 있었다.

주민의식에 기초한 도시 대공원의 이용후 평가 -울산 대공원의 공원효율성 및 이용 만족도를 중심으로- (A Study on the Post Occupancy Evaluation of Urban Grand Park with Reference to the Perception of Residents -Focused on Ulsan Grand Park's Efficiency and User's Satisfaction-)

  • The purpose of this study is to present data on the human activities responding to the physical environment of ‘Ulsan’s Grand Parks by evaluating user behavior and activity, visiting motivations, and user satisfaction. This study was conducted using multi-methods such as interviews and questionnaires surveys. The main findings of this study can be summarized in six parts as follows: \circled1 Behavior patterns showed that the users visited the park left within 30 minutes and the frequency of visits was 1 or 2 times per week. They spent their time mostly on ‘walking’ and ‘picnicking’. The users visited regardless of the seasons or the day of the week. \circled2 The priorities for improvements were analyzed as follows: the users expressed their demands for ‘shadowing facilities’ like shelters and pergolas because they used these facilities frequently. Also, the users would like an ‘event program’, ‘sign system’ and ‘guide program’. \circled3 Analysis of the the user’s perception of the park showed that they perceive the park as ‘representative source of the landscape and open space in Ulsan and place for making contact with nature. \circled4 In examining the visiting motivations of the users of Ulsan Grand Park, it was revealed that people use the park for ‘time with family and friends’, ‘to escape from city life’, ‘to relieve fatigue. As a result of factor analysis, 4 factors were identified such as ‘physical motive(MF1)’, ‘exploratory motive(MF2)’, ‘social motive(MF3)’ and ‘emotional motive(MF4). \circled5 Park users’ evaluation for park facilities showed that people are satisfied with most of the facilities and especially, they have high level of satisfaction for ‘footpaths’, ‘squares’ and ‘picnicspace’. The evaluation of the park user’s of activity reveals that they are content with nearly all the variables. Especially, they have high level of satisfaction for the variables of ‘convenience for dynamic activities’, ‘making of a beautiful atmosphere, ‘accessibility from the outside’ and, ‘convenience in group activitie. Factor analysis of the park user’s of activity revealed 5 factors such as ‘convenience and interest factor (AF1)’, ‘park maintenance, management and use program(AF2)’, ‘visual beauty(AF3)’, ‘safety and accessibility(AF4)’ and ‘crowding(AF5)’. \circled6 Regression analysis was employed to get the predictor factors of overall satisfaction with a result of 60.0%($R^2$). The variance was explained as ‘quality of the picnic space’, ‘convenience and interest factor while using the park’, ‘park program for maintain and management in the park’, ‘visually beauty while using the park’, ‘safety and accessibility of the parks’, ‘quality of the pond’, ‘crowding’, ‘quality of the square’.
