One Year Experience of the Hand Allotransplantation First Performed after Korea Organ Transplantation Act (KOTA) Amendment

  • The revision of the Korea Organ Transplantation Act (KOTA) in 2018 included hand/arm among the organs that can be transplanted. The first hand transplantation since the revision of KOTA took place in January 2021. A 62-year-old male patient experienced hand amputation on July 13, 2018, by a catapult injury. The patient first visited our institute 3 months after the injury. After serial interviews and an overall evaluation, the patient was registered on the hand transplantation waiting list in January 2020. On January 9, 2021, the patient underwent hand transplantation at the right distal forearm level. The total operation time was 17 hours 15 minutes, and the cold ischemic time was 4 hours 9 minutes. Postoperative immunosuppression was administered based on the protocol used for kidney transplantation. Two acute rejection episodes occurred, on postoperative days 33 and 41. Both rejection episodes were reversible with rescue therapy of a higher tacrolimus trough level, steroid pulse therapy, and topical immunosuppressants. Controlled passive range of motion exercise was started on postoperative day 10. Dynamic splint was applied on postoperative day 18. At 1 year, graft maintenance and functional improvement were satisfactory, and the patient showed a Disabilities of Arm, Shoulder and Hand score of 25.8. We successfully performed the first hand transplantation surgery under the KOTA amendment. It came from the organic and effective cooperation of plastic, orthopaedic, and transplantation departments and we believe it will guarantee the future ongoing success.

A 2.5 Gb/s Burst-Mode Clock and Data Recovery with Digital Frequency Calibration and Jitter Rejection Scheme (디지털 주파수 보정과 지터 제거 기법을 적용한 2.5 Gb/s 버스트 모드 클럭 데이터 복원기)

  • In this paper, 2.5 Gb/s burst-mode clock and data recovery(CDR) is presented. Digital frequency calibration scheme is adopted to eliminate mismatch between the input data rate and the output frequency of the gated voltage controlled oscillator(GVCO) in the clock recovery circuitry. A jitter rejection scheme is also used to reduce jitter caused by input data. The proposed burst-mode CDR is designed using 0.11 ${\mu}m$ CMOS technology. Post-layout simulations show that peak-to-peak jitter of the recovered data is 14 ps with 0.1 UI input referred jitter, and maximum tolerance of consecutive identical digit(CID) is 2976 bits without input data jitter. The active area occupies 0.125 $mm^2$ without loop filter and the total power consumption is 94.5 mW.

Influencing Factors on Father-Child Attachment in Marital Satisfaction, Marital Conflict, Parenting Stress, and Parenting Attitudes (결혼만족도, 부부갈등, 양육스트레스, 양육태도가 아버지와 아동의 애착에 미치는 영향)

  • The purpose of this study is to identify the level of father-child attachment and to explore possible factors that may influence such attachment level in marital satisfaction, marital conflict, parenting stress, and parenting attitudes. The data were collected using a structured questionnaire from 131 fathers living in B city, between February 16 and August 16 in 2014. The collected data were analyzed using descriptive statics, ANOVA, Pearson correlation coefficient, and multiple regression with SPSS 21.0 program. The mean of father-child's attachment was $3.79{\pm}0.47$. The level of father-child attachment was different according to monthly income (f=5.25, p<.010). There was a significantly positive correlation between father-child attachment and father's marital satisfaction (r=.28, p=.001). Father-child attachment was negatively correlated with father's marital conflict (r=-.23, p=.01), parenting stress (r=-.44, p<.001), acceptance-rejection (r=-.64, p<.001), and control (r=-.30, p<.001), with statistical significance. The acceptance-rejection attitude of father had significant effects on father-child attachment and accounted for 40% of the total variance of the attachment. These results indicated that decreasing the father's acceptance-rejection attitude could be effective in improving father-child attachment.

Development of A Nurse는s Suffering Experience Scale (말기 암 환자를 간호하는 간호사의 고통경험 척도개발)

  • The purpose of this study was to develop Nurse's Suffering Experience Scale and to test the reliability and validity of the instrument. Method: The subjects used to verify the scale's reliability and validity were 220 nurses who were taking care of the end stage cancer patients, while working at university and general hospitals in Daegu and Kyungbuk province from April 20. to July 10, 2001. The data was analyzed by the SPSS/WIN 8.0 program. Results: A factor analysis was conducted, and items that had a factor loading more than .40, and an eigen value more than 1.0. were selected. The factor analysis classified a total of seven factors statistically, and it's communality was 44%. The explanation of factors based on the conceptual framework and item content are as follows: The first factor was expanding self consciousness, the second factor was forming empathy with family, the third factor was professional challenge, the fourth factor was change of values, the fifth factor was spiritual sublimation, the sixth factor was helplessness, and finally the seventh factor was rejection to death. Cronbach's coefficient to test reliability of the scale was .8665 for total of 44 items. The Scale for Nurse's Suffering Experience developed in the study was identified as a tool with a high degree of reliability and validity. Therefore this scale can be effectively utilized for the evaluation of the degree of nurse's suffering experience in clinical settings.

