ARM Cortex 기반의 AED simulator 플랫폼 개발에 대한 연구 (A Study on the ARM Cortex-based AED simulator platform development)

  • In this paper, we implemented a high performance automated external defibrillator(AED) simulator that has various characteristics of virtual simulated patient(VSP) to verify important operations and functions of the AED. To verify the AED in various environment, the AED simulator should have the characteristics of VSP and can analyze the characteristics of electrical shock rhythm. In this paper, the implemented simulator has twenty seven electrocardiogram scenarios and we showed the implemented simulator has less error rate than conventional simulator in analyzing electrical shock rhythm.

가상현실 프로그램을 사용한 재활치료의 효과성 연구: 체계적 고찰 (A Study on the Effectiveness of Rehabilitation by Virtual Reality Program: Systematic Review)

  • 본 연구는 가상현실 프로그램을 사용한 재활치료의 효과성에 대해 체계적으로 분석하여 국내 임상현장에서 재활치료에 종사하는 의료인, 의료기사 및 연구자들에게 다양한 가상현실 기반의 재활치료와 효과성에 대한 정보를 제공하여 가상현실 기반의 재활치료가 임상에서 적절하게 적용될 수 있도록 하고자 한다. 연구 방법으로 학술연구정보서비스(Research Information Sharing Service; RISS), Google Scholar, Pubmed 데이터베이스를 사용하여 2007년 1월부터 2017년 12월까지의 논문을 검색하였다. 주요 검색어는 'Virtual Reality AND Rehabilitation', 'Virtual reality AND Effect'를 사용하였다. 최종적으로 10편의 해외 학술지에 게재된 논문을 본 연구의 분석 대상으로 선정하였다. 선정된 논문은 PICO (Patient, Intervention, Comparison, Outcome) 형식을 사용하여 정리하였다. 연구 결과 10개의 논문에서 참여한 대상자를 유형별로 분석한 결과, 대상자는 총 4가지로 분류되었으며 그 종류는 뇌졸중, 뇌종양, 파킨슨병, 신장질환이었다. 중재에 사용한 가상현실 기반 도구들은 IREX 3건(30%), X-box Kinect 2건(20%), Nintendo Wii 2건(20%), [Existing tool + VR]이 1건(10%), [Mobile application + VR]이 1건(10%)이었다. 중재 결과 10개의 논문 모두 기능향상이 나타났으며 신체기능이 24회(66%), 정신기능 6회(17%), 인지기능 5회(14%), 일상생활활동이 1회(3%)였다. 따라서 본 연구는 가상현실 기반 재활치료와 그 효과를 정리하여 재활치료서비스의 전문성과 질을 향상시키고, 영역을 확대하는 기초자료로 활용될 수 있을 것이다.

가상현실 시스템(Virtual Reality System)을 이용한 훈련이 척수손상환자의 앉기 균형 및 일상생활동작에 미치는 영향 (The Effect of Training Using Virtual Reality System on Sitting Balance and Activities of Daily Living for the Patient with Spinal Cord Injury)

  • Purpose: This study was examined the effect of training using a Virtual Reality System on the sitting balance and Activities of Daily Living for patients with a spinal cord injury. Methods: The subjects were divided into an experimental (6 persons) and control group (5 persons). The experimental group trained the 5 programs, three times per week for 6 weeks using the Virtual Reality System and five days for week using conventional physical therapy. The control group trained five days for a week using conventional physical therapy. Results: The difference in the mean Spinal Cord Independence Measurement (SCIM) score in the experimental and control groups was increased to 8.33 and 6.60 (p=0.79), respectively. The difference in the mean functional reaching test in experimental and control group increased to 4.21 and 1.09 (p=0.25), respectively. The difference in the mean sitting time in experimental and control group increased to 41.05 and 10.33 (p=0.66), respectively. There was a difference in the mean of all variances but these differences were not statistically significant. Conclusion: These results suggest that training using the Virtual Reality System increased the SCIM, functional reaching test and sitting time in people with a spinal cord injury.

라파엘 스마트 글러브를 이용한 가상현실 재활프로그램이 뇌졸중환자의 상지 기능과 일상생활활동 수행에 미치는 영향 (Effect of Virtual Reality Rehabilitation Program with RAPAEL Smart Glove on Stroke Patient's Upper Extremity Functions and Activities of Daily Living)

  • Purpose : This study examined the effects of a virtual reality rehabilitation program on stroke patients' upper extremity functions and activities of daily living (ADL). Methods : The subjects were equally and randomly divided into an experimental group (n=16) to whom a virtual reality rehabilitation program was applied and a control group (n=16) who received traditional occupational therapy. The intervention was applied five times per week, 30 minutes per each time, for six weeks. Jebsen-Taylor hand function test was conducted and the subjects' Manual Function Test was measured to examine their upper extremity functions before and after the treatment intervention, and a Korean version of modified Barthel index was calculated to look at their activities of daily living. Results : After the intervention, the upper extremity functions and activities of daily living of the participants in both groups significantly improved (p<.05). However, the improvements in these parameters among the participants in the virtual reality rehabilitation program were significantly greater than those in the control group (p>.05). Conclusion : The virtual reality rehabilitation program is a stable and reliable intervention method for enhancing the upper limb functions and activities of daily living of stroke patients.

