개에서 제3안검선 냉동수술이 눈물생산량 및 조직학적 변화에 미치는 영향 (Effects of Cryosurgery of the Third Eyelid Gland on Tear Production and Histological Changes in Dogs)

  • The effects of cryosurgery on tear production and histological changes of the third eyelid were studied in dogs. Clinically normal 12 mixed breeds weighing 2∼6 kg were divided into three groups and treated as follows; 45 seconds double freeze-thaw treated group, 60 seconds double freeze-thaw treated group and 90 seconds double freeze-thaw treated group. The significant decrease of the tear production after cryosurgery was shown in all groups throughout the observed periods(p<0.05). However, there was no difference among groups. The main complications after cryosurgery were chemosis and conjunctival injection. Other complications such as eyelid edema, eyelid depigmentation and keratitis were more preominent in group III compared to those of groups I and II. On histopathological examination, chronic inflammatory changes and regeneration of the third eyelid glands were noted in group I and predominated loss of the third eyelid glands and necrosis of hyaline cartilage were observed in group III However, such changes were less appeared in group II. The results of this study suggested that double freeze-thaw cryosurgery for 60 seconds on the third eyelid glands would be the most effective method for treating tear staining syndrome.

Computed tomographic features of third eyelid gland adenocarcinoma in a dog: a case report

  • A 15-year-old Schnauzer, showing right exophthalmos, was diagnosed as adenocarcinoma originated from the third eyelid gland. On computed tomography, a normal right third eyelid gland was not observed. Instead, a heterogeneous cystic mass invaded the retrobulbar space and displaced the eye globe dorsolaterally. In addition, lysis of the bony nasolacrimal duct was found, which was considered the tumor invasion. These findings indicated that third eyelid gland adenocarcinoma should be considered when a retrobulbar mass is found ventromedial to the globe without observation of the normal third eyelid and accompanies osteolysis of the bony nasolacrimal duct in dogs showing exophthalmos.

  • This report aims to describe a case of tumor-like lymphoplasmacytic conjunctivitis in a 7-year-old spayed-female Pomeranian. On complete ophthalmic examination, a mass with papillary projections was noted on the bulbar surface of the right third eyelid. Debulking of the mass was performed while preserving as much of the third eyelid as possible. On the histopathological examination, the mass was diagnosed as lymphoplasmacytic conjunctivitis with mild epithelial hyperplasia. Although a slight regrowth of the mass was noted 3 weeks after surgery, intralesional injection of triamcinolone acetonide led to its disappearance. There was no further recurrence after 5 months.

Holstein 유우의 안검암종 증레 (A Case of a Holstein Cow with Eyelid Carcinomatosis)

  • A ten-year-old holstein cow was presented because of prolapse of the third eyelid and apparent hyperplasia of the right lower eyelid. Historical findings included increased appetite as well as polyuria and polydipsia for about two weeks. The most remarkable fadings on physical examination were a large periocular proliferative tissue and bleeding. Surgical incision was used both as a biopsy and therapeutic tool in holstein cow with mass. Histopathological examination of the mass revealed squamous cell carcinoma. Blood and milk tests of patient with squamous cell carcinomatosis were peformed, but normal values.

개에서 제3안검 제거 후 전기소작법을 이용한 누점폐쇄 효과 (The Effect of Punctal Occlusion with Thermal Cautery after Third Eyelid Resection in Dogs)

  • To evaluate the effect of punctal occlusion on tear preservation after third eyelid resection, 10 eyes were operated in dogs and observed changes in tear preservation using Schirmer tear test (STT). Punctal occlusion was performed on five healthy dogs with no clinical sings of ocular irritation and no history of dry eye syndrome. The STT was performed for 10 days before surgery, and on the day 0, 1, 3, 5, 7, 9, 11, 13, 17, 21, 25, and 30 after third eyelid resection. The mean STT was $17.4{\pm}0.15$ mm/min($mean{\pm}SEM$) for 10 days before surgery. And, it was $11.5{\pm}0.55$mm/min for 30 days after surgery in puntal occlusion(PO) group. The mean STT was $17.3{\pm}0.32$ mm/min for 10 days before surgery. And, it was more decreased to $6.5{\pm}0.60$ mm/min for 30 days after surgery in control group. The paired t-test was performed. Mean STT after the third eyelid resection was higher in PO group than that of control throughout experimental period (p<0.05). In summary, the effect of thermal punctal occlusion significantly increased on the tear preservation by $28.86{\pm}7.96%$. These results suggest that application of thermal punctal occulsion in dry eye syndrome will promote tear preservation.

Orbital exenteration for a third eyelid gland carcinoma in a dog

  • A 13-year-old neutered male Poodle dog was presented with a third eyelid mass in the left eye. The dog had undergone local resection of the mass about a year prior in a private practice. On cytological examination, the mass was diagnosed as adenocarcinoma. Although lung and lymph node metastases were suspected, based upon the computed tomographic results, exenteration was performed to relieve chronic pain and to improve the dog's quality of life. Exenteration carried a good prognosis with no tumor recurrence until 1 year and 10 months after surgery, when local recurrence occurred near the left zygomatic arch.