Development of Simulation Program of Automotive Engine Cooling System (자동차 엔진냉각계의 해석 프로그램의 개발)

  • A numerical program has been developed for the simulation of automotive engine cooling system. The program determines the mass flow rate of engine coolant circulating the engine cooling system and radiator cooling air when the engine speed is adopted by appropriate empirical correlation. The program used the method of thermal balance at individual element through the model for radiator component in radiator analysis. This study has developed the program that predicts the coolant mass flow rate, inlet and outlet temperatures of each component in the engine cooling system (engine, transmission, radiator and oil cooler) in its state of thermal equilibrium. This study also combined the individual programs and united into the total performance analysis program of the engine cooling system operating at a constant vehicle speed. An air conditioner system is also included in this engine cooling system so that the condenser of the air conditioner faces the radiator. The effect of air conditioner to the cooling performance, e.g., radiator inlet temperature, of the radiator and engine system was examined. This study could make standards of design of radiator capacity using heat rejection with respect to the mass flow rate of cooling air. This study is intended to predict the performance of each component at design step or to simulate the system when specification of the component is modified, and to analyze the performance of the total vehicle engine cooling system.

  • In this paper, the multi-sensor signal processing IC is designed. It consists of an analog multiplexer for selection of multi-sensor signals, active filters for noise rejection and signal amplification, and a sample and hold circuit for interface with digital signal processing. By implementing these circuits with CMOS transistors, integration, low power dissipation and miniaturization of the total signal processing system have been made possible.

  • This paper is aiming at implementation of real-time speaker verification system using DSP board. Dialog/4, which is based on microprocessor and DSP processor, is selected to easily control telephone signals and to process audio/voice signals. Speaker verification system performs signal processing and feature extraction after receiving voice and its ID. Then through computing the likelihood ratio of claimed speaker model to the background model, it makes real-time decision on acceptance or rejection. For the verification experiments, total 15 speaker models and 6 background models are adopted. The experimental results show that verification accuracy rates are 99.5% for using telephone speech-based speaker models.

Consumer Satisfaction and Dissatisfaction in Return Process (반품과정에서의 소비자만족.불만경험)

  • This study investigated consumer satisfaction and dissatisfaction experiences in post-purchase product return process. As an exploratory research the study collected qualitative data of subjective consumer experiences using an open-ended questionnaire and identified and classified themes and patterns of satisfaction and dissatisfaction experiences. A total of 113 cases were categorized into satisfied experiences, unsatisfied experiences, and neutral experiences. Further, satisfied experiences were sub-categorized into kind sales associates, quick and easy return process, and services higher than expected based on sources of satisfaction in the return process while unsatisfied experiences were sub-classified into rejection of return, attitude of sales associates, and hassle of return process. In-store adaptive behaviors and intentions on future behaviors were also observed. The results indicated that service quality in the post-purchase service encounter and consumer expectation were important in determining satisfaction and dissatisfaction in the return process. Insights for research questions were proposed.

On the Comparison of Call Overload Control Methodologies in ATM Networks (ATM 통신망에서의 호 과부하 제어방식 비교)

  • Automatic repeated recalling from individual computing equipments such as PC and workstations, to access ATM networks may result in call overload to networks. Call overload causes to waste network resources and fail to meet user call processing QoS requirements. We evaluate two call connection request methods: the back-off recall method(BRM) which makes recall with random waiting time after rejection, and the direct recall method(DRM) which repeats call connection request whenever call request is rejected. We evaluate their performances by simulation and it shows that as the total number of attached terminals to each access node increases, BRM performs better than DRM in terms of the call acceptance level and effective network bandwidth utilization.

  • Recently, the rapid growth of mobile radio system has led to an increasing demand of low-cost high performance communication IC's. In this paper, we have designed RF front end for wireless LAN receiver employ zero-IF architecture. A low-noise amplifier (LNA) and double-balanced mixer is included in a front end. The zero-IF architecture is easy to integrate and good for low power consumption, so that is coincided to requirement of wireless LAN. But the zero-IF architecture has a serious problem of large offset. Image-reject mixer is a good structure to solve offset problem. Using offset compensation circuit is good structure, too. The front end is implemented in 0.25 ${\mu}m$ CMOS technology. The front end has a noise figure of 5.6 dB, a power consumption of 16 mW and total gain of 22 dB.