대인공포증의 치료를 위한 가상 연설 시뮬레이터의 실험적 제작 (Construction of Virtual Public Speaking Simulator for Treatment of Social Phobia)

  • 대인공포증은 사람과의 대면을 무서워하여 그 상황을 회피하는 증상을 말한다. 이 공포증을 치료하기 위해서는 약물치료방법과 인지·행동 치료방법이 주로 사용되었다. 이런 기존의 방법들은 치료 효율이 떨어지며, 치료시 어려움이 있다는 단점을 가지고 있다. 최근에는 기존의 치료방법의 단점을 극복하기 위해 많은 연구에서 가상현실을 정신치료에 적용하고 있다. 가상환경은 환자에게 적절한 자극을 제공하여 환자로 하여금 공포감을 느끼도록 하고, 환자는 이러한 공포상황에 체계적으로 노출됨으로써 공포증을 극복할 수 있게 된다. 본 연구에서는 개인용 컴퓨터를 기반으로 가상 연설 시뮬레이터를 개발하여 대인공포증 치료에 이용하고자 한다. 가상 연설 시뮬레이터를 구성하기 위해 위치센서, 머리 부착형 디스플레이장치와 사운드 시스템을 사용하였고, 치료를 위한 가상환경은 가상청중 8명이 있는 세미나실을 배경으로 구성하였다. 또한 가상현실 정신치료에서 중요한 요소로 작용하는 몰입감을 높이기 위해 머리 부착형 디스플레이 장치에 위치센서를 부착함으로써 환자의 머리이동이 가상환경에 적용되도록 하였고, 여기에 삼차원 사운드를 적용함으로써 목소리나 박수소리 등 여러 가지 소리들이 사실적으로 느껴지도록 하였다. 또한 가상청중이 환자의 발표내용에 맞게 여러 가지 행동과 표정을 변하게 하였다. 본 연구에서 개발한 가상연설 시뮬레이터는 경제적이고 안전한 정신치료에 이용되는 것에 목적이 있으며, 앞으로의 임상실험을 통한 몰입과 공포유발 인자 및 치료효과의 에 관한 정보를 얻고, 이를 시뮬레이터에 계속 적용하는 연구가 필요할 것이다.

  • Virtual Reality (VR) is a new technology that enables humans to communicate with a computer, It allows the user to see, hear, fuel and interact in a three-dimensional virtual world created graphically. Virtual Reality Therapy (VRT), based on this sophisticated technology, has been recently used in the treatment of subjects diagnosed with acrophobia. Acrophobia is a disorder that is characterized by marked anxiety upon exposure to heights, avoidance of heights, and a resulting impairment in functioning. Conventional virtual reality systems for the treatment of acrophobia have limitations, such as overly expensive devices or somewhat unrealistic graphic scenes. The goal of this study is to develop an inexpensive and more realistic virtual environment in which to perform an exposure therapy fur acrophobia. It runs on a personal computer, and a virtual scene ova bunge-jump tower in the middle of a large city. The virtual scenario includes an open tin surrounded by props beside a tower, which allows the patient to feel a sense of heights. The effectiveness of the VR environment was evaluated through the clinical treatment of a subject who was suffering from the fear of heights. Based on pre- and post- questionnaires and subjective comments from the subject. This virtual reality environment proved to be an effective and realistic tool fur the treatment of acrophobia.

불안장애에 대한 가상현실치료 국내동향분석 (Domestic Trend Analysis of Virtual Reality Therapy for the Treatment Anxiety Disorders)

  • Objectives: Many fields have changed since covid-19, psychotherapy being no exception. With the pandemic associated increase in untact treatment, interest in virtual reality exposure therapy has also increased. The aim of study was to analyze recent literature using virtual reality therapy for the treatment of anxiety disorders. Methods: We searched five databases from dates between October 1 to November 1, 2020. We selected articles related to the treatment of anxiety disorders using virtual reality therapy and analyzed the trend of that, not thesis but domestic articles. Results: There were 6 RCT studies and 3 were case reports where, based on the DSM-4, the participants presented with anxiety disorders including 2 with presentation anxiety, 1 heterosexual anxiety, 1 test anxiety, 1 social anxiety, 1 dental anxiety, 2 Acrophobia, and 1 social phobia. Treatment sessions varied from 1 session to 18 sessions, but 5 studies conducted 4 sessions, and all studies attempted virtual reality exposure for 30-minute time periods. 8 studies used HMD as the VR device and 1 study did not report a device. Conclusions: Anxiety disorders treated using virtual reality included speech anxiety and acrophobia. Median treatment session number was 4 and sessions were less than 30 minutes. The most common VR device used was an HMD. VR psychotherapy showed limitations relating to patient experience including cyber sickness and a lack of immersion. In addition, most of the studies were conducted with patients who visited hospitals utilizing Western medical practices. Currently, virtual reality therapy (VRT) intervention in oriental medicine is lacking. The lack of research in this area suggests analyzing data from VR psychotherapy in oriental medicine could provide novel and useful information.