Marsupialization of the Nictitating Membrane Cyst Following Cherry Eye Repair in a Dog

  • One-year-old male Cocker Spaniel dog was referred for the third eyelid enlargement and inflammation in the left eye (OS). It gradually swelled for 2 weeks after the cherry eye repair by conjunctival mucosa pocket procedure at a private animal clinic. Routine ophthalmic examinations including neuro-ophthalmic examination, Schirmer tear test, intraocular pressure and corneal fluorescein staining were all normal. No lesions were found on slit lamp biomicroscopy and indirect ophthalmoscopy except for third eyelid swelling in the OS. Ultrasonography revealed cystic structure within the OS nictitating membrane. Fluid from the cyst was aspirated and there were no microorganisms or neoplastic changes. Surgical intervention was performed under general anesthesia. On the day of the surgery, there was a deep corneal ulcer in the OS, which had not existed before. Ventral palpebral surface of the third eyelid was incised horizontally to the shaft of the T-shaped hyaline cartilage. And then, a full thickness of the cystic wall was incised and marsupialized. Additionally, a direct suture was performed on the ulcerated cornea. Topical and systemic antibiotics and anti-inflammatory drugs were prescribed. One month after the surgery, the third eyelid swelling and the discharge were improved. Marsupialization of the nictitating membrane cyst relieved the swelling of the third eyelid and inflammation. It could be a simple but effective surgical intervention for the cystic complication of conjunctival mucosa pocket procedure in dogs.

Reconstruction of periorbital defects using a modified Tenzel flap

  • Background: Extensive eyelid defects are extremely challenging to reconstruct. Although numerous procedures for reconstructing periorbital defects have been proposed, no method is universally used. However, the Tenzel flap is the most commonly used technique to reconstruct eyelid defects affecting one-third to two-thirds of the eyelid. Methods: Recognizing the usefulness of the Tenzel method, we adapted it to reconstruct larger defects around the eyes. Seven patients underwent reconstruction with a modified Tenzel flap with an extended concept after wide excision of a malignant skin lesion. The main difference from the conventional method is that the modified Tenzel flap includes the medial portion of the lower lid defect. The design of a modified Tenzel flap begins as a semicircle at the lateral canthal area, in the same way as a classical Tenzel flap, and extends medially along the subciliary line to cover the defect on the medial lower eyelid. The follow-up time ranged from 3 to 28 months. Results: All flaps survived and healed well, with minimal scarring and natural palpebral outlines. Conclusion: Compared to traditional procedures, the modified Tenzel flap has several advantages, including a one-stage operation, a less noticeable scar, and effective prevention of complications such as lower eyelid ectropion.

홍채 인식을 위한 포물 허프 변환 기반 눈꺼풀 영역 검출 알고리즘 (Eyelid Detection Algorithm Based on Parabolic Hough Transform for Iris Recognition)

  • 홍채 인식은 홍채 패턴 정보를 이용하여 사람의 신원을 확인하는 생체 인식 기술이다. 일반적인 홍채 인식 시스템에서 취득된 홍채 영상에는 홍채 패턴 정보를 가리는 눈꺼풀이 포함된다. 이러한 눈꺼풀은 홍채 인식의 성능을 저하시키는 요소이다. 따라서 본 논문에서는 홍채인식의 정확성을 향상시키기 위해 눈꺼풀 검출 알고리즘을 제안한다. 본 연구는 기존의 방법에 비해 다음과 같은 세 가지 차별성과 장점을 가지고 있다. 첫 번째, 눈꺼풀 검출에 문제가 되는 속눈썹과 조명 반사광(specular reflection)을 기존의 방법에 의해 검출한 후에, 선형 보간법(interpolation)을 이용하여 제거하는 방법을 제안함으로써 눈꺼풀 추출의 정확도를 향상하였다. 두 번째, 기존의 알고리즘은 눈꺼풀 후보점을 추출하기 위해 홍채의 넓은 부분을 탐색하므로 영상잡음이나 홍채 패턴 등에 의해 눈꺼풀을 잘못 추출하는 경우가 많았다. 이러한 문제를 해결하기 위하여 본 논문에서는 검출된 홍채의 외곽경계 정보에 의해 초기 눈꺼풀 탐색 영역을 결정하고, 마스크 기법을 이용하여 눈꺼풀 후보점들을 추출함으로써 눈꺼풀 추출 에러를 감소시켰다. 세 번째, 기존의 알고리즘들은 포물선 방정식에 의해 눈꺼풀 영역을 검출하지만, 사용자의 눈의 회전을 고려하지 않았기 때문에 많은 에러가 발생되었다. 따라서 제안하는 알고리즘은 눈의 회전을 고려한 회전된 포물선 방정식을 이용한 허프 변환(Hough transform)을 통해 눈꺼풀을 검출함으로써 이러한 에러 발생을 감소시켰다. CASIA 데이터베이스의 홍채 영상을 사용하여 제안하는 눈꺼풀 검출 알고리즘을 실험한 결과, 위 눈꺼풀의 검출 정확도는 90.82%, 아래 눈꺼풀의 검출 정확도는 96.47%였다.

  • The eyelids perform many complex functions and are esthetically important, since eye is focal point of face. Tumor extirpation or trauma can lead to full-thickness eyelid defect, which Should be reconstructed as soon as possible There are a number of operation methods for lower lid reconstruction, and among them, Hughes procedure is a time-honored method. Its advantages are good cosmetic result and conjunctival repair by using eyelid structures to rebuild eyelid structures. However, this is a two-staged procedure in which the vision of the affected eye is obstructed during the interval between the operations. We experienced a case of trauma-induced full-thickness lower lid defect extending over one-third of lid length treated by Hughes method. The cosmetic and functional result was good and there was no donor site morbidity, and now we report this case with review of literatures.