가상현실을 이용한 치과 임플란트 수술 교육 (Virtual Reality for Dental Implant Surgical Education)

  • 본 연구에서는 실제적인 환자의 모델을 바탕으로 가상현실 기반의 수술환경을 제작하는 방법에 대한 고찰과 함께 치과 임플란트 수술 술기를 배울 수 있는 가상현실을 활용한 수술 모델에 대한 평가를 시행하고자 한다. 환자의 안면 해부학적 모델은 실제 환자의 CT 데이터를 바탕으로 안면부위 골에 대한 삼차원 모델을 먼저 완성한 후 각각의 근육을 표현하고 안면부에 대한 삼차원 스캔 그리고 치아의 3차원 스캔 후 모델링을 통해 각각의 해부학적 구조물을 표현하였다. Unity3D를 활용해 단계별 시나리오를 게임으로 구성하였다. 처음 절개를 시행하는 단계에서부터 환자의 현재 상태에 필요한 상악동 거상술에 대한 술기를 표현하고, 임플란트 식립과정 그리고 골이식술을 하는 과정을 단계별로 포함하였다. 단계별로 중요한 과정의 효과적인 습득을 위해 HMD(Head Mount Display) 및 Leap motion과 연동하여 3차원 모델에 기구를 위치시키고 수술행위를 재현해볼 수 있게하여 몰입감을 높이면서 실제 수술을 하고있는 느낌을 재현하고자 하였다. 총 20명의 수련의를 대상으로 가상현실 시뮬레이션을 수행한 후에 설문조사를 시행하였다. 임플란트 수술 교육 프로그램은 치과 대학생들과 수련의들의 교육 도구로서 수술 술기를 습득하고 평가하는데 사용할 수 있는 가능성을 보여주었다. 통상적으로 수술 술기 교육 방법이 많지 않은 현실에서 HMD 와 Leap motion이라는 비교적 저렴한 기기를 이용하여 몰입감을 높일 수 있는 가상현실 기반의 수술 교육은 장점이 있다고 말할 수 있다.

Optimal Density Assignment to 2D Diode Array Detector for Different Dose Calculation Algorithms in Patient Specific VMAT QA

  • Background: The purpose of this study is to assign an appropriate density to virtual phantom for 2D diode array detector with different dose calculation algorithms to guarantee the accuracy of patient-specific QA. Materials and Methods: Ten VMAT plans with 6 MV photon beam and ten VMAT plans with 15 MV photon beam were selected retrospectively. The computed tomography (CT) images of MapCHECK2 with MapPHAN were acquired to design the virtual phantom images. For all plans, dose distributions were calculated for the virtual phantoms with four different materials by AAA and AXB algorithms. The four materials were polystyrene, 455 HU, Jursinic phantom, and PVC. Passing rates for several gamma criteria were calculated by comparing the measured dose distribution with calculated dose distributions of four materials. Results and Discussion: For validation of AXB modeling in clinic, the mean percentages of agreement in the cases of dose difference criteria of 1.0% and 2.0% for 6 MV were $97.2%{\pm}2.3%$, and $99.4%{\pm}1.1%$, respectively while those for 15 MV were $98.5%{\pm}0.85%$ and $99.8%{\pm}0.2%$, respectively. In the case of 2%/2 mm, all mean passing rates were more than 96.0% and 97.2% for 6 MV and 15 MV, respectively, regardless of the virtual phantoms of different materials and dose calculation algorithms. The passing rates in all criteria slightly increased for AXB as well as AAA when using 455 HU rather than polystyrene. Conclusion: The virtual phantom which had a 455 HU values showed high passing rates for all gamma criteria. To guarantee the accuracy of patent-specific VMAT QA, each institution should fine-tune the mass density or HU values of this device.

Virtual Design Considerations for Fixed Dental Prosthesis Including Axial Contour and Proximal Contact to Maintain Periodontal Health and Physiologic Function: A Narrative Review

  • The axial contour and proximal contact of the prosthesis affect periodontal health, food impaction, and patient satisfaction. This narrative review provides a summary of articles regarding the axial contour and proximal contact of a fixed dental prosthesis on periodontal health and physiologic function. By acquiring a comprehensive understanding of the axial contour and proximal contact of teeth and prostheses, as well as their functional significance, the virtual design of fixed dental prosthesis can be optimized to maintain periodontal health and promote physiologic function effectively.
